1.Diagnostic value of serum Fer, AFP and AFP-L3 combined detection in primary hepatic cancer

Objective To evaluate the diagnostic value of the combined detection of serum Fer,AFP and AFP-L3 in patients with primary hepatic cancer (PHC).Methods The serum levels of Fer,AFP and AFP-L3 were determined in 92 patients with PHC,84 patients with benign liver disease,and 45 control subjects.Results The PHC group serum Fer was (819.4±616.8) μg/L,AFP was (556.9±451.5) μg/L,AFP-L3 was (109.0±62.5) μg/L.The benign liver disease group Fer was (229.3±155.5) μg/L,AFP was (6.0±4.2) μg/L,AFP-L3 was (26.0±49.4) μg/L.The healthy control group Fer was (125.6±110.5) μg/L,AFP was (2.8±0.8) μg/L,AFP-L was (7.0±1.8) μg/L,there was significant difference between the three groups (all P < 0.01).The sensitivity of PHC group alone in detection of serum Fer was 85.9 %,the sensitivity of AFP was 71.0 %,the sensitivity of AFP-L3 was 83.7 %,the combined detection of Fer and AFP or AFP-L3 improved the sensitivity of the test to 90.2 % and 95.7 %,the three combined detection sensitivity was 98.9 %.Conclusion Combined detection of serum Fer,AFP and AFP-L3 is essential in the diagnosis of PHC.
2.Content Determination of Paeonol in Liuwei Dihuang Pill by SPE - HPLC

Objective To develop SPE - HPLC method for the assay of paeonol in Liuwei Dihuang Pill (LDP). Methods Separation was performed on a Hypersil ODS(2) column (250 mm?4. 6 mm, 5?m) . The mobile phase consist of methanol - 2 % acetic acid (55 :45, v/v) . The flow rate was 1 mL/min. The UV detection was set at 275 nm. Results Paeonol had a good linear relation in the range of 0. 03998 ~ 0. 7996 ug, the average recovery of honeyed bolus of LDP was 102. 9 % and the RSD was 1.2%; the average recovery of concentrated bolus of LDP was 101. 5 % and the RSD was 1.8%. Conclusion The method is simple, sensitive and rapid and it can be used to determine the paeonol content of LDP.
3.Risk factor analysis for postoperative brain metastasis in patients with non-small-cell lung cancer

Objective The patients with non-small cell lung cancer ( NSCLC) are still at risk of brain metastasis after surgi-cal treatment.The study was to investigate the clinical characteristics and the risk factors for postoperative brain metastasis in patients with NSCLC in order to provide references for prophylactic cranial irradiation ( PCI) . Methods The clinical and prognostic data of 213 patients with NSCLC in our department of oncology from February 2008 to February 2012 were retrospectively analyzed.The occur-rences of postoperative brain metastasis in NSCLC patients with different characteristics were observed.Cox model was used to investi-gate the risk factors for postoperative brain metastasis in patients with NSCLC. Results The overall cumulative rate of brain metasta-sis was 23.9%(51/213) and the rates of brain metastasis at 1-year, 2-year, 3-year after surgery were 6.1%, 14.1%, 22.5% re-spectively.Univariate analysis showed that the rates of postoperative brain metastasis in NSCLC patients with different ages, histological types, tumor stages and CEA levels were statistically significant (P<0.05).Cox regression showed that adenocarcinoma (HR=2.00, 95%CI:1.11~3.64), lung cancer stageⅢ(HR=3.70, 95%CI:1.67~8.25) and high CEA (≥5.0μg/L)(HR=3.16, 95%CI:1.60~6.25) were risk factors for postoperative brain metastasis in patients with NSCLC.The survival time in NSCLC patients with brain metastasis was remarkably lower than that in patients without brain metastasis (P<0.05). Conclusion The NSCLC pa-tients with adenocarcinoma, III stage disease, or high CEA are more likely to develop brain metastasis after surgery, which should be the focus of PCI.
4.Efficacy of fluoxetine in treatment of patients with the obstructive sleep apnea-hypopnea syndrome

