2.Clinical observation on Xihuang Pill combined with chemotherapy in treating 18 cases of advanced esophageal cancer

Objective: To observe the clinical effects on advanced esophageal cancer by combined Xihuang Pill with chemotherapy. Methods: Dividing 35 advanced esophageal cancer cases randomly into two groups: 18 cases in the treatment group and 17 cases in the control group. Cases in the control group were treated by nedaplatin 40mg/m2,d1,2; 5-Fu400mg/m2,d1-5; CF200 mg/m2,d1-5. While the treatment with Xihuang Pill combined. The difference of quality of life, hematological toxicity, efficacy and symptoms between the two groups was observed. Results: The quality of life, remission rate of some symptom in the treatment group were much better than the control group, and the hematological toxicity and efficacy were the same. Conclusion: Xihuang Pill could improve the quality of life of advanced esophageal cancer patient, and alleviate some symptoms.
3.Impact of the timing of tratheostomy on patients treated with prolonged mechanical ventilation-A metaanalysis of randomized and quasi-randomized controlled trials

Objective To compare the early and delayed tracheostomy in patients treated with prolonged mechanical ventilation in respects of mortality.incidence of nosocomial pneunonia and length of ICU stay.Methods Randomized controlled trials(RCTs)and quasi-randomized controlled trials(quasi-RCTs)were performed by searching throush the Ovide MEDLINE(1996-2006.7),EMBASE(1980-2006.7),Cochrane Database(Issue 2,2006),Chinese Cochrane Centre Database and CBMdisc(1978-2006.7).The published and unpublished data and their references were searched.All RCTs and quasi-RCT of tracheostomy for critically ill patients treated withprolonged mechanical ventilation were included.Data were collected and evaluated by two reviewers independently and separately.RevMan version 4.2 software was used for data analysis.Results Two hundred and eighty-six patients from 4 RCTs and 106 cases from one quasi-RCT were taken for meta analysis.The meta-analysis showed that the early tracheostomy could reduce mortality(OR 0.69,95%CI 0.51,0.95),significantly shorten the duration of mechanical ventilation(WMD-8.49,95%CI-15.32,-1.66)and shorten the length of ICU stay as well(WMD-15.33,95%CI-24.58,-6.08)in patients treated with prolonged mechanical ventilation.but the incidence of nosocemial pneumonia was not different between two sorts of patients(OR 0.91,95%CI 0.70,1.18).Conclusions In case of prolonged mechanical ventilation,the tracheostomy performed at earlier stage may decrease the mortality,and shorten the duration of mechanical ventilation and the length of ICU stay.but cannot reduce the incidence of nosocomial pneumonia.
4.Renal mucinous tubular and spindle cell carcinoma:one case report and literature review

Objective To improve the diagnosis and treatment of renal mucinous spindle cell carcinoma,and the knowledge of the type of kidney cancer.Methods The clinical date of 1 case of renal MTSCCa was reported.The case of patient was a 38-year-old women with abdominal discomfort and hematuresis.Ultrasound tip:the upper pole of the right kidney mixed echo placeholder,tumor and the surrounding is obvious blood flow signal.CT revealed a low density shadow in the upper pole of the right kidney,involving the right renal vein,partial filling defect,visible in the inferior vena cava are visible low density shadow,uneven enhancement after the reinforcement,and has delayed reinforcement.MRI shows:the upper pole of the right kidney huge soft tissue mass,T1WI shows low signal,T2WI show slightly higher signal,uneven enhancement and have delayed reinforcement.Preoperative diagnosis:right renal clear cell carcinoma with right renal vein and inferior vena cava tumor emboli.With kidney cancer radical resection and take out inferior vena cava tumor emboli,in the operation right kidney tumor,about 10 cm × 20 cm,right renal vein and inferior vena cava tumor emboli in formation,inferior vena cava tumor emboli within about 8 cm,the clinical and pathologic feature was analyzed and discussed with litcrature review.Results The operation smoothly.Pathological findings suggested higher grade mucinous tubular and spindle cell carcinoma;microscopically a large number of ingredients mixed bundle arrangement of spindle cells,nuclear fission as easy to see.Immunohistochemical staining:Vimentin ±,Syn +,NSE +,CD99 +,Ki-67 + 70%.According to the 2010 AJCC for kidney cancer TNM staging,the pathological pT3c N0M0.According to the Robson stage]].The patient reserved interferonα-2a associate with interleukin-2 theraph after a month.During treatment in patients with fever and other flu symptoms,weekly review of routine blood,liver and kidney function has no obvious abnormalities.The patient died of pulmonary embolism,liver,lung multiple metastases two months later.Conclusions Renal mucinous tubular and spindle cell carcinoma is a rare cell types in the kidney pathological classification,women in high incidence,higher pathological types,worse prognosis.
5.Clinical Study on Acupuncture Treating Thalamic Pain

