2.Study on the In Vitro Model for Th1 Type Shifting

Objectives To establish an experimental model for Th1 typ e shifting and meet the requirements of studying on the mechanisms of some immun omodulators. Methods The levels of cytokines, IL-12, IFN-ice were detected by using ELISA. Sple en cells of the BALB/c mice were incubated under the following conditions: with different concentrations of T cell mitogen ConA (1 mg/mL, 0.5 mg/mL, 0.25 mg/mL, 0.125 mg/mL), mononuclear phagocyte system activator LPS (50 mg/mL, 5 mg/mL, 0. 5 mg/mL) or LPS (50 mg/mL, 5 mg/mL, 0.5 mg/mL) combined with 0.25 mg/mL ConA. Re sults LPS could induce the production of IL-12 from spleen cells. The lowest concentration that ConA could induce the measurable production of IFN-rom sp leen cells was 0.25 mg/mL. When different concentrations of LPS were combined wi th 0.25 mg/mL ConA, LPS could accelerate the production of IFN- and positively with that of IL-12. Conclusion LPS combined with ConA can induce the activation of spleen cells from mice towards Th1 type response.
3.Expression of keratin 17 in proliferative skin diseases

Objective To investigate the expression and significance of keratin 17 (K17) in proliferative skin diseases,including psoriasis,basal cell carcinoma (BCC),squamous cell carcinoma (SCC) and malignant melanoma.Methods Tissue specimens were collected from the lesions of 14 patients with severe plaque-type psoriasis,16 patients with BCC,16 patients with SCC,8 patients with malignant melanoma,as well as from the normal skin of 17 patients with trauma.Immunohistochemistry was conducted to detect the expression of K17 in these tissue samples.Results K17 was absent in the cytoplasm of keratinocytes in the basal layer,prickle cell layer or granular layer of the normal skin.There was a strong expression of K17 in the prickle cell layer,but a weak or negative expression of K17 in the basal layer of psoriatic skin,and parakeratotic cells did not express K17.In BCC tissues,K17 was absent in carcinoma cells,but visible in peritumoral cells in the prickle cell layer and granular layer.In SCC tissues,K17 was localized in highly differentiated carcinoma cells,but not in lowly differentiated carcinoma cells.There was a strong expression of K17 throughout the epidermis above the melanoma,but a negative expression in the melanoma cells or melanocytes.Conclusion K17 may serve as a molecular marker for the differential diagnosis of some proliferative skin diseases.

6.Relationship between adjacent segment degeneration and clinical outcome after lumbar spine fusion

[Objective] To determine the relationship between the adjacent segment degeneration(ASD)and the clinical outcome after lumbar spine fusion.[Methods]Totally 436 patients undergoing posterolateral lumbar fusion were followed up for 58.13 months(26~114 months).The patients were divided into 3 groups according to the number of the fusion segments:1 segment,2 segments and 3 or more segments.UCLA grading scale was used to evaluate the degeneration of the adjacent segments.Cinical outcome was evaluated according to the criteria established by Hou SH.[Results]Adjacent segment degeneration(ASD)occurred in 58 patients,including 14 in group A,27 in group B and 17 in group C.Forty-nine patients had their ASD above the fusion level and 9 below the fusion level.For all the 14 patients in group A,the degenerative grade increased by 1 grade.Of the 27 patients in group B,16 increased by 1 grade,7 by 2 grades,and 4 by 3 grades.Of the 17 patients in group C,4 increased by 1 grade,8 by 2 grades and 5 by 3 grades.For the clinical outcome,7 patients in group A increased by 1 grade.In group B,13 patients increased by 2 grade and 5 patients increased by 3 grades.For the clinical effect degrees,7 cases decreased by 1 degree in group A.In group B,13 cases decreased by 1 degree and 6 decreased by 2 degrees.[Conclusion]No significant relationship was found between the adjacent segment degeneration and the clinical outcome.Careful attention is needed for avoidance of multi-segmental fusion.

