1.China's social non-profit medical institutions:Motivations, challenges and countermeasures

Social forces participating in non-profit medical institutions will help to insist on the commonweal of public medical and health care and realize the goal of access to basic medical and health services for everyone .At present , social forces participating in non-profit medical institutions have made some prominent improvements under a series of policy support .However , due to the absolute dominance of public hospitals , social forces participating in non-profit medical institutions are still potting a lot of restrictions on access threshold of basic medical services , devel-opment space and policy implementation .In order to promote the healthy development of social forces participating in non-profit medical institutions , we should first recognize its significance and guarantee its development potential , tal-ents construction and development space through implementing preferential policies , improving supervision mecha-nisms, exploring multi-sited license, optimizing allocation of resources and so on .
2.Retrospective analysis on neuroradiological features of central neurocytoma

Objective To evaluate the neuroradiological features of intraventricular central neurocytoma in ten patients.Methods Ten patients with central neurocytoma were imaged using contrast-enhanced and non-enhanced MRI scans,including 4 were also scanned with non-enhanced CT.The imaging data were studied with regard to location,size,margin,signal intensity,enhancement characteristics and presence of calcifications.Clinical data(i.e.presenting signs and symptoms,physical findings and medical history) were collected and histopathological and immunohistochemical studies were performed.Results There were 4 male,6 female in our group.All lesions were located in the lateral ventricles.Three tumors were confined to the left side,three to the right side and four involved both lateral ventricles.The growth of central neurocytoma was of close spatial relation to the septum pellucidum.In MRI,most of the cases showed a heterogeneous hypointensity on T1WI and hyperintensity on T2WI or FLAIR with a well-defined margin.The presence of cystic components,necroses and calcifications had been the cause of these internal heterogeneities.After intravenous administration of gadolinium(Gd-DTPA) all tumors showed a heterogeneous enhancement.CT provided additional information by distinguishing intratumoral calcifications in all four evaluated cases.Conclusion Central neurocytoma should be considered when young patients with lesions in the lateral ventricle,which contain calcifications and show some enhancement.This is especially applicable for tumors involving both lateral ventricles with symmetrical growth around the center of septum pellucidum or for unilateral ventricular tumors with a wide base attachment to the septum pellucidum.
3.Assay of Hydrochlorothiazide Tablets by RP-HPLC

OBJECTIVE:To establish a RP-HPLC method for determination of the content of hydrochlorothiazide tablets.METHODS:Samples were determined by RP-HPLC on a ZORBAX SB-C 18 column with a mixture of phosphate buffer solution(pH=4.0)-methanol(80∶20)as mobile phase and UV detection at226nm.RESULTS:The calibration curves of hydrochlorothiazide was linear from0.006to0.303mg/ml(r=1.0000,n=5),the average recovery and relative standard deviation were100.1%and0.43%(n=9)respectively.CONCLUSION:The method is rapid,simple,accurate and precise.It can be used for assay of hydrochlorothiazide.
4.The relationship between homocysteine and cerebral infaction in patients with type 2 diabetes mellitus

Objective To explore the correlation between serum homocysteine (Hcy) level and diabetes mellitus combined with cerebral infarction (DMCI) in patients. Methods A total of 144 patients with type 2 diabetes mellitus (T2DM) were selected and divided into two groups, patients without cerebral infaction (group A, n=64) and patients with cerebral infaction (group B, n=80). Thirty healthy people were used as control group (group C). The serum Hcy level was detected by enzymatic cycling assay in three groups. The serum levels of cholesterol (TC) and triglyceride cholesterol (TG) were detected by enzymatic determination. The serum levels of high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C) were detected by homogeneous phase method. The serum level of creatinine (Cr) was detected by creatine oxidase method. The level of uric acid (UA) was detected by urinary enzyme peroxidase coupling method. Glycosylated hemoglobin (HbA1c) was detected by high performance liquid chromatography. The carotid artery intima media thickness (IMT) was examined by color Doppler ultrasound, and patients were divided into mild and no stenosis group, moderate stenosis group and severe stenosis group according to the
results. The levels of Hcy were compared between all groups. The correlation of Hcy level and other indicators was analyzed. Results The levels of Hcy and HbA1c were group C

6.Research progress on the anti-infectious mechanism of vitamin D

Clinical epidemiological studies have shown that vitamin D deficiency is associated with infectious diseases, which is confirmed by recent studies implying the role of vitamin D as a key regulator of host defense against infections. Bioactive vitamin D triggers antimicrobial pathways against pathogens in mucous membrane and cells of the human innate immune system. In addition, vitamin D also attenuates excessive inflammation and acquired immunity, and thus limits collateral tissue damage. The observed effects of vitamin D on infectious diseases, particularly airway infections, should be addressed in the management of viral infections. Findings from cross-sectional studies should be confirmed by prospective research. In this review, we summarize the current knowledge regarding the effect of vitamin D on innate and acquired host defense against infections.
7.Effects of bonding after microabrasion with normal acid etching time on losing ratio of orthodontic brackets to dental fluorosis

