1.Effect of transrectal prostatic biopsy on serum prostate specific antigen levels.

OBJECTIVESTo assess the effect of transrectal prostatic biopsy (TPB) on the concentrations of serum prostate specific antigen (PSA).
METHODSTwenty patients with abnormal PSA levels and/or digital rectal examination (DRE) underwent TPB. Serum PSA levels were measured before TPB and at 0.5 h, 1 week, 1 month after TPB, respectively.
RESULTSThe serum PSA levels before TPB and 0.5 h, 1 week, 1 month after TPB were (12.23 +/- 8.62), (34.90 +/- 41.53), (23.59 +/- 20.78) and (11.31 +/- 6.95) micrograms/L, respectively. The serum PSA concentration was significantly higher at 0.5 h after TPB than before (P < 0.05), and then gradually decreased. PSA levels remained higher for at least 1 week in 85% (17/20) patients(P < 0.05), then returned to the baseline at one month after TPB (P > 0.05).
CONCLUSIONSTPB can lead to a dramatic increase of PSA in serum and keep the PSA value high in one week. Then the PSA in serum decreased gradully. Serum PSA level cannot return to baseline until one month after TPB.

2.Establishment of a NOD/SCID mouse model with human immune reconstitution bearing human triple-negative breast cancer.

OBJECTIVETo establish a NOD/SCID mouse model with human immune reconstitution and observe its immune response to human triple-negative breast cancer xenograft.
METHODSTwenty-four NOD/SCID mice without immune leakage were subjected to cyclophosphamide (CTX) treatment 3 days prior to immune reconstitution with human peripheral blood mononuclear cell (PBMC) injection and subcutaneous transplantation of human triple-negative breast cancer MDA-MB-231 cells, CTX treatment and PBMC injection without tumor cell transplantation, MDA-MB-231 cell transplantation only, or no treatments. The tumor growth and immune responses of the mice were observed at regular intervals.
RESULTSCompared with the tumor-bearing mice, the tumor-bearing mice with immune reconstitution showed prolonged incubation period of tumor formation, slower tumor growth rate and increased survival rate. Human IgG and CD3(+) T cells were detected in the peripheral blood of the mice 1 week after human PBMC injection. The percentage of CD3(+) T cells in the spleen cells was 55.3% at 9 weeks in tumor-bearing mice with immune reconstitution and 52.7% in tumor-bearing mice without immune reconstitution. The spleen index of the tumor-bearing mice with immune reconstitution was much higher than that in mice with only immune reconstitution and the control mice (9.64 vs 3.82∓0.31 and 1.51∓0.14 mg/g).
CONCLUSIONA stable NOD/SCID mouse model with immune reconstitution has been established successfully, which shows immune responses to triple-negative breast cancer xenografts and allows studies of immunological therapy study of triple-negative breast cancer.

3.Multicenter randomized control trial on safety of domestic idarubicin for acute leukemia.

OBJECTIVETo evaluate the safety of domestically produced idarubicin in the treatment of acute leukemia by a multicenter randomized control trial.
METHODSThis trial was carried out in the hemotologica department of five hospitals throughout China, with hospitalized patients who suffered from acute myelogenous leukemia ( AML except M3 type) , acute lymphocytic leukemia ( ALL) , chronic myelogenous leukemia-blast (CML-blast) , totally 155 patients. Those with severely cardial, hepatic or renal disfunction or those who had ever treated with > or = 200 mg/m(2) idarubicin were excluded from the trial. All patients signed the letter of consent as required by the Ethics Committee of our government. In this study, 155 leukemia patients were randomly grouped into: 1. test group treated using domestic idarubicin, 2. control group using imported idarubicin. The acute myelogenous leukemia regimen included idarubicin 8 mg/m(2), dl -3 plus cytosine arabinoside 100 mg/m(2), dl - 7 for 1-2 cycles. The regimen for acute lymphocytic leukemia was idarubicin 8 mg/m2, dl - 3; vincristine 2 mg/mr, dl; cyclophosphamide 750 mg/m2, dl ; plus prednisone 60 mg/m(2),dl - 14 for 1-2 cycles.
RESULTSClinical response rate of the tested group treated with domestic idarubicin and control group treated with imported idarubicin was 78. 1% (50/64) vs. 76.9% (50/65) without any statistically significant difference between the two groups(P >0. 05). Grade Ill - IV hematological toxicity rate of the domestic idarubicin group and imported idarubicin group was 74. 0% vs. 73. 1% , respectively (P = 0. 73). Drug-related death was observed in 3 of 77 patients in the domestic idarubicin group (3.9%) due to cerebral hemorrage or septic infection. The incidence of non-hematological toxicities in domestic idarubicin group and imported idarubicin group was 84. 4% vs. 79. 5% for nausea or vomiting, 70. 1% vs. 71. 8% for infection, 42. 9% vs. 41. 0% for mucositis, 33. 8% vs. 33. 3% for alopecia, 28.6% vs. 28. 2% for serum glutamicoxalacetic transaminase abnormalitis, 16. 9% vs. 10. 3% for cardiac toxicity, all without statistically significant differences between these two groups (P > 0. 05). Discontinuation of treatment due to non-hematological toxicity was not neccessary.
CONCLUSIONDomestic idarubicin is comparable to imported counterpart in efficiency and safety for the treatment of acute leukemia. The most severe side effects of domestic idarubicin is hematological toxicity, which should be closely observed and treated in time, while its non-hematological toxicity is tolerable.

