1.Tumor Patients in Hospital with Subsequent Bacteremia:An Investigation

OBJECTIVE To investigate the situation of the tumor patients with subsequent bacteremia in hospital in order to study the some prevention and controlling measures. METHODS Ten cases in the internal medical department for tumor with the central venous pressure apparatus were analyzed and had the symptom of chilling and hyperpyrexia , and then were carried through the germiculture for the central venous catheter. RESULTS Pathogenic microorganism had been isolated from the hemoculture. CONCLUSIONS Once the patients with the central venous pressure apparatus have the symptoms of unknown infection, it must be consider the possibility of contaminated tube and conduct countermeasures in time.
2.Early endoscopic intervention of acute nonvariceal upper gastrointestinal bleeding of senile patients

Objective To investigate the clinical value of early endoscopic intervention in diagnosis and treatment of acute nonvariceal upper gastrointestinal bleeding (ANVUGIB). Methods A retrospective analysis was performed of 150 senile patients with ANVUGIB who were intervened by early endoscopy or selective endoscopy from Janurary 2015 to September 2015. The causes of ANVUGIB were analyzed. The diagnosis rates, success rates of hemostasis, rates of rebleeding, surgery, hemostatic time, blood transfusion volume, hospitalization time, hospitalization cost were compared in early endoscopy group and selective endoscopy group. The quality of life was compared between the two groups by NHP after the patients left hospital for three months. Results Peptic ulcer, gastric cancer, acute hemor-rhagic gastritis were the three major causes of ANVUGIB. The diagnosis rate and successful hemostasis rates of early endoscopy group were significantly higher than that in selective endoscopy group ( < 0.05). The rates of rebleeding and surgery were significantly lower, the blood transfusion volume was reduced, hospitalization time were shorten, and hospitalization cost were lower in early endoscopy group ( < 0.05). The quality of life of early endoscopy group was statistically higher than that in selective endoscopy group ( < 0.05). Conclusion Early endoscopic intervention is safe, effective and identify hemorrhage cause fast in diagnosis and treatment in senile patients with ANVUGIB. It is worthy of widespread promoting.
3.Correlation between the central macular thickness and the visual function in patients with macular edema

Objective To determine the correlation between central macular thickness (CMT) and the visual function in patients iwht macular edema (ME). Methods The clinical data of 42 eyes of 40 patients with ME which were examined by optical coherence tomography (OCT) and microperimetry (MP-1) wereretrospectively analyzed. In 40 patients (42 eyes), diabetic ME (DME) was in 27 eyes,branch retinal vein occlusion was in 11eyes, and central retinal vein occlusion was in 4 eyes. All of the eyes had undergone OCT,MP-1 and best-corrected visual acuity (BCVA) test. Central macular thickness (CMT) was measured by fast macular scans using OCT. Retinal sensitivity (MS) and fixation patterns were evaluated by Mp-1.The position was chosen :2 disc diameters (DD) temporal to the disc and one third of a DD inferior to the centre of the disc. Results The correlation between CMT and BCVA is not significant (r=-0. 429, P=0. 069) as well as the correlation between CMT and MS (r=-0. 433,P=0. 058). The difference of CMT between the unstable and stable group was significant (F = 3. 262, P = 0. 039). The difference of CMT between the central fixation group and preferred retinal locus (PRL) group was significant (F=3. 173,P=0. 044). Conclusions BCVA and MS have no significant correlation with CMT. When CMT increases, the fixation stability decreases, fixation location, changes, and PRL occurs.
4.Advances in the Transformation to Small Cell Lung Cancer from Non-small Cell Lung Cancer Following Acquired Drug-resistance to EGFR Tyrosine Kinase Inhibitors

The use of epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) in the treatment of sensitive EGFR mutation in non-small cell lung cancer(NSCLC)has been proved significant curative effect.However,the ac-quisition of the drug resistance to EGFR-TKIs is almost inevitable, and common drug resistance mechanisms include T790M mutation,cMET amplification,etc.One of the rare resistance mechanisms of EGFR-TKIs is the transformation from NSCLC into small cell lung cancer (SCLC), which account for about 3%-15%. It is an important rare drug resistance mechanism which is not well understood. Therefore, it is necessary to review the present situation and the progress of the this drug resistance mechanism. This article summarizes these hypothesizes from two parts, which are respectively the "common origin" and"transformation time node". At present, two possible mechanisms of this kind of transformation has been proposed, which are respectively the hypothesis of the tumor heterogeneity and the hypothesis of the transformation from NSCLC into SCLC. This article also involves a lot of changes in the level of molecules,such as the lack of RB1 gene,the inactivation of P53 gene and the mutation of PTEN M264I gene,etc.At the same time,this article summarizes the characteristics,the diagnostic methods and the treatment strategy of this kind of transformation. There are still many problems which need further research and resolution.
5.Effect of altered oxidative stress system on liver function after partial splenic embolization

