ObjectiveTo investigate the effects of early activities after acute myocardial infarction (AMI) on duration and expenditure of hospitalization and activities of daily living (ADL) of patients.Methods68 AMI inpatients without complications were divided into the early activity group (33 cases) and control group (35 cases). Patients of the early activity group started early activity training by the bed from the 2nd day after admission, and those of the control group from the 10th day. One week before discharge, ADL of all patients were assessed with Barthel index (BI), and duration and expenditure of hospitalization of two groups were compared.ResultsThe average duration and expenditure of hospitalization in the early activity group were obviously lower than those in the control group (P<0.05～0.01), but the total scores of ADL and the scores of most items at discharge were markedly higher than those in the control group (P<0.05～0.01).ConclusionEarly activity training by the bed can obviously shorten the duration of hospitalization, reduce the medical cost and improve the ADL of AMI patients.

Objective This study was to investigate the diagnostic value of CT-guided percutaneous needle lung biopsy ( PN-LB) in different types of lung cancer and compare the pathological results of CT-guided PNLB and surgical specimens. Methods We performed a retrospective analysis of 324 cases of lung cancer, all confirmed by CT-guided PNLB after imaging manifestation of lung-occupying lesion and treated by surgical resection.We compared the pathological results of CT-guided PNLB and surgical speci-mens. Results The total coincidence rate of the pathological result from CT-guided PNLB with that from surgical specimens was 85.8% (278/324), with even a higher coincidence rate (96.4%) in adenocarcinoma, followed by squamous cell carcinoma (86.1%), and small cell lung cancer (45.5%).Large cell lung cancer and adenosquamous carcinoma were liable to be diagnosed as adenocarcinoma by CT-guided PNLB ( 7/10 and 6/10 ) .McNemar-Bowker test revealed no significant differences in the coincidence rate in adenocarcinoma, squamous cell carcinoma, and small cell lung cancer (κ=0.837, P=0.215). Conclusion CT-guided PNLB has a high coincidence rate with surgical specimens in the pathological diagnosis of lung cancer, but the coincidence rate varies with different pathological types of the tumor.

OBJECTIVETo explore guiding role of three-column theory in manipulative reduction, small splint fixation and early rehabilitative exercises of Colles fracture.

METHODSFrom August 2011 to February 2012,47 patients with Colles fractures were treated by manipulative reduction small splinting fixation and early rehabilitative exercises under the guidance of three-column theory, including 21 males and 26 females aged from 40 to 76 years old with an average of (65.5 +/- 2.3). According to AO fracture classification, 27 patients were type A (including 18 cases with type A2 and 9 cases with type A3) and 20 patients were type C (including 10 cases with type C1, 6 cases with type C2 and 4 cases with type C3). Pain and recovery time of swelling, postoperative complications were observed and recorded, Gartland-Werley scoring system were applied for evaluate functional recovery and biomechanical analysis of wrist joint at 12 weeks after operation.

RESULTSAll patients were followed up for 3 to 9 months with average of 5 months. Pain relief time ranged from 5 to 15 d with average of (7.6 +/- 2.2) d,recovery time of swelling of opisthenar was for 6 to 13 d with an average of (8.9 +/- 1.9) d. Two patients occurred tension vesicle within 3 days after operation, but no other complications occurred. According to Gartland-Werley scoring system, 25 cases got excellent results, 18 cases good and 4 cases moderate at 12 weeks after operation.

CONCLUSIONUnder the guidance of three-column theory, treating Colles fracture by manipulative reduction, small splinting fixation and early rehabilitative exercises can reduce pain and swelling time, promote union of fracture, effectively rehabilitate wrist function, improve clinical efficacy, and fit for concept of biomechanics.

AIM: To explore regulation of lipopolysaccharide (LPS)-induced elevation of Ca 2+ intracellular level in alveolar macrophages(AMs) from patients with chronic bronchitis by Angelica Sinensis and nifedipine.METHODS:AMs was obtained from 7 patients with chronic bronchitis and 6 normal controls by bronchoalveolar lavage and intracellular Ca 2+ level was detected after adding Angelica Sinensis, nifedipine or LPS to the supernatant of AMs loaded by Fura-2. RESULTS: In contrast with normal control group (99.65?32.21 nmol/L), intracellular Ca 2+ level in AMs from chronic bronchitis group (189.47?23.69 nmol/L) was increased significantly in the absence of extracellular Ca 2+ but not 1 mmol/L. Intracellular Ca 2+ level in AMs from chronic bronchitis group were significantly increased by adding 10 ?g/mL LPS to the supernatant of AMs. LPS-induced elevation of intracellular Ca 2+ level in AMs from chronic bronchitis group was completely inhibited by Angelica Sinensis or nifedipine.CONCLUSION: Both Anelica Sinensis and nifedipine may inhibit activation of AMs from patients with chronic bronchitis by reducing LPS-induced elevation of intracellular Ca 2+ level in AMs, suggested that these two medicines may inhibit non-specific inflammation of airways in chronic bronchitis.

Objective To explore the characteristics of defense mechanism in patients with obsessive-compulsive disorder(OCD).Methods Outpatients diagnosed as OCD were tested by Yale-Brown obsessive Compulsive Scale. Among them, 40 patients with score beyond or equal to 16 were selected as research group. 33 healthy individuals were selected as normal group. They were tested with Defense Style Questionnaire.Results ①The factor score of immature defense mechanisms in the OCD group is much higher than that in the normal group (P<0.01); the factor score of mature defense mechanisms in the OCD group is lower than that in the normal group (P<0.05); the factor score of middle defense mechanisms in the OCD group is much higher than that in the normal group (P<0.01). ②Passive aggression, acting out, splitting, regression, somatization in the immature defense mechanisms and relief, false altruism, isolation in the middle defense mechanisms are much more used by the OCD group than that in the normal group; but suppression and humor in the mature defense mechanisms are less used by the OCD group than that of the normal group(P<0.05 or P<0.01). ③Sublimation, suppression and consuming tendency are much more used in male patients than female ones in OCD group(P<0.05).Conclusion The patients with OCD tend to use the immature defense mechanisms and middle defense mechanisms, and there is some difference between male and female patients in using defense mechanisms.
