Objective To investigate the clinical value of 64 slice spiral CT low dose chest scan in the diagnosis of pulmonary tuberculosis.Methods From January 2015 to January 2016,45 patients with suspected pulmonary tuberculosis were studied.All patients underwent routine chest CT scan and low dose chest CT scan.To compare the difference of radiation dose related parameters between normal dose chest CT scanning and low dose chest CT scan.To observe the display of the characteristics of lung disease by routine dose chest CT scanning and low dose chest CT scan.Results In 45 patients,there were a total of 30 patients were diagnosed with tuberculosis.The diagnostic sensitivity,specificity and diagnostic coincidence rate of conventional CT scan and spiral CT low-dose scan were 93.3% and 86.7% respectively.In low dose CT scanning radiation dose related parameters,the effective dose [(1.36±0.22)mSv vs.(5.24±0.68) mSv],CT dose index[(31.8±3.7) vs.(51.2±6.3)] and dose length product[(35.4±5.4)mGycm vs.(137.9±26.1) mGycm]were significantly lower than those of the conventional dose CT scan,the differences were statistically significant (all P<0.05).Conventional dose and low dose CT scan could show the lung lesions of pulmonary tuberculosis.Comparison between the two groups,low dose CT scanning on the burr sign and grinding glass shadow display rate was low.Compared with the conventional dose CT scanning,the difference was statistically significant (P<0.05).Conclusion In the diagnosis of pulmonary tuberculosis,the radiation dose of 64 slice spiral CT is significantly decreased,be able to clear most of the lung lesions.However,the effect is poor in the display of the grinding glass and the thin and short burr.
3.Effects of acupuncture combined with Brunnstrom staging on upper-limb motor function, cerebral arterial blood flow velocity, and brain function remodeling after stroke

Objective: To explore the effects of acupuncture combined with Brunnstrom staging on upper-limb motor function, cerebral arterial blood flow velocity, and brain function remodeling after stroke. Methods: A total of 77 patients after stroke were selected between January 2017 and December 2019 to perform a prospective study. All cases were divided into an observation group and a control group by the random number table method. Both groups were treated with conventional symptomatic treatment for stroke and functional exercise according to the Brunnstrom staging. The observation group was treated with additional acupuncture treatment based on the Brunnstrom staging therapy. The therapeutic efficacy was evaluated after six-week treatment. The traditional Chinese medicine (TCM) symptom scores of dizziness, headache, limb numbness, and language disorders before and after the treatment in the two groups were compared. According to the Fugl-Meyer motor function assessment (FMA), the upper-limb motor function before and after the treatment was compared. And the average systolic blood flow velocity of the anterior cerebral artery (ACA), middle cerebral artery (MCA), and posterior cerebral artery (PCA) in the two groups before and after the treatment were compared. According to the modified Edinburgh-Scandinavia stroke scale (MESSS), the nerve function before and after the treatment was compared. Results: The total effective rate of the observation group was 94.9%, significantly higher than 79.0% of the control group (P<0.05). After the treatment, the TCM symptom scores of dizziness, headache, limb numbness, and language disorders in both groups decreased, and the scores in the observation group were all significantly lower than those in the control group (P<0.05). The FMA score of upper limbs in both groups increased, and the score in the observation group was significantly higher than that in the control group (P<0.05). The average systolic blood flow velocities of the ACA, MCA and PCA in both groups increased, and were significantly higher in the observation group than in the control group (P<0.05). The MESSS score in both groups decreased, and the score in the observation group was significantly lower than that in the control group (P<0.05). Conclusion: Acupuncture combined with Brunnstrom staging is effective for patients after stroke. It can effectively improve the upper-limb motor function and cerebral artery blood flow velocity, promote brain function remodeling, and restore nerve function.
4.Changes of plasma calcitonin gene-related peptide and endothelin levels in patients with pregnancy-induced hypertension

Objective:To evaluate the effect of the plasma cyclic nucleotide in patients with pregnancy-in-duced hypertension. Methods :To observe the level of the plasma cAMP by radio competitive protein bind-ing assay and level of the p lasma cGMP by radioimmunoassay (RIA). Results:The contents of the plasmacAMP and cGMP were significantly higher in PIH group than those in the controls. Plasma cAMP showedthe negative correlation to postpartum MAP. Conclusion:cAMP and cGMP had synergy on antagonizingthe contraction evoked by Ca2+. This was an effective compensatory protective mechanism of PIH.
5.Association study of IL-10 gene promoter polymorphisms and efficacy of risperidone treatment of schizophrenic patients with first episode

