3.Analysis of the current situation of clinical nursing ward rounds
Chinese Journal of Hospital Administration 1996;0(02):-
The paper first sums up the types of clinical nursing ward rounds and points out that at present two types play a dominant role in China, viz. the problem-based nursing ward round and the nursing procedure-oriented nursing ward round. Then it expounds the goal of clinical nursing ward rounds, introduces the forms, participants, and schedules of nursing ward rounds, and analyzes the major problems that currently exist in this aspect. The paper argues that it is imperative for the hospital to reinforce the management of nursing ward rounds and ensure that they are well planned and prepared and highly standardized and focused so as to improve their quality.

Objective To investigate the role and prognostic impact of E-cadherin( E-cad)on in-vasion and metastasis of gastric carcinoma by analyzing E-cadherin expression at DNA,RNA and protein levels. Methods RNA and protein expression of E-cad were tested in 60 cases of gastric carcinoma and adjacent tissues by streptavidin peroxidase( SP )immunohistochemistry and in situ hybridization. DNA methylation of E-cad was tested with methylation-specific polymerase chain reaction( MSP). Results The positive rates of E-cadherin protein and mRNA in gastric carcinoma were significantly lower than those in adjacent tissues. DNA methylation of E-cad negatively correlated with the E-cad protein and RNA expres-sion in gastric carcinoma respectively(r= -0. 355,r = -0. 271). DNA methylation of E-cad negatively correlated with the 5-year survival rate of gastric carcinoma(r = -0. 381,P ﹤0. 01). However,E-cad protein positively correlated with the 5-year survival rate of gastric carcinoma(r=0. 378,P﹤0. 01). Con-clusion The decreased expression of E-cad is one of the most important biological features in gastric car-cinoma. Methylation of E-cad DNA may be an important agent causing low expression of E-cad protein and mRNA and it can be used as an important indicator in predicting invasion and metastasis of gastric cancer.

OBJECTIVETo observe the clinical efficacy on constipation predominant irritable bowel syndrome (IBS-C) treated with acupuncture for regulating the mind and strengthening the spleen and the impacts on the quality of life in the patients.
METHODSSixty patients of IBS-C were randomized into two groups. In the acupuncture group (30 cases), acupuncture was applied to Tianshu (ST 25), Zusanli (ST 36), Shangjuxu (ST 37), Taichong (LR 3), Sanyinjiao (SP 6), Yintang (GV 29) and Baihui (GV 20), once a day, 5 treatments a week and 4 weeks treatment as one session. In the western medication group (30 cases), lactulose oral solution was prescribed, 15 mL each time, three times a day, 4 weeks treatment as one session. The symptom grade scale and the quality of life scale (IBS-QOL) were adopted to observe the changes in clinical symptoms and quality of life before treatment, after treatment and 2-month follow-up after treatment in the patients of the two groups separately.
RESULTS(1) Clinical symptom score: the difference in the symptom score at each time point was significant statistically in intra-group comparison (P < 0.01). In 1 week, 4 weeks of treatment and 2 months after treatment, the clinical symptom scores in the acupuncture group were lower than those in the western medication group (P < 0.05, P < 0.01). (2) IBS-QOL score after treatment was improved as compared with that before treatment in the two groups (P < 0.01). The improvement of IBS-QOL at 2 months after treatment in the acupuncture group was apparently superior to the western medication group (P < 0.05). (3) The total effective rate was 90.0% (27/30) in the acupuncture group and was 83.3% (25/30) in the western medication group. The overall efficacy in the acupuncture group was better than that in the western medication group (P < 0.01).
CONCLUSIONAcupuncture for regulating the mind and strengthening the spleen significantly relieves the clinical symptoms of IBS-C and improves QOL of patients. The total efficacy is superior to lactulose oral solution and presents a certain of long-term efficacy.

Objective To observe the effects of quercetin liposome (LQ) on formation of advanced glycation end prod-ucts(AGEs)and receptor for advanced glycation end products (RAGE) in kidney of diabetic rats. Methods LQ was made by rotary evaporation, and the model of type 2 diabetic rats were established by being fed on high-sugar and high-fat diet combined with intraperitoneally injection of streptozotocin (STZ). Then type 2 diabetic rats were randomly divided into six groups:diabetic model group (group DM), low level of LQ group (group LQ-L ), medium level of LQ group (group LQ-M), high level of LQ group (group LQ-H), positive control group (group aminoguanidine, AG) and control group (group N). After 8 weeks of interventions, blood glucose, body weight, kidney hypertrophy index (KI), blood urea nitrogen (BUN) and serum creatinine (Scr) were measured in each group. ELISA was used to detect serum AGEs, and 24 h urine albumin. The pathologi-cal change of glomerular basement membranes was observed by PAS staining and the expressions of AGEs in kidney was as-sessed by immunohistochemical method. The transcription level of RAGE mRNA in kidney was determined by RT-PCR. Re-sults Compared with the group N, the level of blood glucose, KI, BUN, Scr, serum AGEs and 24 h urine albumin were in-creased significantly in group DM, while the level of body weight decreased. Also the volume of kidney glomerulus increased and glomerular basement membranes thickened, the transcription levels of AGEs and RAGE mRNA in kidney tissue in-creased in DM group (P<0.05). Compared with group DM, the level of blood glucose, KI, BUN, Scr, serum AGEs and 24 h urinary albumin decreased, while the level of body weight increased in all three LQ groups. Meantime, the change of patho-logical morphology of glomerular basement membranes reduced and the expressions of AGEs and RAGE mRNA in kidney tissue decreased in all three LQ groups. All changes in the medium LQ dose group were more obvious than those of other two LQ groups (P<0.05). Conclusion Similar to AG, LQ has effect on inhibiting the action of proteinum unenzymatic glycosyl-ation and on decreasing the production of AGEs in serum as well as the expression of RAGE mRNA in kidney. Therefore, LQ play important protective role in kidneys of diabetic rats.