4.Multi-institutional randomized controlled clinical trial on China made 4-demethoxydaunokrubicin (IDA) in the treatment of acute leukemia.

OBJECTIVETo evaluate the efficacy and safety of IDA (Haizheng Parmacy, China) in the treatment of acute leukemia.
METHODSA multi-institutional single-blind randomized controlled clinical trial was carried out. A total of 155 newly diagnosed patients with AML and ALL were enrolled. The patients were randomly divided into two groups, one was given IDA (n = 77) and the other given zevodas (Pharnacia & Upjohn, n = 78) for comparison.
RESULTSAll the patients enrolled in this trial were eligible for assessment of side effects, and 129 patients for evaluation of overall response rate. In patients treated with IDA vs zevodas, the overall response rate (OR) was 78.1% vs 76.9%, CR was 68.8% vs 67.7%; in AML patients, OR was 82.4% vs 71.8%, and CR was 76.5% vs 64.1%; in ALL patients, OR was 80.0% vs 81.8%, and CR was 68.0% vs 68.2%. There was no sitatistically significant difference in hematologic and non-hematologic toxicities between the two groups.
CONCLUSIONThe efficacy of IDA in the treatment of acute leukemia is comparable to that of zevodas. Both have similar toxic side effects.

5.A prospective multicenter clinical trial of medical and surgical treatment for chronic rhinosinusitis.

OBJECTIVETo demonstrate the clinical outcomes of maximal medical therapy(MMT) and functional endoscopic sinus surgery+ maximal medical therapy (FESS+MMT) for moderate to severe chronic rhinosinusitis without nasal polyps (CRSsNP) in China, to provide clinical evidence for treatment recommendation.
METHODSA prospective multicenter case control study consisting of 90 strictly selected CRSsNP patients were carried out by ENT Hospital of SUN Yet-sen university and 7 other university affiliated hospitals across China from March 2011 to October 2012. All patients were selected to MMT group or FESS+MMT group followed by 3 months treatment and 6 months follow up. Treatment efficacy evaluation indicators included improvement of visual analogue scale (VAS) score, quality of life, CT score and post-operative nasal endoscopic evaluations. SPSS16.0 software was used to analyze the data.
RESULTSAll patients enrolled complied with moderate to severe CRSsNP evaluation criteria. There were no significant differences found in the baseline data between two groups (P > 0.05). (1) At the 3 time points (pre-treatment, 3 months and 6 mongths after treatment) , VAS overall symptom score in MMT group were 6.52 ± 2.61, 2.66 ± 2.10, 2.40 ± 1.56, significant differences were found among them (t value were 2.083 and 2.295, both P < 0.05); in FESS+MMT group the values were 6.99 ± 2.70, 0.95 ± 0.84, 0.60 ± 0.81, significant differences were found among them (t value were 3.582 and 5.196, both P < 0.05); SNOT-20 score in MMT group were 38.61 ± 17.36, 18.59 ± 14.04, 18.40 ± 8.91, significant differences were found among them (t value were 2.737 and 2.657, both P < 0.05); in FESS+MMT group the values were 38.21 ± 19.61, 5.94 ± 5.01, 2.65 ± 2.31, significant differences were found among them (t value were 3.247, 3.319, both P < 0.05). (2) FESS+MMT group relative to the MMT group in VAS overall symptom score and quality of life improvements appeared earlier and were more pronounced. (3) Overall treatment efficacy showed that in MMT group: complete control 14 cases (30.4%), partially control 31 cases (67.4%), uncontrolled 1 cases (2.2%); In FESS+MMT group: complete control 17 cases (38.6%), partially control 26 cases (59.1%), uncontrolled 1 cases (2.3%). (4) Patients' satisfaction survey showed that the number of the patients who were very satisfied with the efficacy in the FESS+MMT group were 2 times higher than the MMT group.
CONCLUSIONS(1) For moderate to severe CRSsNP, both MMT and FESS+MMT treatment can effectively control the overall symptoms and classified symptoms, reduce CT scores and significantly improve the quality of life, the ineffective rate is less than 5%. (2) FESS+MMT group in terms of improving symptoms and the onset time are better than MMT group, especially in improving the stuffy nose, head and face fullness, and mental and physical symptoms are better than MMT group. (3) FESS+MMT group showed better results in patient satisfaction survey compared to the MMT group. Therefore for moderate to severe CRSsNP patients, FESS+MMT therapy could be recommended as the preferred treatment.

6.Clinical application of mastoscopic axillary sentinel lymph node biopsy.

