1.Evaluation of the relationship between donor graft position and recipient position after endothelium keratoplasty by AS-OCT

Background The advantages of endothelium keratoplasty(EK)include without corneal surface incision and sutures,remaining normal corneal topography,minimizing astigmatism and providing a healthy donor endothelial cell count and function.Using anterior segment optical coherence tomography(AS-OCT) to examine the morphology of the corneal graft after EK is helpful for the evaluation of the outcome of EK.ObjectiveThis study applies AS-OCT to observe the relationship of recipient cornea and donor graft and offer a basis for the evaluation of operative outcome.MethodsThe clinical data from 44 eyes of 42 patient who received DSAEK in Peking University Eye Center from September 2007 through April 2009 was reviewed retrospectively.These patients included 30 male and 12 female with the age from 15 to 83 years.Structure of cornea and relationship between recipient cornea and graft disc after surgery were clinically examined and recorded by slit-lamp digital camera and AS-OCT.This study was approved by the Ethics Committee of Peking University.Written informed consent was obtained from all of the patients before the operation.ResultsThe donor graft attached to posterior central surface of recipient in 28 eyes under the slit-lamp microscopy and AS-OCT.The thickness of donor graft was decreased,and graft sticked more closely to recipient cornea and was more transparent with the lapse of time.Complete dislocation of the donor graft in early stage was found in 5 eyes(11.36%) and partial dislocation in 2 eyes(4.55%).Curliness of graft edge occurred in 1 eye(2.27%) and eccentricity of donor graft in 8 eyes(18.18%).ConclusionAS-OCT can display the morphology and structure of cornea as well as the relationship between donor graft and recipient after DSAEK.It is helpful for evaluating the clinical effectiveness of DSAEK.
2.Clinical analysis on 1050 cases with tracheobronchial foreign body.

OBJECTIVE:
To investigate the clinical pathological features of tracheobronchial foreign bodies and to improve the diagnosis and treatment.
METHOD:
A retrospective study was conducted on 1050 cases with suspected tracheobronchial foreign bodies. Each patient was analyzed for age, sex, history, complication and location of the foreign body. Nine hundred and forty-nine out of 1050 cases were diagnosed as tracheobronchial foreign body, in which 13 patients coughed out the foreign bodies before operation and the rest 936 cases were given operation by bronchoscope under general anesthesia.
RESULT:
Among the 949 cases, 936 cases were given operation, with successful removal of foreign bodies in 932 cases (99.6%). One patient suffered from complication (0.1%).
CONCLUSION
For the diagnosis of tracheobronchial foreign bodies, it is very important to collect the detailed history of foreign body inhalation, physical examination and chest roentgenoscopy. Spiral CT with 3-D reconstruction of trachea and bronchus is performed in patients with suspected foreign bodies. Complete surface anesthesia of respiratory tract mucosa is the key procedure in the removal of foreign bodies from respiratory tract by bronchoscope.

3.Suspending of M. temporal, temporal fascia and parietal periosteum to correct late facial palsy.

OBJECTIVETo explore a simply, effective dynamical method to correct late facial palsy.
METHODSThe method of suspending of M. temporalis, temporal fascia was reformed below: (1) To prolong flap of M. temporalis, temporal fascia by parietal periosteum. (2) To elevate the reversal level of compound flap. (3) To fill depressed temporal area by silica gel piece.
RESULTSThe compound flap is united structurally and long enough to transfer. Temporal defect is recontoured. And zygomatic area is no longer protruded.
CONCLUSIONSThe reformative method resists defect of the old one and obtains a dynamical result.

4.Prevalence of allergic rhinitis and survey analysis of health service needs and utilization in Qingdao area.

OBJECTIVETo investigate the need and utilization for health services in patients with allergic rhinitis in Qingdao area.
METHODSAn investigation was conducted by the method of combining multi-stage sampling and cluster sampling. SPSS 13.0 software was used to analyse the data.
RESULTSAmong Two thousand and four hundred questionnaires, two thousand and fifty-two questionnaires were available. The effective rate was 85.5%. The total two-week prevalence in male was 6.5%, while that in female was 6.2%. There was no significant difference between male and female (χ(2) = 0.103, P > 0.05). The total ratio of those who visited doctors was 1.7%. The ratio of those who visited doctors in the first two weeks was 26.7% (35/131). 49.0% of non-users of health service thought it was unnecessary to consult a doctor because of mild symptoms. In the investigation about the allergic rhinitis, 60.2% patients (1235/2052) confessed that they heard of allergic rhinitis, 30.0% patients (616/2052) thought of allergic rhinitis was common cold.
CONCLUSIONSThe number of patients treated for allergic rhinitis was less than its prevalence. The primary reason for low attendance was that the majority of patients with mild symptoms thought it was no need to see a doctor. In order to improve the residents' health services utilization, medical education for allergic rhinitis is necessary.

