1.Effect of Xuebijing injection on the number of T lymphocytes and Toll-like receptors as adjuvant therapy of severe abdominal infections

ObjectiveTo examine the effect of Xuebijing Injection on the number of T lymphocytes and Toll-like receptors as adjuvant therapy of severe abdominal infections.MethodsA total of 78 patients with severe abdominal infection who were hospitalized in the intensive care unit were divided into control group and treatment group. The patients in control group received conventional therapy alone, while the patients in treatment group received Xuebijing Injection (50 mL twice daily for 2 weeks) in addition to conventional therapy. Blood sample was drawn before and after 2-week treatment to determine T lymphocytes. The peripheral blood mononuclear cells were separated to determine the expression of Toll-like receptors (TLR) 2 and TLR4. The Acute Physiology and Chronic Health Evaluation (APACHE) II score and gastrointestinal function score were recorded before and after treatment.ResultsThe levels of CD3+, CD4+ and CD4+/CD8+ of peripheral blood in the Xuebijing-treated patients were signiifcantly higher than those in control group (P<0.01), but the levels of CD8+ was signiifcantly lower than that in control group (P<0.01). The levels of TLR2 and TLR4 mRNA of the peripheral blood mononuclear cells in the treatment group were signiifcantly lower than those in control group (P<0.01). The APACHE II score and gastrointestinal function score in treatment group were signiifcantly lower than those in control group (P<0.01).ConclusionXuebijing Injection as adjuvant therapy of severe abdominal infection can improve the number of T lymphocytes, and reduce the expression of Toll-like receptors.
2.Study on colonic H2 S-producing enzyme CSE and colonic motility in diabetes mellitus

Objective To investigate the relationsheep between colonic H2 S‐producing enzyme cystathionine‐γ‐lyase(CSE) and colonic motility in a rat model of diabetes mellitus(DM) .Methods To obtained diabetic rat models ,all rats were injected intra‐peritoneally (ip) 1% streptozotocin (STZ ,65 mg/kg) ,type 1 diabetes model was established ,nornmol conditions were observed pe‐riodically .Ten days after treatment ,production of colonic longitudinal smooth muscle strips ,organ bath recordings were used to test the colonic motility ;immunofluorescence and double immunofluorescence lableling method were performed to detect the distribution of CSE;Western blot were performed on rat colonic samples devoid of mucosa and submucosa to detect the expression of CSE .Re‐sults DM rats decreased the colonic motility(P<0 .05);CSE was strongly expressed in the cytosols of the circular and longitudinal smooth muscle cells and the nucleus of the myenteric plexus neurons ;CSE distributed in myenteric neurons in the nucleus of rat ,but the proportion of MP had no difference (P>0 .05);DM rats increased the expression of CSE in the colon devoid of mucosa and sub‐mucosa(P<0 .05) .Conclusion The decrease of colonic motility in diabetic rat may be associated with the increased production of H2 S‐producing enzyme CSE .
3.Changes of vasocative intestinal peptide neurons in the myenteric plexus of gastric antrum in rats with se-vere acute pancreatitis

Objective Patients with severe acute pancreatitis ( SAP) always suffer decrease in gastrointestinal motility , even gastrointestinal failure.The article aimed to investigate the changes of vasoactive intestinal peptide (VIP) in the myenteric plexus of the gastric antrum in rats with severe acute pancreatitis ( SAP) complicated by gastrointestinal dysmotility . Methods Twenty Sprague Dawley rats were randomly allocated into two groups:control group and SAP group .Acute pancreatitis was induced by intraductal infu-sion of 5%sodium taurodeoxycholate .24 h after operation , records were made on pathological evaluation of pancreatic lesion , amylase, C-reactive protein ( CRP) , gastric emptying rate and the percentage of VIP neurons in the total neurons . Results Compared with the control group, the gastric emptying rate in the SAP group was significantly lower ([31.49 ±3.96]% vs [70.99 ±1.82]%).The score of pancreatic lesion, amylase, CRP and the percentage of VIP neurons was significantly higher ([14.63 ±1.06]score vs [1.60 ± 0.52]score, [2061.25 ±434.86]U/L vs [360.20 ±30.29]U/L, [140.10 ±17.07]μg/mL vs [35.71 ±6.43]μg/mL, [22.45 ± 4.10]%vs [10.72 ±1.92]%)(P<0.05). Conclusion Plas-ticity took place in VIP neurons in the myenteric plexus of gastric an-trum in rats with SAP complicated by gastrointestinal dysmotility , which was connected with neural mechanism underlying SAP compli-cated by gastrointestinal dysmotility .
4.Effects of different means of liquid recucitation on mirrocircultation in rats with hemorrhagic shock

