Objective To investigate the protective effects of continuous norepinephrine infusion and its mechanism in early stage of lipopolysaccharide induced septic rats.Methods Forty SD rats were randomly divided into 5 groups (an external jugular vein catheterization was performed in every rat a day before intraperitoneal injection):saline control group (n =8),endotoxin group (n =8),low dose norepinephrine therepy group(n =8),middle dose norepinephrine therepy group (n =8),and high dose norepinephrine therepy group (n =8).Saline control group received intraperitoneal injection of 0.9% saline 10 ml/kg,and then received an infusion of 0.9% saline 1 ml/h.Endotoxin group animals were intraperitoneal injected with lipopolysaccharide 10 mg/kg,and then received an infusion of 0.9% saline 1 ml/h.Therapy group animals received an infusion of norepinephrine solution 1 ml/h after being intraperitoneal injected LPS 10 mg/kg.The doses of norepinephrine were 0.06,0.3,0.6 μg/(kg· min) respectively.Blood samples were taken 2 h and 6 h later and the levels of serum TNF-α、IL-1β and IL-10 were detected by enzyme-linked immunoadsorbent assay.The membrane potential and three tricarboxylic acid cycle key enzyme activity [isocitrate dehydrogenase(IDH) and pyruvate dehydrogenase (PDH) activity] of isolated liver mitochondrion were detected 24 h later,and the morphologic changes of mitochondria in liver were observed by electronic microscopy.Results In endotoxin group,the levels of serum TNF-α JL-1 β and IL-10 all increased significantly(P < 0.05) and liver mitochondrial membrane potential,PDH and IDH activity decreased significantly compared with control group(P <0.05,P <0.01).In the middle dose of norepinephrine therepy group,the levels of serum TNF-α and IL-1β all increased significantly (P < 0.05,P < 0.01),the level of serum IL-10 decreased significantly compared with endotoxin group (P < 0.05) ; In the high dose norepinephrine therepy group,the liver mitochondrial membrane potential,PDH and IDH activity increased significantly compared with endotoxin group (P < 0.05).The mitochondrial ultrastructural changes in every group were not obviously.Conclusion In the early stage of septic rats,reversible liver mitochondrion injury can be observed.Norepinephrine infusion can protect early septic rats through attenuating inflammatory reaction and enhancing tricarboxylic acid cycle key enzyme activity and mitochondrial membrane potential level.
3.Relevant research on expression of C5b-9 and disease in gastric adenocarcinoma

Objective To evaluate the correlation between the expression of C5b-9 and the gastric carcinoma.Methods In this study, the human gastric adenocarcinoma (HGAC)tissues (n =32 cases)from 32 patients were evaluated by immunohistochemistry and tissue chip.The result was analyzed and evaluated by Multinomial logistic regression and likelihood ratio.Results Multinomial logistic regression and likelihood ratio testing showed that over-expression of C5b-9 in HGAC tissue was significantly correlated with clinical stage (P =0.007) and tumor stage (P =0.005),but not with tumor metastasis,lymphoid nodal status,sex or age.Conclusion C5b-9 can be the criteria of di-agnosis and clinical staging for gastric adenocarcinoma,which may help in diagnosis and assessment disease severity of human gastric carcinoma.
4.Preoperative assessment of cervical invasion in endometrial cancer:comparison of MRI and D&C

Background and purpose:A pre-surgery diagnosis of cervical invasion in endometrial cancer was usually determined by dilatations and curettage(D&C),so it could help in optimizing surgery,but the reliability of D&C still was controversial.This study was to compare MRI with D&C to estimate their accuracy for the diagnosis of cervical invasion in endometrial carcinoma respectively.Methods:34 endometrial cancer patients had been proved to have cervical invasion in endometrial cancer through examinations of either fractional dilatations and curettages(D&C),magnetic resonance imaging(MRI)before surgery or postoperative histopathologic examination.The results of D&C and MRI were compared with postoperative histopathologic examination,and the sensitivity,specificity were estimated for accuracy of cervical involvement in endometrial cancer.Results:21 of 34 patients were identified as having cervical invasion in endometrial cancer by histopathology after operation.D&C test matches 26.5% of them and MRI test matches 80%.The sensitivity was 35% vs.76.5% and the specificity 7.7% vs.100% for D&C and MRI methods respectively.All of them had significant statistical differences.Conclusions:MRI test is much better than D&C method for preoperative diagnosis of cervical invasion in endometrial carcinoma.
5.Long stem bipolar arthroplasty for elderly patients with intertrochanteric fractures

