1.Discussion on strengthening yin of Chinese herbs with bitter-flavor

Chinese herbs with bitter flavor have the function of purgation and dryness as well as strengthening yin.Strengthening Yin means keeping and maintaining yin fluid.Actually,it is to draw assistance from some Chinese herbs with bitter flavor to purge excessive fire in order to maintain yin qi by heat-clearing and fire-purging,that is to say purging fire to keep or save yin.Strengthening yin of bitter herbs is different from the moistening and nourishing function of sweet-cold herbs.It can be generally divided into three aspects: purging ministerial fire and restoring renal yin,clearing heat and purging fire to maintain yin fluid;purging to preserve yin.Strengthening yin doesn't mean the property and flavor of bitter-cold can nourish yin directly.We should not only emphasize clearing fire by using herbs with bitter in flavor and cold in nature,but also pay attention to nourish yin.It is causal relation that exists between purging fire and maintaining yin.The former is only a means and method,and the latter is objection and effect.Because the bitter-flavor can only damage yin by bitterness and dryness but is not good at benefiting yin,we cannot strengthen yin by using bitter herbs in treating yin-deficiency syndrome without excessive fire.
2.Storage Optimization of PACS in Affiliated Hospital of Medical College of CPAPF

Objective To investigate the demands of PACS system on the storage and the ways to optimize its performance.Methods The storage media,such as CD Rom library and disc array,were integrated,and three-tier network structure was established.Results Online storage through virtual storage technology could be applied to high-capacity,high-quality and high-reliability medical image storage.Conclusion Storage optimization PACS is implemented.
3.Correlation of diagnostic imaging and autopsy findings of eight patients with acquired immune deficiency syndrome

Objective To investigate the imaging findings with pathologic correlation in patients with acquired immune deficiency syndrome(AIDS).Methods Imaging findings,autopsy and pathological data were retrospectively analyzed in eight patients with AIDS.Routine CT scanning of different body parts was performed during their hospitalization.CT scanning was performed from the skull to the pelvis immediately following their death.After routine formalin fixing,7 cardevers were cross sectioned for autopsy in freezing state and 1 for gross autopsy.Tissues were obtained from each sections and organs for pathological examinations.Results The autopsy data showed parasitic infections(5 cases),bacterial infections (3 cases),fungal infections(2 cases),virus infections(2 cases),lymphoma(1 case)and cerebrovascular diseases(1 case)in eight patients with AIDS.The CT scanning demonstrated symmetrical ground glass liked shadows with pulmonary hilus as the center in 5 cases of pulmonary PCP infection; pulmonary patchy shadows,scattering distribution of nodular shadows,extensive military nodular shadows with even distribution and tuberculous pleurisy; cloudy shadows for 2 cases of fungi infection with multiple foci of chronic inflammation;pulmonary net-like parenchymal changes for 2 cases of pulmonary CMV infection;thickened intestinal wall and narrowed intestinal lumen for 1 case of intestinal tumor; low density shadows of brain tissue for 1 case of CMV encephalitis and MRI findings of high T_1 and high T_2 signals as well as MRA findings of broken vascular channels in liquefied areas of brain tissues; patchy low density areas inside a cyst of brain for one case of brain toxoplasmosis infection:multiple small patchy low density areas in cerebral basal ganglia for one case of brain cryptococcus infection.Conclusions In AIDS patients,infection and tumor may occur in various organs resulting in complex symptoms,which makes it more complicated and difficult to make accurate diagnosis.A correlative study of imaging findings and pathological manifestation of AIDS patients at autopsy could be helpful for clinical diagnosis.
4.Mutants of hypoxia-inducible factor-1αaccelerate in vivo angiogenesis in bone defect region

Objective To investigate the functions of triple point-mutants of hypoxia-inducible factor 1α(HIF1α)in angiogenesis in bone defect region under normoxic conditions.Methods Triple point-mutations (the 402,564 and 803 amino acids)in HIF1α coding sequence (CDS)were induced.The triple mutant of HIF1α(402/564/803)was inserted into the adenovirus pAdEasy-1 system to complete viral packaging and titer measurements. The wild-type HIF1α gene and the empty adenovirus vector were packaged in the same way.For the in vitro experiment,the rabbit bone marrow mesenchymal stem cells (MSCs)were divided into four experimental groups:A,MSCs infected with viral solution containing mutant HIF1α;B,MSCs infected with viral solution containing wild-type HIF1α;C,MSCs infected with viral solution without any HIF1α;and D,MSCs without viral infection. The efficiency of infection was observed by the expression of human renilla reniformis green fluorescent protein (hrGFP).The expression levels of HIF1α mRNA and protein in infected cells in each experimental group were measured.For the in vivo experiment,the MSCs were divided into the same four groups and infected with the virus solutions from each group and cultured under normoxic conditions.At 72h after the infection,the MSCs were used as seed cells and transplanted into Apatite-wollastonite magnetic bioactive glass-ceramic (AW MGC)vector to construct artificial tissue-engineering scaffolds,and then the scaffolds were implanted into the in vivo rabbit radial bone defect model.The animals from each group were sacrificed 8 weeks after the surgery and the tissues from the implantation region were harvested for evaluation of angiogenesis.Results The 402,564 and 803 amino acids in CDS area were point mutated into alanine;three types of recombinant adenovirus were successfully constructed, packaged and characterized.The expression levels of HIF1αmRNA were significantly higher in Group A and Group B than in Group C and Group D (P <0.05 ).There was no significant difference between Group A and Group B group or between Group C and Group D (P >0.05 ).The HIF1α protein expression in Group A was significantly higher than that in the other three groups (P <0.05);however,there was no significant difference among groups B,C and D (P >0.05).There was prominent angiogenesis in bone defect region in Group A,but not in the other three groups.Conclusion ① Triple point-mutants of HIF1αefficiently expressed functional proteins under normoxic conditions.② Triple point-mutants of HIF1αeffectively promoted in vivo angiogenesis in bone defect region.
5.Establishment and evaluation of prognostic model of severe traumatic brain injury

