2.Establishment and preliminary application of measuring method for three-dimensional kinematics of the hindfoot joint in vivo

Objective To establish a new method for measuring the 3-D kinematics of hindfoot joints in vivo by using reverse engineering software together with the theory of rigid body kinematics.Methods CT images were gathered from 9 feet of 5 healthy volunteers in both the initial position (neutral position) and the terminal position (extremely inversion-adduction-dorsiflexion position).The 3-D digital modules of hindfoot joints in the initial position and terminal position were established with MIMICS 10.01 software.The data of reconstructed digital modules was inputted into the GEOMAGIC 10.0 software in STL format for twice registration,and then their relatively displacement and changes of angle in 3-D space between the two positions were calculated Results The rotation range of the tibiotalar joint was 3.89° ±2.77° in eversion,5.29°±4.47° in abduction,10.77°+5.70° in dorsiflexion,and the relative displacement was 0.78±0.59 mm towards lateral ankle,0.18±0.75 mm towards the hindfoot,(0.65±0.71) mm towards the proximal limbs;the range of subtalar joint was 16.46°±2.94° in inversion,12.77°±1.81° in adduction,6.33°±4.32° in plantarflexion,and the relative displacement was 5.50±1.45 mm towards medial ankle,1.96±1.77 mm towards forefoot,0.43±1.18 mm towards distal limbs; the range of talonavicular joint was 38.82°±5.98° in inversion,19.71°±6.33° in adduction,5.09°±6.89° in plantarflexion,and the relative displacement was (9.77±1.73) mm towards medial ankle,3.13±1.29 mm towards hindfoot,4.64±1.42 mm towards proximal limbs.Conclusion This method measuring 3-D kinematics of hindfoot joints in vivo is non-invasive and easy to operate.
3.Nickel-titanium memory-shape compression anastomosis clip for gastrointestinal anastomosis: a Meta-analysis and sequential analysis

BACKGROUND:Nickel-titanium memory-shape compression anastomosis clip (Ni-Ti CAC) has been used in gastrointestinal anastomosis, but its efficacy and safety still remain controversial.OBJECTIVE:To evaluate the efficacy and safety of Ni-Ti CAC in gastrointestinal anastomosis.METHODS:A computer-based online research of PubMed, EMbase, Cochrane Library, CBM, CNKI, and VIP databases was performed for articles published before January 15th, 2017 using the keywords of nickel-titanium, compression anastomosis clip, anastomosis, gastric, jejunum, ileum, small intestine, colon, rectum, and large intestine in English and Chinese, respectively. The randomized controled trials about Ni-Ti CACversus conventional methods for gastrointestinal anastomosis were included. Meta-analysis of the anastomosis time, time of exsufflation, and hospitalization time was performed, and sequential analysis was conducted on TSA v0.9 software.RESULTS AND CONCLUSION:A total of 18 eligible randomized controlled trials were enroled, involving 1860 patients. Ni-Ti CAC could reduce the anastomosis time [MD=-3.83, 95%CI(-6.48,-1.19),P=0.004] and time of exsufflation [MD=-0.14, 95%CI(-0.22,-0.05),P=0.002], but there was no significant difference in the hospitalization time [MD=-0.83, 95%CI(-1.82, 0.16), P=0.10]. The quality was ranked as low level based on GRADE system. The time of exsufflation of Ni-Ti CAC was superior to that of conventional method, which was confirmed by sequential analysis. One case of death was reported and incision infection was the most common adverse effects; additionaly, pulmonary embolism and abdominal pain occurred. To conclude, Ni-Ti CAC can facilitate gastrointestinal anastomosis, accelerate the time of exsufflation, and holds a good safety. However, more multicenter and high-quality randomized controlled trials are needed.
4.Meta-analysis of Shenqi Fuzheng Injection Combined with Chemotherapy for Colorectal Cancer

OBJECTIVE:To evaluate the clinical efficacy and safety of Shenqi fuzheng injection combined with chemotherapy in the treatment of colorectal cancer.METHODS:Retrieved from PubMed,EMBase,Clinical Trials,Cochrane Library,CJFD,CBM,Wanfang database and VIP,RCTs about Shenqi fuzheng injection combined with chemotherapy (trial group) vs.chemotherapy alone (control group) in the treatment of colorectal cancer were included.Meta-analysis was performed by Rev Man 5.3 statistical software after data extraction and quality evaluation with Cochrane system evaluator manual.RESULTS:A total of 25 RCTs were included,involving 1 987 patients.Results of Meta-analysis showed that objective remission rate of trial group was significantly higher than control group [RR=1.19,95% CI (1.02,1.39),P=0.02];the improvement of survival quality was significantly better than control group [RR=1.72,95%CI(1.49,1.99),P<0.001];CD4VCD8+ was significantly higher than control group [MD=0.40,95% CI (0.29,0.50),P<0.001];the incidence of gastrointestinal reaction was significantly higher than control group [RR=0.59,95%CI(0.52,0.68),P<0.001];the incidence of liver and renal injury was significantly lower than control group [RR=0.52,95%CI(0.41,0.67),P<0.001],with statistical significance.CONCLUSIONS:Shenqi fuzheng injection combined with chemotherapy can improve objective remission rate of colorectal cancer patients,survival quality and immune function,and reduce the occurrence of toxic reation.
5.Low-molecular-weight heparin calcium with application of alprostadil combined in the treatment of hypoparathyroidism after thyroid surgery