Objective To investigate the effects of different means of liquid resuscitation on microcirculation in rats with hemorrhagic shock. Method A model of rats with hemorrhagic shock was established in 40 Wistar rats. The rats were randomly divided into 4 groups according on different means of liquid resuscitation, and killed 24 hours after the examination, group A: without liquid resuscitation; group B: low-molecular-weight-40; group C: 7.5% hypertonic saline,group D:low-molecular-weight-40 + hyperonic saline. As well as the microcirculation in kidney was observed using electron microscope. Results In group without resuscitation, the microcirculation in kidney damaged severely; While in group with hypertonic or low molecule dextran alone, there was harm with dif-ferent degrees, and the microcirculation in kidney was nearly perfect in group with both hypertonic and low molecule dextran alone. Conclusions Different means of liquid resuscitation may infect the microcirculation in kidney, which with both hypertonic and low molecule dextran can protect the microcirculation in kidney.
5.Ability of 18F-FDG PET/CT radiomic features to differentiate EGFR mutation status in patients with lung adenocarcinoma

Objective:To explore and compare the value of radiomic features based on 18F-fluorodeoxyglucose (FDG) PET and CT in distinguishing epidermal growth factor receptor (EGFR) mutation status in patients with lung adenocarcinoma. Methods:Pretreatment 18F-FDG PET/CT images and EGFR gene status of 114 patients (64 males and 50 females, aged range: 35-84 (average age: 61) years) with primary lung adenocarcinoma between January 2017 and December 2017 were retrospectively collected. The volume of interest was drawn manually slice by slice, then the features were extracted by the LIFEx software. The parameters were screened by least absolute shrinkage and selection operator (LASSO) method for 200 times, and ten-fold cross-validation was used to select the best tuning parameter λ. Three models, namely M PET, M CT, M PET+ CT, were constructed by binary logistic stepwise regression. The receiver operating characteristic (ROC) curve was generated and the corresponding area under the curve (AUC), sensitivity, specificity and accuracy were calculated. The AUCs of three models were compared by Delong test. Results:Totally, 53.51%(61/114) patients were with wild type EGFR and 46.49%(53/114) patients had EGFR mutation. There were 3, 3, 7 parameters selected to form M PET, M CT, M PET+ CT, respectively. The AUCs for M PET, M CT, M PET+ CT were 0.730, 0.752 and 0.866 respectively. When the cut-off values were 0.427, 0.522, 0.378 for M PET, M CT and M PET+ CT, the Youden index were up to the maximum as 0.420, 0.405, 0.630, with sensitivities of 83.0%(44/53), 58.5%(31/53), 92.5%(49/53), specificities of 59.0%(36/61), 82.0%(50/61), 70.5%(43/61) and accuracies of 70.2%(80/114), 71.1%(81/114), 80.7%(92/114), respectively. There was no significant difference between AUC of M PET and M CT ( z=-0.320, P>0.05). The differences of AUCs between M PET+ CT and M PET, M PET+ CT and M CT were statistically significant ( z values: 2.963, 2.523, both P<0.05). Conclusions:PET, CT and PET+ CT radiomic features are all associated with EGFR gene expression in lung adenocarcinoma. M PET+ CT has the highest predictive efficiency.
6.Microbial lipid production by Rhodosporidium toruloides in a two-stage culture mode.

To shorten the cultivation time and reduce the consumption of raw materials for microbial lipid production, oleaginous yeast Rhodosporidium toruloides AS 2.1389 was cultivated using a two-stage culture mode, in which the cell propagation and lipid accumulation were separated. The yeast cells recovered from the propagation culture were re-suspended in glucose solution for lipid accumulation, through which lipid content over 55% of the dry cell weight was achieved, the longer the propagation stage was, the higher the lipid content. Analysis of the lipid indicated that the long-chain fatty acids with 16 and 18 carbon atoms were major components, suggesting that the lipid can be an alternative feedstock for biodiesel production.

7.Survey and analysis of the present situation of caregivers' caring activities and supportive behaviors of family caregivers for patients with diabetic kidney disease

Objective:To investigate the current status of care activities and supportive behaviors for family caregivers of patients with diabetic kidney disease and to analyze the influencing factors.Methods:A total of 280 family caregivers for diabetic kidney disease were chosen by convenience sampling in a Nephrology Ward of a ClassⅢ Grade A general hospital and a Diabetic Nephropathy Department of a diabetes specialist hospital in Tianjin from February to October 2019. The general information questionnaire, Barthel Index, Diabetes Caregiver Activity and Support Scale, Social Support Rating Scale, General Self-Efficacy Scale, Zarit Caregiver Burden Interview Scale were applied to conduct an investigation, and to explore the factors affecting the caring activities and supportive behaviors of family caregivers of patients with diabetic kidney disease.Results:The total score of caring activities and supporting behaviors of 280 family caregivers of patients with diabetic kidney disease was (61.23±8.07) . Multiple stepwise linear regression analysis showed that patients' self-care ability, family caregivers' self-efficacy, social support, caring burden, gender, education level were the factors influencing the caring activities and supportive behaviors of family caregivers (adjusted R2=0.558, F=59.769, P<0.001) . Conclusions:Family caregivers need to improve their caring activities and supportive behaviors further, and clinical staff should pay more attention to patients with following features: poor self-care abilities, low self-efficacy, low levels of social support, heavy caring burdens, female, and low levels of education, as well as to provide health education and training from the perspective of family caregivers to assist them in improving their caring activities and supportive behaviors.
8.Staged and segmented two hybrid surgeries for total repair of Debakey type Ⅰ aortic dissection: a series of 10 cases

