1.Epidemiological characteristics of 45 norovirus clusters in Baoshan District

Objective:
To investigate the epidemiological characteristics of 45 norovirus clusters in Baoshan District, Shanghai Municipality from 2019 to 2021, so as to provide the evidence for improving the management of norovirus clusters.
Methods:
The epidemiological characteristics and laboratory testing of 45 norovirus clusters were collected in Baoshan District from 2019 to 2021, and the temporal distribution, population distribution, distribution of places of norovirus clusters, clinical manifestations, laboratory testing results, origin of norovirus infection and transmission routes of norovirus clusters were analyzed using a descriptive epidemiological method.
Results:
A total of 45 norovirus clusters involving 29 379 individuals were reported in Baoshan District from 2019 to 2021, and 435 cases with norovirus infections were identified, with an average attack rate of 1.48%. Norovirus infection most often occurred from November to April of the following year, and the infection predominantly occurred among students (408 cases, 93.79%). There were 25 norovirus clusters in kindergartens (55.56%), 18 clusters in primary and middle schools (40.00%), one cluster in a social care facility and one cluster in a geriatric ward. Vomiting was the predominant clinical symptom (416 cases, 95.63%). The positive rate of norovirus was 37.35% in 597 samples. There were 40 norovirus clusters caused by the norovirus GⅡ genotype (88.89%), and the median duration of the norovirus cluster was 6 days (interquartile range, 1 day). In addition, 82.22% of the norovirus clusters reported to centers for disease control and prevention within 1 day,and 88.89% of the clusters were transmitted through human to human transmission or vomitus of cases.
Conclusion
The norovirus clusters predominantly occur in winter/spring and among students in Baoshan District from 2019 to 2021. Kindergartens and primary and middle schools are main places of norovirus infections, and the norovirus GⅡ genotype is the main type of pathogens. Human-to-human transmission and exposure to patients' vomitus are the main routes of transmission.
2.Influencing factors for medication compliance in patients with comorbidities of chronic diseases: a meta-analysis

Objective:
To systematically evaluate the influencing factors for medication compliance in patients with comorbidities of chronic diseases, so as to provide the evidence for improving medication compliance.
Methods:
Literature on influencing factors for medication compliance in patients with comorbidities of chronic diseases were retrived from CNKI, Wanfang Data, VIP, SinoMed, PubMed, Web of Science, Cochrane Library and Embase from inception to January 20, 2024. After independent literature screening, data extraction, and quality assessment by two researchers, a meta-analysis was performed using RevMan 5.4 and Stata 16.0 softwares. Literature were excluded one by one for sensitivity analysis. Publication bias was assessed using Egger's test.
Results:
Initially, 7 365 relevant articles were retrieved, and 35 of them were finally included, with a total sample size of about 150 000 individuals. There were 30 cross-sectional studies and 5 cohort studies; and 11 high-quality studies and 24 medium-quality studies. The meta-analysis showed that the demographic factors of lower level of education (OR=2.148, 95%CI: 1.711-2.696), lower economic income (OR=1.897, 95%CI: 1.589-2.264), male (OR=0.877, 95%CI: 0.782-0.985), living alone (OR=2.833, 95%CI: 1.756-4.569) and unmarried (OR=2.784, 95%CI: 1.251-6.196); the medication treatment factors of polypharmacy (OR=1.794, 95%CI: 1.190-2.706), potentially inappropriate medication (OR=2.988, 95%CI: 1.527-5.847), low frequency of daily medication (OR=0.533, 95%CI: 0.376-0.754) and adverse drug reactions (OR=3.319, 95%CI: 1.967-5.602); the disease factors of long course of disease (OR=2.118, 95%CI: 1.643-2.730), more comorbidities (OR=1.667, 95%CI: 1.143-2.431) and cognitive impairment (OR=2.007, 95%CI: 1.401-2.874); and the psychosocial factors of poor belief in taking medication (OR=1.251, 95%CI: 1.011-1.547), poor self-rated health (OR=1.990, 95%CI: 1.571-2.522) and being guided by healthcare professionals (OR=0.151, 95%CI: 0.062-0.368) were the influencing factors for medication compliance in patients with chronic comorbidities.
Conclusion
The medication compliance in patients with comorbidities of chronic diseases is associated with demographic factors, pharmacological factors, disease factors and psychosocial factors, mainly including living alone, adverse drug reactions, course of disease, number of comorbidities and medication beliefs.
3.Clinical characteristics and oral care needs of perioperative patients with oral cancer

