1.The Analysis of High Frequency Sensorineural Hearing Loss Caused by Piston Technique of Stapes Surgery in Patient with Otosclerosis

Objective To examine the causes of high frequency sensorineural hearing loss caused by piston technique of stapes surgery.Methods The retrospective analysis involved in thirty cases(49 ears)with stapedectomy from February 2004 to 2009 at our hospital.Pure tone audiometry was performed for each patient who underwent stapedectomy both during the two days preoperation and 1,3,6 months postoperation.Results One month after operation at 8000 Hz,the air conduction hearing threshold was 8 dB less than that before,4 000 Hz average bone conduction hearing threshold was 10 dB less than that before(P < 0.01).The average high-frequency hearingthreshold of ≥40 years old group was noticeably declined than that of 40 years old group(P < 0.01).There was no association between the tinnitus and high frequency hearing loss.(P >0.05).Conclusion Transient high-frequency sensorineural hearing losses occurred to most patients under going stapedectomy,but showed significant improvement,3 months after the surgery.The average high-frequency hearing threshold of ≥40 years old group was seriously declined than that of 40 years old group.

Objective To study the vestibular function in patients with sudden deafness .Methods Retrospec-tive analysis of 436 cases of patients with sudden deafness ,being divided into two groups according to whether the merger of vertigo .147 cases with vertigo group ,75 cases of which were mild to moderately severe hearing loss .In-clud the rise type of 28 cases ,26 cases of flat type ,21cases of decline type .72 cases of which were severe hearing loss .In 289 cases without vertigo ,157 cases had mild moderate and severe hearing loss .Includ the rise type of 36 cases ,57 cases of flat type ,64 cases of decline type .One hundred and thirty two cases of which were severe hearing loss .Use video electronystagmogram (VNG) to test the vestibular function .Process the data with SPSS 13 .0 soft-ware .Results One hundred and twenty three(83 .7% )cases of the vertigo group suffered from vestibular function decline ,with 51 cases of mild to moderately severe hearing loss ,72 cases of severe and above .165 (57 .1% )cases of the without vertigo group suffered from vestibular function decline .There were 45 cases of mild to moderately se-vere hearing loss ,120 cases of severe and above .35 patients with vertigo patients with BPPV ,28 cases of which happened within 1 week in sudden deafness occured and 80% of which were vestibular function decline .Conclusion Patients with sudden deafness with vertigo suffered from vestibular function damage .Patients with sudden deafness without vertigo also suffered from vestibular function damage .And the more serious the hearing loss ,the higher the vestibular function decline opportunities .
3.Hidden hearing loss in tinnitus patients with normal audiograms: implications for the origin of tinnitus.

OBJECTIVE:
To investigate hidden hearing loss in tinnitus patients with normal audiograms by means of auditory brainstem response (ABR) and explore the origin of tinnitus.
METHOD:
Pure tone thresholds, ABR thresholds, amplitude of wave I and wave V of ABR were analyzed in 40 tinnitus patients and 15 controls.
RESULT:
There was no significantly difference in pure tone thresholds and ABR thresholds between those tinnitus patients and controls while a reduced amplitude of wave I and normal amplitude of wave V of ABR in the tinnitus patients became evident.
CONCLUSION
Tinnitus patients with normal audiograms have hidden hearing loss at the level of primary auditory nerve and the generation of tinnitus is likely attributed to a homeostatic response of neurons in brainstem.

4.Correlation between serum Metrnl and interleukin-6 in patients with acute cerebral infarction

Objective As a new adipokine,Metrnl is closely related to atherosclerosis and inflammatory response.This study aims to explore the correlation between serum Metrnl level and acute cerebral infarction and inflammatory factors.Methods 139 patients with acute ischemic stroke were selected as ACI group,and 130 people with physical examination were selected as N-ACI group.The serum Metrnl level was measured by ELISA,and the common risk factors of stroke were detected.The correlation between serum Metrnl level and acute cerebral infarction and common risk factors was compared by statistical methods.Results The level of serum Metrnl in patients with ACI was lower than that in the control group (P<0.001).After adjusting for confounding factors,the difference was still significant,indicating that low level of Metrnl was significantly correlated with acute cerebral infarction.In the acute cerebral infarction group,the level of serum Metrnl was negatively correlated with BMI,HCY,LDL,AIP,and was negatively correlated with inflammatory markers Hs-CRP and IL-6.Regression analysis showed that Metrnl was a protective factor for acute cerebral infarction.Conclusion The level of serum Metrnl is significantly correlated with the occurrence of acute cerebral infarction.At the same time,the level of serum Metrnl is negatively correlated with inflammatory factor IL-6.As a new biomarker of acute cerebral infarction,Metrnl may become a new diagnostic and therapeutic target.
5.Survey of the quality of life, psychological status, and treatment needs of patients with rosacea

