Objective To investigate the role of CTLA4-Ig and anti-CD40L monoclonal antibody in the acute rejection of pancreaticoduodenal transplantation model of rats.Methods Pancreaticoduo- denal transplantation model was established from the donor F344 rats to the Lewis recipients(diabetes models).The models were divided into 4 groups:groups A,B,C and D with 12 rats in each group. Two days after transplantation,recipients were injected intraperitoneally with saline,CTLA4-Ig(200?g),anti-CD40L monoclonal antibody(200?g),CTLA4-Ig(200?g)combined with anti-CD40L monoclonal antibody(200?g)respectively.On the day 1,4,7,10 after transplantation,the grafts were harvested for histopathological examination and RT-PCR to determine the levels of interleukin (IL)-2,interferon(IFN)-?,IL-4 and IL-10;The blood CD3~+,CD4~+and CD8~+ T cells were detected by flow cytometry.On the day 4 after transplantation,the CD4~+ CD25~+ T cells in the grafts were de- tected by flow cytometry.Results As compared with group A,the severity of the rejection of grafts in groups B,C and D were depressed;Down-regulation of IL-2 was observed in the groups B,C and D, and the levels in group D were lowest.Down-regulation of IFN-7 was detected in the groups B,C and D,but there was no significantly difference between groups D and B or groups D and C.Up-regulation of IL-4 was observed in the groups B and C,and the levels in group D were lower than in groups A,B and C.Up-regulation of IL-10 was observed in groups B and C,and there was significant difference between groups D and B or groups D and C.The CD3~+,CD4~+ and CD8~+ T cells in groups B,C and D were less,but more CID4~+ CD25~+ T ceils in transplanted pancreas were observed,more notably in group D than in group C.Conclusions Combined use of CTLA4-Ig and anti-CD40L monoclonal anti- body can remarkably diminish the severity of the rejection,which might be mediated by altering the balance in Th1/Th2 and increasing the number of CD4~+ CD25~+ regulatory T cells.Co-stimulation blockade with CTLA4-Ig and anti-CD40L monoclonal antibody induction seems to be an attractive strategy to control allograft rejection.

Objective To examine the induction effects of bone marrow mesenchymal stem cells(BMSCs) transfected with bone morphogenetic protein 7 (BMP7) gene differentiating into chondrocytes. Methods We observed the phenotype of cells which were stained with alcian blue and HE climbing to the six pore plate with invert microscope. The glycosaminoglycan (GAG) value in culture medium was detected in control group,BMP7 transfect and culture medium induced groups after 7,14 and 21 days using standard curve method. Standard curve was described using galacturonic-acid as reference substance. The content of collagen Ⅱ was detected by ELISA method. Results HE and Alcian blue staining showed that BMP7 gene transfection group and the group induced by fluid possess the characteristics of chondrocyte. BMP7 induced BMSCs differentiation to chondrocyte which secrete specific protein called collagen Ⅱ and GAG. Content of GAG were (17.1±3.4),(39.5±5.4),(40.8±6.1)mg/L in control group,BMP7 gene transfected group and induced group,collagen Ⅱ were (89.7±14.3),(152.8±14.5),(155.5± 19.3)μg/L in these three groups separately. Comparing with control group,GAG and collagen Ⅱ of BMP7 gene transfected group and culture medium induced group increased obviously(all P < 0.05),but there was no significant difference between BMP7 gene transfeeted group and culture medium induced group (P > 0.05). Conclusion This active protein induces BMSCs differentiating into chondrocyte,in a level similar to that of inducing medium.