The treatment of oral cancer patients mainly involves surgery in combination with radiotherapy and chemotherapy. This paper reviews the clinical features of perioperative patients with oral cancer, including oral flora imbalance, oral complications after radiotherapy and chemotherapy, the presence of oral incisions (and flaps), special dietary needs, and airway management. In connection with the above characteristics, this article analyzes the necessity of three aspects of oral care to improve the patient’s comfort and prevent pulmonary and surgical site infections, with the goal of providing a reference for oral care research on patients undergoing oral cancer surgery and laying a foundation for the construction of comprehensive oral care programs during the perioperative period for patients with oral cancer.
4. A qualitative research on the experience of oral hygiene care of perioperative oral cancer patients

5.Human osteoarthritic chondrocytes up-regulate the expression of osteoprotegerin in osteoblasts via the Indian hedgehog signaling pathway

BACKGROUND:Upregulation of hedgehog protein signaling can increase the expression of osteoarthritis markers,Runx2,a disintegrin and metalloproteinase with thrombospondin motifs,collagen type X alpha 1,and matrix metalloproteinase 13,while inhibition of hedgehog proteins attenuates the severity of osteoarthritis.It is speculated that osteoarthritic chondrocytes can influence bone formation by affecting osteoblasts through the Indian hedgehog protein(IHH)signaling pathway. OBJECTIVE:To investigate the effect of human osteoarthritic chondrocytes on subchondral osteoblasts. METHODS:Tibial plateau specimens from patients with osteoarthritis were collected.Chondrocytes were extracted using enzymatic digestion,and osteoblasts were extracted using enzymatic pre-digestion + bone block method.Chondrocytes were identified by toluidine blue staining and immunofluorescence and osteoblasts were identified by alkaline phosphatase staining and immunofluorescence.Chondrocytes were cultured in sodium alginate beads to maintain chondrocyte phenotype and co-cultured with osteoblasts.The co-culture system was added with IHH signaling pathway inhibitor(cyclopamine,10 nmol/L)and activator(purmorphamine,10 nmol/L)separately.After 48 hours of co-culture,osteoblasts from each group were collected,mRNA expressions of Gli1,osteoprotegerin,Runx2,parathyroid hormone-related peptide,alkaline phosphatase,receptor activator of nuclear factor-kB ligand(RANKL)and osteocalcin were detected by qRT-PCR,and protein expressions of GLi1,oseoprotegerin and RANKL in osteoblasts were detected by western blot. RESULTS AND CONCLUSION:The mRNA expression levels of GLi1,osteoprotegerin and RUNX2 in osteoblasts were significantly increased,while the mRNA expression levels of parathyroid hormone-related peptide were decreased(P<0.05)when co-cultured with human osteoarthritic chondrocytes.The mRNA and protein levels of Gli1 were significantly decreased after the addition of IHH signaling pathway inhibitor(cyclopamine)(P<0.05),and the mRNA and protein levels of Gli1 were significantly increased after the addition of IHH signaling pathway activator(purmorphamine)(P<0.05).Osteoprotegerin showed the same trend as Gli1 in the experiment.The osteoprotegerin/RANKL ratio followed the same trend as osteoprotegerin.To conclude,human osteoarthritic chondrocytes can promote the expression of Gli1,osteoprotegerin,Runx2 and other proteins in osteoblasts.The upregulation of osteoprotegerin is related to the IHH signaling pathway.Osteoarthritic chondrocytes can up-regulate the expression of osteoprotegerin in osteoblasts through the IHH signaling pathway and thus up-regulate the osteoprotegerin/RANKL ratio,which will contribute to bone formation in subchondral bone.
6.18F-FDG PET/CT manifestations of primary systemic anaplastic large cell lymphoma

