1.Differentiation of muscle-derived stem cells into neuron-like cells induced by ciliary neurotrophic factor and Compound Salvia Miltiorrhiza Injection in vitro

BACKGROUND: The low survival rate of neuron cells is one of the main mechanisms of stem cell allograft, which might lead to the failure of allograft. Nuclear factor-KB (NF-kB) is one of main transcription factors for cell signaling transduction and participates in call proliferation and differentiation.OBJECTIVE: To study the differentiation of muscle-derived stem cells (MDSCs) into neuron-like cells induced by ciliary neurotrophic factor (CNTF) and Compound Salvia Miltiorrhiza Injection in vitro, and the expression of NF- kB.DESIGN, TIME AND SETTING: The experiment at cell molecular level was performed at the Oral Science Laboratory of the Second Affiliated Hospital of Liaoning Medical College from March to May 2008.MATERIALS: A total of 10 Sprague Dawley neonatal rats aged 7 days were supplied by Experimental Animal Center, Liaoning Medical University. CNTF (Sigma, USA) and Compound Salvia Miltiorrhiza Injection (Chiatai Qingchunbao Pharmacantical Co.,Ltd., China) were used in this study.METHODS: Rat MDSCs were harvested in vitro, pudfied by differential adherence and enzyme digestion, and incubated in 6-well plate. Samples in the induction group were incubated in DMEM containing CNTF for 24 hours. The medium was changed.Subsequently, samples were dnsed three times, and then incubated in serum-free DMEM supplemented with Compound Salvia Miltiorrhiza Injection for 5 hours. Samples in the control group were treated with serum-free DMEM.MAIN OUTCOME MEASURES: Neurofilament protein and NF-KB inhibitor protein expression were detected using reverse transcription-polymerase chain reaction (RT-PCR) and Westem blotting.RESULTS: No neurofilament protein expression was found in MDSCs before induction, and neurofilament protein-positive MDSCs were detected following induction. 2.Therapeutic efficacy observation of warm needling moxibustion plus spine subtle adjusting manipulation for cervical radiculopathy

Objective: To observe the clinical efficacy of warm needling moxibustion plus spine subtle adjusting manipulation for cervical radiculopathy. Methods: A total of 70 patients with cervical radiculopathy were randomized into an observation group and a control group, with 35 cases in each group. The observation group was treated with warm needling moxibustion plus spine subtle adjusting manipulation, while the control group was treated with warm needling moxibustion alone. The treatments were performed three times a week, and for four weeks in total. The visual analog scale (VAS) was scored before and after treatment. And the clinical efficacy of the two groups was compared after treatment. Results: The total effective rate was 97.1% in the observation group, versus 88.6% in the control group. The difference between the two groups was statistically significant (P<0.05). After treatment, the VAS scores in both groups significantly decreased (P<0.01), and the score in the observation group was significantly lower than that in the control group (P<0.05). Conclusion: Warm needling moxibustion plus spine subtle adjusting manipulation has a better effect in the treatment of cervical radiculopathy than warm needling moxibustion alone.
3.Imaging measurement for internal fixation design of axial lumbosacral vertebral fusion viaposterior rectal space

BACKGROUND: The minimally invasive axial lumbar interbody approach (AxiaLIF) for L4–S1 fusion has been applied in America and Europe, and has obtained satisfactory curative efficacy. Because of significant anatomical differences between Chinese and Europeans and Americans, whether AxiaLIF is appropriate for Chinese remains unclear. Moreover,there are some problems in the application of AxiaLIF, so how to optimize AxiaLIF is a key to its promotion in China.OBJECTIVE: To provide anatomical data for the design of axial screws suitable for Chinese through measuring the mid axial line of the lateral lumbar radiograph and cross sections of L5 and S1 on lumbar CT in normal Chinese population.METHODS: The lateral lumbar radiographs from Chinese healthy population were selected, including 35 males and 30 females, the axial height of S1, the disc distance between L5 and S1, and the axial height of L5 were measured so as to provide anatomical data for designing the length of the axial screw. The transverse and sagittal diameters of L5 and S1 in the lumbar CT of 26 adult healthy males and 24 healthy females were measured to provide anatomical data for designing the diameter of the axial screw.RESULTS AND CONCLUSION: (1) The axial height of S1 in males and females was (26.76±3.94) mm and (22.91±2.91) mm, respectively (P < 0.05). The disc distance between L5 and S1 was (12.62±1.90) mm for males and (11.92±1.78) mm for females (P > 0.05). The axial height of L5 was (29.12±2.18) mm for males and (26.91±2.47) mm for females (P <0.05). (2) The transverse diameter of S1 was (49.14±4.14) mm for males and (46.11±4.44) mm for females (P < 0.05).The transverse diameter of L5 was (41.34±4.31) mm for males and (43.12±3.71) mm for females (P < 0.05). The sagittal diameter of L5 was (34.48±2.32) mm for males and (33.03±3.48) mm for females, and the sagittal diameter of S1 was (35.65±4.28) mm for males and (33.53±3.26) for females, (both P > 0.05). (3) That is to say, this study provides the anatomical data for designing the axial screws suitable for the lumbar fusion of Chinese by measuring the mid axial line of the lateral lumbar radiographs and the cross sections of L5 and S1 on lumbar CT. The image measurement method can be used to analyze the preoperative images of the patients to predict the feasibility of the surgical approach and pre-select the internal fixation model for personalized screw positioning.
4.Protective Effect of L-Ornithine-L-Aspartate on Ischemia-reperfusion Injury after Partial Hepatectomy