Objective:To investigate the impact of rosacea on patients' quality of life and mental health, and to analyze variations across different subtypes, gender, sun exposure, smoking status, ethnicity, and educational levels, as well as patients' demands for treatment.Methods:It was a cross-sectional study from October 2022 to January 2024, 177 rosacea patients from the Department of Dermatology, the First Affiliated Hospital of Kunming Medical University were surveyed. The cohort comprised 36 males and 141 females, with a median age of 35 (29, 44) years. Data collected through questionnaires included general information, Dermatology Life Quality Index (DLQI), Generalized Anxiety Disorder-7 (GAD-7), Patient Health Questionnaire-9 (PHQ-9), and Patient Need Questionnaire (PNQ). Statistical analysis was conducted on the collected data.Results:The median DLQI score was 10 (3, 15), indicating moderate impact. The median GAD-7 score was 5 (2, 7), suggesting mild anxiety; while the median PHQ-9 score was 6 (2, 9), indicating mild depression. Female patients scored higher in both DLQI and GAD-7 than males ( P<0.05). Papulopustular subtypes had higher DLQI and GAD-7 scores compared to other subtypes ( P<0.01). Patients with lower education levels scored higher in DLQI and GAD-7 than those with higher education ( P<0.01), and non-smokers had higher GAD-7 scores than smokers ( P<0.05). Significant positive correlations were found between DLQI and PHQ-9 ( r=0.39, P<0.01), and DLQI and GAD-7 ( r=0.51, P<0.01), as well as between PHQ-9 and GAD-7 ( r=0.59, P<0.01). Regarding treatment needs, 159 patients (89.8%) expressed a strong desire for a clear diagnosis and treatment plan, and the same number wished for a complete resolution of all skin lesions. Female patients consistently demonstrated significantly stronger demands in all 28 treatment need items (all P<0.05). Conclusions:Rosacea significantly affects patients' quality of life and mental health, particularly among females, individuals with papulopustular subtypes, and those with lower education levels. There is a strong demand for definitive diagnostic and therapeutic plans, especially among female patients.
6.Relationship between childhood obstructive sleep apnea-hypopnea syndrome and tympanogram.

OBJECTIVE:
To explore the correlation between childhood obstructive sleep apnea-hypopnea syndrome and tympanogram.
METHOD:
A prospective clinical study was carried out. Polysomnography and tympanogram were analyzed in 72 cases.
RESULT:
Among the 144 ears 37 ears had type C or B tympanogram. There was no correlation between tympanogram and LSaO2. It was found correlation between tympanogram and AHI (P < 0.05).
CONCLUSION
Hypoxemia may affect middle-ear function. It was necessary to do the examination of acoustic impedance routinely in children with moderate and severe OSAHS.

7.Characteristics of Cerebral Blood Flow in Sleep Deprivation Based on Arterial Spin Labeling

Purpose To investigate the difference of cerebral blood flow between sleep deprivation and rest wakefulness.Materials and Methods Fifty subjects were recruited from universities in Xi'an from October 2020 to December 2021.The psychomotor vigilance test(PVT)task was used to measure sustained attention.Arterial spin labeling technique was used to analyze and compare the relative cerebral blood flow(rCBF)between sleep deprivation and rest wakefulness.The correlation between altered rCBF of specific brain regions and PVT task performance after sleep deprivation was analyzed.Results Compared with rest wakefulness,rCBF in bilateral dorsolateral prefrontal lobe,bilateral parietal lobule,left orbital middle frontal gyrus,bilateral middle temporal gyrus,right posterior central gyrus,and bilateral angular gyrus was significantly decreased after sleep deprivation.The rCBF of bilateral thalamus,left precuneus,right medial prefrontal lobe,left posterior cingulate gyrus,and left inferior temporal gyrus was significantly increased(FDR corrected,P<0.05,cluster size≥20 voxels).The changes of rCBF in left dorsolateral prefrontal lobe and right parietal lobule were significantly negatively correlated with the PVT task performance(r=-0.56,P<0.001;r=-0.64,P<0.001),and the change of rCBF of left precuneus was significantly positively correlated with the PVT task performance(r=0.72,P<0.001).Conclusion The abnormal changes of CBF in default mode network,frontoparietal network-related brain regions and thalamic may be the important neural mechanism of sustained attentional decline after sleep deprivation.
8.Involvement of oral flora in the role of betel nut-extract-induced ulcers in rats

Objective A model for studying oral ulcers induced by betel nut-extract was constructed in rats.Changes in the structure and diversity of oral flora were observed to explore the involvement of oral flora and local inflammatory factors in the pathogenesis of oral ulcers induced by betel nut-extract and to provide theoretical support for the prevention and treatment of oral ulcers in the clinic.Methods Thirty SD rats were randomly divided into normal,model and intervention groups(Guilin watermelon cream,8 mg/d for 7 days),with 10 rats/group.The oral mucosa of rats was subcutaneously injected with 10 g/mL of betel nut-extract to generate an oral ulcer model.The histomorphological changes were observed,and ulcer area and ulcer scores were assessed.Local oral tissue tumor necrosis factor-α(TNF-α),interleukin(IL)-2 and IL-8 levels were determined.Oral mucosal tissues were sampled for HE staining and analyzed for the structural distribution of oral flora and the diversity of microbial communities using high-throughput sequencing method.Results Compared with rats in the normal group,those in the model group had an increased ulcer area,significantly increased ulcer scores(P<0.01),and significantly increased levels of TNF-α,IL-2 and IL-8 in the oral mucosal tissues(P<0.01).The amount Streptococcus(P<0.05)and Veillonella(P<0.001)in the oral saliva of the model group rats was significantly reduced.The model group rats showed oral mucosal epithelial cell hyperplasia or focal necrosis,mucosal lamina propria edema,and hemorrhage accompanied by mass neutrophil and monocyte infiltration.Compared with the model group rats,the intervention group rats had significantly reduced ulcerated area(P<0.05,P<0.01)and ulcer scores(P<0.05).And oral mucosal tissue levels of TNF-α(P<0.01),IL-2(P<0.05)and IL-8(P<0.05),as well as significantly increased Streptococcus(P<0.001)and Veillonella(P<0.01)and significantly reduced Staphylococcus(P<0.01)in the oral saliva.The degree of lesions in the oral mucosal tissues was significantly improved in the intervention group.Conclusions Betel nut-extract can be used to successfully reproduce a rat model of oral ulcer,and it is speculated that the development of oral ulcers after exposure to betel nut-extract may be related to an imbalance in the oral flora and local tissue inflammatory mediators.
