1.Xiyanping aerosol inhalation for pulmonary infection in patients with acute stroke

Objective To investigate the therapeutic effectiveness of Xiyanping aerosol inhalation for pulmonary infection in patients with acute stroke. Methods Sixty-three patients with pulmonary infection after acute stroke were enrolled and randomly allocated to either a treatment group or a control group. The control group was treated with anti-infective and apophlegmatisant on the basis of standardized stroke treatment, the treatment group was treated with Xiyanping aerosol inhalation on the basis of the treatment in the control group. All patients were treated for 14 days. The resolution times of symptom and signs, and the changes of inflammatory parameters were compared. Results The total effective rate in the treatment group was significantly higher than that in the control group (93.75%vs. 74.19%;χ2=4.510, P=0.034). The resolution times of symptom and signs, such as cough and excessive phlegm (5.94 ± 1.25 d vs. 6.73 ± 1.48 d;t=2.292, P=0.025),fever (2.72 ± 0.11 d vs. 3.25 ± 0.18 d;t=12.046, P<0.01), pulmonary rale (5.22 ± 1.15 d vs. 7.21 ± 1.21 d;t=6.693, P<0.01), and dyspnea (1.17 ± 0.49 d vs. 2.82 ± 0.61 d;t=11.855, P<0.01) in the treatment group were significantly shorter than those in the control group. White blood cell count (2.55 ± 1.02 ×109/L vs. 3.91 ± 1.25 ×109/L;t=4.738, P<0.01), granulocyte proportion (4.92% ± 1.32% vs. 6.25% ± 1.16%; t=4.738, P<0.01) and lymphocyte proportion (58.62%± 6.15%vs. 65.94%± 6.17%;t=4.716, P<0.01) in the treatment group were significantly lower than those in the control group. Conclusions Xiyanping aerosol inhalation can shorten the resolution times of symptom and signs, and improve the therapeutic effectiveness in patients with pulmonary infection after acute stroke.
2.Study on the Protective Effect of Bilobalide on Ischemical Reperfusion Injury in Rats Undergoing Liver Transplantation

OBJECTIVE:To observe the effect of Bilobalide on ischemical reperfusion injury in orthotopic liver transplantation model rats and to discuss its protective effect on ischemical reperfusion injury of rats undergoing liver transplantation.METHODS:The model of orthotopic liver transplantation was established by in rats by setting up the bridge between portal vein and left renal vein and building blood bypass by inlaying of canal in the inferior vena,after modeling,the rats were treated with Bilobalide,then levels of NO(active medium of blood vessel)and ET1 before ischemia and at 10 min,30min and 2h after reperfusion were determined by nitratase reduction method;serum levels of ALT,AST,ALP,LDH enzymology,ATP and MDA in the liver tissues were determined as well.The hepatic tissue sample was taken at 2h and fixed with formalin to be made into specimen for observation of the ultrastructure of liver cells and hepatic lobules under electron microscope.RESULTS:After being treated by Bilobalide,serum NO level was elevated and the pathological elevated levels of ALT,AST and LDH were brought down,the microcirculation in ischemical reperfusion injury rats was improved and the extent of damage in the ultrastructure of liver cells was lessened in rats.CONCLUSION:Bypass of portal vein and inferior vena cava prior liver transplantation is helpful for the prevention of ischemical reperfusion injury of liver.The balance of NO/ET1 is possibly a factor influencing the blood flow in the microcirculation of transplanted liver.Bilobalide was proved to be of protective effect on ischemical reperfusion injury in rats.

4.Local massage with safflower oil for phlebitis following peripheral intravenous catheters

ObjectiveTo evaluate the curative effect of local massage with safflower oil in patients with phlebitis following peripheral intravenous catheters.MethodsA total of 71 patients with phlebitis following peripheral intravenous catheters wererandomly divided into 2 groups, a safflower oil group with 36 cases, and a magnesium sulfate group with 35 cases. The magnesium sulfate group was treated by local external application of 33% magnesium sulfate on phlebitis at the puncture site, while the safflower oil group was treated by external application of safflower oil 3~5 cm around the peripheral vein puncture site and massage. Both groups were treated for 48 h. Visual Analog Scale (VAS) was used to assess pain degree, marking method to label the localred swelling area.ResultsThe VAS score (0.81±0.13vs.0.94±0.11;t=4.543,P<0.01) at 48 h after the treatment, and the local red swelling area at 24 h (3.62±1.22 cm2vs.4.42±1.72 cm2;t=2.335, P=0.022) and 48 h (1.07±0.25 cm2vs.3.26±1.07cm2;t=11.952,P<0.01) after the treatment in the safflower oil group were significant lower or smaller than the magnesium sulfate group.ConclusionsLocal massage with safflower oil can effectively alleviate the severity of phlebitis, relieve symptoms, reduce the score of VAS and local red swelling area, and promote the damaged tissue to restore normal.
5.Morphological characteristics of expressions of DKK1 in different periods of tooth development in postnatal mice and their significances

