Hepatic resection has been the main form of the treatment of liver benign and malignant tumors. With the popularity of applications of minimally invasive techniques in surgery in recent years, laparoscopic hepatectomy(LH) application has also achieved a rapid development. It' s advantages are known as a small open incision, light systemic response and rapid recovery, meanwhile it can also attain the same efficiency as the open surgery does. However, there also exist disadvantages, such as complicated conduct, expensive instruments, and bleeding problems. This article reviews the process of development of laparoscopic liver resection, and elaborates its current status, existing problems, and evaluation of its treatment and efficiency.

The analysis of the results of the renal replacement therapies,renal transplantation for 23 cases,hemodialysis for 33 cases,in 56 elderly patients with chronic renal function failure was performed.The total mortality in transplanted group with a higher early mortality is lower than that in hemodialysis group.The living quality of transplanted group was better than of the hemodialysis group.The associated diseases prior to the replacement therapies were the contributions to the death.It was believed that the age has not been served as a limitation for elderly patients receiving renal transplantation. As a replacement therapies the operation should be choosed first by elderly patients.A strictly preoperative physical examination should be done and a postoperatively special cure plan that is suited to the elderly drawn up.

Objective To evaluate open acromioplasty for treating acromion impingement syndrome combined with rotator cuff injury. Methods From March 2004 to August 2007, 20 cases of acromion impingement syndrome combined with rotator cuff injury were treated in our department. During open acromioplasty, we repaired the rotator cuff, and reconstructed the insertion site with anchor screws for patients who had been diagnosed as complete rupture of superspinatus tendon. Postoperative rehabilitation program was conducted and all patients were followed up at our outpatient clinic. ASES (American Shoulder and Elbow Surgeons) scoring system was used to assess the outcome of the patients. Results Eighteen patients were followed up regularly for an average of 11 (6 to 13)months. The mean ASES score was 90. 5 postoperatively. A significant difference was found between the preoperative and postoperative ASES scores in this group (t= 13.865, P<0.01). Conclusion Open acromioplasty can achieve good outcome in treatment of acromion impingement syndrome combined with rotator cuff injury, and can be carried out by primary hospitals.

To determine energy compounds in myocardial tissue simultaneously, we determined standard Creatine(Cr), Creatine phosphate(PCr), Hypoxanthine(Hypo), Nicotinamide adenine dinucleotide (NAD), Adenosine monophosphate (AMP), Adenosine diphosphate (ADP), Adenosine triphosphate (ATP), Cyclic-adenosine monophosphate(cAMP), and other compounds related to energy metabolism by a rapid isocratic method.The result showed that standard curves of the eight compounds were linear and passed through the origo for all examined concentrations (r=0.999).A recovery (88.1～117.6%) and replication (CV%=0.97～3.78%) for the present method was found to be accord with methology request.

Developing changes of physiology and biochemistry in myocardium and brain tissue after myocardial infarction in rats was observed. The result showed that BP、 ?dp/dtmax and BBF were dccreased significantly at 6 hours after ischemia. The BBF was returned to normal after 72 hours but BP and ?dp/dtmax wcrc kept lower for 7 days. The MDA was clearly higher at 72 hours and returned to normal at 7th days in myocardium and brain tissue. BUt the NAGase reached to the peak at 7th days and was returncd to normal at 14th days. The changes of MDA and NAGase were correlated to the changes of heart fuetion after myocardial infarction. It suggested that cardiogcnic cerebral ischemia is a real fact.

OBJECTIVE:To study the effects of mifepristone on the expression of inflammatory factors in endometriosis (Ems)model rats. METHODS:SD rats were randomly divided into normal group(normal saline),model group(normal saline), mifepristone low-dose,medium-dose and high-dose groups [0.65,1.30,2.60 mg/(kg·d)],with 10 rats in each group. Except for normal group,Ems model was induced in other groups,and they were given relevant medicine intragastrically for consecutive 4 weeks. The volume of the ectopic focus was compared before and after treatment. The serum contents of TNF-α,IL-6 and IL-8 were detected by ELISA method. The phosphorylation of NF-κB p65 and the expression of COX-2 and PGE2 were detected by West-ern blot. The expression of NF-κB p65 mRNA in ectopic focus was detected by RT-PCR. RESULTS:Compared with normal group,the volume of the ectopic focus,the serum contents of TNF-α,IL-6 and IL-8,the expression of COX-2 and PGE2 protein, the phosphorylation of NF-κB p65 and the expression of NF-κB p65 mRNA in ectopic focus were increased in model group(P<0.01). Compared with model group,the volume of the ectopic focus,the serum contents of TNF-α,IL-6 and IL-8,the expression of COX-2 and PGE2 protein,the phosphorylation of NF-κB p65 and the expression of NF-κB p65 mRNA in ectopic focus were de-creased in mifepristone groups(P<0.01),in dose-dependent manner. CONCLUSIONS:Mifepristone can reduce the volume of the Ems ectopic focus,via blocking NF-κB signaling pathway and reducing the expression of inflammatory factors.

