1.Tension-free mesh-plug repair for inguinal incarcerated hernia

Objective To evaluate the clinical effect of tension-free mesh-plug repair in the treatment of inguinal incarcerated hernia. Methods The mesh & plug tension-free hernia repair was administered in 45 patients with incarcerated indirect hernia using the Bard Mesh PerFix Plug. Results The operation time was 48~98 min (mean, 75 min). The patients got out of bed 12~24 hours after surgery. Wound pain lasted 2~4 days. Postoperative subcutaneous hydrops occurred in 2 patients and dysuria happened in 2 patients. The postoperative hospital stay was 3~10 days (mean, 5 days). Follow-up for 5~18 months (mean, 12.5 months) in 39 patients found no recurrence. Conclusions Tension-free mesh-plug repair has advantages of reliability, minimal invasion, rapid recovery and low recurrence rate. This method can be applied to those with incarcerated hernia with satisfactory outcomes.

Thyroid-associated ophthalmopathy (TAO) is an inflammatory eye disease associated with thyroid autoimmune disorder.The traditional treatments have yielded limited efficacy but with some side effects.Recently,researches have shown that some new immunosupressants may be used for the treatment of TAO with significantly improved symptoms,and reduction in clinical activity score.

The study of mechanical properties on heart valves can provide an important theoretical basis for doctors to repair heart valves and prosthetic valve materials research. In this paper, we present the current status of the mechanical property study methods of heart valve, expound the methods and special requirements about uniaxial tensile test and biaxial tensile test of the heart valve, and further discuss several establishment methods of heart valve constitutive models. We also discuss the development trend of heart valve mechanics.

7.Effect of the 13-cis isotretinoin on androgen receptor levels in acneic patients

Objective In order to explore the effect of 13-cis isotretinoin on androgen receptor in acneic patients and provide the theoretical basis for acne treatment. Methods Radiological binding assay (RBA) was used to study the specific AR binding capacities between the experimental group and the control group before and after taking medicine. The serum testosterone level was measured with radioimmunoassay. Results 16 weeks after taking medicine, the AR level in the acne patients decreased significantly compared with that before taking 13-cis RA (P<0.05),but the affinity change was not found, the testosterone levels did not evidently change either. Conclusion The 13-cis RA can decrease the levels of AR and depress the hyperplasia of sebaceoous glands.

9.Qualitative study on the real psychological experience of family members of patients in neurological intensive care unit

Objective:To deeply understand the real experience of family members of patients in the neurosurgery intensive care unit during the period of accompanying them, and to provide reference for managers to formulate nursing countermeasures to improve the coping ability of family members.Methods:Descriptive phenomenological research method in qualitative research was adopted to conduct in-depth interviews with family members of 12 patients admitted to a neurosurgery intensive care unit in Shanghai, manage the interview data with Nvivo11 software, and use Colaizzi's seven-step analysis method to summarize and analyze the topic.Results:There were four themes: adhere to and do not give up, adhere to was due to the trust and affirmation of the medical care, do not give up was because it was difficult to give up blood kinship, adhere to the hope of life because of patients; timing change of emotional experience, first aid period: helpless to rescue and expecting the condition to stabilize, complication peak: understanding the characteristics of the disease and paying attention to the treatment progress, stable period: uncertain prognosis and desire for emotional companionship.The powerlessness of the future; learn to self psychological adjustment.Conclusions:The family members of patients with severe neurological diseases have both negative experience and positive feelings, and the accompanying experience and needs at different stages of the disease are changing dynamically. According to the cognition and needs of the family members, exploring and developing palliative medicine, tapping the potential of the family members from a positive perspective and improving coping ability are one of the ways to maintain the stable mental health of the family members.
10.Experimental study on the structural damage of the blood-brain barrier induced by overpressure blast in rat

Objective To investigate the influences of blast wave overpressure created by explosion on rat's blood-brain barrier (BBB),and to provide an experimental evidence for further elucidating the mechanism of craniocerebral blast injury,to help direct the clini- cal therapeutic strategy.Methods Twenty male Sprague-Dawley rats were randomly divided into four groups:15min,12h and 24h post- explosion groups and normal control group with five rats for each group.Rats were injured with blast wave produced by explosion.Evan 's blue(EB)was intravenously injected into the injured rats,and the overpressure-induced leakage of BBB was examined under the fluores- cence microscope and by quantitative observation with opacimeter.Results Extensive impairment of BBB could be seen all over the brain 15 min after the overpressure explosion,and bright red EB fluorescent patches appeared as a method distributing along the capillary vessels. These patches were more distinct near the cerebral cortex,and with the lapse of time,their number decreased gradually from the inner to the outer areas.The changes in EB contents in brain tissue matched with that of the BBB.The EB contents in brain tissues in 15min group were(330.4?53.6)?g/g,and it was much higher compared with that of other groups(P