Objective:To study the expression characteristics of Dickkopf1 (DKK1) in different time and space during tooth development of the postnatal mice, and to provide the theoretical basis for clarifying the mechanism of Wnt signaling pathway in regulating the tooth development.Methods:The postnatal Kunming mice at days 0.5, 6.5, 12.5, 18.5, 24.5, and 30.5 respectively after birth were selected and divided into various groups by time,three in each group.The mice in each group were sacrificed and the paraffin sections of mandibular bone including the first molar were prepared at the thickness of 5 μm, followed by HE staining and immunohistochemical staining in order to detect the expressions of DKK1 in tooth tissue and periodontal tissue.Results:At 0.5 d after birth, the mandibular first molar tooth germ was in the bell stage.At 6.5 d the enamel development of mandibular first molar was almost completed, and the epithelium root sheath extended to the root direction.At 12.5 d the dentin development of crown was completed, with the root formatted about 1/3. At 18.5 d the root had formatted about 2/3.At 24.5 d the root had reached the full length.At 30.5 d the apical foramen was narrow, and the root development was basically completed.There was no DKK1 expression at 0.5 d, but it expressed in the odontoblasts and predentin at 6.5 d. From days 12.5 to 30.5,the expressions of DKK1 were positive in periodontal ligament, alveolar bone, and cellular cementum as odontoblasts, which were gradually increased with the prolongation of time.However, no expression of DKK1 was detected in the pulp.Conclusion:DKK1 shows regular expressions at different tooth developmental stages after birth, suggesting its potential role in the growth of dentin and periodontal tissues.
6.Recent therapeutic drug progress on neuroendocrine prostate cancer

Neuroendocrine prostate cancer(NEPC) is a prostate cancer subtype with a very high degree of malignancy and a special molecular phenotype.NEPC is not sensitive to endocrine therapy, and there are currently no specific drugs, so there is a lack of effective clinical treatment.New advances in NEPC therapeutic include chemical therapy, targeted drug therapy based on molecular phenotype and other non-targeted drug therapy. This article summarizes the current treatment methods, pharmaceutical, and clinical research results for NEPC, aiming to deepen clinicians' more comprehensive understanding of NEPC patients' treatment strategies.
7.Current status and related research progress of mechanical thrombectomy in large core ischemic stroke of anterior circulation

In recent years,mechanical thrombectomy has been the most important research progress in the treatment of acute cerebral infarction,especially the positive results of five endovascular therapy studies in 2015 has rewritten its clinical guidelines.However,the focus of these studies was mainly on the small vessel infarction(SVI),and the inclusion criteria of these studies include the following aspects:ASPECTS ≥6 points,Alberta Stroke Program Early CT Score within 6 hours after stroke onset,the infarct volume<70 mL within 6-24 hours after stroke onset,and the presence of image mismatch or the presence of mismatch between clinical condition and perfusion imaging.The above studies excluded patients with ASPECTS<6 points or infarct volume ≥70 mL of large core infarction(LCI).With the continuous progress of the endovascular treatment of acute ischemic stroke(AIS),the mechanical thrombectomy therapy strategy has crossed from the"time window"to the"tissue window",meanwhile,the therapeutic goal of mechanical thrombectomy has also moved from treating SVI to a new era of treating LCI that has been a very hot topic recently.Whether endovascular treatment is beneficial for patients with LCI remains uncertain.This paper aims to make a comprehensive review concerning the relevant research progress in endovascular therapy for anterior circulation large core ischemic stroke,including the imaging determination and study inclusion criteria of LCI,the postoperative blood pressure management,and the factors influencing ineffective recanalization and prognosis.(J Intervent Radiol,2023,32:1256-1262)
8.Effect of L-carnitine combined with citicoline on the therapeutic effect of neonatal hypoxic-ischemic encephalopathy and serum NO and ET-1