BACKGROUND:For patients in intensive care unit (ICU), mechanical ventilation is an effective treatment to survive from acute illness and improve survival rates. However, long periods of bed rest and restricted physical activity can result in side effects. This study aimed to investigate the feasibility of early rehabilitation therapy in patients with mechanical ventilation. METHODS:A randomized controlled trial was carried out. Sixty patients, with tracheal intubation or tracheostomy more than 48 hours and less than 72 hours, were admitted to the ICU of the Affiliated Hospital of Medical College, Qingdao University, from May 2010 to May 2012. These patients were randomly divided into a rehabilitation group and a control group. In the rehabilitation group, rehabilitation therapy was performed twice daily, and the training time and intensity were adjusted according to the condition of the patients. Early rehabilitation therapy included heading up actively, transferring from the supine position to sitting position, sitting at the edge of the bed, sitting in chair, transferring from sitting to standing, and ambulating bedside. The patient's body mass index, days to first out of bed, duration of mechanical ventilation, length of ICU stay, APACHE Ⅱ score, highest FiO2, lowest PaO2/FiO2 and hospital mortality of patients were all compared between the rehabilitation group and the control group. The differences between the two groups were compared using Student's t test. RESULTS:There was no significant difference in body mass index, APACHE Ⅱ score, highest FiO2, lowest PaO2/FiO2 and hospital mortality between the rehabilitation group and the control group (P>0.05). Patients in the rehabilitation group had shorter days to first out of bed (3.8±1.2 d vs. 7.3±2.8 d; P=0.00), duration of mechanical ventilation (5.6±2.1 d vs. 12.7±4.1 d; P=0.005) and length of ICU stay (12.7±4.1 d vs. 15.2±4.5 d; P=0.01) compared with the control group. CONCLUSION:Early rehabilitation therapy was feasible and effective in improving the outcomes of patients with mechanical ventilation.

Moving cup of cupping therapy is studied in the present paper systematically. According to the experiences of clinical practice, duration, speed and intensity of moving cup has been quantified. Fifteen basic manipulations have been evolved, and 10 protocols for clinical application recommended. "Moving cup according to 3 factors of duration, speed and intensity" has been advanced on the basis of acupuncture therapy. And it has been concluded that the effects of warming needling, simple cupping, scraping, massage and medication can be achieved by moving cup with different techniques and clinical protocols.

A strain was isolated from internal organ of died porcine about 8 weeks with purulent pneumonia,arthritis,pyogenic arthritis and endocarditis in April 2007.Objectives of the study are to confirm the genus of the strain,pathopoiesis,and drug sensitivity.The mainly study methods:the first,the strain was identified by the phenotype and the characteristics of the biochemistry,sequence 16S rDNA genes of the strain was analyzed by molecular biology technology,finally animal experiment and drug sensitivity testing were done.The results of the phenotype and the characteristics of the biochemistry showed that it is greatly similar to Actinomyces hyovaginalis,16S rRNA sequence analysis exhibited the homology achieved to 99.2% com-pared with group III strains of Actinomyces hyovaginalis,and the phyletic evolution analysis also indicated that it has mostly relationship with group III strains of Actinomyces hyovaginalis.Animal experiment dis-covered it has highly pathogenicity to Mus musculus albus;Drug sensitivity testing showed that it is hyper-sensitive to Erycin,Gentamicin and Amikacin.So,the result of the study confirmed that the strain is Actin-omyces hyovaginalis III with the pathogenicity.

Objective To discuss the clinical application and efficacy of digitization prototyping titanium meshes in the repairing of earthquake-induced skull defect. Methods The clinical data of 66patients with skull defect caused by earthquake were analyzed. These patients were divided into group A (treated with traditional handwork shaping titanium meshes, n=1S) and group B (treated with digitization prototyping titanium meshes, n=51). The efficacy and complications were compared.Results Compared to group A, group B had a shorter operation time, a shorter recovery time, a lower postoperative incidence rate of subcutaneous hematocele, hydrops and infection (P＜0.05). The digitization prototyping titanium meshes were significantly superior to handwork shaping titanium meshes (P＜0.05). And patients with skull defect repaired at early stage had a higher recovery rate of neurological function than those at advanced stage (P＜0.05). Conclusion Using digitization prototyping titanium meshes to repair the skull defect can enjoy anatomical reduction, shortened operation time, and fewer postoperative complications. In addition, an early repair in earthquake-induced skull defect can not onlydisburden patients' mind, but also prevent or even reverse secondary brain damage of skull defect,therefore, the patients' neurological function can be improved and their lives can be better.

