1.Recent progress on study of the selection of intraocular lens in the bilateral sequential cataract surgery

Bilateral sequential cataract surgery is very common in clinical,but the ophthalmologists often ignore how to choose the timing of operation and whether the first eye can be used to improve the intraocular lens (IOL) power calculation for the second one.Despite the refinements in IOL selection with the use of third-generation IOL formulas,5% of eyes still result in a postoperative refractive surprise of ± 1.00 D or more.This paper introduced the methods of using refraction error of the first eye to improve the IOL power calculation for the second eye,and the refractive error of the second eye can be improved by modifying the IOL power to correct up to 50% of the error from the first eye in details.Because of the variation of the postoperative refractive surprise within a month,the interval of three to four weeks between two eyes cataract surgery is highly recommended.These conclusions may provide the advantageous guidance for us in the selection of IOL in bilateral cataract surgery.
2.Optimization of coding sequences and expression of antimicrobial peptide magainin II in Escherichia coli and Pichia pastoris.

The antimicrobial peptide magainin II is expressed in the skin of the African clawed frog, Xenopus laevis, and exhibits a broad spectrum of antimicrobial activity as well as tumoricidal properties at low concentrations. In addition, magaininII plays a synergistic role during antimicrobial and tumoricidal processes with another antimicrobial peptide PGLa that is also expressed in Xenopus laevis. The optimized cDNA sequence of magainin II and magainin II-PGLa hybrid peptide according to E. coli or Pichia pastoris codon usage frequency were synthesized and sub-cloned into prokaryotic expression vector pGEX and Pichia pastoris secreted expression vector pPIC9k. The resulting recombinant plasmids were named as pGEX-magainin II and pPIC9k-magainin II-PGLa. The GST-magainin II fusion protein was highly expressed in E. coli. Furthermore, magainin II was successfully purified by digestion with PreScission Protease to cleave the GST tag. Additionally, our data obtained from the ELISA revealed that magainin II -PGLa hybrid peptide was successfully expressed in Pichia pastoris. These experiments establish a useful system for further studies of these antimicrobial peptides.

3.Cluster ensemble algorithm based on dual neural gas applied to cancer gene expression profiles.

The microarray technology used in biological and medical research provides a new idea for the diagnosis and treatment of cancer. To find different types of cancer and to classify the cancer samples accurately, we propose a new cluster ensemble framework Dual Neural Gas Cluster Ensemble (DNGCE), which is based on neural gas algorithm, to discover the underlying structure of noisy cancer gene expression profiles. This framework DNGCE applies the neural gas algorithm to perform clustering not only on the sample dimension, but also on the attribute dimension. It also adopts the normalized cut algorithm to partition off the consensus matrix constructed from multiple clustering solutions. We obtained the final accurate results. Experiments on cancer gene expression profiles illustrated that the proposed approach could achieve good performance, as it outperforms the single clustering algorithms and most of the existing approaches in the process of clustering gene expression profiles.

5.Clinical significance of changes in peripheral lymphocyte count after surgery in early cervical cancer

Objective To evaluate the clinical significance of changes in peripheral lymphocyte count after surgery in early cervical cancer.Methods The 123 patients with stage Ⅰ bl and Ⅱ al treated by abdominal type Ⅲ radical hysterectomy from May 2008 to December 2012 were reviewed.The median age of patients was 43 years old (range:30 to 66 years).The median follow-up was 25 months with a range of 5-61 months.Peripheral blood samples were obtained on pre-operative,post-operation day 3 and 7.The log-rank test was used to compare the homogeneity of progression-free survival functions across strata defined by categories of prognostic variables.The Cox proportional hazard model was used to assess the significance of potential prognostic factors for progression-free survival.Results Univariate analyses preoperative lymphocyte count (P =0.012) and lymph nodes metastases status (P =0.001) and parametrial invasion (P =0.013) were significant risk factors for progression-free survival rate.On multivariate analyses,preoperative lymphocyte count [hazard ratio (HR) =6.087,95% CI:1.743-21.251,P =0.005] and lymph nodes metastases status (HR =5.984,95% CI:1.803-19.802,P =0.003) were independent risk factor of progression-free survival rate.Conclusion Peripheral lymphocyte counts after cervical cancer surgery may be a important prognostic factor.
6.Effect of modified electroconvulsive therapy on working memory in schizophrenics

Objective To explore the effect of modified electroconvulsive therapy (MECT) on working memory in schizophrenia patients.Methods 2-back test paradigm was set to measure the performances of working memory.A total of 30 cases of schizophrenia were recruited and measured with accurate rate and reaction time on this tasks before and after treatment with MECT.Differences were analyzed between patients through MECT treatment and 30 patients through drug treatment.Results Repeated measure ment analysis of variance on the accurate rate ((66.70±9.82) %,(67.61±6.83) %,(80.72± 12.08) %) and reaction time((1415.69± 258.51) ms,(1328.65±236.08) ms,(1094.45± 259.44) ms) of 2-back test were significant among three times of MECT (F=19.163,P<0.001 ; F=13.084,P<0.001).Schizophrenia patient's performance of error rate and reaction time of 2-back test were higher at the end of 2 weeks after treatment((80.72± 12.08)%;(1094.45± 259.44)ms) than before MECT treatment ((66.70±9.82) % ; (1415.69±258.51) ms).The difference between them were significant(t =-4.369,P<0.001 ; t=5.596,P<0.001).Schizophrenia patients performance of error rate and reaction time of 2-back test were higher at the end of 2 weeks after treatment((80.72± 12.08)%;(1094.45±259.44) ms) than at the end of 1 day after treatment((67.61±6.83)%;(1328.65±236.08) ms).The difference between them were significant(t=-5.416,P<0.000; t=3.824,P=0.001).Schizophrenia patients performance of error rate of 2-back test were higher at the end of 2 weeks after treatment than drug treatment group ((73.67±10.41) %),and the difference between them were significant(t=2.424,P=0.018).Conclusion Working memory functions in schizophrenia patients may be improved in varying degrees with MECT.
7.The effects of valsartan on the proliferation,migration,p-ERK1/2 and p-P38 MAPK expression of vascular smooth muscle cells

