1.Microsurgery techniques for insular glioma.

OBJECTIVE:
To explore the microsurgical techniques for insular glioma without damaging its surrounding normal structures.
METHODS:
We retrospectively analyzed 54 patients with insular gliomas who underwent microsurgical operation by trans-syvian fissure approach between May, 2003 and August, 2008 in Xiangya Hospital. We discussed the techniques in the operation and summarized how to protect the key blood vessels, distinguish and protect the surrounding normal structures.
RESULTS:
There were 36 complete removals,14 secondary complete removals, and 4 partial removals.Six patients had complications after the craniotomy who had temporal speech disorder (aphasia mostly began to recover about 10 days after the craniotomy),4 patients had opposite side paralysis worsening (3 recovered normally and 1 improved after 6 months),4 had light paralysis, and another 3 had paralysis and speech disorder.
CONCLUSION
The microsurgery by means of trans-syvian fissure approach can well expose the anatomical relation between tumor and its surrounding structures,so that we can remove the tumor and protect the surrounding normal tissues as much as we can.

2.Clinical Observation of Additional Use of Cypate Gingival Fluid in Periodontal Basic Therapy before Surgery in the Treatment of Pregnancy Gingival Tumor

OBJECTIVE: To observe the clinical efficacy and safety of additional use of Cypate gingival fluid in periodontal basic therapy before surgery in the treatment of pregnancy gingival tumor. METHODS: A total of 86 patients with pregnancy gingival tumor in Second Affiliated Hospital of Qiqihar Medical University during Jan. 2015 to Jan. 2016 were divided into observation group and control group according to random number table, with 43 cases in each group. Control group received ligation and oral micro-electrosurgical surgery according to the volume of tumor. Observation group was given Cypate gingival fluid-based periodontal basic therapy (3-5 mL for gargling, 2-3 min each time, 3-5 times each day, 5 d) and then receive surgery. Clinical efficacies, gingival related indexes (gingival index, sulcular bleeding index, probing depth), quality scores of life, ADR, the occurrence of complications were observed in 2 groups. The followed-up was conducted 1 year after surgery, and the recurrence of gingival tumor was compared between 2 groups. RESULTS: No case was found shedding in the study. The total response rate of observation group was 97. 67%, which was significantly higher than 79. 07% of control group (P<0. 05). After treatment, the gingival related indexes of 2 groups were decreased significantly compared to before treatment, while quality scores of life were increased significantly compared to before treatment, observation group was significantly better than control group (P<0. 05). No severe ADR or complication was found in 2 groups during treatment. Recurrence rate of gingival tumor in observation group was 2. 33%, which was significantly lower than 13. 95% in control group 1 year after treatment (P<0. 05). CONCLUSIONS: Compared with surgery alone, additional use of Cypate gingival fluid in periodontal basic therapy before surgery has better clinical efficacy and lower recurrence rate, with good safety.
3.Chondrosarcoma original from the zygomatic arch: A case report and literature review.

Chondrosarcoma original from the zygomatic arch is a very rare disease with high malignancy. Surgery is the main means of treatment at present for duo to its poor sensitivity to radiochemotherapy. We reported a young patient who was recovery well in a 4-years follow-up without radiochemotherapy after a total resection of the tumor.

4.Expression and characterization of NADPH-cytochrome P450 reductase from Trametes versicolor in Escherichia coli.

Trametes versicolor has strong ability to degrade environmental organic pollutants. NADPH-cytochrome P450 reductase (CPR) of T. versicolor transfers electron to cytochrome P450s (CYPs) and participates in the degradation process of organic pollutants. Sequence analysis showed that the genome of T. versicolor contains 1 potential CPR and multiple potential CYP sequences. To further study the molecular mechanism for the involvement of T. versicolor CPR in the cellular degradation of organic pollutants, a CPR gene from T. versicolor was cloned and heterologously expressed in Escherichia coli. Subsequently, the main properties of the recombinant enzyme were investigated. A truncated CPR protein lacking the predicted membrane anchor region (residues 1-24), named CPRΔ24, was overexpressed as a soluble form in E. coli. The recombinant CPRΔ24 protein showed a molecular weight consistent with the theoretical value of 78 kDa. Recombinant CPRΔ24 was purified using a Ni²⁺-chelating column followed by size exclusion chromatography. The specific activity of the purified CPRΔ24 was 5.82 U/mg. The CPRΔ24 enzyme displayed the maximum activity at 35 ℃ and pH 8.0. It has different degrees of tolerance against several types of metal ions and organic solvents. The apparent Km and kcat values of recombinant CPRΔ24 for NADPH were 19.7 μmol/L and 3.31/s, respectively, and those for the substrate cytochrome c were 25.9 μmol/L and 10.2/s, respectively, under conditions of 35 ℃ and pH 8.0. The above research provides the basis for exploring the functional mechanism of T. versicolor CPR in the degradation pathway of environmental organic pollutants.

6.Carotid endarterectomy combined with vertebral artery transposition treating vertebral artery V1 segment stenosis combined with ipsilateral carotid artery stenosis

Objective To explore the treatment outcome of carotid endarterectomy combined with vertebral artery transposition in patients with severe stenosis to occlusion of the vertebral artery V1 segment and the ipsilateral carotid artery. Methods From June 2017 to September 2020, patients with severe stenosis to occlusion of the vertebral artery V1 segment and the ipsilateral carotid artery treated with carotid endarterectomy combined with vertebral artery transposition in Fuwai Hospital were retrospectively analyzed. Results Finally 12 patients were enrolled, including 10 males and 2 females with an average age of 67.8±6.0 years. Twelve patients were successfully operated and the follow-up time was 1-3 years. The stenosis degree of the V1 segment of the vertebral artery decreased from 83.5%±11.8% to 24.9%±14.3% (P<0.001). The stenosis degree of carotid artery decreased from 85.6%±11.0% to 0% (P<0.001). Postoperative follow-up showed that the symptoms of symptomatic patients before surgery improved. The 1-year and 3-year patency rates were 100.0%, and there were no peripheral nerve injury complications, perioperative deaths or strokes. Conclusion Carotid endarterectomy combined with vertebral artery transposition can treat ipsilateral carotid artery stenosis and vertebral artery stenosis at the same time, improve blood supply to the brain, improve patients' symptoms and has high promotion value.
