1.Identification of BAC clones for array CGH in human cancer by FISH technique

Objective To evaluate the quality of BAC clones for CGH microarrays in the detection of tumors.(Methods Chromosome) preparations were made from peripheral blood of the healthy volunteers in the conventional manner.The locations of BAC(RPCI-11 library and CTC library) within the region of interest were compiled from information archived by the UCSC and the NCBI.Probes were labeled by nick-translation with biotin-16-dUTP or digoxigenin-11-dUTP.Precise localization of each BAC was confirmed using normal metaphase chromosomes by FISH technique.The copy number and molecular organization of the region of 223 BAC clones which were crucial in the development and progression of human cancers were investigated.Results The FISH analysis indicated the normal BAC clones accounted for 81.62% of the total(186/223);the abnormal clones with additional FISH noises accounted for 13.45%(30/223);those with wrong localization pattern was 3.58%(8/223),and those with no bacterial growth was 1.35%(3/223),respectively.Conclusion FISH technique is effective and useful in the identification of BAC clones for array CGH.
2.Analysis of therapeutic effect ofShengji-Yuhong ointment in the treatment of patients with pressure ulcer

Objective To analyze the therapeutic effect ofShengji-Yuhong ointment in the treatment of patients with pressure ulcer.Methods 86 patients of pressure ulcer were randomly divided into a control group and an observation group, with 43 cases in each. After debridement, the wound was covered with vaseline gauze in the control group, whileShengji-Yuhong ointment in the treatment group. 10 days constituted 1 course of treatment, and both groups were treated for 3 courses. The blood supply of the whole blood viscosity, plasma viscosity, erythrocyte aggregation index detection; white blood cell count (WBC), erythrocyte sedimentation rate (ESR), and C reactive protein (CRP) were observed in order to observe the control condition of the patients with wound infection.Results The total effective rate was 95.3% (41/43) and 74.4% (32/43) in the observation group and control group respectively, with significant difference between two groups (χ2=5.800,P=0.016). After treatment, the whole blood viscosity (high-shea) (4.06 ± 1.38 mPa?svs. 4.74 ± 1.62 mPa?s,t=2.095), the whole blood viscosity (medium-shea) (3.71 ± 1.22 mPa?svs. 4.34 ± 1.41 mPa?s,t =2.216), blood reduction viscosity (1.13 ± 0.22 mPa?svs.1.44 ± 0.51 mPa?s,t=3.660), the whole blood viscosity (medium-shea) (4.16 ± 0.48 mPa?svs. 4.51 ± 0.89 mPa?s,t=2.270) obviously compared with group before treatment decreased (P<0.05). The patients in the observation group in the whole blood viscosity (medium-shea) (3.71 ± 1.22 mPa?svs. 4.16 ± 0.48 mPa?s,t=2.251), and blood reduction viscosity (1.13 ± 0.22 mPa?svs. 1.32 + 0.31 mPa?s,t=3.278) in the observation group were obvious better than the control group (P<0.05). After the treatment the WBC, CRP, ESR in the observation group were decreased significantly than the control group (t=5.947, 7.198, 12.064,P<0.01).ConclusionShengji-Yuhong ointment can effectively control the PU infection in the wound, improve wound tissue under the blood circulation, and promote wound healing.
3.Ventral bladder mucosa onlay graft urethroplasty for the treatment of panurethral stricture

Objective To investigate the efficacy of ventral bladder mucosa onlay graft urethroplasty for the management of panurethral stricture.Methods From August of 2005 to July of 2013,11 cases of panurethral stricture were treated by ventral bladder mucosa onlay graft urethroplasty.The median age of the patients was 53 years (22-72 year),The median stricture length was 15 cm (12-18 cm).The patient was placed in the lithotomy position,Penile urethra was exposed by circumcoronal incision and degloving of skin,Bulbar urethra was exposed by inverted Y-shaped perineal incision.The strictured urethral segment was then opened ventrally in the midline up to at least 1 cm proximally into the healthy urethra.An appropriate size bladder mucosa graft was harvested,and was quilted to the splited urethra edge,the graft width was 1.5-2.0 cm.Two F10 fenestrated silicone catheters were left as urethral stents,a suprapupic cystostomy tube was left.The urethral stent was removed 4 weeks postoperatively.Follow-up was performed every 3 months for the first year,and annually thereafter.Success was defined as normal voiding with a maximum flow rates ≥ 15 ml/s,and the patients required no further instrumentation,including dilation or urethrotomy.Results The mean follow-up was 18 months (range,9-36 months),the overall success rate was 10/11.One patient developed urethral meatus stenosis 3 months postoperatively,and was managed by meatal dilatation.Conclusion Ventral bladder mucosa onlay graft urethroplasty can be used for the management of panurethral stricture,Bladder mucosa is an alternative substitution for complex urethral reconstruction.
4.Explorations on improving the quality of medical genetics teaching for international students

The teaching methods were explored to improve the quality of medical genetic teaching for foreign students according to the common problems during the teaching process. The negative effects of communication barriers in medical genetic teaching could be reduced by interactive teaching or problem-based learning in groups,in which the ability to resolve problems by themselves could be improved. In order to improve the teaching systematicness and teaching quality,the teaching contents in class should be from simple to deep,covering genetic laws,pathogenesis,diagnosis and control measure of genetic diseases. From the perspective of practical application and combining with the construction of self-de-signed teaching textbook and cases, the quality of medical genetic teaching ultimately could be further improved.

