1.Analysis of the curative effect of adrenaline injection combined with titanium clips by gastroendoscopy for acute nonvariceal upper gastrointestinal bleeding

Objective To evaluate the curative effect of titanium clips and adrenaline injection by gastroen-doscopy for acute nonvariceal upper gastrointestinal bleeding ( ANVUGIB). Methods 65 patients with ANVUGIB were randomly divided into group of titanium clips(31 cases) and group of injection combined with titanium clips(34 cases) for treatment of bleeding. The rate of curative effect of hemostatic and rehaemorrhagia was compared. Results All the patients of the two groups were succeed at the first time hematischesis, the hemostatic rates were all 100%. The titanium clips group has 6 cases rebleeding,and the rehaemorrhagia rate was 19.4% ,the injection combined with titanium clips group has 1 case rebleeding, and the rehaemorrhagia rate was 2. 9%. There was significant difference between the two groups. Conclusions The curative effect of injection with titanium clips was reliable, and was superior to treatment with titanium clips alone.
2.Preparation and in vitro Property of Voriconazole Sulfonated Butyl Ether-β-Cyclodextrin Inclusion Compound

Objective: To prepare voriconazole (VCZ) sulfonated butyl ether-β-cyclodextrin (SBE-β-CD) inclusion compound and study the properties in vitro.Methods: VCZ SBE-β-CD inclusion compound was prepared respectively by a stirring method, an ultrasonic method and a grinding method, and the one with the highest inclusion rate and inclusion compound yield was chosen as the final preparation method.The formula and preparation process were optimized by an orthogonal design.The inclusion compound was identified by differential scanning calorimetry and solubility determination, and the in vitro dissolution was determined as well.Results: The stirring method had the highest inclusion rate and inclusion compound yield, and the optimal preparation and formula conditions were as follows: the inclusion temperature was 25℃, the stirring time was 4 h and the amount ratio of VCZ to SBE-β-CD was 1∶1.After the optimization, the inclusion rate was (86.14 ± 0.69)%, and the yield of inclusion compound was (97.11 ± 0.31)%.After the inclusion, the characteristic peak of VCZ disappeared and the solubility of VCZ increased significantly.Compared with those of VCZ, the dissolution rate and amount of VCZ inclusion compound both increased notably.Conclusion: VCZ SBE-β-CD inclusion compound can be prepared by the stirring method, which lays foundation for the further studies on VCZ eye drop.
3.Correlation between postprandial hyperglycemia and cardiovascular disease

Objective: To investigate the relationship between postprandial glucose and cardiovascular disease.Methods: Ninety-four patients were divided into an IFG,an IGT and a DM group,and observed for the incidence of cardiovascular disease and the risk indexes of cardiovascular disease,such as CIMT,TC,TG,HDL-C,LDL-C,BMI,SBP,DBP and CRP.The correlation between postprandial glucose and the risk indexes were analyzed. Results: The risk indexes were obviously higher in the IGT and DM groups than in the IFG group(P0.01).Postprandial glucose was correlated positively with the risk indexes,but negatively with HDL-C.Conclusion: Postprandial hyperglycemia increases the incidence of cardiovascular disease.
4.Detection of interleukin-17 and tumor necrosis factor-alpha and synovium pathological changes in patients with rheumatoid arthritis and significance

ObjectiveTo investigate the levels of serum interleukin -17(1L-17) and tumor necrosis factor -alpha( TNF- α ) and synovium pathological changes in patients with rheumatoid arthritis ( RA ) and its clinical significance.MethodsThe levels of serum IL-17 and TNF- α in 50 patients with active phase of RA ( active phase of RA group),50 patients with stable phase of RA ( stable phase of RA group ) and 50 normal controls (NC group) were measured by enzyme-linked immunospecific assay(ELISA),and the synovium was detected by pathological examination.ResultsThe levels of serum IL- 17 and TNF- α in active phase of RA group [(42.60 ± 11.30),( 113.20 ± 13.11 ) mg/L]were significantly higher than those in stable phase of RA group [( 19.60 ± 5.75),( 14.50 ± 5.33) mg/L]and NC group [(7.40 ± 3.32),( 10.90 ± 2.24 ) mg/L](P <0.01 ),the level of serum IL-17 in stable phase of RA group was significantly higher than that in NC group ( P < 0.05),but the level of TNF- α had no significant difference between stable phase of RA group and NC group ( P > 0.05).The synovium was significantly different between RA patients and NC group.Conclusion IL-17 and TNF-α are more sensitive index for detecting the activity of RA and may play important rolesinmonitoring the therapeutic effects and prognosis of RA.
5.The effects of bone marrow mesenchymal stem cell transplantation on pancreatic fibrosis in rats with chronic pancreatitis

