1.Visual field analysis in early mild Parkinson's disease

Objective To evaluate visual field changes in early mild Parkinson's disease.Methods A total of 66 eyes of 33 cases with early mild Parkinson's disease and 72 eyes of 36 age-matched normal individuals were enrolled into the study.Humphrey Field Analyzer Ⅱ was applied for central visual field test.The visual field indices of mean deviation (MD) and pattern standard deviation (PSD) were analyzed to evaluate the location and the characteristics of visual field defect in this study.Results Visual field indices MD (-3.4±2.5) dB was significantly changed in patients with PD when compared to the controls (-0.6 ± 1.7) dB.PSD (4.3±2.6) was significantly higher in patients with PD than that in the control group (2.1 ± 1.8) dB.Glaucoma hemifield test (GHT) assessment was within normal limits in the controls.Of the 33 patients (66 eyes) in PD,GHT showed outside normal limits in 31 eyes,borderline in 8 eyes,and within normal limits in 27 eyes.31 eyes outside normal limits appeared glaucomatous visual field defects,in which 16 with nasal step and 5 with arcuate defect.Conclusions Visual field indices including MD and PSD in early mild patients with PD were significantly worse than that in the controls group.GHT abnormalities could be found in early mild PD patients with visual field defects,including pericentral scotoma and nasal step,which mimicked glaucomatous changes.
2.The Origination of Progenitor Cells of Utricular Regenerative Hair Cells in Rat

Objective To investigate the cell origination of progenitor cells of the mammalian regenerative hair cell.Methods Utricular sensory epithelial of postnatal day 1 wister rats was isolated by mechanical dissociation.The explants were digested by thermolysin,then were transfrred to an aliquot containing 0.125% trypsin and 0.125% collagenase for incubation to harvest the pure utricular sensory epithelial cell(USEC).USEC were cultured in Dulbecco Modified Eagle Medium(DMEM),and observed daily by inverted microscope.Immunocytochemical staining with cytokeratin 18,vimentin,Brn3a and Calretinin,ultrastrctural examination with transmission electron microscope,and reverse transcription PCR with mRNA of p27kipll,AchRa9 and Myosin Ⅶa.The markers of hair cells and supporting cells were used to identify the origination and characteriztion of USEC.Results USEC showed a large,flat,polygonal epithelial morphotype with big,round neuclei.USEC monolayer showed "cobblestone-like" appearance.Some cells showed "Dome" formation,probably due to fluid collection underneath the cell monolayer.USEC expressed cytokeratin 18 but did not express vimentin,and had rich microvilli and complex tight junction,which indicated the epithelial origination of USEC.USEC expressed Brn 3a,calnevimin and mRNA of p27kipl,AchRa9,myosin Ⅶ.Conclusion USEC coexpressed the characteristic markers of the hair cell and supporting cells.This indicates that the culture cells may represent progenitor cell of hair cell and the progenitor cell may come from supporting cells.

4.CT findings of secondary pulmonary lymphoma:report of 20 cases

Objective To analyze the features of CT images of secondary pulmonary lymphoma. Methods The dats of totally 20 patients with secondary pulmonary lymphoma which were proven through lung puncture pathology or clinical treatment were retrospectively analyzed. There were 19 non-Hodgkin lymphoma cases and 1 Hodgkin lymphoma case. All cases received thorax CT scanning, and 13 of them had enhanced CT scanning. Results There were many patterns in our CT findings for secondary pulmonary lymphoma, mixed pattern in 9 cases, mass (nodule) in 4 cases, pneumonic-alveolar in 3 cases, bronchovascular-lymphatic in 3 cases, and miliary in 1 case. There were 13 cases accompanying with mediastinal lymphadenovarix, 5 cases with pleura invasion, 11 cases with pleural effusion, and 2 cases with pericardium involvement. Mild enrichment was found in 7 cases and obviously enrichment was seen in 1 case. Conclusion Diversity is the characteristic feature of secondary pulmonary lymphoma. Combined with other thorax findings, CT scaning can make a definite diagnosis.
5.Approaches to improving the network teaching of medical imaging

