1.Recent progress on study of the selection of intraocular lens in the bilateral sequential cataract surgery

Bilateral sequential cataract surgery is very common in clinical,but the ophthalmologists often ignore how to choose the timing of operation and whether the first eye can be used to improve the intraocular lens (IOL) power calculation for the second one.Despite the refinements in IOL selection with the use of third-generation IOL formulas,5% of eyes still result in a postoperative refractive surprise of ± 1.00 D or more.This paper introduced the methods of using refraction error of the first eye to improve the IOL power calculation for the second eye,and the refractive error of the second eye can be improved by modifying the IOL power to correct up to 50% of the error from the first eye in details.Because of the variation of the postoperative refractive surprise within a month,the interval of three to four weeks between two eyes cataract surgery is highly recommended.These conclusions may provide the advantageous guidance for us in the selection of IOL in bilateral cataract surgery.
2. Pharmacokinetics of fondaparinux sodium in Chinese patients after major thoracic surgery

OBJECTIVE: To characterize the pharmacokinetics of a single dose of 2.5 mg fondaparinux sodium in Chinese patients after thoracic surgery. METHODS: Thirteen patients were enrolled in the study and were given a single dose of 2.5 mg fondaparinux sodium via a subcutaneous injection. Plasma concentrations of fondaparinux sodium were measured throughout a 24 h period. The pharmacokinetic analysis was performed using WinNonlin software. RESULTS: The following pharmacokinetic parameters were derived; t1/2(7.92 ± 3.13) (4.21-15.89) h, tmax(2.52 ± 0.93) (1.38-4) h, p-1 (0.73 ± 0.18) (0.45-1.02) mg · L-1, AUClast(6. 56 ± 2.19) (3.07-9. 75)mg · h · L-1, Vz/F(3.54 ± 1.00) (2.24-5.73) L, CL/F(0.34 ± 0.14) (0.16-0.63) L · h-1, and MRTlast (6.98 ± 2.1) h. The pearson correlation coefficient, r was 0.7283(P = 0.005) between the serum creatinine(Scr) and mean residence time(MRTlast) of fondaparinux. CONCLUSION: The disposition of fondaparinux is highly variable in patients. A statistical significant correlation between Scr and MRTlast of fondaparinux was observed. Fondaparinux sodium administered by intravenous injection is well tolerated in patients.
3.Review of arsenic trioxide effect on solid cancer

Clinical trials have demonstrated that arsenic trioxide(As2O3) was effective in the treatment of acute promyelocytic leukemia(APL), based on this facts, studies have been carried out to study its role in the treatment of solid cancer at home and abroad. Primary in vitro study showed that As2O3 could treat the cancer of digestive system such as hepatoma, esophageal carcinoma, gastric cancer, and the cancer of urinary and reproductive system such as cervical cancer, ovarian cancer, bladder transitional carcinoma,as well as lung cancer, breast cancer, prostate cancer, etc. The mechanism of As2O3 actions may be the induction of apoptosis, the inhibition of growth and the promotion of differentiation of solid cancers. This article reviews on As2O3 in treatment of solid cancer.
4.Transabdominal ultrasonography in the diagnosis of achalasia Transabdominal ultrasonography in the diagnosis of achalasia

Objective: To assess the clinical application of transabdominal ultrasonography for the diagnosis of achalasia. Methods: The subjects were divided into 3 groups, with 8 patients suffered from achalasia as patient group, 10 normal individuals and 8 patients with cardiac carcinoma as controls. All subjects received water provocative test to study the lower esophagus and cardia. Sonographic findings were compared between the 3 groups. After treatment by administration of drugs, pneumatic balloon dilation, or intrasphincteric injection of botulinum toxin, the 8 patients were re-examined by TAUS for longitudinal comparison. Results: (1)The normal subjects had no dilation of cardia and lower esophagus before drinking water, but their cardias opened rapidly with the drinking action. (2)All the patients of achalasia were found to have dilation of lower esophagus before taking water. During 10 min continuous TAUS scanning, 5 of 8 demonstrated absence of relaxation of cardias and absence of water from the esophagus into the stomach. The other 3 showed slight and intermittent relaxation of cardias with water flow into the stomach. Thirty minutes after water provocative test, all the 8 patients had water retardation within the lower esophagus. (3)The patients of cardiac carcinoma had an ultrasound feature of cardiac ring enlargement and thickening of cardiac wall without dilation of lower esophagus before water provocative test. Water passed the cardia more slowly. Conclusion: TAUS is a readily accepted examination. It can quickly give reliable diagnostic clues to achalasia. TAUS can also be used for making assessment of therapeutic effect on achalasia.
5.Analysis of the setup errors of a stereoscopic two-dimensional kilo-voltage XGS-10 system for head-and-neck region intensity-modulated radiotherapy

