2.Application of improved quality evaluation method to Fufang Danshen products

AIM:To apply improved quality evaluation method for improving current quality criteria of Fufang Danshen products. METHODS:Considering both hydrophilic and lipophilic active components-danshensu,protocatechuic aldehyde,salvianolic acid B,cryptotanshinone and tanshinone ⅡA were selected as five markers. A HPLC method was developed to simultaneously determine their contents,which were used to compare the products from different companies,and different batches from the same company. Similarity factor method was used to evaluate the similarities of in-vitro dissolution profiles. Moreover,accelerated and long-term stability tests were performed to predict the shelf-life of Fufang Danshen products. RESULTS:All of the Fufang Danshen products passed the content requirements of Chinese Pharmacopeia. However,the content variations of the hydrophilic components were much higher than those of the lipophilic components. In dissolution tests,most of the hydrophilic components were dissolved completely while all of the lipophilic components could not be detected. The f2 values showed that there were no similar dissolution profiles among different brands of Fufang Danshen products. The contents of five markers degraded compatible with first-order kinetics processes in both the accelerated and long-term stability tests. The predicted shelf-life of Fufang Danshen products were shorter than those labelled on the products. CONCLUSION:It is suggested that more markers with defined content range instead of lower content limit are required for better control of Fufang Danshen products. Dissolution and stability tests need to be added into the quality control criteria in the Chinese Pharmacopeia.
3.Analysis of Composition and Medication Regularities of Prescriptions for Treatise on Febrile Diseases Based on TCM Inheritance Platform System(V2.5)Software

OBJECTIVE:To investigate the composition and medication regularities of prescriptions for Treatise on Febrile Dis-eases. METHODS:The prescriptions included in Treatise on Febrile Diseases were collected and inputted into TCM inheritance platform system(V2.5)software,and prescription database was established. The medication and the compatibility regularity of pre-scriptions were excavated using the function of software integration. RESULTS & CONCLUSIONS:83 kinds of TCM involved in 112 prescriptions ranked by frequency,among which 24 ingredients were used more than 4 times. Commonly used TCM groups were summarized to evolve 16 key groups and 8 new prescriptions. Four properties of TCM are mainly the property of“Wen and Han”,and five tastes mostly are“Sweet,Bitter,Spicy”. TCM with the property of“Wen and Sweet”taste are more than those with the property of“Wen and Spicy”,and TCM acting on spleen are more than those acting on lung.
4.Radiofrequency ablation for hepatocellular carcinoma: our perspectives on patient selection and curative results

Radiofrequency ablation (RFA) has been playing an important role in the treatment of hepatocellular carcinoma (HCC). It is minimally invasive, easily repeatable and potentially curative. Many centers now accept it to be the best therapeutic choice for patients with early-stage HCC when liver transplantation or surgical resection is not suitable. In this article, we reviewed the indications, techniques , and clinical results of RFA in the treatment of HCC.Suitable patient selection, good pre-treatment planning,complete ablation, and careful post-treatment follow up are important. Techniques, such as artificial pleural effusion and ascites, contrast-enhanced ultrasonography-guided RFA,laparoscopic or open surgical approaches have reduced complications and expanded the indications for RFA.
5.Construction of a novel bioartificial liver system and its functional evaluation in vitro

Objective To construct a novel bioartificial liver (BAL) system and evaluate its functions in vitro. Methods Chinese experimental minipig hepatocytes were isolated by in situ recirculating collagenase perfusion method and 1.0?10~(10) hepatocytes were cultured in serum-free medium with restriction of(attachment), and using spinner method to form hepatocyte(spheroids).cted by inoculating the hepatocyte(spheroids) into cell circuit of a hollow fiber bioreactor from BIOLIV A3A. Observing the number and viability of the(hepatocytes), the changes of alanine aminotransferase (ALT), total bilirubin (TBI), albumin (ALB) in(circulating) hepatocyte suspension and(RPMI1640) medium; in addition, lidocaine metabolism test was(determined),(during) 6h circulation of the system. (Results) There were no significant differences in number and viability of the hepatocytes before and after 6h(circulation). The BAL system has relatively strong albumin synthesis and lidocaine(metabolism) functions. (Conclusions) The BAL system that we developed had ability to support liver functions and could be used in the treatment of liver failure, or to provide temporary liver support for candidates of liver(transplantation).