Objective To observe the 18F-FDG PET/CT manifestations of primary systemic anaplastic large cell lymphoma(ALCL).Methods A total of 21 patients with primary systemic ALCL were enrolled,and PET/CT manifestations were observed.Results Among 21 cases of ALCL,there were 15 cases of ALK+and 6 cases of ALK-.Affected lymph nodes in multiple site were observed in 19 cases,mainly located in the neck(n=13),mediastinum(n=12 cases)or retroperitoneum(n=12),while single site affected lymph node was notice in 2 cases.Extranodal organs/site involvements were found in 12 cases,including 6 cases of soft tissue(such as skin,muscles,etc.),4 cases of bone,14 cases of organs,including 4 cases of lung,3 cases of liver,2 cases of pancreas,2 cases of kidney,2 cases of gastrointestinal tract and 1 case of thyroid.Among 21 cases of ALCL,19 with irregular lymph node morphology and fused into clusters,17 with uniform density,3 with necrosis and 1 with calcification.All ALCL lesions in 21 cases showed hypermetabolism,the maximum standard uptake value(SUVmax)and the mean standard uptake value(SUVmean)of the affected lymph node was 17.04±9.94 and 9.96±6.15,respectively,while the metabolic tumor volume(MTV)and total lesion glycolysis(TLG)of all lesions was 92.54(67.61,249.21)cm3 and 723.46(419.78,1 461.17)g,respectively.The maximum diameter of the affected lymph node was not significantly correlated with SUVmax nor SUVmean(both P>0.05),but positively correlated with MTV and TLG of all lesions(r=0.696,0.767,both P<0.001).Ann Arbor staging was positively correlated with the maximum diameter,SUVmax and SUVmean of the affected lymph node,also MTV and TLG of all lesions(r=0.467,0.458,0.702,0.780,0.664,all P<0.05).Conclusion 18F-FDG PET/CT manifestations of primary systemic ALCL were characteristic,with significant changed metabolic parameters,including SUVmax,SUVmean,MTV and TLG.
7.Association between Yang Deficiency Syndrome and the End-point Events of Diabetic Kidney Disease: A Retrospective Cohort Study

ObjectiveTo investigate the impact of yang deficiency syndrome on the progression to end-point events of diabetic kidney disease (DKD). MethodsA retrospective study among patients with stage Ⅳ DKD admitted to Dongzhimen Hospital of Beijing University of Chinese Medicine from September 1st, 2016 to September 30th, 2021 was conducted. Data on the patients' general information, clinical indicators including duration of diabetes, duration of proteinuria, history of smoking and drinking, hemoglobin (HGB), fasting blood glucose (FBG), albumin (ALB), serum creatinine (Scr), urea nitrogen (BUN), uric acid (UA), cholesterol (TC) , triglycerides (TG), low-density lipoprotein (LDL), 24-hour urine protein quantification (24h-UTP) and estimated glomerular filtration rate (eGFR), and TCM syndromes including symptoms, tongue and pulse, and syndrome scores were collected. The patients were divided into exposure group (yang-deficiency group) and non-exposure group (non-yang-deficiency group). The general information, clinical indicators and incidence rates of end-point events were compared, and the impact of yang deficiency syndrome on the end-point events of stage Ⅳ DKD was analyzed. Survival analysis was performed using Kaplan-Meier method, and multivariate Cox proportional risk models were used to identify independent predictors of end-point events. ResultsA total of 160 patients with stage Ⅳ DKD were included in the study, including 43 cases of yang deficiency syndrome and 117 cases of non-yang deficiency syndrome. Compared to those in the non-yang deficiency group, the waist circumference, BUN and the incidence of end-point events in the yang deficiency group were significantly higher (P<0.05 or P<0.01). Spearman correlation analysis showed that yang deficiency syndrome was positively correlated with incidence of end-point events of stage Ⅳ DKD (r = 0.167, P = 0.035). Furthermore, 24h-UTP and BUN levels were also positively correlated with end-point events in stage Ⅳ DKD patients (P<0.01), while ALB and HGB levels were negatively correlated (P<0.01). Kaplan-Meier survival curves showed that yang deficiency syndrome was associated with an increased risk of end-point events (Log Rank P = 0.011). Moreover, 24h-UTP levels ≥3500 mg, BUN level ≥8 mmol/L, ALB level <30 g and HGB level <11 g were all associated with the increase of the risk of end-point events (P<0.05 or P<0.01). Multivariate Cox regression analysis showed that yang deficiency syndrome was an independent risk factor for patients with stage Ⅳ DKD to progress into end-point events (HR = 2.36, 1.32 to 4.21; P = 0.004), as well as 24h-UTP ≥ 3500 mg, BUN ≥ 8 mmol/L, HGB<11 g and ALB<30 g (P<0.05 or P<0.01). ConclusionsFor stage Ⅳ DKD, patients with yang deficiency syndrome are more likely to have end-point events, which is an independent risk factor for the progression into end-point events.
8.The association of serum level of interleukin⁃18 with structural progression in knee osteoarthritis