Objective To investigate the effects and mechanisms of bone marrow mesenchymal stem cells (BMSCs) on pancreatic fibrosis in rats of chronic pancreatitis. Methods Thirty healthy male SD rats were randomly divided into three groups:control group, model group and transplanted group (n=10 for each group). Chronic pancratitis rat model was induced by retrograde injection of oleic acid into the biliopancreatic duct. The sham operation group was treated only with solvent. Transplanted group was given BMSCs through caudal vein injection at 1 week and 5 weeks after the model induction. All rats were weighed at 1 week, 4 weeks and 8 weeks in three groups. After 8-week feeding,pancreatic tissues were harvested for HE and picric-sirius staining. The contents of transforming growth factorβ1 (TGF-β1), typeⅠcollagen and typeⅢcollagen were detcted by using ELISA. Results Compared with the control group, the weights of rats were decreased at 4 weeks and 8 weeks in model group and transplantated group (P<0.05). There were no significant differences in body weights between model group and transplantated group (P>0.05). The pancreatic fibrosis score and pathological injury were ameliorated signicantly in transplanted group. The contents of TGF-β1, typeⅠcollagen and typeⅢcollagen in pancreas were increased in model group than those of control group and transplanted group (P<0.05). Conclusion BMSCs transplantation can reduce the collagen secretion and reduce the degree of pancreatic fibrosis in rats with chronic pancreatitis, which may be related to the inhibition of the release of TGF-β1.

For the purposes of meeting the needs of COPD patients for health education, decreasing times and costs of re-hospitalization, and increasing patient satisfaction and nurses' working enthusiasm. In 2011, our hospital established a specialist nursing clinic for chronic obstructive pulmonary disease (COPD), and two nursesentitled-deputy directorswith rich clinical experiences for more than fifteen years and strong communication skills were se-lected to take charge of the clinic, and they were responsible for nursing assessment, making individualized pul-monary rehabilitation plan, health guidance, long-term follow up, etc., for discharged stable COPD patients and COPD patients with no need for hospitalization. Patient satisfaction was improved from 83% to 96% after implemen-tation.

Objective To determine the difference between target and measured concentration of remifentanil given by target-controlled infusion (TCI) and evaluate the performance of a new type Ⅰ TCI system for Chinese. Methods Thirty-six ASA Ⅰ or Ⅱ patients aged 40-60 yr weighing 50-70 kg undergoing elective lung resection were randomly divided into 2 groups according to target remifentanil concentration: group Ⅰ 6 ng ? ml-1 and group Ⅱ 8 ng?ml-1. The patients were premedicated with intramuscular midazolam 0.05 mg?kg-1 and atropine 0.5 mg. Anesthesia was induced with remifentanil and propofol both given by TCI. The target concentration of propofol (effect-site concentration) was set at 3 ?g?ml-1 and remifentanil (plasma concentration) at 6 or 8 ng? ml-1. When the patients lost consciousness, vecuronium 0.1 mg?kg-1 was given i. v. to facilitate intubation. The patients were mechanically ventilated and PETCO2 was maintained at 30-40 mm Hg. Anesthesia was maintained with TCI of propofol and remifentanil and intermittent i. v. boluses of vecuronium. Target plasma concentration of remifentanil remained unchanged during anesthesia. BIS value was maintained at 45-55 by modifying target propofol concentration. Arterial blood samples were taken before and 5, 10, 20, 40, 60, 90 and 120 min after TCI remifentanil was started for determination of blood remifentanil concentration by high performance liquid chromatography.The performance error (PE) was determined for each measured blood remifentanil concentration. The performance in the population was determined by median absolute performance error (MDAPE), median performance error (MDPE) and the wobble (the median absolute deviation of each PE from the MDPE).