[Objective]To evaluate the results of displaced neck fractures with internal fixation(AO compress cannulated screws) and total hip replacement.[Method]Form 1998.5～2003.10,forty-four patients(mean age 75.2 years) who had an acute displaced femoral neck fracture were treated with internal fixation(21 hips) and total hip replacement(23 hips).The hip function,including the pain,movement and walking,was evaluated by Charnley Score(Charnley improved Merle d' Aubign? score).The health-related quality of patients' life was rated with use of the EQ-5D index.[Result]Forty patients were followed up at least twentyfour months.The ratio of complications in fixation group,defined as nonunion,osteonecrosis and peri-implant fracture,was 36.8%.The ratio of complications in total hip replacement group,defined as dislocation,periprosthetic fracture,or radiographic signs of loosening of the femoral or acetaular component,was 4.7%.There was significance different between two group(P

Objective To observe the effect of Ganshu Granule on different models of fatty liver in rats. Methods Rats with acute fatty liver induced by alcohol and carbon tetrachloride, rats with chronic fatty liver induced by alcohol and high-fat diet, and rats with acute fatty liver induced by ethionine were used, and the effects of different doses of Ganshu Granule on serum lipid metabolism, intrahepatic lipid metabolism and histopathological changes in liver were observed. Results In fatty liver rats induced by alcohol and carbon tetrachloride, Ganshu Granule could reduce the level of total cholesterol (TC), triglycerides (TG), aspartate aminotransferase (AST), alanine aminotransferase (ALT) and alkaline phosphatase (ALP) in serum and could decrease total fat and glycogen(Gn) in liver obviously. In fatty liver rats induced by alcohol and high-fat diet, Ganshu Granule could significantly decrease the level of TC, TG, ALP, LDL and ALT in serum and could reduce the content of total fat, TG and Gn in liver obviously. In fatty liver rats induced by ethionine, Ganshu Granule could decrease the level of TC, TG and ALT in serum, raise the level of HDL in serum and could decrease the content of total fat, TG and Gn in liver obviously. The histopathologic results showed the degree of hepatic steatosis and hepatocellular necrosis were lessened in all of the treated groups. Conclusion Ganshu Granule is effective in preventing and treating fatty liver.

BACKGROUND:Femoral tunnel suspensory fixation device for anterior cruciate ligament reconstruction has a certain clinical comparison. However, there are a few fixture researches on the expansion of bone tunnel and graft changes.
OBJECTIVE:To observe the short-period effect of femoral tunnel side fixtures Endobutton and Tightrope used in autologous hamstring anterior cruciate ligament reconstruction on bone tunnel and implant change.
METHODS:Twenty-three patients with anterior cruciate ligament injuries received one-stage hamstring reconstruction, including 13 cases of femoral tunnel fixation with Endobutton and 10 cases with Tightrope. Tibial tunnel used extrusion screws. We observed operation time, recent effects after operation (Lysholm knee score, IKDC score) and the postoperative change of femoral tunnel diameter and graft length.
RESULTS AND CONCLUSION:(1) Twenty-three cases were fol owed up for 9-12 months, averagely 10 months. (2) Joint motion was not limited in both groups. No significant difference in IKDC and Lysholm scores was determined between the two groups (P>0.05). (3) Graft length and femoral tunnel diameter were significantly higher in the Endobutton group during final fol ow-up compared with that at 1 week postoperatively (P<0.05). (4) These results suggested that the recent effects were similar between Tightrope and Endobutton fixation, but Tightrope fixation device reduces the length of graft and the extent of bone tunnel enlargement.

This thesis analyzes the current problems in the medical morality and ethics among medical staff,including the insufficiency of service conception,excessive emphasis on personal economic benefits,and the absence of basic morality,and points out the importance of medical morality and ethics construction.Concrete suggestions are also raised from the aspects of strengthening the moral education of serving the people,establishing an overall supervision mechanism,and generally improving medical service quality.