Objective :
To investigate the cross⁃sectional associations of serum interleukin( IL) Ⅳ18 with cartilage volume , cartilage defects , bone marrow lesions ( BML) and biomarkers of cartilage degradation in patients with
knee osteoarthritis (OA) , and to provide new ideas and new methods for clinical diagnosis and treatment.
Methods:
The study included 151 patients with knee OA , a general questionnaire survey was conducted , and the knee strucral was photographed by magnetic resonance imaging (MRI) . The cartilage volume was measured by OsiriX software in 3D⁃FLASH sequence , and cartilage defect and BML were determined in T2⁃weighted sequence. Serum IL-18 and matrix metalloproteinase ( MMP) Ⅳ3 , 13 levels were measured by enzyme⁃linked immunosorbent assay(ELISA) . SPSS software was used for statistical analysis.
Results :
In multivariable analyses , serum IL⁃18 level
was consistent at divided part of joint (femorotibial joint and the patella femoral joint , all P < 0. 05) . Serum IL⁃18 level was positively associated with cartilage defect and BML at media femorotibial area (all P < 0. 01) . Serum IL⁃18 level was positively associated with MMP⁃3 (β = 0. 31 , 95% CI:0. 001 - 0. 010) and MMP⁃13 (β = 0. 86 , 95% CI:0. 08 - 0. 10 , all P < 0. 01) .

9.Characteristics of the chloroplast genome of Dracaena marginata and phylogenetic analysis.

Dracaena marginata is a widely cultivated horticultural plant in the world, which has high ornamental and medicinal value. In this study, the whole genome of leaves from D. marginata was sequenced by Illumina HiSeq 4000 platform. The chloroplast genome were assembled for functional annotation, sequence characteristics and phylogenetic analysis. The results showed that the chloroplast genome of D. marginata composed of four regions with a size of 154 926 bp, which was the smallest chloroplast genome reported for Dracaena species to date. A total of 132 genes were identified, including 86 coding genes, 38 tRNA genes and 8 rRNA genes. Codon bias analysis found that the codon usage bias was weak and there was a bias for using A/U base endings. 46 simple sequence repeat and 54 repeats loci were detected in the chloroplast genome, with the maximum detection rate in the large single copy region and inverted repeat region, respectively. The inverted repeats boundaries of D. marginata and Dracaena were highly conserved, whereas gene location differences occurred. Phylogenetic analysis revealed that D. serrulata and D. cinnabari form a monophyletic clade, which was the closest relationship and conformed to the morphological classification characteristics. The analysis of the chloroplast genome of D. marginata provides important data basis for species identification, genetic diversity and chloroplast genome engineering of Dracaena.