Objective To build the prognostic model of STBI and provide guidance for making clinical decision .Methods A total of 486 patients with STBI was collected .Six months after injury ,according to GOS ,the patients were divided into five levels :good , mild-to-moderate disability ,serious disability ,persistent vegetative state and death .Ordinal regression analysis were used to find out the main factors of different populations .Results There were significant differences between different levels in nine factors .The re-sult of ordinal regression analysis showed that GCS ,age ,rescue time and reaction of pupil to light stood as independent variables in prognosis of patients with STBI(P<0 .05) .The outcomes from the prognostic model were pretty much exactly the same as the ac-tual results(Kappa= 0 .640 ,P= 0 .026) .Conclusion Though using the prognostic model of STBI ,the present study found out prognosis was closely related to GCS ,age ,rescue time and reaction of pupil to light in patients with STBI ;thus it could provide guidance for prognosis evaluation .

The positive significance of the system of overall responsibility by the attending physician in the management of the hospital reform is also identified.

8.A prospective study On etiology of chronic cough

Objective To investigate the characteristic of etiology of chronic cough in Shenzhen.Methods The chronic cough etiology was analyzed in 136 cases with the guidance of cough diagnosis and treatment guidelines(2009 editions)published by Chinese Medical Association.The cough was the main or sole symptom,the duration was no less than 8 weeks and chest X-ray film was normal.Results The causes of chronic cough was confirmed in 125 patients and was not definitely diagnosed in 11 patients by inspection and treatment.Cough due to single cause was found in 104 patients(83.20%,104/125),due to compound causes was found in 21 patients(16.80%,21/125).The first 4 etiologies were cough variant asthma(CVA)with 57 patients(36.31%,57/157),upper airway cough syndrome(UACS)with 41 patients(26.11%,41/157),eosinophilic bronchitis(EB)with 17 patients(10.83%,17/157),occupational injury(including harmful,toxic substances inhalation,etc.)with 10 patients(6.37%,10/157).Conclusions The most common cause of chronic cough in Shenzhen is CVA,UACS,EB.Due to the developed industrialization,there is a lack of understanding the cough course of inhaling more harmful and toxic gases and substances in the manufacturing process.So this should be paid more attention.
9.Effect of Xinmailing Solution on neuronal cell injury induced by glutamate

AIM: To detect the effects of Xinmailing Solution and MK-801 on injury of neuronal cell induced by glutamate. METHODS: Cultured neuronal cell injuried by glutamate was prepared and the content of malondialdehyde and nitrite in cell supernatant was measured. Morphology changes were also observed with discrepancy microscope at the same time. RESULTS: Xinmailing Solution and MK-801 attenuated cell injury induced by glutamate,and inhibited increase in malondialdehyde and nitrite in cell supernatant. CONCLUSION: Xinmailing Solution had a protective effect on neuronal cell at cell level.
10.Case-control Study on Small Splint and Plaster External Fixation after Closed Manipulative Reduction in the Treatment of Elderly Type C Colles Fractures

Objective To compare the clinical effects of small splint fixation after closed manipulative reduction with plaster external fixation in the treatment of elderly type C Colles fractures.Methods Totally 96 elderly patients with type C Colles fractures were randomly divided into two groups, 48 patients in each group. Both groups were treated with closed manipulative reduction. The fractures were externally fixed with splint in the treatment group, and those were externally fixed with pipe plaster in the control group. The figure of radius length, palmar tilt and ulnar deviation were detected respectively in pre-reduction, the same day with reduction, and the day when external fixation was removed (6th week), in order to evaluated the drop of anatomical position. The therapeutic effects were evaluated according to Gartland-Werley wrist score when external fixation was taken off after 6 months.Results In both groups, figures of radius length, palmar tilt and ulnar deviation evidently increased in the day after reduction and the day of external fixation removal, compared with pre-reduction (P<0.05). Compared with the day after reduction, the radius length, palmar tilt and ulnar deviation of the day of external fixation removal all dropped in both groups (P<0.05). But the drop degree in control group was significantly higher than treatment group (P<0.05). For the joint function, 9 cases for excellent, 26 for good, 6 for fair, and 4 for bad in the therapy group. The excellent and good rate was 77.8% (35/45). In the control group, 4 cases for excellent, 22 for good, 10 for fair and 9 for bad. The excellent and good rate was 57.8% (26/45). The curative effect of treatment group was remarkably superior to control group (P<0.05).Conclusion Closed manipulative reduction combined with splint external fixation is more effective than pipe plaster in treatment of elderly type C Colles fractures.