8.Study on the aberrant methylation and protein of p16 gene in patients with colorectal cancer

Objective To explore whether hypermethylation in the promoter of p16 gene and protein of p16 were associated with development and clinicopathological characteristics of colorectal cancer. Methods Methylation-specific PCR ( MSP) and immunohistochemistry SP were used to detected hypermethylation of p16 gene and p16 protein in tumor tissues from 32 patients with colorectal cancer. Results The hypermethylation of p16 gene was detected in 40. 6% of tumor tissues. The protein of p16 promoter was detected in 75% of tumor tissues. The hypermethylation of p16 gene was detected in 63% in Dukes stages of C and D tumors. The protein of p16 promoter was detected in 69% of tumor tissues. The hypermethylation of p16 gene was detected in 25% in the stages of A and B tumors. The protein of p16 promoter was detected in 81% of tumor tissues. The hpermethylation of p16 gene was detected in 100% in low differentiated carcinomas. The protein of p16 promoter was detected in 20% , the hypermethylation of p16 gene was detected in 30% , in the high and mediate differentiated carcinomas, the protein of p16 promoter was detected in 85%. Furthermore, the hypermethylation of p16 gene was detected in 63% in the lymph node metastasis and 25% in without lymph node metastasis. The protein of p16 promoter was detected in 65% in rectum and 100% in colon. Conclusions Our study demonstrated that p16 hypermethylation and protein were associated with the development of colorectal cancer and could be used as a putative prognostic indicator for this malignancy.
9.Effects of Bawei Shangke Huoxue Tablets on Hypercoagulable State of Patients with Hip Fracture

Objective To explore the effects of Bawei Shangke Huoxue Tablets on hypercoagulable state of patients with hip fracture. Methods Seventy-two hip fracture patients at hypercoagulable state were randomly divided into treatment group and control group, 36 cases in each group. Both groups were given conventional orthopedic treatment including raising up the affected limb, external application of Huoxue Zhitong Paste on the affected area, and instructions for affected limb functional exercise. Additionally, the treatment group was given oral use of Bawei Shangke Huoxue Tablets together with subcutaneous injection of low-molecular-weight heparin calcium, and the control group was given subcutaneous injection of low-molecular-weight heparin calcium. Before and after treatment, we observed the plasma D-dimer (D-Di) and blood von Willebrand factor(vWF) levels as well as the incidence of deep venous thrombosis (DVT) of both groups before and after treatment. Results (1) All of the cases of both groups completed the clinical trial, and no severe adverse reaction such as increased bleeding occurred. (2) On the admission day, the D-Di and vWF levels of both groups were higher than the normal levels, and were decreased obviously one day before the operation (P < 0.05). On the first post-operative day, the levels were increased as compared with those before operation (P < 0.05), and then decreased on the twelfth post-operative day (P < 0.05) compared with those on the first post-operative day). The treatment group had better effect on improving D-Di and vWF levels than the control group (P < 0.05). (3) The results of follow-up for 3 months showed that 61 cases got the follow-up, 31 cases from the treatment group and 30 from the control group. The DVT incidence of treatment group was 9.68% and that of the control group was 16.67%, the difference being insignificant(P>0.05). Conclusion Bawei Shangke Huoxue Tablets are effective on improving hypercoagulable state of patients with hip fracture, and can be used at early stage and are beneficial to lowering the risks of anticoagulant drugs. Bawei Shangke Huoxue Tablets plus low-molecular-weight heparin calcium have certain effect on preventing DVT.
10.Small external fixation devices combined with flap for fracture and dislocation of articulations interphalangeaes

Objective To explore the outcome of small external fixation devices combined with flap for fracture and dislocation of articulationes interphalangeaes.Methods Twelve cases (13 fingers) with fracture and dislocation of articulationes interphalangeae were treated with small external fixation devices combined with flap.Blood vessel grafting and crossing anastomosis were performed.The area of the flap was 1.0 cm ×1.5 cm -2.5 cm × 3.0 cm.Results Follow-ups lasting from 6 months to 2 years showed that all fingers and flaps were survived postoperatively.Fixation average time of small external fixation was 37 days.Fracture healing was found in all cases with satisfactory effect and with better apperance.Conclusion Small external fixation devices combined with flap was proper for fracture and dislocation of articulationes interphalangeae.