7.The role of IL-6 in the co-cultivation of porcine hepatocytes with bone marrow mesenchymal stem cells

Objective To investigate the role of IL-6 in the co-cultivation system of porcine hepatocytes with bone marrow mesenchymal stem cells (MSCs) in vitro.Methods Mononuclear cells were isolated from bone marrow aspirate of swines (n = 3) by 1.077 g/ml density gradient centrifugation.MSCs of passage 3 and primary hepatocytes harvested by a two-step in situ collagenase perfusion technique were inoculated in Millicell culture inserts at a seeding ratio of 1:2,and functional changes of heterotypic interactions were characterized.Levels of IL-6,TNF-α,and TGF-α were measured respectively.Results Hepatocyte viability was greater than 95%.Hepatocyte performance levels such as albumin secretion and urea synthesis were all significantly enhanced in co-culture group compared with hepatocyte homo-culture (P<0.05).The IL-6 level also significantly increased in co-culture group (P < 0.01).A significant decrease of hepatic functions was observed upon neutralization of IL-6 in co-culture.Conclusion MSC-derived IL-6 plays a key role in the up-regulation of hepatocyte functions in this co-culture.
8.Experimental study on the maintenance of porcine hepatocyte morphology and function by culturing with bone marrow mesenchymal stem cells

Objective To establish porcine hepatocyte co-culture system with bone marrow mes-enchymal stem cells (MSCs) in vitro for the ideal cell source of bioartificial liver. Methods Mononu-clear cells were isolated from bone marrow aspirate extracted from the anterior superior lilac spine of swines (n=3) by 1.077 g/ml density gradient centrifugation. MSCs of passage 3 and primary hepato-cytes harvested by a two-step in situ collagenase perfusion technique were randomly distributed, and the morphological and functional changes of heterotypic interactions were characterized. Results The purity of the third passage MSCs and primary hepatoeytes was more than 90 % and 99%, respectively.Hepatocyte viability was greater than 95 %. A rapid attachment and self-organization of three-dimen-sional hepatocyte spheroids were encouraged in co-culture. Heterotypic junctions remained similar to that of hepatocytes in vivo. Hepatocyte performance levels such as albumin secretion and urea synthe-sis were all significantly enhanced in co-culture group compared with hepatocyte homo-culture (P<0.05). The best hepatic function levels were achieved on day 2 and moderately decreased in the following co-culture days. Conclusion Co-cultivation of porcine hepatocytes and MSCs may preserve hepatocyte morphology and function, which could contribute to the functional bioartificial liver.
9.Chemical constituents from roots of Pueraria lobata.

OBJECTIVETo study the chemical constituents in the roots of Pueraria lobata.
METHODVarious chromatographic methods were employed to separate of chemical constituents and the spectroscopic methods were used to elucidate the structure.
RESULTTwenty-two compounds were isolated and characterized as beta-sitosterol palmitate (1), beta-sitosterol (2), lupeol (3), lupeone (4) , puerarol (5), diisobutyl phthalate (6), bis(2-ethylhexyl) phthalate (7), sophoracoumestan A (8), coumestrol (9), allantion (10), dadzein (11), formononetin (12), 3'-methoxy daidzein (13), ononin (14), 3'-hydroxy dadzein (15), genistin (16), dadzin (17), 8-methoxy ononin (18), sissotorin (19), (-)-puerol B 2-O-glucopyranoside (20), (6S,9R)-roseoside (21) and sucrose (22), respectively.
CONCLUSIONCompounds 1, 5-8, 15, 18, 21 and 22 were isolated from P. lobata for the first time, and componds 1, 5-7, 15, 18, 21 and 22 were isolated from the genus Pueraria for the first time.

