Cytomegalovims(CMV) infection is widely spread in our country.The initial CMV infection often occurs in childhood,can cause a variety of respiratory diseases in children,seriously threaten the health of children.This article describes the relationship between CMV infection and respiratory disease in children with pneumonia,wheezing,bronchial and pulmonary dysplasia,and reviews the progress in the diagnosis and treatment of cytomegalovirus infection,in order to improve the understanding of human CMV infection in respiratory diseases of children.
2.Negative feedback effection of DNA methylation on expression of DNMT1 in patients with systemic lupus erythematosus

Objective:To analyse the whole DNA methylation status , expression of DNMT1 and DNA methylation status of DNMT1 promoter from patients with systemic lupus erythematosus ( SLE) .To assess the negative feedback effection of DNA methylation on the expression of DNMT1 in the patients with SLE.Methods:The whole DNA methylation status in T cells from 34 SLE patients and 23 healthy controls was assessed by the specific monoclonal antibodies to 5-methylcytosine (5-mc) and the DNA methylation status of DNMT1 promoter was assessed by the Bisulfite Conversion Kit and sequencing.Real time RT-PCR was applied to analyse DNMT 1 mRNA levels in T cells from the patients and controls.Results: SLE patients had siginificantly whole DNA hypomethylation than controls(P<0.05),and the whole DNA methylation was negative correlated with the SLE-DAI.There was no difference in levels of DNA methylation of DNMT1 promoter between patients and controls ,also no correlation between the levels of DNA methylation of DNMT 1 promoter and DNMT1 mRNA.Conclusion:There was no negative feedback effection of DNA methylation on the expression of DNMT 1 in the patients with SLE.The mechanism of lower expression of DNMT 1 in patients with SLE should be studied further.
3.Vitrectomy for Giant Traumatic Avulsion of Vitreous Base

Objective To describe the clinical features and observe the effect of vitrectomy on traumatic avulsion of vitreous base with giant retinal dialysis.Methods Seven cases with giant retinal dialysis(rang 120~150?) secondary to traumatic avulsion of vitreous base and PVR grade B to C 3 were retrospectively reviewed.All eyes had recieved vitrectomy,scleral buckle and lenses removed,and six eyeballs underwent postoperative sequential fluid-gas exchange.Results In most cases,the dialyses and avulsed vitreous base developed in the temponate quadrants and complicated lenses dislocation,pupilloparalysis,and vitreous hemorrhage.All cases attained success after disappearance of the intraocular tamponate.The major complication was elevated intraocular pressure,which was transient.Conclusions Giant retinal dialysis may result from traumatic avulsion of vitreous base.These dialysis often become clinical manifestation for several days or longer following ocular contusion.Vitrectomy is a very effective management for these cases.
4.Clinical efficacy of peppermint oil capsules combined with Domperidone in the treatment of bile reflux gastri-tis after gastric surgery

Objective To observe the clinical efficacy of peppermint oil capsules combined with domperi-done in the treatment of bile reflux gastritis(BRG)after gastric surgery.Methods Totally 47 patients with BRG after gastric surgery were treated with peppermint oil capsule (0.9g,tid)and domperidone(10mg,tid).Each patient's gas-troscopic manifestations and histopathology were rechecked after 4 weeks.The efficacy was observed on the improve-ment of symptoms and gastric mucosal injury,and on the reduction of bile reflux.Results 42 patients attained satis-factory therapeutic effect with the total efficiency rate of 89.4%.No adverse effects during the treatment period. Conclusion Peppermint oil capsules combined with domperidone shows better efficacy in treatment of BRG after gastric surgery.
5.Histological subtypes classification of non-small cell lung cancers using 18F-FDG PET-based radiomics