Objective: To evaluate the clinical effect of acupuncture in managing thalamic pain syndrome. Methods: Eighty patients with thalamic pain syndrome were randomly allocated into two groups, with 40 cases in the treatment group receiving combined scalp and body acupuncture treatment and 40 cases in the control group receiving single body acupuncture treatment. The intervention was given for 28 d in total, and the effects were evaluated after 14-day and 28-day treatments. Results: The inner-group comparison of McGill Pain Questionnaire (MPQ) showed significant differences after the treatment d 14 in both of the two groups (P<0.05), and after the treatment d 28, the differences were even more significant (P<0.01). After 28-day treatment, compared with pre-treatment, the Transcranial Doppler (TCD) test detected significant changes in the two groups (P<0.05). The total effective rate was 72.5% in the treatment group versus 47.5% in the control group, and the difference was statistically significant (P<0.05). The 3-month follow-up study found the relapse rate was 0 in the treatment group and 15.8% in the control group, and the difference was significant (P<0.05). Conclusion: Acupuncture is effective in treating thalamic pain syndrome and combined scalp and body acupuncture can achieve even better results.
6.Action mechanisms of active vitamin D3 on protecting the liver of type 2 diabetes mellitus rats

BACKGROUND:Active vitamin D3 plays an important role in the development of type 2 diabetes and its complications. OBJECTIVE: To explore the influence of active vitamin D3 on the liver of type 2 diabetes melitus rats, and its mechanisms. METHODS:A total of 35 male Sprague-Dawley rats were randomly divided into normal control group, diabetes group and vitamin D3 group. In the diabetes group and vitamin D3 group, rats received high fat and high sugar diet and intraperitoneal injection of streptozotocin to prepare rat models of diabetes melitus. In the vitamin D3 group, rats were intragastricaly given calcitriol dissolved in peanut oil 0.03 μg/kg per day. In the normal control group and diabetes group, rats were intragastricaly given peanut oil. 8 weeks later, rats were sacrificed and serum was isolated. Fasting blood glucose, fasting insulin, total cholesterol and triacylglycerol levels were measured. Insulin resistance index in the steady-state model was calculated. The liver was retained, and subjected to hematoxylin-eosin staining, immunohistochemical staining, and real-time fluorescent quantitative PCR. RESULTS AND CONCLUSION: (1) Compared with the diabetes group, triacylglycerol and insulin resistance index were lower in the vitamin D3 group (P < 0.05). (2) Compared with the normal control group, sweling of the liver cels and fatty degeneration with inflammatory cel infiltration were found. Protein expression of JNK and C-Jun and phosphorylation levels, mRNA expression of JNK and C-Jun, tumor necrosis factor α and interleukin-1β increased in the diabetes group (P < 0.05). Liver cel sweling and fatty degeneration lessened, inflammatory cel infiltration reduced in the vitamin D3 group. Simultaneously, the expression of above factors was lower in the vitamin D3 group than in the diabetes group (P < 0.05). (3) Results suggested that the protective effect of vitamin D3 on the liver of rats with type 2 diabetes was possibly associated with its effect on downregulating JNK/C-Jun signaling pathway.
7.In vitro hemodynamic evaluation of the cryopreserved stented homograft bioprosthetic heart valve

Objective: To evaluate hemodynamic function of the cryopreserved stented homograft bioprosthetic valve , compared to Perfect bovine pericardial bioprosthetic heart valve. Methods: Three sorts of stented homograft aortic and pulmonary valve (21 #, 23 #, 25 #) preserved by liquid nitrogen were prepared. In vitro pulsatile flow tests were performed on the valve in accordance with ISO/FDA guideline. Effective orifice area (EOA), transvalve pressure gradient and regurgitation ratio were recorded at various flow volume, compared with Perfect bioprosthetic valve. Results: There was no difference between the transvalve pressure gradient of the stented homograft aortic and pulmonary valve, but they were higher than Perfect bioprosthetic valve. The stented homograft aortic and pulmonary valve showed the larger regurgitation ratio than perfect valve, but there was no difference between two types of valve. EOA of the stented homograft aortic and pulmonary valve were smaller than Perfect valve's, there was no discrepancy between them. Though actual orifice area (AOA) of the stented homograft valve was smaller than Perfect's, but EOA/AOA ratio was no difference between two types of valve.Conclusion: The stented homograft valve has satisfactory hemodynamic performance. There is no significant discrepancy between homograft aortic and pulmonary valve's hemodynamic function.

9.Construction of the stented homograft valve preserved by liquid nitrogen

0.05). The mechanical strength of dacron and thread were not significantly changed. Rb/Rc was approximately 1.2, H/Rc was approximately 1.4 and ?was approximately 10. These parameters accord with standard of prosthetic valve design. Conclusion The stented homograft valve can be cyropreserved by liquid nitrogen and accord with standard of prosthetic valve design.
10.Effect of Chinese Herbal Compound Formula on Plasma Endothelin and Serum Nitric Oxide of Diabetic Rats

Objective To investigate the effects of Chinese herbal compound formula on plasma endothelin and serum nitric oxide of diabetic rats. Methods The selected SD male rats were induced by streptozotoc (53 mg/kg i.p) and the model rats were divided at random into model contrasted group (MCG), Chinese herbal compound formula treated group (CTG) and ramipri treated group (RTG) while the normal contrasted group (NCG) was set up.CTG were supplied with decoctions of Chinese herbal compound formula, RTG were supplied with solution of ramipri, MCG and NCG were supplied with water.