7.Enlightenment from the surveillance in post extreme emergencies and disasters

Objective The detailed analysis of the surveillance in post extreme emergencies and disasters (SPEED) provides practical reference for China to establish a disaster medical rescue information monitoring system with Chinese characteristics. Methods The SPEED system under the scene of disaster medical rescue information monitoring is analyzed in detail. The SPEED system design, work flows, system implementation and other aspects are analyzed and summarized in this paper, and suggests the enlightenment of SPEED system for Chinese disaster medical rescue information monitoring work. Results The SPEED system is an information monitoring system for the early stages of disasters. It provides monitoring for diseases caused by disasters, and life and health trends. It has a complete data collection mechanism, a comprehensive personnel training system, a complete system function, and an implementation strategy involving multi-layer, multi-region, and multi -sector. It is a powerful tool for disaster medical rescue and management personnel to obtain information in time. In the field of disaster medical rescue, a similar public-facing information monitoring system in China is still not perfect. Conclusion Learning the design flows and establishment mode of the SPEED system can provide reference for China to establish a disaster medical rescue information monitoring system with Chinese characteristics.
8.Establishment of a rat osteoporotic model by ovariectomy combined with glucocorticoid exposure

BACKGROUND:There are many postmenopausal women taking hormone, which leads to much loss of bone mass, further inducing fragility fractures. The studies on the hormone exposure combined with ovariectomy-induced osteoporotic model are still immature, and the related molecular mechanism remains unclear. OBJECTIVE: To establish the rat osteoporotic model induced by ovariectomy combined with glucocorticoid exposure and to explore the underlying molecular mechanism. METHODS: Thirty 3-month-old female Sprague-Dawley rats were randomly divided into blank control, sham and model groups (n=10 per group). The rats in the blank control group received no intervention; rats in the sham group were clipped off a little of coeliac adipose tissue; the model rats received bilateral ovariectomy and 4-week administration of glucocorticoid. RESULTS AND CONCLUSION:At 4 weeks after modeling, compared with blank control and sham groups, the model group showed significantly lower bone mineral density of the femur, number of bone trabeculae and bone volume/total volume, and significantly wider bone trabecular spacing. Additionally, the model group revealed the damaged bone trabecular structure and thiner cortical bone. The expression level of Runx2 was downregulated whereas both collagen type 1α1 and peroxisome proliferators activated receptor γ mRNA were upregulated in the model group. These findings suggest that ovariectomized rats exposed to glucocorticoid rapidly develop femur osteoporosis, maybe by downregulating the expression of Runx2, as well as upregualting collagen type 1α1 and peroxisome proliferators activatedreceptor γ mRNA.
9.Analysis of core indicators of fatal gastrointestinal rebleeding in emergency from the perspective of big data

Objective To explore a method of screening the core indicators in the emergency database that can be used to evaluate the in-hospital fatal gastrointestinal rebleeding by using the big data algorithm. Methods Based on the emergency database of the Chinese PLA General Hospital, through the big data retrieval technology, all the 647 patients diagnosed as gastrointestinal bleeding in the emergency database were enrolled, except those who were admitted to the hospital for the first time and whose hemoglobin (Hb) was less than 90 g/L or did not undergo Hb test. Among them, there were 313 in the rebleeding group (fatal rebleeding in the hospital) and 334 in the non-rebleeding group (no fatal rebleeding in the hospital). General data of patients were collected, including gender, age, physical signs, blood gas, test index collection data, and the identification of gastrointestinal rebleeding. The fusion algorithm of rough set algorithm, genetic algorithm, and cellular automaton algorithm were used to calculate the key indicators that affect gastrointestinal rebleeding. Results A total of 499 indicators were calculated by machine fusion algorithm, after screening 5 times repeatedly, 24 key indicators were screened out, 3 of which were vital signs, including systolic blood pressure (SBP), diastolic blood pressure (DBP), temperature (T); 7 key indicators of blood routine, including white blood cell count (WBC), eosinophil (EOS), monocyte (MONO), Hb, hematocrit (HCT), red cell distribution width (RDW), mean corpuscular hemoglobin (MCH); 3 key indicators of coagulation, including prothrombin time (PT), plasma fibrinogen (FIB), activated partial thromboplastin time (APTT); 5 key indicators of biochemical, including myoglobin (MYO), chloride, glucose (GLU), serum albumin (ALB), total bilirubin (TBil); and 6 key indicators of blood gas, including pH, lactate (Lac), oxygen saturation (SO2), base excess (BE), bicarbonate (HCO3-), partial pressure of carbon dioxide (PaCO2). Conclusions Using big data technology, 24 core indicators for evaluating the fatal gastrointestinal rebleeding in hospitals can be screened out from the emergency database, providing new ideas and methods for clinical diagnosis of the disease.
10.A cross-disciplinary collaborative "Datathon" model to promote the application of medical big data

Medical practice generates and stores immense amounts of clinical process data, while integrating and utilization of these data requires interdisciplinary cooperation together with novel models and methods to further promote applications of medical big data and research of artificial intelligence. A "Datathon" model is a novel event of data analysis and is typically organized as intense, short-duration, competitions in which participants with various knowledge and skills cooperate to address clinical questions based on "real world" data. This article introduces the origin of Datathon, organization of the events and relevant practice. The Datathon approach provides innovative solutions to promote cross-disciplinary collaboration and new methods for conducting research of big data in healthcare. It also offers insight into teaming up multi-expertise experts to investigate relevant clinical questions and further accelerate the application of medical big data.