OBJECTIVE:To explore the protective effect of L-ornithine-L-aspartate on ischemia-reperfusion injury after partial hepatectomy. METHODS:104 patients underwent partial hepatectomy(vessel occlusion in portal fissure)were randomly divided in-to control group(53 cases)and trial group(51 cases). Control group was given routine liver-protective drugs,and trial group was additionally given L-ornithine-L-aspartate 10 g added into 5%Glucose injection 250 ml intravenously before surgery,for 7 consecu-tive days. The fasting peripheral venous blood was collected in 2 groups on first day before surgery and first,forth and sixth day af-ter surgery to detect liver function;the changes of main aminopherase index(ALT and AST)were compared between 2 groups at different portal fissure vessel occlusion time after surgery. RESULTS:There was no statistical significance in the difference of liver resection range,blood loss,first porta hepatis vessel occlusion time and anesthesia time in both groups during operation (P>0.05). Compared to before surgery,liver function indexes raised to different extent in 2 groups after surgery,with statistical signifi-cance(P<0.05). The levels of AST and ALT in trial group were significantly lower than in control group on first,forth and sixth day after surgery,while albumin level was significantly higher than control group,with statistical significance(P<0.05). Total bili-rubin levels of both groups were approximate basically,there was no statistical significance(P>0.05). Among those patients under-went porta hepatis vessel occlusion with Pringle method and with occlude occlusion time ≥15 min,the AST and ALT level of con-trol group was higher than those of trial group after surgery,but albumin level was below trial group,with statistical significance (P<0.05). CONCLUSIONS:L-ornithine-L-aspartate could improve liver function fast and effectively for partial hepatectomy pa-tients,especially for the patients underwent porta hepatic vessel occlusion with Pringle method for a long time(obvious ischemia-re-perfusion injury).
6.Relationships between HBV-M patterns and liver function,HBV-DNA in patients with chronic HBV infection of different stages

Objective To investigate the distribution of five indicators of hepatitis B(HBV-M)and its relationship with liver function parameters and HBV-DNA load in patients with chronic HBV infection in different stages.Methods The serum samples were collected from 456 patients infected with HBV.The HBV-M,liver functional parameters and HBV-DNA level were quantita-tively detected.According to the stages of disease,the patients were divided into 3 groups including chronic hepatitis B group(inclu-ding mild subgroup,moderate subgroup and severe subgroup),liver cirrhosis group(including compensatory subgroup,decompen-sated subgroup)and hepatocellular carcinoma goup.Results The ratio of HBsAg,HBeAg,HBcAb positive pattern(135 pattern) and HBsAg,HBeAb,HBcAb positive pattern(145 pattern)in the three groups were statistically different(P <0.05).In each chron-ic hepatitis B subgroup,both ALT and AST levels of 135 pattern were significantly higher than the other two patterns(P <0.05). In each liver cirrhosis subgroup and hepatocellular carcinoma group,there were statistically significant differences in ALB and TBIL levels between the three patterns(P <0.05 ).In each group,the HBV-DNA level of 135 positive pattern were significantly higher than the other two patterns(P <0.05).Conclusion With the advancement of chronic hepatitis B,there is a downtrend in the ratio of 135 pattern and increasing trend in the ratio of 145 pattern.If the stage of hepatitis B is discriminated,ALT,AST,ALB TBIL and HBV-DNA level were closely related to HBV-M pattern.
7.The evaluation of the left transthoracic approach in simultaneous antireflux surgery at the time of CDH repair in infancy

