OBJECTIVETo evaluate the clinical therapeutic effect of acupuncture treatment of children cerebral palsy with gaining in wisdom and activating channels method.
METHODSSixty cases were randomly divided into a gaining in wisdom and activating channels group, a JIN's 3-needling group and a routine acupuncture group, 20 cases in each group. The gaining in wisdom and activating channels group was treated with point group of the head selected as main points combined with Dazhui (GV 14), Jinsuo (GV 8), Mingmen (GV 4), Yaoyangguan (GV 3), Hegu (LI 4), Neiguan (PC 6), Sanyinjiao (SP 6), etc., the JIN's 3-needling group was treated with Four-shen needling and Dazhui (GV 14), Shenzhu (GV 12), Fengfu (GV 16), etc. were selected as main acupoints in the routine acupuncture group. Comprehensive Function Evaluation for Disabled Children was used for assessment of comprehensive function of disabled children.
RESULTSThe total effective rate of 90.0% in both the gaining in wisdom and activating channels group and the JIN's 3-needling group, with no significant difference between the two groups (P > 0.05), were better than that of 60.0% in the routine acupuncture group (P < 0.05).
CONCLUSIONAcupuncture treatment with gaining in wisdom and activating channels method can significantly improve the comprehensive function of disabled children with more obvious therapeutic effects.

Objective To study the incidence of moderate and severe preoperative anxiety and its related risk factors in patients undergoing elective general anesthesia. Methods A total of 562 pa-tients undergoing selective anesthesia 225 males and 337 females, aged 18-85 years,ASA physical status Ⅰ-Ⅲ,were selected in the affiliated hospital of Xuzhou medical university.State-trait anxiety inventory (STAI)was used to assess the degree of preoperative anxiety and the patients were divided into mild anxiety group (SAI ≤ 37,n=294)and moderate and severe anxiety group (SAI > 37,n=268)according to SAI score. The factors related to preoperative anxiety such as age,gender,edu-cational background, marital status, history of previous surgery, presence of pain, preoperative diag-nosis of tumor and presence or absence of hypertension were collected. Results The incidence of moderate and severe preoperative anxiety in our study was 268 (47.7%).Logistic regression analysis showed that female (OR=1.846,95%C I 1.298-2.624),single (OR=2.208,95%C I 1.218-4.004) might be the risk factors of preoperative anxiety. Conclusion Females and single individuals may be independent risk factors for preoperative anxiety.

Objective To investigate the MRI manifestations of secondary toxoplasmosis encephalitis in HIV patients and summarize the characteristics of MRI changes. Methods MRI images of 3 5 HIV patients with secondary toxoplasmosis encephalitis were analyzed retrospectively. Results Of 35 patients,27 showed multiple lesions and 8 showed single lesion.Lesions mainly involved the brain gray matter j unction or basal ganglia.MRI showed lamellar, nodular or round lesions, which showed low signal on T1WI,and slightly higher and unevenly high signal on T2WI.Irregular liquid signal or nodular slightly lower signal was showed on T2WI in 14 cases, and short T1signal was showed in 15 cases.Circular lesions could be seen"cattle eye"in 18 cases. For 11 cases of enhanced scan of a single lesion,a ring enhancement was displayed, and coexistence of patchy, nodular and annular enhancement were showed in multiple lesions,in which parts of the multi-circular enhancemens were clustered. Conclusion MRI manifestations of HIV patients with secondary toxoplasmosis encephalitis show primarily multiple lesions on the gray matter j unction or basal ganglia. The lesions show high signal on T1WI and"cattle eye"on T2WI,and existence of flaky, nodular and ring enhanced lesion or clustered enhancement are showed on enhanced imaging, which is of a certain specificity.

OBJECTIVETo introduce a new prefabricated flap with matched colour, texture, thin enough thickness, large enough dimension and reliability for reconstruction of massive defect of face and neck.
METHODSThe patients with massive scar of face and neck were selected for treatment with prefabricated flap. Flap prefabrication involved two stages. The "sandwich" structure including the descending branch of the lateral femoral circumflex vessels and surrounding muscle fascia was harvested from the thigh and anastomosed to superior thyroid artery or facial vessels. Flap prefabrication was performed by inserting the fascia flap between the cervicothoracic skin and the tissue expander placed beneath the skin. After a period of expansion, the flap was transferred to the recipient site based on the implanted vessels. The results including complications were examined during follow-up.
RESULTSNine patients received this treatment. The average dimensions of fascia flap harvested was 6.3 cm x 11.2 cm. After mean interval of 16.7 weeks, the expanders were filled to a mean volume of 1670cc. The size of prefabricated flap ranged from 12 cm x 15 cm to 15 cm x 32 cm. In all cases, the flap efficiently covered the entire defect of the face and neck, and the donor site of the flap is closed primarily. All of the flaps developed venous congestion in some degree after the second operation. Partial flap necrosis occurred in two cases. Three flap was thinned to contour the bulky pedicle. During follow-up, the transferred flap was matched well to the adjacent skin. The reconstructed face restored nature contour and expression. Muscle weakness or paraesthesia was not found in the donor thigh.
CONCLUSIONSCervicothoracic Prefabricated Flap, is reliable and versatile in the reconstruction of massive soft tissue deficits with restoration nature surface and expression of the face and neck.

OBJECTIVETo explore the feasibility, effect, and clinic value of total laparoscopic gastric resection in patients with benign gastric disease.
METHODSThe clinical materials of 50 cases underwent total laparoscopic gastric resection (LG group) and 104 cases open surgery (OG group) between January 2002 and June 2006 were compared. The operation time, intraoperative blood loss, mean time of stay in hospital and postoperative complications were studied.
RESULTSAll operations in LG group were successfully preformed with laparoscopic technique, and the mean operation time was 105 min, mean intraoperative blood loss was 50 ml, and mean hospital stay was 7 days. Incisional wound infection occurred in 2 cases and no serious complications occurred in this group. In OG group, the mean operation time was 118 min, mean intraoperative blood loss was 108 ml, and mean hospital stay was 12 days. Wound infection occurred in 7 cases, disorder of gastric emptying was found in 3 cases, fistula of anastomotic stoma happened in 1 case and bowel obstruction occurred in 1 case. There was significant difference in operation blood loss and hospital stay between the two groups (P < 0.05).
CONCLUSIONSLaparoscopic gastric resection is a safe and feasible minimally-invasive surgery, it brings less pain, less bleeding, shorter hospital stay.