5.Investigation and analysis of resource allocation for diagnostic radiology/radiotherapy and radiation protection in Jiaxing city

Objective To analyze the diagnostic radiology equipment number,radiological staff structure,protection allocated at the medical institutes in Jiaxing city,with the purpose to provide basic data for the further optimization of resource allocation.Methods As required by the national radiation health monitoring program and using the stratified random sampling method,a total of 111 hospitals at grades Ⅲ,Ⅱ,Ⅰand other types were used to select 50%radiological institutions in Jiaxing.Unified questionnaires were used to investigate the basic situation in these hospitals,such as diagnostic radiology equipment,configurable protection equipment,radiation worker number and diagnostic frequency.Results The hospital number at grades Ⅲ,Ⅱ,Ⅰand other types accounted separately for 13.5%,21.6%,46.9%and 18.0%of the total.The equipment numbers for radiotherapy,nuclear medicine,interventional radiology and X-ray imaging at grade Ⅲ hospitals accounted for 100%,100%,57.9%and 38.0%,respectively,significantly higher than others.The numbers of conventional photofluorography and fluoroscopy equipment at grades Ⅰhospitals accounted for 34.2%and 50%,slightly higher than others ;CT and mammography devices were distributed at the grades ⅡandⅢhospitals.The frequencies at X-ray diagnosis,CT diagnosis and interventional therapy and diagnosis,radiotherapy,and nuclear medicine diagnosis and therapy were 178.9,157.5,3.1,1.5,2.2 and 0.1 per thousand person-year in Nanhu district,which were higher than in the other areas of Jiaxing city.Conclusions Grade Ⅲhospitals have more diagnostic radiology devices and radiation workers than in non-grade-Ⅲ hospitals.These hospitals were all equipped with adequate amount of protection equipment.The frequency of diagnostic radiology in Nanhu district hospital was significantly higher than that in other district hospitals.
6.Transformation of strictosamide to vincoside lactam by acid catalysis.

AIM:
To identify the structure of the acid-catalyzed product of strictosamide and explore the reaction mechanism.
METHODS:
The acid-catalyzed reaction process of strictosamide was monitored by HPLC, and a macroporous resin was used to purify the reaction solution. The structure of the product was confirmed by MS, NMR, and ROESY spectra.
RESULTS:
The acid-catalyzed transformation yield from strictosamide to vincoside lactam was 52%.
CONCLUSION
The reaction mechanism of the transformation from strictosamide to vincoside lactam may be related to the stability of the three-dimensional configuration of the compound. These results offer a new way to obtain vincoside lactam from the widely distributed indole alkaloid strictosamide by acid-catalysis.

7.Monitoring and analysis of total α and total β levels in drinking water around Qinshan Nuclear Power Plsnt in 2018

Objective To ascertain the background levels of total α and total β radioactivity in drinking water around Qianshan Nuclear Power Plant (QNPP). Methods Under the National Health and Wellness Committee′s program, 11 monitoring points within 30 km of QNPP were selected in Haiyan County in 2018. The samples of raw water, factory water and tap water were collected in the first half and the second half of the year, respectively, and determined and analyzed according to Standard Examination Methods for Drinking Water - Radiological Parameters (GB/T 5750.13-2006). The total α and total βlevels in the tested drinking water were evaluated under the Standards for Drinking Water Quality ( GB 5749-2006) . Results A total of 22 water samples were collected from 11 sampling points in April and September 2018, respectively. The residue rate obtained after drinking water was dried varied, dependent on different types of water, which could be up to 0.0337%. The total α levels in different types of water ranged from 0.008 to 0.04 Bq/L, averaged on (0.015±0.009) Bq/L, whereas the total β levels ranged from 0.014 to 0. 320 Bq/L with an average of ( 0. 188 ± 0. 068) Bq/L. There was statistically significant difference in totalαlevels between raw water and factory water ( Z=-2.286, P<0.05) , without statistically significant difference between residue rate and totalαand totalβlevels ( P>0.05) . The background levels of total α and total β radioactivity in 93 drinking water samples in 2016 and 2017 were also investigated, combined with a total of 115 water samples from different water bodies in 2018. There was statistically significant difference in total α and total β levels in tap water between 2017—2016 and 2018 ( Z=-2.976, -2.031, P<0.05), between 2016 and 2017 (Z=-2.042, -3.214, P<0.05) and between 2017 and 2018 (Z=-20. 112, -2.511, P<0.05). Conclusions The operation of nuclear power plant has no impact on radioactivity level in drinking water around QNPP. The drinking water around QNPP is safe.
8.Significance of Targeted Sequencing Assay for Patients with Suspected Myeloid Malignancies.

OBJECTIVE:
To investigate the clinical significance of the targeted next-generation sequencing assay for patients with suspected myeloid malignancies.
METHODS:
A total of 39 hematopenia patients with suspected myeloid malignamies in Department of Hematology of The Affiliated Huai'an No.1 People's Hospital of Nanjing Medical University from January 2018 to April 2019 were treated, 20 hot spot genes of myelodysplastic syndrome (MDS) were detected.
RESULTS:
Regarding the diagnostic type, there were 7 cases of idiopathic cytopenia of undetermined significance (ICUS), 8 cases of clonal cytopenias of undetermined significance (CCUS) and 24 cases of myeloid myeloid malignancies which included 18 cases of MDS, 4 cases of myelodysplastic/myeloproliferative neoplasms (MDS/MPN) and 2 cases of acute myeloid leukemia. Positive mutation was detected in 70.8% (17/24) of myeloid malignancy patients , and 72.7% (16/22) in MDS and MDS/MPN patients. The main mutation types were ASXL1, TET2 and RUNX1. Compared with gene negative group, there were no significant differences in sex, age (<60 years old or ≥60 years old), proportion of bone marrow blast cells (<5% or≥5%) and cytogenetics (good, medium and poor) (P>0.05). Furthermore, all 8 CCUS patients showed positive mutation, and the incidence of double or multiple mutation in CCUS group was significantly lower than that of the MDS and MDS/MPN group (37.5% vs 54.5%) (P=0.002). The mutation types between the two groups were similar, and there was no significant difference in variant allele frequency (P>0.05).
CONCLUSION
Our results suggest that there are high rates of double or multiple mutations in myeloid malignancies, especially in patients with MDS and MDS/MPN. Targeted sequencing assay can improve the diagnosis of myeloid malignancies, and guide clinical treatment.