Objective To investigate association of IL-10 gene promoter polymorphisms with first episode schizophrenic and efficacy of risperidone.Methods 513 schizophrenic patients with first episode(patient group) and 627 health controls(control group)participated in this study.Genotyping for IL-10 gene label single nucleotide polymorphism(SNP) sites-592A/C (rs1800872),-1082A/G (rs1800896),-819T/C (rs1800871) were performed by polymerase chain reaction and restriction fragment length polymorphism (PCR-RFLP)genotyping technology.513 patients were administered orally risperidone for 8 weeks.Clinical efficacy were evaluated with PANSS.Results The frequences of rs1800896 genotypes (332:166:15,453:161:13 ; x2 =7.55,df=2,P=0.02) and alleles(830:196,1067:187; x2 =7.09,df=1,P=0.008,OR=1.35,95% CI=1.08 ~ 1.68) showed significant differences between patient group and control group,however only allele differences showed statistical significance after Bonferroni correction (P=0.024).The frequencies of rs1800872and rs1800871 genotypes and alleles had no significant differences between patient group and control group(P>0.05).There were no significant differences of haplotype frequences between patient group and control group (P> 0.05).There were no significant differences of genotypes (134:209:38,61:59:12;x2 =5.16,df=2,P=0.08;244:129:8,88:37:7;x2 =4.57,df=2,P=0.10; 188:158:35,54:64:14;x2=2.80,df=2,P=0.25) and alleles (477:285,181:83;x2 =3.03,df=1,P=0.08;617:145,213:51 ;x2 =0.01,df=1,P=0.92;534:228,172:92;x2 =2.22,df=1,P=0.14) frequences of rs1800872,rs1800896,rs 1800871 between effective group (381 cases) and invalid group (132 cases) of Risperidone efficacy (P> 0.05),while ATC haplotype frequences had significant difference between two groups(x2 =7.501,P=0.006,OR (95% CI) =2.420(1.262-4.640)).Conclusion IL-10 gene rs1800896 polymorphism may be associated with susceptibility to schizophrenia.The clinical curative effect of risperidone in the treatment of schizophrenia is associated with ATC haplotype,ATC haplotype may be good predictors to the overall curative effect of risperidone.
6.Expression of cytokines in the degenerative cartilages and synovial tissues of osteoarthritis patients

BACKGROUND:The inflammation of osteoarthritis is mediated by chondrocytes and the synovial tissue-secreted cytokines. Articular cartilage and synovial tissue contains a variety of cytokines, which play important regulatory role in the repair of articular cartilage injury.
OBJECTIVE:To analyze the relationship between chondrocytes, synovial tissue-secreted cytokines and osteoarthritis, as wel as the effect of chondrocytes and synovial tissue-secreted cytokines on osteoarthritis.
METHODS:A computer-based search was conducted in Wanfang database (www.wanfangdata.com.cn), PubMed database (www.ncbi.nlm.nih.guv/pubmed) by the first author from 2005 to 2010. The key words were“osteoarthritis, degeneration, cartilage tissue, cytokines”. A total of 146 articles were obtained through computer search, then reading the titles and abstracts for initial screen, 86 articles with the unrelated research purposes were eliminated, 40 articles with the repetitive content were excluded, and final y 21 articles related to the effect of various cytokines in the degenerative cartilage and synovial tissue of the osteoarthritis patients were included for further analysis.
RESULTS AND CONCLUSION:The cytokines mainly refers to the activated immune cel s and some stromal cel-secreted non-specific regulate immune responses and inflammatory responses-mediated smal molecule proteins, including lymphokines produced by the lymphocytes, as wel as the single nuclear factor and other cytokines produced by monocytes macrophages. Synovial cel-secreted cytokines may partial y explain the pathological process of osteoarthritis and play an important role in joint inflammation. Although, more and more scholars have pay attention to the effect of synovial cel s and cartilage cel-secreted cytokines, but mainly studies the effect of exogenous cytokines on chondrocytes or synovial cel s, and the effect of endogenous cytokines in the pathogenesis of osteoarthritis has not been extensively studied.
7.Correlation between vitamin D and liver cirrhosis classification in hepatic diabetes

