1.Predictive value of emergency suspected sepsis risk stratification score for prognosis of patients with sepsis

Objective:To explore the value of using risk-stratifcation of mergency department suspected sepsis score(REDS) score in the prognosis of patients with sepsis.Methods:The clinical data of sepsis patients hospitalized in Huangshan Shoukang hospital from January 2018 to February 2020 were collected for retrospective case-control study.The REDS, acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score and mortality in emergency department(MEDS) of all cases were calculated.To evaluate the value of REDS in prognosis of patients with sepsis by comparing REDS with APACHEⅡ and MEDS.Results:Finally 103 patients were enrolled in the study, including 65 males (63.11%) and 38 females (36.89%); the age ranged from 42 to 93 years old, the median (quartile) was 74 (65, 82) years old; 72 patients (69.91%) survived (survival group), 31 patients (30.09%) died (death group). There were significant differences between survival group and death group in REDS((5.28±2.10) score vs.(7.87±2.53) score), MEDS score ((6.64±3.32) score vs.(9.06±3.3) score), APACHE Ⅱ score ((18.96±6.72) score vs.(21.87±6.15) score) (t value were5.404, 3.381 and 2.067, respectively; P value were<0.001, 0.001 and 0.041, respectively). Youden index was biggest(0.381), when REDS was 7.5.The area under the receiver operating characteristic curve was 0.777 (95% CI: 0.678-0.876, P<0.001). When APACHE Ⅱ score was 19.5, Youden index was the largest (0.177), and the area under the ROC of APACHEⅡwas 0.614(95% CI: 0.499-0.729, P=0.068). When the MEDS score was 10.5, Youden index was the largest (0.341), and the area under the receiver operating characteristic curve was 0.696 (95% CI: 0.590-0.802, P=0.002). The area under the curve of working characteristics of the subjects with red score was larger than that of APACHE Ⅱ score ( Z=1.987, P=0.046). Conclusion:REDS has a good effect in judging the condition and prognosis of patients with sepsis, which is equivalent to MEDS score, but better than APACHEⅡ.

3.The clinical effect of simvastatin combined with ferulate in treatment of early diabetic nephropathy

Objective To investigate the clinical efficacy of simvastatin combined with ferulate in treatment of early diabetic nephropathy (DN).Methods 106 patients with DN were randomly divided into two groups by random number table,and each group had 53 cases.The control group was given simvastatin,while the observation group was given simvastatin combined with ferulate,and the clinical outcomes were compared between the two groups.Results The total effective rate of the observation group was 92.45%,which was significantly higher than 73.58% of the control group(x2 =8.992,P < 0.05) ; after treatment,the Upro,SCr,BUN and CRP levels of the observation group were (50.72 ± 7.74) mg/24h,(70.01 ± 8.56) μmol/L,(3.87 ± 0.54) mmol/L and (3.51 ± 0.63) mmol/L,which were significantly lower than (83.74 ± 7.92) mg/24h,(84.33 ± 9.01) μmol/L,(5.03 ± 0.62) mmol/L and (5.52 ± 0.61) mmol/L of the control group,the difference was statistically significant (t =7.73,6.59,8.62,9.30,all P < 0.05).Conclusion Simvastatin combined with ferulate in treatment of early diabetic nephropathy (DN) has significant effect,and it can effectively improve the clinical symptoms and protect renal function,and it has better clinical value.
4.Trends in incidence of stroke among residents in Jinhua Cityfrom 2015 to 2020

