Objective To investigate the association of the tumor necrosis factor ligand superfamily member(TNFSF)4 gene polymorphisms rs3861950 and cerebral infaretion in Hunan area. Methods The frequencies of the genotypes of rs3861950 were detected utilizing real-time fluorescent PCR method based on TaqMan probe.Subiects examined were composed of 287 patients and 285 healthy individuals. Results There were significant differences in TNFSF4 gene rs3861950 C→T site polymorphism and allele frequency between the subjects and the controls, the distribution of CC genotype was significantly higher in the former (7.7%)than in the later(2.1%,X2=9.553,P=0.008),so was the frequencies of C allele(0.190 vs 0.137,X2=5.887,P=0.015).Moreover,there were significant differences in the distribution of genotype and the frequencies of allele between the subgroup of cerebral thrombosis and control group ( for the distribution of genotype X2=9.396 P=0.009,for the frequencies of allele X2=6.134,P=0.013).Logistic regression analysis showed that the risk factor of CC genotype was 3.7 times higherthan others(P=0.002.OR 3.706).Conclusions TNFSF4 gene SNP rs3861950 is associated with cerebral infarction and C allele is considered to be one of independent risk factors in the Han population in Hunan Province.

Objective To observe the effects of X-blance therapy with uninjured side ear acupoint press in hospitalized patients with unilateral radicular sciatica. Methods A total of 73 patients were randomly divided into experimental group (37 cases) and control group (36 cases) according to the sequence of hospitalization. Both groups were given the conventional treatment and nursing care. Besides of this, the experimental group received X-blance therapy with uninjured side ear acupoint Press. Visual analogue scale ( VAS) and Chinese version of oswestry disability index ( CODI ) were compared between the two groups before and after the treatment. Results The VAS, CODI and the 3 coarse scores of every demension of CODI ( pain, single function and comprehensive function) were respectively (2. 53 ± 2. 37), (19. 63 ± 12. 37), (1. 83 ± 1. 69), (3. 78 ± 3. 24), (3. 22 ± 2. 13) in the experimental group after 3 weeks intervention, and (3. 70 ± 2. 59), (27. 93 ± 15. 88), (2. 70 ± 1. 93), (5. 46 ± 3. 63), (4. 41 ± 2. 76) in the control group, with statistically significant difference (t= -2. 009,-2. 39,-2. 042,-2. 084,-2. 046, respectiuely;P<0. 05). Conclusions X-blance therapy with uninjured side ear acupoint press can effectively reduce pain and improve disability of the patients with radicular sciatica.

Objective To investigate the correlations of risk factors of age,delirium-inducing factors, dementia, delirium severity, and disease severity with the prognosis of elderly patients with delirium. Methods A total of 112 patients aged 70 years and older with initial onset of delirium during hospital stay were enrolled in this study. They were hospitalized in Dalian Municipal Central Hospital from Jan.2013 to Dec.2015.The severity of delirium was evaluated by delirium rating scalerevised-98(DRS-R-98).The acute physiology and chronic health enquiry(APACHE-Ⅱ)score and the sequential organ failure assessment(SOFA)score were recorded within 48 h after delirium onset. After periods of hospitalization of Jan.2013 to Dec.2015,patients were followed up until 31 Dec 2017.Correlations of age,delirium-inducing factors and dementia with survival time and long-term viability after delirium onset were statistically analyzed. Results Age,delirium-inducing factors, cognitive impairment, duration of cognitive impairment, type of dementia, and delirium severity had no correlations with the lifetime and long-term viability in patients with delirium. While,APACHE Ⅱ score for assessment of severity degree of delirium(P <0.001,r =-0.390) and SOFA score(P <0.001,r=-0.638)were negatively correlated with the lifetime in patients with delirium.SOFA score (P =0.004) and delirium recurrence (P < 0.001) were significantly correlated with the long-term viability in patients with delirium. Conclusions The severity and recurrence of delirium are strongly correlated with the lifetime and quality of life in patients with delirium.Compared with APACHE Ⅱ score,SOFA score may have a more important clinical application value in predicting the prognosis of patients with delirium.