AIM: To evaluate the response of obstructive sleep apnea hypopnea syndrome (OSAHS) to fluoxetine. METHODS: After a baseline polysomnography (PSG) and Epworth Sleepiness Scale (ESS) documented OSAHS, 10 patients were treated with fluoxetine (20 mg?d -1 ) for four weeks, a repeating PSG and ESS was performed to evaluate the action of fluoxetine. RESULTS: After 4 weeks fluoxetine treatment, the proportion of REM sleep time was decreased by 9.2 %, and the assessment of ESS was significantly decreased (P 0.05 ). The apnea/hypopnea index (AHI) fell in average 24.84 per hour,the mean SaO 2 and minimum SaO 2 significantly increased by 3.2 % and 16.6 %, respectively, and desaturation event index significantly decreased 18.24 per hour (P 0.05 ). The treatment efficiency of fluoxetine to OSAHS was 50%. CONCLUSION: Short term use of fluoxetine in patients with OSAHS is associated with better sleep quality, improvement in oxygenation,and reduction of episodic apneas or hypopneas.
5.Prediction Indexes of Hepatitis B Virus Intrauterine Infection

[Objective] To investigate the value of HBV-M and HBV DNA of newborns born to HBsAg-positive mother, which were tested before combined immunization of hepatitis B. [Method] A total of 420 infants born to HBsAg-positive mothers delivered in Obstetric Department of the Third Affiliated Hospital of Sun Yat-Sen University from June 2006 to February 2008 were followed up at least 6 months and rechecked HBV-M to confirm the diagnosis of HBV intrauterine infection, which included 33 HBsAg or HBV DNA positive newborn babies and 6 newborns with both HBsAg seropositive and HBV DNA seropositive. [Result] HBV intrauterine infection rate was 0.95%. Using newborn both HBsAg positive and HBV DNA positive as diagnostic criterion to diagnose HBV intrauterine infection, the positive likelihood ratio was 208.3, while using newborn HBsAg positive or HBV DNA positive as diagnostic criterion, it was 14.3. [Conclusion] Newborn both HBsAg positive and HBV DNA positive obtained before combined immunization of hepatitis B may predict HBV intrauterine infection, and it may play as a clinical index of preliminary diagnosis of HBV intrauterine infection.
6.Diagnosis value of expression of inserum macrophage inhibitory factor 1 combined with alpha-fetoprotein isoforms 3 in primary liver cancer

Objective Study on the diagnosis value of expression of serum macrophage inhibitory factor 1 (MIC-1) combined with alpha-fetoprotein isoforms 3 (AFP-L3) in primary liver cancer detected by enzyme linked immunosorbent assay (ELISA).Methods MIC-1 and AFP-L3 concentrations were detected by ELISA from selected 116 patients of primary liver cancer.Results were compared both in combined and sigle detection.Results In primary liver cancer group AFP-L3 concentration [(127.12±51.43) ngmml] was significantly higher than that in normal control group [(27.11±7.26) ng/ml,P < 0.001].With AFP-L3 > 38.0 ng/ml as the critical value,the sensitivity was 85.34 % (99/116),specificity was 88.33 % (53/60) and the diagnostic accuracy was 86.36 % (152/176).In primary liver cancer group MIC-1 concentration [(3140.43±1138.23) pg/ml]was significantly higher than that in normal the control group [(701.88±302.34) pg/ml,P < 0.001],the sensitivity was 91.38 % (106/116),specificity was 85.00 % (51/60),the diagnostic accuracy was 89.20 %(157/176).The two combined detection sensitivity was 83.62 % (97/116),specificity was 91.67 % (55/60),diagnostic accuracy was 86.36 % (152/176).Conclusion MIC-1 combined with AFP-L3 concentration detection can improve the specificity of the diagnosis of primary liver cancer,which has certain clinical value.
7.Follow-up study on hepatitis B virus mother-to-child transmission in 1355 HBsAg-positive pregnant women

Objective To investigate hepatitis B virus (HBV) mother-to-child transmission rate in hepatitis B virus surface antigen (HBsAg)-positive pregnant women.MethodsA total of 1355 HBsAg-positive pregnant women and their 1360 newborns (included 5 twins)were collected prospectively.All newborns received hepatitis B immunoglobulin (HBIG) 200 U intramuscularly within 6 hours of birth as early as possible,and were administered with routine 10 μg recombinant hepatitis B vaccine (at 0,1,6 months of birth).The venous blood HBV markers and HBV DNA levels were detected in all newborns at 0,7,12 months of age.The measurement data were analyzed by t test.Qualitative data were analyzed by chi square test,rank sum test or Fisher exact test.Results The intrauterine HBV infection rate of 1360 infants were 1.54% (21/1360) during 12 months of follow-up.The rate of intrauterine infection in HBeAg positive mothers was significant higher than that of HBeAg negative mothers (4.44% vs 0,χ2 =35.99; P<0.05); the rate of intrauterine infection in HBV DNA positive mothers was significant higher than that of HBV DNA negativemothers (3.13% vs 0,χ2 =21.84; P<0.05).When maternal serum HBV DNA≥1 × 107 IU/mL,the rate of intrauterine infection was 6.01 %,which was significantly higher than that of maternal serum HBV DNA< 1 × 107 IU/mL (χ2 =39.43,P<0.05).ConclusionsAfter strict combined active-passive immunization,the rate of HBV intrauterine infection is 1.54%.When mothers are HBeAg positive or with high level of HBV DNA,the rate of HBV intrauterine infection increases significantly.Intrauterine infection is the main cause of failure in immunoblockade of HBV mother-to-child transmission.
8.Role and mechanism of resveratrol in a rat model of complex regional pain syndrome type Ⅰ