OBJECTIVETo evaluate the technique and significance of mastoscopic axillary sentinel lymph node biopsy.
METHODSSixty-two patients with breast cancer use methylene blue to test axillary sentinel lymph node. Sentinel lymph node was moved with endoscopy, and endoscopic axillary lymph nod dissection was performed. Pathological examination of sentinel lymph node and axillary lymph node was made with HE. To evaluate detection rate and false negative rate in sentinel lymph node.
RESULTSAmong the 62 patients, 61 were confirmed by endoscopic axillary sentinel lymph nod biopsy. Detection rate was 98.4%. Thirty-five cases were no metastasis, 27 cases were metastasis, false negative rate was 0.
CONCLUSIONSMastoscopic axillary sentinel lymph node biopsy has a high detection rate, good efficiency of cosmetic and lower complications. It has higher sensitivity than traditional axillary lymph nod dissection and provide accurate lymph node stages.

7.Study on the relationship between hepatitis C virus infection and sharing injection equipment, sexual behavior among injecting drug users.

OBJECTIVETo study hepatitis C virus (HCV) transmission through different modes of sharing injection equipment and sexual behavior among injecting drug users (IDUs) in Liangshan of Sichuan province.
METHODSA community-based survey was conducted to investigate past and current demographic data, injection equipment sharing patterns and sexual behavior of IDUs. Blood samples were also taken to test for HCV. The survey was conducted between Nov 8 and Nov 29, 2002. 379 subjects were screened through outreach recruitment and peer informing. SPSS (11.5) was used for data analysis.
RESULTSHCV prevalence was 71.0% (269/379). Needles or syringes sharing in the past three months and past syphilis infection were strongly associated with HCV transmission after univariate analysis using chi-square test. Trend analysis indicated that HCV infection rate increased along with the increase of needles or syringes sharing, sharing of rinse water and the number of peers sharing the equipments. Data from multivariate logistic regression showed that sharing of needles or syringes and history of syphilis infection were significantly associated with HCV transmission. No significant difference was found between HCV infection and sexual behavior after univariate analysis using chi-square test.
CONCLUSIONFurther sero-epidemiological prospective cohort studies should be conducted to clarify the relationship between different modes of sharing injection equipment, sexual behavior and HCV infection.

8. Influence of fat and muscle on sit-ups in female college students

Objective To explore the influence of muscle and fat distribution on sit-up ability of female college students. Methods A total of 1560 female students majoring in non-physical education in a Guangxi University were randomly selected. Body composition was measured by the bioresistance antibody component meter. According to the National Physical Health Test Standard, the height, weight and sit-ups of female college students were measured. The students were divided into four groups according to the score of sit-ups:<60 points, 60-69 points, 70-79 points, ≥80 points. SPSS 22.0 statistical software was used to analyze the data. Results There were statistically significant differences in body fat rate, total fat mass, trunk fat mass, trunk fat rate, visceral fat area, visceral fat mass and subcutaneous fat mass in different sit-ups (F value were 3.414, 2.914, 2.881, 3.347, 3.064, 3.235, 2.766, P<0.05). That was, the fat parameters in the 60-69 group were the highest and ≥80 the lowest. However, there was no significant difference between total muscle mass and trunk muscle mass in each score segment. After correcting for age, height, and weight factors, except for visceral fat area and visceral fat mass, the correlation between other fat parameters and sit-up score were negatively correlated, which were statistically significant (r<0, P<0.05). There was a positive correlation between each muscle parameter and sit-up performance (r>0, P<0.05). Compared with other parameters, the correlation coefficient between trunk muscle mass and sit-ups was the largest. Conclusion The distribution of fat and muscle has different effects on sit-up ability. Female college students' sit-up ability is greatly affected by subcutaneous fat and trunk muscle, and has nothing to do with visceral fat.

10.Comparative Analysis of Subtotal Thyroidectomy for Graves′ Disease Leaving a Unilateral Remnant Based on the Upper Pole

【Background】 The aim of this prospective randomized study was to evaluate the feasibility of subtotal thyroidectomy leaving a unilateral remnant based on the upper pole. 【Methods】 Patients who underwent the subtotal thyroidectomy and isthmusectomy leaving either a unilateral remnant based on the upper pole(GroupⅠ ,59 patients)or with the bilateral dorsal thyroid tissue remained(Group Ⅱ,54 patients)were prospectively compared in operation time, blood loss,recurrence,and postoperative complications.【Results】The operation time remained similar between the two groups,but the blood loss,the reoperation time,recurrent laryngeal nerve injury,and recurrence in GroupⅠwere much less than those in Group Ⅱ. In addition ,no postoperative hemorrhage occurred in GroupⅠ. Three patients (5.56%) underwent postoperative hemorrhage in Group Ⅱ. Two patients(3.39%)in GroupⅠand 4 patients(7.40%)in Group Ⅱ experienced transient hypocalcemia.【Conclusion】In terms of blood loss,reoperation time,postoperative complication, and recurrence ,subtotal thyroidectomy with recurrent laryngeal nerves identification and the unilateral superior pole remnant of the gland provides a better outcome than subtotal thyroidectomy with bilateral dorsal thyroid tissue remnant.