Objective To evaluate the clinical result of long stem bipolar femur prosthetic replacement for elderly patients with intertrochanteric fractures.Methods A total 48 patients of intertrochanteric fractures,at age of 70 years or above,were treated with cemented long stem bipolar femur prosthetics.The surgery time,blood loss,Harris hip score and complication rate were observed.Results The average surgery time,the blood loss,Harris hip scores of the arthroplasty were 70 minutes,400 ml and 87 ml,respectively,without any complications.Conclusion Long stem bipolar femur prosthetic replacement can gain good hip function and decrease the complications of fracture in treating intertrochanteric fractures.
6.Preliminary study on impact of high altitude on gastric mucosa and its mechanisms

Objective To investigate the impact of high altitude on human gastric mucosa and its underlying mechanisms. Methods Twenty-five native Tibetans with chronic mountain sickness CMS residing at high altitude (4500 m) were examined for ultrastrueture and pathological changes of gastric mucosa. The other 25 native Tibetans residing at low altitude (<2000 m) were served as control.The expression of hypoxia-inducible factor (HIF)-1α in human gastric mucosa was detected with immunohistochemistry. Results The color of gastric mucosa presented dark in patients with CMS.The histopathological examination showed significant differecnce in incidence of cnronc body gastritis and atrophic gastritis between CMS group and control group (P<0. 05). The transmission electronic microscopy revealed that the epithlieal cells of gastric mucosa were impared in CMS group compared to control group. The expression of HIF-1α protein was increased in CMS group in comparison of control group (102.94± 34.04 vs 14.04 ± 6.53, P<0.05). Conclusions The high altitude may involve in damage of gastric mucosa and the up-regulation of HIF-1α protein may involve in its mechanism.

The human bone morphogenetic protein 7 (hBMP 7) gene was reconstructed in retroviral vector and transferred into incasing cells PT67 by liposome mediated method.The clones of the cells transferred with BMP 7 were selected by G418, and targeted rabbit bone marrow stem cells were infected with the virus granules which secreted from PT67 cells and also selected by G418. The mRNA and protein of BMP 7 gene in transferred cells were analyzed with hybridization in situ and immunohistochemistry. BMP 7 retrovirus vetor was successfully reconstructed. Cells transferred by PLNCX 2 hBMP 7 expressed abundant human BMP 7 mRNA and protein in the cytoplasm. However positive findings were not found in those cells that were not transferred. It may be used to increase the osteogenic capability of BMSc in the study of bone tissue engineering.

Overall enhancement of the public health system ranks a key task and goal for the ongoing health reform.This paper described the public health development in Shenzhen amid the ongoing health reform.Shenzhen has achieved the following objectives as required in the reform:better public service by public health institutions,availability of major and primary public health services as required by the state and city,overall elevation of public health service capabilities,and significant drop of disease morbidity and mortality.Challenges ahead include room of improvement in public health service network,incentive mechanism of public health service providers,and that of public health service delivery capability.
9.Imaging evaluation of calf arteries in patients with peripheral arterial occlusive disease by using time-resolved angiography with interleaved stochastic trajectories on MR scanner