Objective To investigate the role and mechanism of resveratrol in a rat model of complex region-al pain syndrome type Ⅰ(CRPS1). Methods 50 male SD rats were divided into control group(group A),sham operation group(group B),model group 1(group C),model group 2(group D),and model group 3(group E). Groups A,B and C received 5%DMSO;group D received ISO-1 of 1 mg/(kg·d);and group E received resveratrol of 20 mg/(kg · d)for 14 days. Pain behaviors were assessed on days 0,7,and 14. Serum levels of MIF and TNF-αwere detected by ELISA. MIF ,total ERK1/2 and p-ERK1/2 in sciatic nerve were detected by Western blot. Re-sults On days 7 and 14 after treatment,resveratrol injection,similar to ISO-1,significantly improved the pain threshold;serum levels of MIF and TNF-α were significantly decreased;expressions of MIF ,total ERK1/2 and p-ERK1/2 in sciatic nerve were also decreased significantly in group E,which were significantly lower than group C (P < 0.05). Conclusions Resveratrol can significantly improve pain threshold ,decrease expressions of MIF and p-ERK1/2 in a rat model of CRPS1,which might be involved in the inhibition of ERK signaling pathway.
9.mTORC1 inhibitor inhibit human pancreatic neuroendocrine tumors cell proliferation by influence glutamine metabolism

Objective To evaluatethe effect of mTORC1 inhibitor on the proliferation in human pancreatic neuroendocrine tumors(pNET)cell line BON,to explore the function of glutamine metabolism in it.Methods In vitro cultured human pancreatic neuroendocrine tumors(pNET)cell line BON,BON cells were treated with different concentrations of rapamycin(1,5,10,25,50, 100 nM)for 12,24 h.Then CCK-8 assay are used to calculate the growth inhibitory rate.Rapamycin treated with BON 12 h,test the glutamine uptake level compared with control.Then deprive of glucose and/or glutamine,CCK-8 assay were used in observation of cell proliferation,cell cycle distribution was analyzed by flow cytomety.Results Rapamycin significantly inhibited the growth of BON cells in a time-and dose-dependent manner(P <0.05).Meanwhile,rapamycin can reduce the glutamine uptake level compared with control.BON obviously depends on glutamine for growth,without glucose and glutamine group have obvious difference in growth rate(P <0.05).Conclusion mTORC1 inhibitor can inhibit BON cells proliferation and influence the glutamine uptake lev-el.suggesting that mTORC1 inhibitor might inhibit BON cells proliferation by influenced the glutamine metabolic pathway.
10.Characters of the fibroblast-like cells cultured from the mobilized peripheral blood cells

AIM: To explore the characters of fibroblast-like (F-L) cells cultured from granunocyte clony stimunating factor (G-CSF)-mobilized peripheral blood cell (PBC) harvests. METHODS: The adherent cells in the PBC harvests were cultured for 2 week in the mediums of RPMI-1640/L-DMEM/G-CSF or interleukin-3 (IL-3) plus RPMI-1640, the cultured F-L cells were analyzed by flow cytometry (FC). RESULTS: The adherent non-confluent F-L cells obtained from the four groups were similar in their phenotypes: CD33+, CD11c+, CD64+, CD14+, CD45+, HLA-DR+, CD86+, CD34-, CD38-, CD3-, CD19-, CD56-, CD29-, CD44-, CD105-. The F-L cells are similar to monocytes except CD38-and were distinct from dendritic cells (DC) or mesenchymal stem cells (MSC). CONCLUSION: The cultured F-L cells are macrophages rather than DC or MSC. G-CSF, rhIL-3 enhances their numbers.