Objective:
To analyze the incidence and trends for incidence of stroke in Jinhua City from 2015 to 2020, so as to provide insights into stroke control.
Methods:
The data pertaining to incidence of stroke among registered residents in Jinhua City from 2015 to 2020 were collected from Zhejiang Provincial Information Management System for Surveillance on Chronic Diseases. The incidence of stroke was estimated and standardized to the Sixth National Population Census in 2010. The trend in incidence of stroke was evaluated using annual percent change (APC).
Results:
Totally 120 993 stroke cases were reported in Jinhua City from 2015 to 2020, and the annual mean crude incidence and standardized incidence of stroke were 415.68/105 and 339.67/105, respectively. There were no significant trends in crude incidence (APC=0.30%, t=0.234, P=0.827) and standardized incidence of stroke (APC=-0.99%, t=0.946, P=0.398). The crude incidence of stroke was significantly higher in men than in women (457.38/105 vs. 372.62/105; χ2 =1 262.329, P<0.001), and the crude incidence of stroke appeared a tendency towards a rise with age (χ2 trend=377 708.263, P<0.001), with a tendency towards a decline among residents at ages of 75 years and older (APC=-2.66%, t=3.078, P=0.037). There were 94 038 residents with ischemic stroke (77.72%) and 24 176 residents with hemorrhagic stroke (19.98%), and both the crude incidence (APC=1.78%, t=3.440, P=0.026) and standardized incidence of hemorrhagic stroke (APC=2.66%, t=3.911, P=0.017) appeared a tendency towards a decline.
Conclusions
The incidence of stroke remained relatively stable in Jinhua City from 2015 to 2020; however, the overall incidence was still high. Ischemic stroke was the predominant type of stroke, and men and middle-aged and elderly people are at high risk of stroke.
5.Analysis on acupoint characteristics of medical records in Zhenjiu Dacheng.

Twenty-nine medical records of acupuncture and moxibustion at the end of volume in Zhenjiu Dacheng are precious clinical examples. The characteristics of selecting acupoints are less and ingenious with precious matching on the basis of syndrome differentiation and examining pathogenesis detailedly, according to meridians with diseases locations recognized clearly, keeping adopting acupoints on regular meridians combined with extra points and making good use of specific points, and attaching importance to acquired essence with protecting spleen and stomach. Valuable experience is provided for physicians of later generations to treat diseases.

6.The Effect of Hand Twelve Jing-Well Points Bloodletting on Blood-brain Barrier Permeability in PMCAO Rats

Objective To investigate the effect of hand twelve Jing-Well points bloodletting on blood-brain barrier (BBB) permeability in the early stage of focal cerebral ischemic injury in rats with permanent middle cerebral artery occlusion (pMCAO). Method Male Wister rats were randomized into sham operation, model and bloodletting groups. A model of middle cerebral artery occlusion was made by Longa's method. 2%Evans blue (EB) was injected into the caudal vein at three time points:5, 24 and 72 hrs after model making. EB exudation amount was assessed by measurement of EB absorbance in brain tissue to observe BBB permeability. Result Cerebral EB exudation amount was significantly higher in the model group of rats than in the sham operation group (P<0.01) and significantly lower in the bloodletting group than in the model group (P<0.01). Conclusion Hand twelve Jing-Well points bloodletting can reduce blood-brain barrier permeability in pMCAO rats. Its effect is highly significant at 24 and 72 hrs after ischemia.
7.Correlation of the S100B level with cardiac function and chronic heart failure

Objective To investigate the correlation of the S100B level with cardiac function and chronic heart failure.Methods A total of 80 elderly chronic heart failure (CHF) patients who accepted treatments in our hospital from March 2012 to December 2013 were recruited,and 80 subjects undergoing health examinations served as the control group.The New York Heart Association (NYHA) functional classification in the experimental group was determined.Serum levels of S100B and NT-proBNP,left ventricular end diastolic diameter (LVEDD) and left ventricular ejection fraction (LVEF) were compared between the groups,and the correlation of the S100B level with NT proBNP,LVEF and the NYHA functional classification were analyzed.Results The serum S100B level was increased in the experimental group as compared with the control group [(91.4 ±31.9) pg/L vs.(74.9±26.0) pg/L,t=1.980,P=0.000].The serum NT-proBNP level was higher,LVEDD was longer,and LVEF was lower in the experimental group than in the control group (all P<0.01).The serum S100B level was positively correlated with NT-proBNP (r=0.309,P=0.003) and the NYHA functional classification (r=0.508,P=0.001),and negatively correlated with the LVEF (r=-0.192,P=0.004).Conclusions The serum S100B level is increased in patients with CHF and negatively correlated with cardiac function,suggesting that the S100B protein may serve as a valuable tool for the diagnosis and assessment of CHF.However,more studies are needed to provide more evidence.