Objective To investigate the effects of losing ratio of orthodontic brackets bonding after prolonged etching time (prolonged etching time method) and microabrasion with normal acid etching time (etching after microabrasion method) to different degree of dental fluorosis.Methods A total of 30 patients with dental fluorosis were selected from the cases treated in the Department of Stomatology,Lianyungang Hospital of Traditional Chinese Medicine.They were classified as 3 groups from mild to severe degree according to Dean's criteria,10 people per group.Patients within each group were randomly assigned to 2 subgroups,5 people per subgroup;prolonged etching time method and etching after microabrasion method were used,respectively,in each subgroup.The total and first losing ratios of brackets at five time points (1,2,4,12 and 24 weeks) were observed.The losing ratio differences of orthodontic brackets between the two disposing measures bonding to different degree of dental fluorosis were analyzed.Resuts The total bracket loss rates were 38.3% (46/120) and 21.4% (25/117),respectively,in severe dental fluorosis disposed by prolonged etching time method and etching after microabrasion method.It was significantly higher than that of the same processing method of mild and moderate dental fluorosis groups,in which the total bracket loss rates were 12.5% (15/120),18.1% (21/116),7.6% (9/118) and 5.8% (7/120),respectively,and the differences were statistically significant (x2 =21.12,11.87,8.96,12.24,all P < 0.01).The total loss rate of etching after microabrasion method was lower than that of prolonged etching time method in mild dental fluorosis group,the difference was not statistically significant (x2 =1.56,P > 0.05).The total loss rates of etching after microabrasion method were lower than that of prolonged etching time method in moderate and severe dental fluorosis groups,and the differences were statistically significant (x2 =8.49,8.13,all P < 0.05).The medians of first bracket loss rates were 8.3% and 4.3%,respectively,in severe dental fluorosis disposed by prolonged etching time method and etching after microabrasion method.They were significantly higher than that of the same processing method of mild and moderate dental fluorosis groups,in which the medians of first bracket loss rates were 2.5%,4.3%,1.7% and 0.8%,respectively,and the differences were statistically significant (U =2.619,2.627,2.312,2.627,all P < 0.05).The average percentage of first bracket loss rates of etching after microabrasion method was lower than that of prolonged etching time method in mild dental fluorosis group,the difference was not statistically significant (U =-0.949,P > 0.05).The average percentages of first bracket loss rates of etching after microabrasion method were lower than those of prolonged etching time method in moderate and severe dental fluorosis groups,and the differences were statistically significant (U =-2.214,-2.410,all P< 0.05).Conclusions Whether in prolonged etching time method subgroup or etching after microabrasion method subgroup,the more serious injury of dental fluorosis is,the higher loss rate of orthodontic bracket will be.For the clinical treatment of mild and severe dental fluorosis,the effect of etching after microabrasion method is better than prolonging etching time method.
8.Posterolateral approach for Maisonneuve injury associated with posterior malleolus fracture

Objective To investigate the surgical treatment of Maisonneuve injury associated with posterior malleolus fracture under direct visualization via the posterolateral approach.Methods Between January 2008 and June 2015,27 patients with Maisonneuve injury associated with posterior malleolus fracture were treated at our department.They were 18 men and 9 women,aged from 20 to 60 years (average,40.1 years).All fractures and syndesmoses received open reduction under direct visualization and internal fixation via the posterolateral approach.The posterior malleolus fractures were fixed with screws or plate and the syndesmoses fixed using cortical bone screws.Functional results were evaluated using the Baird-Jackson ankle scoring system.Results The patients were followed up for 8 to 30 months (average,18 months).No wound infection,necrosis,dehiscence,lose of reduction or implant failure was found during follow-up visits.All cases got bony union at the last visit.The excellent and good rate was 92.6%.Conclusion Since the posterolateral approach allows good access to the Maisonneuve injury and posterior malleolus fracture under direct visualization,it facilitates accurate direct fracture reduction,stable fixation and early exercise.
9.Study on the relationship between ?-fibrinogen gene-455G/A polymorphism and cerebral infarction

Objective To study the correlation between ?-fibrinogen gene variation (-455G/A) and plasma fibrinogen level and cerebral infarction.Methods 86 hypertension patients with cerebral infarction(infarction group),85 hypertension patients without cerebral infarction(hypertension group) and 90 healthy people(control group) were enrolled in this trial. The polymorphism of ?-fibrinogen gene-455G/A located at the start of transcription from all subjects was analyzed by polymerase chain reaction and restriction fragment length polymorphism (PCR-RFLP) technique. Plasma fibrinogen level was measured with prothrombin time (PT) assay.Results The fibrinogen level in infarction group(4.82?0.26 g/L) was significantly higher than that in control group(4.37?0.19 g/L)( P 0.05).The A-455 allele frequency in infarction group was significantly higher than those in control group and hypertension group (all P 0.05). Conclusion Plasma fibrinogen level is affected by the ?-fibrinogen gene -455G/A polymorphism. The fibrinogen A-455 allele may be an independent risk factor of cerebral infarction.
10.Research advances in gene-enhanced cartilage tissue-engineering