Objective To analyze the discrepancies between position adjustments obtained with the stereoscopic 2DKV XGS-10 system and the Varian OBI system for head-and-neck region IMRT treatments,and to compare for image acquisition and registration time.Methods CBCT images were obtained with OBI system and 2DKV images were acquired by XGS-10 system for 30 head-and-neck patients prior to Varian21EX IMRT treatment.The images were registered with planning image for localization,and position adjustments were given in LR,SI and AP directions,then the discrepancies between them were analyzed.On the comparison of the two different systems,the Pearson coefficient was used to analyzed the correlation and 95% CI analysis to discern the consistence.Results Analysis of images acquired for the 30 patients yielded the following results:position adjustments with XGS-10 system were (-1.03 ± 2.15) mm,(0.86 ± 2.59) mm,(0.42 ± 1.66) mm in LR,SI and AP directions,whereas (0.00 ± 1.68) mm,(1.53 ± 2.12) mm,(0.10 ± 1.54) mm with CBCT in LR,SI and AP directions.The discrepancies were (-1.03 ± 1.24) mm,(-0.68 ± 1.78) mm and (0.32±1.61) mm in LR,SI and AP directions.The correlation coefficients between them were 0.817,0.731 and 0.495 in LR,SI and AP directions.95% CI were (-1.47--0.59),(-1.32-0.04),(-0.26-0.90) mm.The average image acquisition and registration time were 10 s and < 15 s in XGS-10 system,with 3 min and 8 min in OBI system.Conclusions Both of XGS-10 system and OBI system could be used to improve patient position accuracy,but XGS-10 system could cut down the total time.
6.α7nAChR is involved in anti-inflammation of physiological concentrations of glucocorticoids

AIM:To explore the role of α7 nicotinic acetylcholine receptor (α7nAChR) in anti-inflammation of glucocorticoids (GCs) at physiological concentrations .METHODS: MTT assay was used to measure the viability of BV-2 cells, which were processed by hydrocortisone at different concentrations .On the basis of inflammatory model induced by LPS in BV-2 cells, experimental groups were divided as follows:(1) control;(2) LPS;(3) GCs+LPS;(4) methyl-lycaconitine ( MLA)+GCs+LPS.The levels of TNF-αand IL-1βin the cell supernatants were detected by ELISA .RE-SULTS:Hydrocortisone at concentrations of 2 000 and 1 000 nmol/L decreased the cell viability to (76.9 ±5.5)% and (90.8 ±7.3)%, respectively, indicating the cellular injury by GCs at over-physiological doses.LPS significantly induced the releases of TNF-αand IL-1βin a time-and dose-dependent manner in BV-2 cells.Hydrocortisone at physiological con-centrations (500 and 250 nmol/L) reduced the releases of TNF-αand IL-1βin BV-2 cells stimulated by LPS, and MLA at concentration of 10 nmol/L antagonized the anti-inflammatory effect of GCs .CONCLUSION:α7nAChR is involved in the anti-inflammatory effect of the physiological concentrations of GCs .
7.Relationship between IL-33 and childhood autoimmune disease

Interleukin-33 is regarded as the orphan receptor ST2 that belongs to the cytokine family of interleukin-1 in 2005. IL-33 is found to play a potentially pathogenic roles in the autoimmune disease, allergic disease, vascular disease, inflammatory disease and it might be a useful sera marker for the diagnosis and prognosis in related diseases. Here we make a review on the relationship between IL-33 and autoimmune disease in recent years.
8.Related substances in orlistat detected with UPLC-MS/MS.