Objective To investigate the effect of altered oxidative stress system on liver function after partial splenic embolization(PSE).Methods Twenty-nine patients with liver cirrhosis and hypersplenism who received PSE were selected.Peripheral venous blood was drawn from the patients before and at 1 week,1 month,and 3 months after PSE,and the indexes of oxidative stress system factors including malondialdehyde(MDA),superoxide dismutase(SOD),advanced oxidiation protein products(AOPPs),and gluta-thione peroxidase(GSH-Px)were detected,as well as liver function indexes.Results There were positive correlation between SOD activity and total bilirubin(TBil)and model for end-stage liver disease(MELD)scores at 1 week postoperatively(TBil:r=0.725,P<0.05;MELD:r=0.764,P<0.05).There was positive correlation between GSH-Px activity and alanine aminotransferase(ALT)at 1 month postoperatively(r=0.777,P<0.05),however,the AOPPs was negatively correlated with ALT and aspartate aminotransferase(AST)at 3 months postoperatively(ALT:r--0.900,P<0.05;AST:r=-0.957,P<0.05).Conclusion PSE can improve the body oxidative stress system and enhance the body's antioxidant response,and then improve the liver function.
6.Minimally invasive radiofrequency ablation for Kasabach-Merritt phenomenon

Objective:To evaluate the therapeutic effect of radiofrequency ablation in the treatment of severe hemangioma with thrombocytopenia and coagulation abnormality (Kasabach-Merritt phenomenon, KMP).Methods:From April 2018 to April 2021, 11 cases of KMP in our department were retrospectively analyzed. The patients underwent MRI, color ultrasound and hematology examination. Radiofrequency ablation was performed for all patients under general anesthesia. The comprehensive, multi-level and multi-point ablation was accurately implemented under the whole-process intraoperative monitoring of color ultrasound. Postoperative ice pad was applied for the treatment area, and dressing change was carried out every day.Results:All 11 cases of KMP were treated with radiofrequency ablation, including 5 males and 6 females, aged 1 d to 2 months, with a median age of 15 d. Intraoperative color Doppler ultrasound monitoring showed that the tumor was completely ablated in the single operation. All patients were followed up for 0.5 to 2 years. The tumor was cured without recurrence. The local function was unaffected. The local scar formation was mild and the appearance was satisfactory.Conclusions:Radiofrequency ablation has great clinical value in the treatment of KMP. It makes up the shortcomings and avoids the disadvantages of traditional surgery and other conservative treatment methods. Good clinical results can be achieved by radiofrequence ablation with satifactory function and appearance.
7.Effect of silencing syndecan-1 on migration, invasion and angiogenesis of venous endothelial cells

Objective:To investigate the expression of syndecan-1 (SDC1) in human venous malformations and the effect of syndecan-1 (SDC1) silences on migration, invasion and angiogenesis of human umbilical vein endothelial cells (HUVECs).Methods:Pathological samples of venous malformation were selected from 20 patients with hemangioma surgically resected in the Third Affiliated Hospital of Zhengzhou University from December 2020 to June 2021 (10 males and 10 females, aged 1-57 years, median age 7 years ). HUVECs was purchased from American Type Culture Collection (ATCC ). Immunohistochemistry was used to detect the expression of SDC1 in venous malformations. The cytological experiments of HUVECs cultured in vitro were divided into two groups, transfected SDC1 small interfering RNA (siRNA) (si-SDC1 group) and control siRNA (control group), respectively. The siRNA with the best silencing effect was selected by real-time quantitative PCR and Western Blot method for follow-up experiments. Then the ability of HUVECs migration, invasion and angiogenesis was detected by scratch test, Transwell migration and invasion test and angiogenesis test, respectively.Results:SDC1 was positive in 13 of the 20 venous malformations and was mainly expressed in the cytoplasm of venous endothelial cells. Compared with HUVECs cells in NC group, the expression of SDC1 mRNA and protein in HUVECs cells in si-SDC1 group decreased, indicated that the synthesized siRNA targeting SDC1 could silence the expression of SDC1 , and the ability of HUVECs migration, invasion and angiogenesis were enhanced in si-SDC1 group.Conclusions:SDC1 was positively expressed in venous malformations. After silencing SDC1, the migration, invasion and tube formation ability of HUVECs were enhanced.

(3) The migration, invasion and angiogenesis abilities of HUVECs were enhanced in si-SDC1 group( P<0.05). Conclusions:SDC1 was expressed in most venous malformations. After silencing SDC1, the migration, invasion and tube formation abilities of HUVECs were enhanced.