Objective To observe the correlation between vitamin D and liver cirrhosis classification in hepatic diabetes.Methods Thirty-eight patients with hepatic diabetes,48 patients with type 2 diabetes and 30 cases of healthy controls were collected.The level of serum 25-(OH)D3 was determined,and the Child-Pugh score was calculated.The correlation between pathogenesis and pathogenetic condition of hepatic diabetes was analyzed.Results The levels of serum 25-(OH)D3 in hepatic diabetes patients,type 2 diabetes patients and healthy controls were (38.23 ± 12.47),(63.33 ± 13.58) and (86.14 ± 16.25) μ mol/L,and there was statistical difference (P < 0.05).There was negative correlation in hepatic diabetes between serum 25-(OH)D3 level and liver cirrhosis Child-Pugh score (r =-0.363,P < 0.05).Conclusion Low level of vitamin D may be one of the pathogenesis of hepatic diabetes.
8.The serum levels of creatine kinase, brain natriuretic peptide, and neuropeptide Y in severe pneumonia pediatric pa- tients combined with heart failure

Objective To investigate the levels of creatine kinase (CK-MB), brain natriuretic peptide (BNP), and neu-ropeptide Y (NPY) in severe pneumonia pediatric patients combined with heart failure (HF). Methods Pneumonia pediatric patients admitted from December 2010 to December 2014 had been enrolled and divided into pneumonia group (P) (n=32), se-vere pneumonia group (SP) (n=20) and severe pneumonia combined HF group (HF) (n=36). Twenty healthy children served as control group (C). Serum level of CK-MB was detected by enzyme rate method and were measured by ELISA. BNP and NPY were measured again during the recovery period in 18 cases in HF group. Results The serum levels of CK-MB, BNP and NPY were signiifcantly different among the four groups, (F=25.19~277.94, P<0.01). The serum levels of CK-MB, BNP and NPY were signiifcantly higher in HF group than in SP, P and C group (P<0.05). The serum level of CK-MB was signiifcantly higher in SP group than that in P and C group (P<0.05) and there was no difference between P and C group (P>0.05). The serum levels of BNP and NPY were not statistically signiifcant between SP and P and C group (all P>0.05). In HF group, the serum levels of CK-MB, BNP, and NPY were signiifcantly decreased in 18 cases after treatment. The serum level of BNP was positively correlat-ed with CK-MB and NPY (r=0.681, 0.525, all P<0.01) and the NPY and CK-MB levels were also positively correlated (r=0.545, P<0.01). Conclusions The Detection of the serum levels of CK-MB, BNP and NPY can help diagnose severe pneumonia com-bined with HF. The BNP maybe the most sensitive indicator.

10.TREATMENT OUTCOMES OF OPEN PELVIC FRACTURES ASSOCIATED WITH SEVERE PERINEAL INJURIES

Objective The aim of this study was to evaluate the management outcomes of open pelvic fractures asso-ciated with extensive perineal injuries.Methods We retrospectively studied 15 cases with open pelvic fractures associated with extensive perineal injuries (urethral and anal canal laceration) admitted between August 2009 and September 2013. Mechanism of injury, Injury Severity Score, associated injuries, hemodynamic status on arrival, resuscitation and transfusion requirements, operative techniques, intra-and postoperative complications, length of intensive care unit and hospital stay, and mortality were recorded in a computerised database for further evaluation and analysis.Results Two patients (13.3%) died; the first with a type Ⅲ anteroposterior compression (APC) pelvis fracture was due to uncontrolled hemorrhagic shock, while the latter was due to septicaemia.We had one case with acute renal failure which improved with medical treatment.One patient (6.6%) had sciatic nerve palsy associated with type Ⅱ lateral compression (LC) pelvis fracture.Five hemodynami-cally unstable patients (33%) underwent explorative laparotomy for suspected intra-abdominal injuries.All of these patients had intra -abdominal fluid based on FAST examination.Three of them had large expanding pelvic hematoma and two had spleen injury.Conclusion Open pelvic fractures with extensive perineal injuries of mortality rates are high.Early diagnosis and appropriate treatment, including reanimation, colostomy, cystostomy, vigorous and repeated irrigation and debridement, and fixation by an external fixator can improve the outcomes and reduce the mortality rate .