Objective:To distinguish lung adenocarcinoma (ADC) from squamous cell carcinoma (SCC) using 18F-fluorodeoxyglucose (FDG) PET-based radiomic features. Methods:A retrospective analysis was performed in 182 patients (109 males, 73 females, age (59.0±8.3) years) with non-small cell lung cancer (NSCLC) who underwent 18F-FDG PET/CT scan between January 2018 and December 2019 in Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology. All patients had been diagnosed pathologically with lung ADC or SCC. The patients were divided into a training set ( n=91) and a validation set ( n=91) using simple random sampling method. Radiomic features were extracted from the PET images of segmented tumors using the Python package. The minimum redundancy maximum relevance feature selection algorithm and least absolute shrinkage and selection operator were employed to select informative and non-redundant features, and a radiomics signature score (rad-score) was developed. Differences of rad-score between groups were compared by Mann-Whitney U test. Multivariate logistic regression was applied to select the important factors. A combined model was constructed based on the clinical variable and radiomics signature. The predictive performance of models was analyzed and compared using receiver operating characteristic (ROC) curves and Delong test. Results:Four radiomic features, namely HHL_first order_maximum, LHL_first order_entropy, HHH_ gray level dependence matrix_large dependence high gray level emphasis (GLDM_LDHGLE), HHL_GLDM_LDHGLE (H/L represent the high/low pass filter) were selected to build the rad-score. The rad-score showed a significant ability to discriminate between different histological subtypes in the two sets(training set: -1.30(-1.70, -1.04) vs -0.60 (-1.11, 0.20), z=-4.61, P<0.001); validation set: -1.31(-1.66, -0.96) vs -0.73(-1.02, -0.24), z=-4.76, P<0.001). The area under the curve (AUC) of the rad-score were equal to 0.815 (95% CI: 0.723-0.906) in the training set, and 0.813 (95% CI: 0.726-0.901) in the validation set, respectively, which were larger than those of the clinical variables (smoking had the best prediction performance, training set: 0.721 (95% CI: 0.617-0.810), validation set: 0.726 (95% CI: 0.623-0.814)), however, the difference was not significant ( z values: 1.319, 1.324, both P>0.05). When the clinical variable (smoking) and radiomics signature were combined, the complex model showed a better performance in the classification of histological subtypes, with the AUC increased to 0.862 (95% CI: 0.785-0.940; sensitivity: 88.00%(22/25), specificity: 72.73%(48/66)) in the training set and 0.854 (95% CI: 0.776-0.933; sensitivity: 75.00%(21/28), specificity: 84.13%(53/63)) in the validation set. The AUC values were significantly higher than those of the clinical variable (smoking; training set: z=3.257, P<0.001; validation set: z=3.872, P<0.001). Conclusion:Individualized diagnosis model incorporating with smoking and radiomics signature can help differentiate lung cancer subtypes in a non-invasive, repeatable modality.

Objective: To evaluate the role of enteral nutrition with dietary fiber and medium chain triglycerides in postoperative patients with gastric cancer. Methods: Forty patientswith gastric cancer receivedenteral diet with dietary fiber and medium chain triglycerides for 7 days. Enteral feeding was initiated 48hours after surgery. Host biochemistryindicators, nutritional parameters and immunity were monitored. Results: With surgical trauma, patients'nutritional status and immunity were impaired, and after nutritional support, the nutritional and immune parameters increased significantly. Conclusions: In postoperative patientswith gastric cancer, enteral nutrition with dietary fiber and medium chain triglycerides can improve the nutritional status and immunity.

Objective To investigate whether gastric hypersensitivity exists in functional dyspepsia patients, and to observe the number of mast cells in proximal gastric mucosa in gastric hypersensitivity patients and the change of mast cell degranulation after stimulation so as to evaluate the role of mast cells in gastric hypersensitivity. Methods Gastric sensation threshold was determined using barostat in 40 functional dyspepsia (FD) patients and 15 controls. Twenty three patients were divided into two groups on the basis of perception threshold: hypersensitivity group (FD H) and normal sensation group (FD N). Mast cells were stained by immunohistochemistry method, mast cell number was calculated by microscopy and the average optical density valves( A ) of mast cell were obtained by microspectrophotometry (MSP). The ultrastructure of mast cell was observed under electromicroscopy. Objective To screen single nucleotide polymorphism(SNP)of DNA methyltransferase 1 gene in patients with systemic lupus erythematosus(SLE).and to investigate the significance of abnormal expression of DNA methyltransferase in the pathogenesis of SLE.Methods Peripheral blood mononuclear cells(PBMCs)were obtained from 11 patients with SLE and 12 normal human controls.DNA was extracted from the PBMCs.and PCR was performed to amplify some exons(1 to 7)and corresponding introns followed by direct sequencing.Results There was a 21397T/C SNP in the intron 7 between exon 7 and exon 8 of DNA methyltransferase 1 gene in patients with SLE.The frequency of CC homozygote increased in patients with SLE compared with that in normal controls(5/11 vs 1/12,X~2=4.10,P<0.05).Conclusions There is a 21397T/C SNP in DNA methyltransferase 1 gene in patients with SLE.which may be a component of genetic background underlying the abnormal expression Of DNA methyltransferase 1.