In view of the problems existing in medical image network teaching, such as redundant and rigid curriculum system, obsolete structure, untimely updates, lack of supervision system and poor stability of network security and so on, we explored how to improve the medical image network teaching from the fusion of traditional teaching, optimizing curriculum system, perfecting the medical imaging system of autonomous learning, encouraging online communication, creating a good atmosphere, increasing invest-ment in software development and multi-sectoral collaboration, in order to complete the transfer of knowl-edge more effectively, and promote the good development of medical imaging teaching.
6.The efficacy of Helicobacter pylori eradication and folic acid intervention in treatment of atrophic gastritis

Objective To investigate the effect of folic acid combined with Helicobacter pylori (Hp) eradication therapy on chronic atrophic gastritis.Methods From December 2009 to March 2011 at the First Affiliated Hospital of Nanjing Medical University,184 patients with endoscopic and pathological diagnosis of chronic atrophic gastritis (90 Hp positive and 94 Hp negative) were selected.Hp positive patients were divided into group A and group B.Forty-three patients in group A were treated with standard triple Hp eradication therapy and follow by folic acid therapy for three months.Forty-seven patients in group B and Hp negative patients received three months of folic acid therapy.The clinical symptoms of each group were scored before treatment,one month after folie acid therapy and three months after folic acid therapy and analyzed by t test. Patients of each group received gastroscopy before treatment and three months after medicine withdrawal. Endoscopic scores,pathological scores and t test were recorded.The serum levels of pepsinogen ( Ⅰ,Ⅱ ) and gastrin 17 in venous blood of 55 Hp negative patients were detected by enzyme-linked immunosorbent assay (ELISA) method before treatment and three months after medicine withdrawal.Results Compared with three months therapy (1.15 ± 0.03),after one month folic acid therapy (1.55 ± 0.04) was statistically significant in clinical symptoms score of all patients (t =8.18,P<0.01).By the end of therapy,clinical symptom score of group A (1.06 ± 0.04) was lower than that of group B (1.56 ±0.08),and the difference was significant (t=6.00,P<0.01).There was significant difference in endoscopic scores of all patients between before treatment (1.57±0.95) and after treatment (1.00±0.76,t=11.12,P<0.01).The differences in each pathological score of all patients (inflammatory scoring,active scoring,atrophy scoring,intestinal metaplasia scoring,atypical hyperplasia degree scoring) were significant between before treatment and after treatment (t=5.51,6.90,7.53,6.34,2.90,respectively,all P<0.01).The serum level of pepsinogen Ⅰ before treatment of 55 Hp negative patients [(1.03±0.19) nmol/L] was lower than that after treatment [(2.24±0.33) nmol/L],and the difference was statistically significant (t=3.19,P<0.01).After treatment the level of gastrin 17 [(0.86±0.05) nmol/L] was higher than that before treatment [(0.47±0.05) nmol/L],and the difference was statistically significant ( t =5.33,P< 0.01 ).Conclusion Folic acid in combination with Hp eradication therapy can be favorable to atrophic gastritis,which may promote the secretion of pepsinogen and gastrin.
7.Mutation analysis of the ATP2A2 gene in a Kazakh family with Darier's disease

Objective To analyze mutations in the ATP2A2 gene in a Kazakh family with Darier's disease.Methods Clinical data were collected from 49 members from a family with Darier's disease,and peripheral blood samples were obtained from 44 family members and 100 unrelated healthy people.Genomic DNA was extracted from these blood samples.PCR and DNA sequencing were performed to detect mutations in the ATP2A2 gene.Results Darier's disease was inherited in an autosomal dominant manner in this family.A G→A heterozygous mutation (1288-1G→A) was identified at position 1288-1 at the splice site in exon 12 of the ATP2A2 gene in 11 patients in this family,but not in 33 healthy members or 100 healthy controls.Conclusion Darier's disease in this family may be caused by the heterozygous mutation (1288-1G→A)at the splice site in exon 12 of the ATP2A2 gene.
8.Pharmacokinetic comparison of roxithromycin under normoxic and hypoxic conditions in rats by UPLC/MS/MS