Objective To explore the value of time-resolved angiography with interleaved stochastic trajectories (TWIST)in providing hemodynamic indices and morphological imaging of calf arteries in patients with peripheral arterial occlusive disease(PAOD) with 3.0 T MR scanner. Methods Forty patients with confirmed or suspected PAOD underwent TWIST MRA for the calf arteries and conventional contrast-enhanced MRA(CE-MRA) for the whole lower peripheral arteries. TWIST MRA data were used to determine the bolus arriving time of the popliteal artery, the time difference of the bolus arrival in the popliteal artery between the two legs, calf artery transit time, and the mean peak enhancement time of calf arteries. The mean value of peak enhancement time of calf arteries was correlated with ankle-brachial index(ABI). The calf arteries were divided into 13 segments. The visible score of arterial segment was recorded, and the degree of arterial stenosis was graded too. In 16 patients, DSA was used as the gold standard to evaluate the sensitivity and specificity of TWIST MRA and CE-MRA. Results The bolus arriving time of the popliteal artery was (0.7±7.6) s the time difference of the bolus arrival in the popliteal artery between the two legs was (2.1±2.5) s, the calf artery transit time was (35.6±16.9) s, the mean value of peak enhancement time of calf arteries was (52.6±17.6) s. ABI was acquired from 24 patients and 48 legs, which correlated well with the mean value of peak enhancement time in calf arteries (r=0.627,P<0.01). The mean visible score was (2.83±0.48) of TWIST MRA images and (2.78±0.43) of CE-MRA images. There was no significant difference between them (P>0.05). On CE-MRA, there were 38 legs with varying degrees of early venous enhancement; in contrast, there was no venous contamination on TWIST MRA. The sensitivity and specificity of TWIST MRA were 96.7% (118/122) and 100.0% (72/72) in showing normal artery and mild stenosis, 94.1% (32/34) and 96.2% (154/160) in showing severe stenosis, and 94.7% (36/38) and 98.7% (154/156) in showing occlusion. The sensitivity and specificity of CE-MRA were 95.9% (117/122) and 100.0% (72/72) in showing normal artery and mild stenosis, 91.2% (31/34) and 95.6% (153/160) in showing severe stenosis, and 94.7% (36/38) and 98.1% (153/156) in showing occlusion. Conclusion TWIST MRA is a preferable choice for calf arterial occlusive disease, which provided nice morphological images and useful hemodynamic indices with simple operation.
10.Chromogranin A derived peptide CGA47-66 inhibits hyper-permeability of blood brain barrier in mice with sepsis

Objective To explore the effect of chromofungin (CHR), a chromogranin A (CGA) derived peptide CGA47-66, on hyper-permeability of blood brain barrier in septic mice. Methods 120 healthy male C57BL/6 mice were randomly divided into groups, with 12 mice in each group. Seventy-two mice were used for dynamic observation of the contents of water and Evan blue (EB) in brain tissue after being treated with lipopolysaccharide (LPS). Another 48 mice were divided into normal saline control group (NS group), LPS induced sepsis model group (LPS group), low-dose CHR pretreatment group (CL+LPS group), and high-dose CHR pretreatment group (CH+LPS group). The septic model was reproduced by intraperitoneal injection of 10 mg/kg LPS 0.1 mL, and the mice in NS group was given equal volume of normal saline. The mice in CL+LPS group and CH+LPS group were intraperitoneally injected with 15.5 μg/kg and 77.5 μg/kg CHR 10 minutes before LPS injection. Six hours after LPS injection, 4 mL/kg of 2% EB was injected via caudal vein, the contents of water and EB in brain tissue were determined, and EB immune fluorescence in brain tissue was determined to assess the changes in permeability of blood brain barrier. Brain pathology was observed with hematoxylin and eosin (HE) staining. Results With the extension of time after LPS injection, the contents of water and EB in brain tissue were gradually increased, and the time of difference with statistical significance appeared earlier when compared with that of control group in the contents of water than that in EB contents (3 hours and 6 hours, respectively). The contents of water and EB in brain tissue in LPS group were significantly increased as compared with NS group [water content: (79.77±0.62)% vs. (78.28±0.44)%, P < 0.01; EB content (μg/g): 13.87±4.50 vs. 7.13±1.76, P < 0.05]. CHR pretreatment with either of two dosages could reverse the increase in water and EB contents in brain tissue induced by LPS, and the effect was more significant in CH+LPS group [water content: (78.15±0.73)% vs. (79.77±0.62)%, EB (μg/g): 7.09±2.59 vs. 13.87±4.50, both P < 0.05]. It was shown by EB fluorescence observation that the fluorescence signal displayed only in the meninges in NS group, and EB fluorescence was widely distributed in brain parenchyma in LPS group, indicating that the EB leakage in LPS group was more marked than that of NS group. In CHR pretreatment groups, EB fluorescence was decreased in brain parenchyma, indicating that EB leakage was significantly less marked, while it was more obvious in high dose CHR group. It was shown by HE staining that cerebral blood vessel structure was intact in NS group, and the gap around blood vessel was not significant increased. On the other hand, brain structure in LPS group appeared loose, with widening of small perivascular spaces and obvious edema. Brain edema in CHR pretreatment groups was improved as compared with that of the LPS group, and it was more apparent in high dose CHR group. Conclusions LPS induced change in blood brain barrier permeability in mice in a time-dependent manner. Exogenous CGA derived peptides CHR can inhibit LPS induced hyper-permeability of blood brain barrier in septic mice, thus reduces brain edema, protects the brain tissue, and the effect is more obvious with a high dose of CHR (77.5 μg/kg).
