1.Antifilaggrin autoantibodies in rheumatoid arthritis
Chinese Journal of Rheumatology 2000;0(06):-
Objective To investigate the significance of antifilaggrin autoantibodies (AFA) in rheumatoid arthritis (RA).Methods Filaggrin was extracted from the skin of the patients who had breast cancer undergoing mammectomy.AFA were detected at the sera titer 1∶10 with Western blotting, and its clinical significance was investigated with the corresponding clinical materials. Results Antifilaggrin autoantibodies (AFA) were detected in 39 9%(71/178) of RA patients, and 6 cases of control group, with a specificity of 0 969.AFA were present in 48 4%(15/31) of the early and 29 2% of seronegative RA.AFA positive patients exhibited higher frequency of Sjgren′s syndrome; The average and onset ages of AFA positive patients were older than those of the negative ones, and there was correlation between AFA and age.The frequency and average titer of RF patients were higher in AFA positive than in negative ones.AFA was significantly correlated to RF,AKA and APF.Conclusion AFA are valuable diagnostic parameter for RA,especially in the early and seronegative RA.AFA is significantly correlated to AKA and APF,but they are not totally overlapped,None of the three antigens used bear all the epitopes recognised by antifilaggrin autoantibodies.Immunoblot detection of AFA,a simple and reliable test, may be complement to conventional IIF detection of AKA and APF.

BACKGROUND: Alendronate has been recently used for treating osteoporosis by inhibiting the activity of osteoclasts, but further clinical observation is necessary to compare its therapeutic effect with exclusive use of calcium supplement.OBJECTIVE: To observe the therapeutic effect of oral alendronate (tianke tablet) on the clinical symptoms and bone mineral density (BMD) of middle-aged and elderly patients for comparison with calcium gluconate.DESIGN: A self-controlled study and controlled trial with concurrent patients.SETTING: Department of Orthopedics, Shenzhen Hospital, Peking UniversityPARTICIPANTS: Sixty-eight middle-aged and elderly patients with osteoporosis were admitted in the Department of Orthopedics, Shenzhen Hospital of Peking University between July 1999 and July 2000. From these patients 62 at the age over 45 years without secondary osteoporosis were selected for this study, who were randomly divided into oral alendronate group (n=32) and oral calcium gluconate group (n=30).INTERVENTIONS: On the basis of comprehensive therapies, the patients in the alendronate group were given 10 mg alendronate daily and those in calcium gluconate group had 20 mL oral calcium gluconate solution (containing 100 mg calcium gluconate, equivalent to 9 mg calcium in every 10 mL) three times daily for three months. According to bodily pain relief, occurrence of new fracture and improvement in BMD, the therapeutic effect was assessed in three grades.RESULTS: Thirty patients in the alendronate group and 26 in the calcium gluconate group completed the study. Alendronate treatment for 3months significantly increased BMD of the patients [(0.716±0.082) g/cm2in comparison with that before treatment [(0.667±0.083) g/cm2, t= 2.473,P < 0.01], whereas the BMD underwent no obvious changes after treatment with calcium gluconate [(0.671±0.081) g/cm2 before vs (0.680±0.073) g/cm2after treatment, t=1.812, P > 0.05]. Significant difference in BMD after treatment was noted between the two groups (t=2.384, P < 0.01). The effective rate was significantly higher in alendronate group than in calcium gluconate group (X2=14.9, P=0.005), but 7 patients complained of abdominal discomfort in the former group and the patients in calcium gluconate group reported no adverse effects.CONCLUSION: Alendronate can inhibit bone absorption, promote the recovery of bone matrix, and increase the bone mass, producing better effect than exclusive use of calcium gluconate oral solution in treatment of the osteoporosis in the middle-aged and the elderly.

OBJECTIVE:To prepare warfarin sodium granules(WSG)and to establish it’s quality control methods .METHO_DS:To prepare WSG with part.aeq.geometric method,to optimize the amounts of all sorts of agents in the formulation in terms of dissolution time and abilityof WSG by orthogonal experiment. To determin the content of WS by UV. RESULTS:The formulation was designed and optimized, the WSG’s size were evenly. The concentration of WS was linear within the range of 5.92~17.86?g/ml with r=0.9 998(n=7),the average recovery rate was 101.26%~101.84%(n=5)with the RSD lower than 1.0%.The solution dissolved by granules is stable within 10 days.CONCLUSIONS:The preparation method is simple and feasible, the content determination methods is accurate, realible and sensitive, suitable for the quality control of WSG.The dissolved solution can be divided in accurate doses.

Aim To investigate the effect of fluvastatin(FV)on the expression of IL-1? mRNA of left ventricular(LV) myocardium in rats with heart failure after acute myocardial infarction(AMI).Methods 6 h after ligating left coronary artery, surviving AMI female SD rats were randomly assigned to: ①AMI control;②AMI+FV;③sham-operated group, which was randomly selected as non-infarction control. After 8 weeks of therapy,we assessed cardiac function, hemodynamics, ventricular remodeling parameters and IL-1? mRNA expression in the infarcted and non-infarcted area(IA,NIA).Results Compared with the sham-operated group,LV end-diastolic dimension(LVEDD),LV end-diastolic volume(LVEDV),E-wave,E-wave deceleration,E/A ratio,LV end diastolic pressure(LVEDP),relative weight(LVRW),right ventricular relative weight(RVRW),collagen volume fraction(CVF) and IL-1? mRNA in NIA were all significantly increased in the AMI group(all P

Objective: To explore the effect and mechanism of DMDAI-L in inducing the HL-60 cells apoptosis. Methods:Caspase-3 activity in HL-60 cells was measured with the enzymatic visible substrate DEVD-pNA. The fluorescence changes of mito-chondrial membrane potential (△Ψm) in HL-60 cells were investigated with the fluorescent probe JC-1. Results:The caspase-3 activ-ity was significantly increased in HL-60 cells after the DMDAI-L treatment at the concentration of 1. 25, 2. 5, 5, 10 and 20μg·ml-1 for 24h(P<0. 05). DMDAI-L could significantly reduce the mitochondrial membrane potential in HL-60 cells. Conclusion: The mechanism of DMDAI-L in inducing HL-60 cells apoptosis may involve the activation or regulation of caspase-3 activity as well as the reduction of mitochondrial membrane potential in HL-60 cells within certain concentration and time range.

Objective To analyze the safety and effectiveness of using lumbar drainage (LD) in patients with subarachnoid hemorrhage (SAH).Methods Data from 44 patients with SAH were collected.Of these patients, the LDs group consisting 19 patients in whom a lumbar drain had been placed for cerebro-spinal fluid (CSF) drainage, whereas the control group was composed of 25 patients who received no form of CSF drainage or treated solely with an external ventricular drainage (EVD).Incidence of new cerebral infarction of the two groups was measured on sequential CT and complications of LDs group. Results This retrospective review demonstrated a statistically significant beneficial effect in LDs group on CT,reducing the incidence from 32% to 5%.A similar degree of benefit was observed in two group patients with hydrocephalus.Complications with lumbar drains were rare and yielded no permanent sequelae. Conclusion With rare complications and definite safety,LDs are effective to reduce vasospastic cerebral infarction of SAH patients when appropriate patient selection and good technique are employed.