Objective To investigate the application value of low-molecular-weight heparin calcium combined with alprostadil in the treatment of hypoparathyroidism(HP)after thyroid surgery.Methods Sixty patients with HP after thyroid surgery were randomly divided into two groups:study group(30 cases)and control group(30 cases).The study group received low-molecular-weight heparin calcium and alprostadil,along with oral calcium supplements and calcitriol or an intravenous infusion of calcium gluconate,for three consecutive days after surgery.The control group received only oral calcium supplements and calcitriol or intravenous calcium gluconate.The values and changes in parathyroid hormone(PTH),postoperative drainage volume,and postoperative complications were compared between two groups.Results There were no significant difference in PTH levels at 3 days,1 week,1 month,and 6 months after surgery between two groups(P>0.05).The difference in PTH levels between 3 days and 1 day after surgery was(6.77±2.61)ng/L in study group and(1.29±1.12)ng/L in control group,with a significant difference(P<0.05).The difference in PTH levels between 1 week and 1 day after surgery was(10.51±3.66)ng/L in study group and(2.65±2.17)ng/L in control group(P<0.05).There were no significant difference in PTH levels at 1 month,6 months,and 1 day after surgery(P>0.05).Comparison of postoperative drainage volume:The postoperative drainage volume of study group was(90.07±22.24)ml,and that of control group was(88.20±24.09)ml,with no statistical significance(P>0.05).There was no significant difference in postoperative complications between two groups(P>0.05).Conclusion In cases of HP after total thyroidectomy,timely use of low-molecular-weight heparin calcium and alprostadil can accelerate the recovery of PTH and reduce patient numbness,which is safe,effective,and worthy of clinical promotion.
6.Correlation between talonavicular joint movement and changes of the medial longitudinal arch.

OBJECTIVETo explore the characteristics of the talonavicular joint movement in vivo and its effects on changes of the medial longitudinal arch.
METHODSFoot CT images in the initial position (neutral position) and terminal position (maximum varus-adduction-dorsiflexion position) were acquired from 9 cases (5 healthy volunteers, including 4 males and 1 female) during foot varus-adduction-dorsiflexion motion. Based on the principle of rigid body kinematics, the CT data were reconstructed and analyzed with Mimics and Geomagic reverse engineering software. The changes of the talonavicular joint in three-dimensional in 6 degrees of freedom were calculated to determine its correlation to the medial longitudinal arch angle.
RESULTSDuring foot varus-adduction-dorsiflexion motion, the talonavicular joint underwent varus-adduction-plantarflexion motion, with the motion range of 38.82∓5.98° in varus, 19.71∓6.33° in adduction, and -5.09∓6.89° in plantarflexion. During talonavicular joint motion, the medial shift of the navicular was significantly correlated to the changes of foot medial longitudinal arch (P<0.05).
CONCLUSIONDigital technology can solve the problem of measurement of talonavicular joint three-dimensional motion in vivo. Though as a ball-and-socket joint, the talonavicular joint mainly rotates around the sagittal axis, and its movement is a major factor to cause changes of foot medial longitudinal arch.

7.Associations between floods and bacillary dysentery cases in main urban areas of Chongqing, China, 2005-2016: a retrospective study.

BACKGROUND:
Understanding the association between floods and bacillary dysentery (BD) incidence is necessary for us to assess the health risk of extreme weather events. This study aims at exploring the association between floods and daily bacillary dysentery cases in main urban areas of Chongqing between 2005 and 2016 as well as evaluating the attributable risk from floods.
METHODS:
The association between floods and daily bacillary dysentery cases was evaluated by using distributed lag non-linear model, controlling for meteorological factors, long-term trend, seasonality, and day of week. The fraction and number of bacillary dysentery cases attributable to floods was calculated. Subgroup analyses were conducted to explore the association across age, gender, and occupation.
RESULTS:
After controlling the impact of temperature, precipitation, relative humidity, long-term trend, and seasonality, a significant lag effect of floods on bacillary dysentery cases was found at 0-day, 3-day, and 4-day lag, and the cumulative relative risk (CRR) over a 7-lag day period was 1.393 (95%CI 1.216-1.596). Male had higher risk than female. People under 5 years old and people aged 15-64 years old had significantly higher risk. Students, workers, and children had significantly higher risk. During the study period, based on 7-lag days, the attributable fraction of bacillary dysentery cases due to floods was 1.10% and the attributable number was 497 persons.
CONCLUSIONS
This study confirms that floods can increase the risk of bacillary dysentery incidence in main urban areas of Chongqing within an accurate time scale, the risk of bacillary dysentery caused by floods is still serious. The key population includes male, people under 5 years old, students, workers, and children. Considering the lag effect of floods on bacillary dysentery, the government and public health emergency departments should advance to the emergency health response in order to minimize the potential risk of floods on public.