Objective To explore the effects of L-carnitine combined with citicoline on the therapeutic effect of neonatal hypoxic-ischemic encephalopathy(NHIE)and serum nitric oxide(NO)and endothelin-1(ET-1).Methods A total of 95 children with HIE admitted to the hospital from April 2020 to January 2023 were selected and were divided into observation group(47 cases)and control group(48 cases)by random number table method.The control group was given citicoline,and the observation group was given citicoline combined with L-carnitine.The therapeutic effect was evaluated on 14 days after treatment,and the recovery time of original reflex,muscle tone and consciousness was calculated.The levels of oxidative stress indexes[malondialdehyde(MDA),superoxide dismutase(SOD)]and vascular endothelial function indexes(NO,ET-1)were detected before treatment and 14 days after treatment.Neonatal neurobehavioral score(NBNA)was used to evaluate the neurological function of the children,and the safety of the treatment regimen was ob-served.Pearson correlation analysis was used to analyze the correlation between NBNA and vascular endothe-lial function indexes.Results The total effective rate in the observation group was 91.49%,which was higher than 72.92%in the control group(P<0.05).The original reflex recovery time,muscle tone recovery time and consciousness recovery time of NHIE children in the observation group were shorter than those in the control group(P<0.05).There was no significant difference in NBNA scores between the two groups before treatment(t=1.225,P=0.224).After treatment,the NBNA score in the observation group was higher than that in the control group(t=6.223,P<0.001).After treatment,MDA level decreased and SOD level in-creased in two groups(P<0.05).After treatment,the level of MDA in the observation group was lower than that in the control group,while the level of SOD was higher than that in the control group(P<0.05).After treatment,the levels of NO and ET-1 were decreased in both groups(P<0.05).The levels of NO and ET-1 in the observation group were lower than those in the control group after treatment(P<0.05).The adverse drug reaction rates in observation group and control group were 17.02%and 8.33%,respectively,and there was no significant difference between two groups(P>0.05).Pearson correlation analysis showed that serum NO,ET-1 and NBNA score in NHIE children were negatively correlated(r=-0.546,-0.608,P<0.05).Conclusion L-carnitine combined with citicoline could improve the therapeutic effect of NHIE,shorten the re-covery time of clinical manifestations,and improve nerve function,oxidative stress and vascular endothelial function without increasing drugs and adverse reactions.In addition,vascular endothelial function indexes are negatively correlated with NBNA score,which could be used as auxiliary reference indexes for judging NHIE.
9.Challenges and factors that influencing causal inference and interpretation, based on Mendelian randomization studies

Genetic variation is used as instrumental variable to investigate the causal relationship between exposure and outcome, which can avoid issues of confounding and reserve causation regarding Mendelian randomization studies. However, the instrumental variables in Mendelian randomization studies must satisfy three core assumptions—the relevance assumption, the independence assumption, and the exclusion restriction assumption. In addition to the plausibility of core assumptions, the application of Mendelian randomization studies in causal inference is also subject to other limitations. Findings from the Mendelian randomization studies should be interpreted in the context of existing evidence from other sources. In this article we provide an overview of the assumptions, limitations, and interpretation on causal inference that related to Mendelian randomization studies that can be applied in studies of the same kind.
10.Ultrastructural study of ampulla of mouse inner ear

Objective:To observe the ultrastructure of the ampulla, and analyze its physiological and pathological significance.Methods:In this study, 20 Kunming mice were used, and scanning electron microscopy was used to observe the ultrastructure of the ampulla of inner ear.Results:Otoconia was found among the cilia bundles of different haircell(intercilla otoconia of ampulla). The cupula was attached to the lateral wall of the ampulla, and easily to be separated; after separated, a kind of slender crystal(surface otoconia of ampulla) could be seen between the cupula and lateral wall of the ampulla, both sides of ampullary crest were covered with slender crystals too. On the canal side of the ampulla wall, there was more particulate matter attached to the wall near the bottom of ampullary crest, partially embedded in the wall, and less on the utricle side of the ampulla wall.Conclusions:The observation of the ultrastructure of the ampulla is helpful for better understanding the physiological functions of the semicircular canals and the ampulla, and better understanding the pathogenesis and solution of some vertigo diseases.