Objective:To examine the efficacy and experience of staged and segmented two hybrid surgeries for total repair of Debakey type Ⅰ aortic dissection (TIAD).Methods:This study was a retrospective case series. The clinic data of 10 patients with acute TIAD who were admitted to the Department of Cardiac Surgery, Second Hospital of Lanzhou University or the First People′s Hospital of Lanzhou, between January 2016 and August 2022, were retrospectively studied. Ten patients underwent hybrid surgeries in two hospitalizations (stages), including 7 males and 3 females with an age of (60±7) years (range: 49 to 71 years). In stage 1, the first type Ⅱ hybrid arch repair was performed to treat the ascending, total arch, and descending thoracic aorta for acute TIAD without circulatory arrest. In stage 2, the second hybrid surgery including infrarenal abdominal aorta replacement, visceral arteries bypass and endovascular thoracoabdominal aortic repair was performed to treat residual thoracoabdominal aortic dissection after the first hybrid operation (segmented). Basic data, preoperative concomitant diseases, high-risk factors, surgical approaches and postoperative complications of all important organs, as well as CT imaging were analyzed.Results:There was no death in the 20 hybrid surgical procedures. In stage 1 type Ⅱ hybrid surgery, 4 cases underwent reconstruction of the aortic sinutubular junction, while Bentall and David surgery was performed for 3 cases, respectively. A patient received coronary artery bypass grafting. Then all patients were sequentially treated with arch debranching and thoracic aortic endovascular repair. Postoperative complications included renal insufficiency (4/10), hemofiltration (1/10), hypoxemia (4/10), neurologic event (1/10) and type Ⅱ endoleak (1/10). Complete false lumen thrombosis occurred in 9/10 of the patients. All complications recovered successfully at discharge and the average hospital stay was (21±4) days (range: 16 to 28 days) in the first hospitalization. At stage 2, the second hybrid surgery was successfully performed in all patients. No paraplegia, hepatic or renal insufficiency, or endoleak occurred. However, branch graft embolism of the left renal artery was found in one patient 3 days after laparotomy, as well as of superior mesenteric artery in another. Superior mesenteric artery occlusion was successfully treated by endovascular recanalization. Complete false lumen thrombosis occurred in all patients. Although all patients had different degrees of intestinal dysfunction, they were gradually relieved at discharge, and the average hospital stay was (19±2)days (range:16 to 21 days) in the second hospitalization. During follow-up, CT angiography showed aortic remodeling in all patients.Conclusion:Staged and segmented two hybrid surgeries are safe and feasible for total repair of Debakey type Ⅰ aortic dissection and are associated with acceptable early and midterm outcomes.

10.Molecular tracking of long-distance transmission events of COVID-19 by lorry drivers in Fujian province

Objective:To confirm the cross-provincial long-distance transmission events of Coronavirus Disease 2019 (COVID-19) by lorry drivers, the origin of infections of the cases and the transmission routes were tracked.Methods:Nasopharyngeal swab specimens from five lorry driver cases of COVID-19, found in Zhangzhou city in March, 2022 when the local outbreaks occurred in adjacent Quanzhou city, Fujian province, were collected to perform 2019 novel coronavirus (2019-nCoV) targeted genome amplification and followed by next-generation sequencing. The sequences were submitted to online 2019-nCoV analysis platforms to classify the type of variant and mutation sites. Phylogenetic tree for the viruses were constructed by phylogenetic analysis software. Combined with the epidemiological investigation, the origin of infections of the cases and the transmission routes were deduced.Results:Five complete genome sequences, with 29 770-29 839 bp in length and 99.53% average genomic coverage, of 2019-nCoV were successfully obtained. The viruses were all Omicron variants and further divided into three different subclades of BA.2. Of the five strains of 2019-nCoV, three were highly similar to the viruses of two distinct lineages co-circulated in Quanzhou city during the period of local outbreak of COVID-19, respectively. Phylogenetic analysis also revealed that the viruses from three infected lorry drivers were highly homologous to that from local outbreaks in Quanzhou city. The viruses from the rest two cases had seven to fourteen nucleotide mutations (corresponding to 5-7 amino acid substitutions) when compared with the viruses in local outbreaks in Quanzhou city, which excluded the involvement of the two cases into the transmission chains of local outbreaks. Combined with the field epidemiological investigations, the result revealed that the origin of infection of 2019-nCoV of the two sporadic lorry driver cases was outside of Fujian province.Conclusions:With the aid of high-throughput sequencing and bioinformatics technology combined with field epidemiological investigations, we speculated in this study that at least three origins of infection of 2019-nCoV in five lorry driver cases and cross-provincial long-distance transmission via two sporadic cases infected outside Fujian province when they returned.