8.Wnt/b-Catenin Promotes the Osteoblastic Potential of BMP9 Through Down-Regulating Cyp26b1 in Mesenchymal Stem Cells

BACKGROUND:
All-trans retinoic acid (ATRA) promotes the osteogenic differentiation induced by bone morphogenetic protein 9 (BMP9), but the intrinsic relationship between BMP9 and ATRA keeps unknown. Herein, we investigated the effect of Cyp26b1, a critical enzyme of ATRA degradation, on the BMP9-induced osteogenic differentiation in mesenchymal stem cells (MSCs), and unveiled possible mechanism through which BMP9 regulates the expression of Cyp26b1.
METHODS:
ATRA content was detected with ELISA and HPLC–MS/MS. PCR, Western blot, and histochemical staining were used to assay the osteogenic markers. Fetal limbs culture, cranial defect repair model, and micro–computed tomographic were used to evaluate the quality of bone formation. IP and ChIP assay were used to explore possible mechanism.
RESULTS:
We found that the protein level of Cyp26b1 was increased with age, whereas the ATRA content decreased. The osteogenic markers induced by BMP9 were increased by inhibiting or silencing Cyp26b1 but reduced by exogenous Cyp26b1. The BMP9-induced bone formation was enhanced by inhibiting Cyp26b1. The cranial defect repair was promoted by BMP9, which was strengthened by silencing Cyp26b1 and reduced by exogenous Cyp26b1. Mechanically, Cyp26b1 was reduced by BMP9, which was enhanced by activating Wnt/b-catenin, and reduced by inhibiting this pathway. b-catenin interacts with Smad1/5/9, and both were recruited at the promoter of Cyp26b1.
CONCLUSIONS
Our findings suggested the BMP9-induced osteoblastic differentiation was mediated by activating retinoic acid signalling, viadown-regulating Cyp26b1. Meanwhile, Cyp26b1 may be a novel potential therapeutic target for the treatment of bone-related diseases or accelerating bone-tissue engineering.
9.Peripheral Blood Inflammation Indicators as Predictive Indicators in Immunotherapy of Advanced Non-small Cell Lung Cancer.

BACKGROUND:
Lung cancer is the leading cause of cancer-related death, of which non-small cell lung cancer (NSCLC) is the most common type. Immune checkpoint inhibitors (ICIs) have now become one of the main treatments for advanced NSCLC. This paper retrospectively investigated the effect of peripheral blood inflammatory indexes on the efficacy of immunotherapy and survival of patients with advanced non-small cell lung cancer, in order to find strategies to guide immunotherapy in NSCLC.
METHODS:
Patients with advanced non-small cell lung cancer who were hospitalized in The Affiliated Cancer Hospital of Nanjing Medical University from October 2018 to August 2019 were selected to receive anti-PD-1 (pembrolizumab, sintilimab or toripalimab) monotherapy or combination regimens. And were followed up until 10 December 2020, and the efficacy was evaluated according to RECIST1.1 criteria. Progression-free survival (PFS) and overall survival (OS) were followed up for survival analysis. A clinical prediction model was constructed to analyze the predictive value of neutrophil-to-lymphocyte ratio (NLR) based on NLR data at three different time points: before treatment, 6 weeks after treatment and 12 weeks after treatment (0w, 6w and 12w), and the accuracy of the model was verified.
RESULTS:
173 patients were finally included, all of whom received the above treatment regimen, were followed up for a median of 19.7 months. The objective response rate (ORR) was 27.7% (48/173), the disease control rate (DCR) was 89.6% (155/173), the median PFS was 8.3 months (7.491-9.109) and the median OS was 15.5 months (14.087-16.913). The chi-square test and logistic multi-factor analysis showed that NLR6w was associated with ORR and NLR12w was associated with ORR and DCR. Further Cox regression analysis showed that NLR6w and NLR12w affected PFS and NLR0w, NLR6w and NLR12w were associated with OS.
CONCLUSIONS
In patients with advanced non-small cell lung cancer, NLR values at different time points are valid predictors of response to immunotherapy, and NLR <3 is often associated with a good prognosis.