Objective To investigate the effect of valsartan on the proliferation and migration of rat aortic smooth muscle cells and the expression of p-ERK1/2 ,p-P38 promoted by angiotensinⅡ (AngⅡ ) .Methods VSMCs from the rat thoracic aorta were cultured by attachment-block culture .VSMCs from3-5 passages were used .The proliferation of VSMCs was tested by MTT meth-od .Pipettetip wounding injury was used to access the migration of VSMCs .The expression of ERK1/2 ,p-ERK1/2 ,P38 and p-P38 of VSMCs was detected by Western Blot .Results (1) AngⅡ could significantly promote the proliferation of VSMCs .The prolifer-ation of VSMCs promoted by AngⅡ could be inhibited by Valsartan and PD98059 .Valsartan inhibited the proliferation of VSMCs in a concentration-dependence .SB23015 could significantly promote the proliferation of VSMCs .(2 ) The migration activity of VSMCs in the Ang Ⅱ was increased significantly .Valsartan and PD98059 inhibited Ang Ⅱ-induced migration of rat VSMCs and SB23015 was on the contrary .(3) Valsartan and PD98059 could inhibit AngⅡ-induced VSMCs intracellular p-ERK1/2 protein ex-pression ,and this role was enhanced by SB23015 .Valsartan and SB23015 could inhibit Ang Ⅱ-induced VSMCs intracellular p-P38 protein expression ,and PD98059 had no significant effect .(4)VSMCs had no significant effect on cell proliferation ,migration and p-ERK1/2 ,p-P38 protein expression under valsartan alone .Conclusion (1)Valsartan could inhibit Ang Ⅱ-induced proliferation and migration of VSMCs in a concentration-dependence .(2)The effects that valsartan inhibits the proliferation and migration of Ang Ⅱ-induced VSMCs might be related to its inhibiting Ang Ⅱ-induced p-ERK1/2 protein expression .(3)p-P38 might play a negative reg-ulatory role on the AngⅡ-induced p-ERK1/2 activation .
8.Role of n-3 polyunsaturated fatty acid in the combined treatment of breast cancer

Breast cancer is a type of heterogenetic neoplasm.Metabolic dysfunction plays a pivotal role in the pathogenesis and progress of breast cancer.N-3 polyunsaturated fatty acid (PUFA) could change fatty acid metabolism of breast cancer cells and modulate expression of oncogenes and anti-oncogenes through various ways.It may provide therapeutic opportunities in the treatment of chemotherapy,radiotherapy,endocrine therapy and molecule targeted therapy of breast cancer.
9.Endovascular treatment of lower extremity atherosclerosis obliterans

Atherosclerosis obliterans of lower extremity artery mainly involve the aorto-lliac artery,femoral-popliteal artery and below-knee arteries.At present the major treatment of the disease is endovascular treatment.Due to anatomical differences between the three arteries,the principles and effect of endovascular treatment are different.The endovascular treatment of iliac- femoral lesions,because of its patency rate not lower than surgery,has become the preferred way.On account of the high incidence of femoral- popliteal lesions and the dissatisfaction for patency rate,the endovascular treatment of femoral-popliteal lesions has become a hot research.Endovascular therapy in the below-knee arteries is still in the exploration,currently a small balloon-based.
10.Clinical analysis on 130 cases of primary tumor of the duodenum

ObjectiveTo investigate the clinical features,early diagnosis and therapeutic methods of primary tumor of the duodenum.MethodsThe clinical data of 130 patients of primary tumor of the duodenum who were hospitalized in Peaking University Third Hospital from 1974 to 2010 were summarized.There were 30 cases of benign tumors and 100 cases of malignancy. ResultsMore than 90% of primary tumors of the duodenum were located at the duodenal bulb and descendent part.59% of malignant tumors were located in the area of duodenal papilla. The mean diameter of benign tumors was less than 2 cm compared with more than 3 cm in malignancies. Upper abdominal pain,jaundice,black stool,intestinal obstruction and abdominal mass were among the most common clinical manifestations. Jaudice was most common in papilla tumors,and intestinal obstruction and palpable abdominal mass often indicate advanced stage.Duodenofiberscopy combined with air barium double contrast radiography increase the diagnosis rate.Among the 30 benign cases,11 cases underwent simple tumor resection,14 cases received endoscopic tumor resection and 3 cases underwent pancreaticoduodenectomy.Resection rate and the 5-year survival rate was 100%. Of the 100 malignant cases,56 cases underwent standard pancreaticoduodenectomy,3 cases received pylorus preserving pancreaticoduodenectomy,9 cases underwent simple tumor resection and 12 cases received segmental duodenectomy.Resection rate was 84%.The 5-year survival rate was 46% in 59 cases receiving Whipple's procedure.ConclusionsPrimary tumor of the duodenum lacks specific clinical features. Simple tumor resection and segmental duodenectomy can be done for benign cases, while pancreaticoduodenectomy is the therapy of choice for malignant cases.