Aim To study and compare the pharmaco-kinetic parameters of roxithromycin under normoxic and hypoxic rats. Methods A highly effective and rapid ultra-performance liquid chromatography with tandem mass spectrometry ( UPLC-MS/MS) method with posi-tive electrospray ionization source was successfully de-veloped and validated for quantification of roxithromy-cin in rat plasma. Sprague-Dawley rats were randomly divided into the hypoxia and normoxic groups. Each rat obtained a single dose of roxithromycin with 10 mg · kg-1 via intragastric administration. The pharmacoki-netic parameter comparison between normoxic and hy-poxic groups was calculated by SPSS software using in-dependent sample t test method. Results The main pharmacokinetic parameters of roxithromycin between the normoxic and hypoxic rats were:the AUC(0-t) 7 576 and 3 761 μg·h·L-1 , MRT(0-t) 5. 6 and 7. 7 h, T1/2 3. 4 h and 3. 9 h, CL 1. 5 and 3. 0 L · h-1 · kg-2 , tmax3. 1 and 3. 4 h, Cmax 1 116 and 372 μg·L-1 , re-spectively. The levels of Cmax and AUC of roxithromy-cin in hypoxic rats were statistically lower than those in normoxic rats. Conclusion The exposure level of rox-ithromycin in hypoxic rats markedly decreased. Our re-sults may provide an important experimental basis to adjust the dosage for roxithromycin in hypoxic clinical practice.
9.Effect of resveratrol on ROS production and PECAM-1 expression in ox-LDL-stimulated platelets

Aim To investigate the effect of resveratrol on ROS level and PECAM-1 expression in ox-LDL-stimulated platelets. Methods The expression of PE-CAM-1 , Sirt1 and p38 MAPK phosphorylation in ox-LDL-stimulated platelets was determined by Western blot. The level of ROS was measured by immunofluo-rescence kit. Results ox-LDL induced platelet aggre-gation by 14%, whereas resveratrol inhibited platelet aggregation by 50%. Resveratrol decreased ROS level by 3 . 2 fold and completely suppressed PECAM-1 expression in ox-LDL-treated platelets. Resveratrol re-covered Sirt1 expression in ox-LDL-treated platelets. EX527 ( a Sirt1 inhibitor ) increased ROS level and PECAM-1 expression in ox-LDL-stimulated platelets. Meanwhile, resveratrol also suppressed p38MAPK phosphorylation induced by ox-LDL. Conclusion Resveratrol can inhibit platelet aggregation, decrease ROS production and PECAM-1 expression in ox-LDL-stimulated platelets. The mechanism maybe associated with recovery of Sirt1 expression. Moreover, resveratrol can decrease PECAM-1 expression, which may be linked to abolishing p38MAPK phosphorylation.
10."Exploration and practice of the ""5+3"" medical personnel training mode in the department of radiology"

It has been a general trend to open the 5+3 medical personnel training mode of radiology in China.But at present,there are still a lot of problems during the process,such as,lack of training plan,unified teaching materials and examination index for reference,lack of learning consciousness and self-discipline among the students who participate in the standardization training,lack of adequate teaching resources in the standardization training base,and so on.In order to cultivate qualified and high-quality radiologists,this paper discussed the ways of forming a new set mode of 5+3 medical personnel training in the radiology department through exploration and practice,including strengthening teachers' construction and building a professional teachers troop,reforming the teaching method of standardization training,improving the teaching base infrastructure,establishing and perfecting the assessment evaluation mechanism.