The study aims to identify related substances in orlistat pharmaceutical substance from different production technologies. Advanced technologies including UPLC-QTOF-MS and UPLC-QTrap-MS were used and the structures or molecules of 10 kinds of related substances in orlistat were identified. The experimental data were valuable to the quality control and development of orlistat.
9.Characteristics and risk factors of thyroid dysfunction induced after interferon therapy in patients with chronic viral hepatitis

Objective To investigate the characteristics and risk factors of thyroid dysfunction induced after interferon therapy in patients with chronic viral hepatitis.Methods The clinical data of 96 chronic viral hepatitis patients received interferon therapy were analyzed retrospectively.Serological markers of thyroid function and thyroid autoantibodies changes were observed,and were followed up for 1 year after treatment.Logistic regression analysis was performed to evaluate risk factors of thyroid dysfunction.Results Among the 96 cases,84 cases didn't develop thyroid dysfunction after interferon therapy(normal group),and 12 cases developed thyroid dysfunction after interferon therapy (abnormal group),the incidence was 12.5% (12/96),there were 5 cases of Hashimoto's thyroiditis,3 cases of Graves disease,3 cases of destructive thyroiditis,and 1 case of non-autoimmune hypothyroidism.The time to develop thyroid dysfunction was 2-7 (3.8 ± 1.9) months after treatment,the duration of thyroid dysfunction was 1-11 (4.2 ± 0.9) months.Five patients received endocrine therapy,and 2 patients stopped interferon therapy.All patients regained normal thyroid function during 1-year followup after end of treatment.Multivariate Logistic regression analysis showed that female gender (OR =3.767) and anti-thyroid peroxidase antibodies (OR =1.117) were the independent risk factors for thyoid dysfunction.Conclusions Hashimoto's thyroiditis,Graves disease,destructive thyroiditis and non-autoimmune hypothyroidism are the main types of thyroid dysfunction induced after interferon therapy in patients with chronic viral hepatitis.The thyroid function and thyroid autoantibodies should be closely monitored during treatment in patients with chronic viral hepatitis receiving interferon therapy,especially in female and patients who have thyroid autoantibodies.
10.Application of enlargement of plastic surgery for dural closure with radial neuro-patch in standardize large decompressive craniectomy

ObjectiveTo explore the application of enlargement of plastic surgery for dural closure with radial neuro-patch in standardize large decompressive craniectomy (SLDC).MethodsTwenty-one patients (observation group) who needed a SLDC for various reasons were treated with radial incision in dura to release the high intracranial pressure and enlargement of plastic surgery for dural closure with radial neuropatch from March 2009 to June 2010.The neuro-patch were used as the dural substitutes.Twenty-one patients were treated with SLDC only for the similar reasons (control group).General healing effects were compared betweentwo groups.ResultsIn 6 hours and 72 hours after surgery,the rate of obvious effectiveness in observation group was significantly higher than that in control group [33.33%(7/21 ) vs.14.29%(3/21 ),33.33%(7/21 ) vs.19.05%(4/21 ) 、61.90%( 13/21 ) vs.28.57%(6/21 ),61.90%( 13/21 ) vs.38.10%(8/21 )](P < 0.05).The rate of intracranial infection,cerebrospinal leak and epilepsy in observation group [0,0,4.76% ( 1/21 )]was significantly lower than that in control group [14.29% (3/21),14.29% (3/21),19.05% (4/21)](P <0.05).The rate of subcutaneous or subdural cerebrospinal fluid accumulation,cenencephalocele,cerebral ventricular malformation,brain hernia through the skull window had no significant difference between two groups [19.05%(4/21 ),0,4.76%( 1/21 ),0 in observation group;23.81%(5/21 ),9.52%(2/21 ),9.52%(2/21 ),4.76%( 1/21 ) in control group) (P > 0.05 ).After followed up of 6-12 months,the rate of the well general outcome in observation group was significantly better than that in control group [85.71% ( 18/21 ) vs.71.43% ( 15/21 )](P < 0.05 ).The rate of moderate-critical function handicapped in control group was significantly higher than that in observation group [23.81%(5/21 ) vs.9.52%(2/21 )](P<0.05 ).ConclusionsThe SLDC is effective for decreasing the intracranial pressure,and the enlargement of plastic surgery for dural closure with radial neuro-patch can improve the efficiency and decrease the incidence of complicating diseases.This therapy can improve the patients' general quality of survival and is worth to be popularized in clinic.
