1.Treatment of humerus surgical neck fracture with anatomy plate and locking compression plate

Objective To compare the therapeutic effect of anatomy plate and locking compression plate in the treatment of humerus surgical neck fracture. Methods Fifty one patients with humerus surgical neck fracture were enrolled into the study and divided into two groups randomly to accept anatomy plate or locking compression plate treatment. In the anatomy plate treatment group ( n = 35 ), 19 cases were type- Ⅲ fractures and 16 cases were type-Ⅳ fractures according to Neer classification. In the locking compression plate treatment group (n = 16) ,9 cases were type-Ⅲ fractures and 7 cases were type-Ⅳ fractures according to Neer classification. Results In anatomy plate group,28 cases were followed up for 4 months to 48 months ( average period was [21 ± 2] months). According to the Constent score criteria, 19 cases got excellent outcome,8 cases got good outcome,1 case got fair outcome. In locking compression plate group, 14 cases were followed up for 1 month to 24 months (average period was[12 ± 1] months). Ten cases got excellent outcome,4 cases got good outcome. Conclusion Both anatomy plate and locking compression plate treatments are good for the humerus surgical neck fracture.
2.Development of a peer evaluation form and a self-evaluation form to acquire oral presentation skills

1) We have developed two forms to evaluate students' oral presentation skills (important but hard to teach in the medical school curriculum): one is a peer review form for an audience to evaluate a presenter's performance, and the other is a form for a presenter to evaluate his or her own performance.2) The evaluation process is simple: evaluators fill out the forms by checking the items for evaluation. With these evaluation forms students can get tips for improving their presentations because technical suggestions are written near each item.3) The forms were beneficial for both students and instructors, because students could get tips for improving their presentations, and instructors could concentrate their efforts on scientific content after the students' presentations.
3.Secondary infections in hepatitis B patients with acute-on-chronic liver failure and its relationship with the prognosis

Objective To investigate the characteristics of secondary infections in hepatitis B patients with acute-on-chronic liver failure(ACLF)and its impact on the prognosis. Methods Infection sites, clinical and etiological characteristics were retrospectively reviewed in 186 hospitalized patients with ACLF from the Third Affiliated Hospital of Sun Yat-sen University during January to December 2007. Logistic regression was used to analyze risk factors of secondary infections. Results In 186 patients with ACLF, 160 patients(86.0%) were complicated with infections, and the common sites of infections were abdominal cavity, biliary tract, lung and intestinal tract. The rates of secondary infections were higher in patients with serum albumin(Alb)≤30 g/L, total bilirubin(TBil)>342 μmol/L, prothrombin time(PT)>28 seconds, and those complicated with one or more complications(χ~2=5.4, 7.3, 21.3 and 14.7, P<0.05). The fatality rates of patients with and without infections were 74. 5%(119/160) and 42.3%(11/26), respectively, and the difference was of statistical significance(χ~2=10.9,P=0.000). Patients with multi-organ infections had a higher fatality rate(79.8%, 79/99)than those with one organ infections(65.6%, 40/61), and the difference was also significant(χ~2=4.0, P=0.045). Conclusion Patients with ACLF are liable to infection, and the severity is closely related with the prognosis.
4.CHANGES OF PLASMA ADRENOMEDULLIN AND ADRENOTENSIN CONCENTRATIONS IN PATIENTS WITH CONGESTIVE HEART FAILURE

To investigate the changes of plasma adrenomedullin(ADM) and adrenotensin(ADT) levels and their clinical significance in the pathological process of congestive heart failure(CHF), plasma levels of ADM and ADT in 45 patients with CHF before and after treatment were measured by specific radioimmunoassay. 6.FURTHER EVIDENCE THAT GENOTYPE I AND GENOTYPE II OF CRYPTOSPORIDIUM PARVUM ARE DISTINCT

Three new genes of Cryptosporidium parvum were cloned, including a gene encoding methionine aminopeptidase, one encoding chaperonin containing T-complex protein 1 delta (TCP-1 delta) and one with unknown function. DNA sequence analysis indicated that these genes are quite conserved, but there were some base pair differences between genotype I and genotype II isolates. These differences were confirmed by PCR-restriction fragment length polymorphism (PCR-RFLP) analysis of the 3 genes from 41 isolates collected from different hosts and geographical origins. In brief, the band patterns generated by endonuclease Hind III or Hinf I restrictions of the gene of methionine aminopeptidase, Sac I restriction of the gene of chaperonin, or Ava II restriction of the unknown gene could differentiate the isolates of C. parvum into genotype I and genotype II. PCR primers based on these genes amplified only C. parvum genes. Even a single oocyst was detectable with these PCR primers. Thus the results provided further evidence that genotype I and genotype II are distinct, and our three new primers can be used to detect and characterize C. parvum isolates with high sensitivity.
7.Association of the polymorphism of A9570G in ACE2 gene with the genetic predisposition of diabetic nephropathy

Objective To study the association of the polymorphism of A9570G in ACE2 gene with the genetic predisposition of diabetic nephropathy (DN). Methods 312 patients with type2 diabetes(T2DM) were included in the study. All the patients were divided into DM group and DN group according to if onset of DN. PCRRFLP were used to observe the association of polymorphisms of A9570G in ACE2 gene with DN. Results G allele frequencies of male subjects were 57.9% in DM patients and 67.5% in DN patients (X2 = 1. 559 ,P =0.212) ,52. 4% and,52.1% in female DM and DN patients (X2 =0. 003 ,P =0.955). In female patients,genetype in DN patiants had no difference from that in DM patients (X2 = 0. 109, P = 0. 947). Conclusion The polymorphism of A9570G in ACE2 gane may not associated with the genetic predisposition of DN.

10.Study of finasteride application during the off-cycle in patients with advanced prostate cancer treated with intermittent androgen blockade.

ObjectiveTo investigate the finasteride application during the off-cycle of intermittent androgen blockade (IAB) in patients with advanced prostate cancer treated with IAB.MethodsEighty-seven patients with advanced prostate cancer were divided into two groups: forty-nine patients received IAB (group A), and thirty-eight patients underwent IAB and finasteride during the off-cycle of IAB (group B). The time of treatment cycle and the time to disease progression were compared between the two groups. ResultsThe patients in group A completed 89 treatment cycles and the mean cycle length was (12.8±5.4) months [treatment time and non-treatment time were (6.6±3.5) months and (7.1±4.8) months, respectively]. The patients in group B completed 85 cycles and the mean cycle length was (15.3±5.9) months [treatment time and non-treatment time were (6.9±3.2) months and (9.2v±3.9) months, respectively]. There was a significant difference between group A and B in the mean cycle length and the non-treatment time (P=0.0428,P=0.03).The 3-year progression rate was ( 34.8±3.5 )% in group A and ( 28.4±2.7)% in groups B ( P=0.035). ConclusionsThe application of finasteride during the off-cycle of IAB in patients with advanced prostate cancer treated with intermittent androgen blockade (IAB) can delay progression of advanced prostate cancer.