Objective The aim of this study was to evaluate the left transthoracic approach in simultaneous antireflux surgery at the time of CDH repair in Infancy.Methods Between June 2008 and June 2012,18 patients underwent a left transthoracic approach in the treatment of type Ⅲ and Ⅳ hiatal hernia.,including 1 gastric volvulus.Clinical presentation in these patients included vomiting(n =12) 、pulmonary infections (n =9)、symptomatic anemia、failure to thrive (n =18).All the patients were evaluated before and after the surgery on clinical presentation,symptoms and functional assessment.Surgical techniques included extensive mediastinal esophageal dissection,Nissen fundoplication,resection of the hernial sac,crural closure.Results The average of operative time was 90 minutes.The mean blood loss was 5 ml.The average length of stay was 15 days.The hours stayed in PICU were 21.5 h.Mean follow-up was 22 months.There was no hospital mortality and hernia recurrence.Only one patient suffered gastroesophageal reflux disease after operation,and was controlled with antireflux medications.Conclusion Transthoracic is the optimal operative approach for treating the esophageal hiatal hernia,with relatively low postoperative morbidity and recurrence rates.Advantages of the transthoracic approach include the facilitation of Nissen fundoplication、esophageal lengthening procedures,and excellent exposure for the crural suturing.
8.Effect of Gimeracil and Oteracil Porassium combined with cisplatin in the treatment of old patients with advanced gastric cancer

Objective To investigate the clinical efficacy of S-1 combined with cisplatin in the treatment of old patients with advanced gastric cancer.Methods 85 patients were randomly divided into two groups.Experimental group was treated by S-1 combined with cisplatin,while control group was treated by 5-FU and cisplatin.Short-term efficacy and adverse reactions were evaluated after four cycles.Results All patients had succcssfully accomplished chemotherapy.Compared with the control group ( RR =52.4%,DCR =81.0% ),RR and DCR in experimental group ( RR =67.4%,DCR =93.0% ) had statistically significant difference ( P < 0.05 ).The top three of adverse reactions and complications in patients of the two groups were:nausea and vomiting,low WBC,low hemoglobin concentration.Compared with the control group,the toxicity in experimental group had no statistically significant difference ( P >0.05).All patients were followed up with a meau of( 15.5 - 2.7 ) months.The 1 year survival rate was 60.5%(26/43) in the experimental group; the 1 year survival rate was 52.4% (21/42) in the control group.There was no statistically significant difference( P > 0.05 ).Conclusion The S-1 combined with cisplatin had shown better efficacy in the treatment of old patients with advanced gastric carcinoma,and the toxicity was relatively mild and tolerable.
9.Effect of Fingolimod on Angiotensin Ⅱ-induced Renal Oxidative Stress in Rats

Objective To evaluate the effect of fingolimod(FTY720) on angiotensin Ⅱ(AngⅡ)-induced renal oxidative stress in rats and its possible mechanisms.Methods Thirty-six male Sprague-Dawley rats were divided into three groups through random number tables:AngⅡ infusion group(twelve rats,infusion of AngⅡvia subcutaneous osmotic pumps),0.9% sodium hydrochloride solution infusion group(twelve rats,infusion of equal volume of 0.9% sodium hydrochloride solution via subcutaneous osmotic pumps) and FTY720 intervention group(twelve rats,intra-gastric administration of FTY720 besides AngⅡ infusion).Urine samples were collected for 24 hours every week.Blood samples and kidneys were collected when rats were sacrificed at day 14 and 28 d respectively.MDA and T-SOD assay Kits were used to detect the concentrations of MDA and T-SOD in serum and kidney homogenates.Renal pathological changes were observed by light microscope.The expression of Nox4 in the kidneys was evaluated by immunohistochemistry.Results Compared with 0.9% sodium hydrochloride solution infusion rats of the same period,AngⅡ-infused rats developed significant proteinuria.The concentration of serum creatinine,urea nitrogen,AngⅡ and MDA were increased(P<0.05),T-SOD was significantly decreased on days 14 and 28(P<0.05),the serum albumin was significantly increased on day 28(P<0.05),while FTY720 partially reversed these changes.The renal tissue of AngⅡ-infused rats presented mesangial cells proliferation,mesangial matrix deposition,tubular epithelial cells effacement,sclerosis and atrophy in part of the glomerular.FTY720 can alleviate these changes.Immunohistochemistry showed that Nox4 was primarily located in the distal tubule.The expression of Nox4 was significantly increased in AngⅡ-infused rats as compared with that of 0.9% sodium hydrochloride solution infused group rats,and FTY720 administration prevented the change effectively.Conclusion AngⅡ-induced renal injury could be attenuated by FTY720 via preventing oxidative stress.
10.Primary lymphoma of thyroid gland

OBJECTIVE To analyze the reasons of misdiagnosis and surgical complications of primary lymphoma of thyroid gland(PLT). METHODS The clinical data of 10 patients with PLT in our hospital from 1992 to 2006 were retrospectively analyzed. RESULTS All of 10 patients were misdiagnosed,the major surgical complication was hoarseness. CONCLUSION The misdiagnosis of PLT is often encountered. Mastering the clinical features,using fine-needle biopsy,rapid biopsy combining immunocytochemical method can diagnose the primary lymphoma of thyroid gland.
