Objective To evaluate the value of Feridex in demonstrating small focal lesions of the liver on MRI and to discuss the optimal scan time after intravenous administration of Feridex. Methods Small focal lesions in the liver were found on ultrasound or CT in 17 patients. Conventional MR plain scan and Gd-DTPA enhanced scan were performed. One to three days later, T 2 weighted imaging was repeated at 0.5 h, 3 h, and 6 h after Feridex was administered intravenously over 30-40 min at a dose of 0.05 ml/kg. The pre- and post-contrast T 2WI were compared and analyzed in terms of lesion detectability and signal change. Results Twenty-one lesions with diameter smaller than 1 cm were found on the Feridex enhanced images, and only 8 lesions were found on the conventional MR images. The small hepatic cancers, focal nodular hyperplasia (FNH), and regenerated nodules had different signal changes on the Feridex images, which made the differential diagnoses easier. The signal of the liver decreased significantly (P0.05). Conclusion Fridex enhanced MR images could not only improve the detection of small focal lesions, but also provide evidence for differential diagnoses.

Objective To investigate the culture and proliferation of dendritic cells from human cord blood and malignant pleural effusions and identify the phenotype and the function of them in vitro. Methods Existing DCs were obtained from cord blood and malignant pleural effusions. The DCs were cultured with IL-4, GM-CSF and TNF-?. The morphological properties of DCs were observed by invert optical microscope. The phenotypes of DCs were analysed by flow cytometry. The proliferative bioactivities of T cells activated by DCs were observed with mixed lymphocyte reaction (MLR). Results No remarkable differences were found in the morphology and cellular phenotype of the DC, derived from two different sources, but there was remarkable difference between their culture time. The DCs from two different sources could activate homologous lymphocytes to proliferate mostly. But the capacity to stimulate the proliferation of homologous lymphocytes had remarkable difference. Conclusion Mature DCs could be induced from cord blood and malignant pleural effusions.

Objective:To investigate the protective effects of Ghrelin on LPS-induced apoptosis of human alveolar epithelial A549 cells, along with the possible molecular mechanisms. Methods: CCK-8 assay was used to examine the cell viability of A549 treated by LPS.Apoptosis of A549 cells was measured by TUNEL.NO(Nitric oxide)production was detected by NO-specific fluorescent probe 3-Amino,4-aminomethyl-2′,7′-difluoresceindiacetate(DAF-FM DA).Western blot was also performed to examine the expressions of iNOS(inducible nitric oxide synthase),AKT,ERK,p-AKT,p-ERK and apoptotic proteins, such as Bcl-2,Bax,and cleaved caspase-3.Results: LPS exposure impaired cell viability and increased apoptosis of A549 cells significantly in concentration-and time-dependent manners accompanied with increased Bax and cleaved caspase-3 production, coupled with decreased Bcl-2 levels.Meanwhile,LPS promoted iNOS expression and the production of NO.Ghrelin pretreatment ameliorated LPS-caused alterations in the ratio of Bax/Bcl-2 and cleaved caspase-3 expression.TUNEL analysis showed that Ghrelin could decrease the apoptosis induced by LPS in A549(P<0.05).Simultaneously,LPS remarkably decreased the expression of p-AKT and p-ERK in A549 cells, which was abrogated by Ghrelin pretreatment. However,Ghrelin had no significant effect on NO production induced by LPS.Conclusion: Ghrelin reduces LPS-induced apoptosis of human alveolar epithelial cells partly through activating the AKT and ERK pathway, but the level of iNOS derived NO could not be reduced.

Objective To analyze the potential factors facilitating post-pyloric placement of spiral naso-jejunum tube in critically ill patients. Methods A retrospective study was carried out in patients requiring enteral nutrition (EN) from Apr 2005 through Dec 2011 in Intensive Care Unit (ICU).Severity of illness was assessed with APACHE Ⅱ score (acute physiology and chronic health evaluation Ⅱ).A selfpropelled spiral naso-jejunum tube was placed and observed for 24 hours. The forward movement and place of the tube tip was checked by bedside X-ray.The APACHE Ⅱ score, therapeutic measures, agents administered within 24 hours after tube insertion were recorded. The patients were divided into the success group and the failure group identified by bedside X-ray whether the tube tip entered into jejunum or not.Univariate analysis and multivariate Logistic regression analysis were used to find out the potential factors impacting on the success or failure in post-pyloric placement of naso-jejunum tube. Results A total of 508 patients composed of 337 male and 171 female, and aged (62.0 ± 19.2) years with APACHE Ⅱ score of (21.9 ± 7.3) were enrolled for study. The placement was successful in 205 (40.4%) of 508 patients.Univariate analysis showed that APACHE Ⅱ score ≥ 20,sedatives and analgesics, catecholamines, prokinetics, artificial airway and mechanical ventilation were potential factors facilitating the post-pyloric placement of naso-jejunum tube.Multivariate logistic regression identified that APACHE Ⅱ score ≥ 20,sedatives and analgesics and prokinetics were independent factors facilitating the post-pyloric placement of naso-jejunum tube. Conclusions The success rate of self-propelled spiral nasojejunal tubes insertion was relatively low.The prokinetics contributed higher success rate of naso-jejunum tube placement than factors of APACHE Ⅱ score ≥ 20,sedative and analgesic, catecholamine drugs, artificial airway and mechanical ventilation. There were no effects of age and gender on the placement of naso-jejunum tube.