10.Effects of structured triglyceride on postoperative recovery of patients with primary liver cancer after hepatectomy: a prospective study

Objective To investigate the effects of structured triglyceride on hepatic function,nutritional status,inflammatory reaction and immunologic function of patients with primary liver cancer after hepatectomy.Methods A prospective,double-blind,randomized,controlled clinical trial was conducted based on the clinical data of 80 patients with primary liver cancer who underwent hepatectomy at the Drum Tower Hospital from January 2011 to December 2012.All the 80 patients were allocated into the testing group (40 patients) and the control group (40 patients) based on a random number table,and received isometrical nitrogen and isocaloric parenteral nutritional support therapy for 7 days after operation.Structured triglyceride (STG) and physical mixed medium chain/long chain triglycerides (MCT/LCT) were applied to the patients in the testing group and the control group,respectively.The hepatic function,nutritional status,inflammatory reaction,immunologic function and prognosis of patients were compared between the 2 groups.The measurement data with normal distribution were presented as (x) ± s.The comparision between groups was evaluated with an independent sample t test.The trend analyses for variables were done using the repeated measures ANOVA.The measurement data with skew distribution were described as M(P25,P75) and analyzed by the analysis of variance.The count data were analyzed using the chi-square test.Results All the 80 patients who were screened for eligibility were randomly divided into the testing group (40 patients) and the control group (40 patients).The alanine transaminase (ALT),aspartate transaminase (AST),prealbumin,choline esterase,C-reactive protein,IgG,IgM,IgA,CD3 +,CD4 +,CD8 + and CD4 +/CD8 + in the 2 groups showed a range of variations before operation and at postoperative day 5 and 7.The ALT,AST,prealbumin,choline esterase,C-reactive protein,IgG,IgM,IgA,CD3 +,CD4 +,CD8 + and CD4+/CD8 + from preoperation to postoperative day 7 were from (36 ± 10)U/L,(37 ± 13)U/L,(233 ±42)mg/L,(154±32)U/L,(8.1 ±1.6)mg/L,(14±5)g/L,(3.2±1.3)g/L,(2.1 ±1.3)g/L,55.6%±6.8%,37.5%±2.0%,25.9%±2.9% and 1.5 ±0.5 to (83 ±33)U/L,(63 ±42)U/L,(208 ±37)mg/L,(145 ±26)U/L,(30.3 ±8.6)mg/L,(12 ±3)g/L,(2.8 ±1.6)g/L,(2.0 ±1.2)g/L,46.2%±3.9%,33.4%±3.1%,27.9%±3.0% and 1.2 ±0.4 in the control group,and from (36 ± 12)U/L,(36 ± 14)U/L,(231 ±45)mg/L,(149 ±25)U/L,(7.6 ± 1.8)mg/L,(13 ±5)g/L,(3.3 ±0.7)g/L,(2.1 ± 1.1)g/L,54.2%±6.3%,37.6%±3.5%,24.8%±3.4% and 1.5 ±0.4 to (43 ±30)U/L,(40 ±33)U/L,(255 ±44)mg/L,(153 ±27)U/L,(21.7 ±11.2)mg/L,(17 ±4)g/L,(4.1 ± 1.7)g/L,(2.4 ± 1.9)g/L,57.9%±3.2%,39.2% ± 2.9% 22.7% ± 3.4% and 1.7 ± 0.4 in the testing group,respectively,showing significant differences between the 2 groups (F =94.71,86.40,64.22,16.77,8.18,37.23,11.52,22.58,50.30,26.44,13.16,44.84,P < 0.05).There were no significant differences in the level of ALT,AST,prealbumin,choline esterase,C-reactive protein,IgA,CD8 + and CD4 +/CD8 + at postoperative day 5 between the 2 groups (t =1.24,1.03,0.91,1.02,0.53,0.68,1.61,0.98,P>0.05).There were significant differences in the IgG,IgM,CD3+and CD4 + at postoperative day 5 between the 2 groups (t =2.58,3.15,3.87,2.46,P < 0.05).There were significant differences in the levels of ALT,AST,prealbumin,choline esterase,C-reactive protein,IgG,IgM,IgA,CD3 +,CD4 +,CD8 + and CD4 +/CD8 + at postoperative day 7 between the 2 groups (t =4.83,2.79,5.58,2.12,3.10,2.53,4.17,2.35,3.45,2.98,3.12,3.75,P <0.05).The body weight of patients at postoperative day 7 and duration of hospital stay in the control group were (57 ± 6) kg and (9.9 ± 1.5) days,which were significantly different from (61 ± 8) kg and (8.8 ± 1.3) days in the testing group (t =3.67,2.71,P < 0.05).Conclusion Structured triglyceride can effectively improve the recovery of hepatic function,nutrition status and immunologic function and reduce inflammatory reaction for the patients with primary liver cancer after hepatectomy.