Objective To investigate the distribution of common allergens in patients with allergic rhinitis(AR) from 2006 to 2010 in Wuhan area,and provide the objective evidence for the prevention and treatment of AR.Methods The medical records of skin prick test (SPT) performed on 2707 AR patients from 2006 to 2010 were restrospectively analysed,and the positive rate of different allergens and changing trends in this time were compared.SPSS 17.0 software was used to analyse the data.Results There were significant differences among the Dermatophagoides pteronyssinus positive rate ( x2 =12.11,P ＜ 0.05 ) and Dermatophagoides farinae positive rate( x2 =11.11,P ＜0.05 ) in the past 5 years.Meanwhile,there was an upward trend in the positive rate of dust mite,which the positive rate of Dermatophagoides pteronyssinus increased from 84.5％ in 2006 to 90.5％ in 2010 (x2 =6.88,P ＜ 0.05 ),positive rate of Dermatophagoides farinae increased from 81.5％ in 2006 to 89.0％ in 2010 ( x2 =9.68,P ＜ 0.05 ) ; There were significant differences among the Mugwort and Ragweed positive rate of 5 years ( x2 =194.10,P ＜0.05 ; x2 =67.06,P ＜ 0.05 ).There were significant differences among the mold Ⅰ and mold Ⅱ positive rate of 5 years between (x2 =18.95,P＜0.05; x2 =36.62,P＜0.05).Meanwhile,there was an upward trend in the positive rate of mold and fluctuant trend in the positive rate of spring-pollen.Conclusions Nearly five years,dust mites is still the most common allergens in AR patients,presenting upward trend ; the positive rate of mold presenting upward trend; the positive rate of wormwood and guineawood presenting downward trend ; the positive rate of pollen presenting fluctuant trend.

BACKGROUNDThe level of c-Myc is closely associated with high pathological grade and the poor prognosis of gliomas. Vascular endothelial growth factor (VEGF) is the most important angiogenic factor that potently stimulates the proliferation and migration of vascular endothelial cells. This study aimed to address the biological importance of c-Myc in the development of gliomas, we downregulated the expression of c-Myc in the human glioblastoma cell line IN500 and studied the in vitro effect on cellular growth, proliferation, and apoptosis and the expression of VEGF and the in vivo effect on tumor formation in a xenograft mouse model.

METHODSIN500Δ cells were stably transfected with shRNA-expressing plasmids for either c-Myc (pCMYC-shRNA) or as a control (pCtrl-shRNA). Following establishment of stable cells, the mRNA expressions of c-Myc and VEGF were examined by reverse transcription (RT)-PCR, and c-Myc and VEGF proteins by Western blotting and immunohistochemistry. Cell-cycle progression and apoptosis were determined by flow cytometry. The in vivo effect of targeting c-Myc was determined by subcutaneous injection of stable cells into immunodeficient nude mice.

RESULTSThe stable transfection of pCMYC-shRNA successfully knocked down the steady-state mRNA and protein levels of c-Myc in IN500, which positively correlated with the downregulation of VEGF. Downregulating c-Myc in vitro also led to G1-S arrest and enhanced apoptosis. In vivo, targeting c-Myc reduced xenograft tumor formation and resulted in significantly smaller tumors.

CONCLUSIONSc-Myc has multiple functions in glioblastoma development that include regulating cell-cycle, apoptosis, and VEGF expression. Targeting c-Myc expression may be a promising therapy for malignant glioma.

OBJECTIVETo investigate the prevalence and drug resistance of Streptococcus (S.) pneumoniae in patients infected in communities and molecular epidemiology with BOX-polymerase chain reaction (PCR) in Chongqing areas.

METHODSA total of 680 clinical specimens from sputum and throat/nasal swabs were collected from patients seen from September 2000 to March 2001. Antibiotic susceptibility was determined by agar dilution test. BOX-PCR was used for molecular typing of S. pneumoniae.

RESULTSA total of 39 isolates of S. pneumoniae were collected with the isolation rate of 5.7%. Of the 34 S. pneumoniae strains, two showed low-level resistance to penicillin (MIC 0.125 mg/L), one to levofloxacin, but many to macrolide and clindamycin (nearly 70%). All the strains were susceptible to beta-lactams and vancomycin. BOX-PCR typing demonstrated a high discriminatory potential and easy to be accurately analysed. 35 S. pneumoniae strains (include ATCC49619) were divided into 25 distinct types, representing 29 subtypes with A (n = 3) as the predominant type. 2 penicillin-resistant strains were shown to be different types.

CONCLUSIONPenicillin resistant rate of S. pneumoniae was low in Chongqing, but macrolide and clindamycin resistant strains were common while BOX-PCR typing was a suitable technique to type S. pneumoniae. No dominant antibiotic resistant strains were found in Chongqing.
