1.Application of data envelopment analysis in assessment of nursing efficiency of a general hospital

Objective To study the effect of data envelopment analysis evaluation on nursing efficiency in general hospitals.Method The input indexes including the number of enrolled nurse,indirect nursing time,average annual nursing consumables and average annual continuing education time and the output indexes including hospitalized patient number and year-round nursing income of 10 nursing units in our hospital during October 2012 to October 2013 were collected and analyzed with C2R,C2GS2 model and C2R/ C2GS2 ratio from Data Envelopment Analysis.Result About 5 nursing units were effective in terms of overall efficiency,8 ones in terms of unit technical efficiency and 5 one in terms of nursing unite scale efficiency.Conclusions We need improve nursing efficiency in our hospital.Nursing managers should pay attention to the results and improve working process aiming at the practical situations.
2.Effect of insulin combined with Edaravone therapy on cerebral vasospasm after experimental subarachnoid hemorrhage

Objective To investigate the effect of insulin combined with Edaravone therapy on delayed cerebral vasospasm (CVS) after experimental subarachnoid hemorrhage (SAH).Methods 40 New Zealand white rabbits were randomly separated into sham-operated group (n=8),SAH group (n=10),insulin group (n=10) and insulin+Edaravone group(n=12).SAH models were made by injecting blood twice into cisterna magna.30 min later of the first blood injected,the insulin 0.2 U/kg was subcutaneous injected in insulin group and insulin+Edaravone group,3 times daily for 7 d;while,the insulin+Edaravone group received Edaravone 0.5 mg/kg (intravenous injected in ear vein) twice a day for 7 d.7 d after the model made,the basilar artery cross sectional area was measured and the pathological changes were obseved to estimate the degree of CVS.The expression of insulin receptor(InRa) in vascular endothelial cell was detected by immunohistochemical staining.Results The basilar artery cross sectional area between insulin+Edaravone group[(0.46?0.3)mm2]and the sham-operated group[(0.48?0.4)mm2] was no significant difference;but they were obviously bigger than those in SAH group[(0.25?0.3) mm2] and the insulin group [(0.30?0.3)mm2](all P
3.Prediction of the Shelf Life of Compound Sodium Citrate Injection by Initial Average Rate Stability Test

OBJECTIVE:To predict the shelf life of compound sodium citrate injection by initial average rate stability test. METHODS: The initial average rate stability test was performed at 7 different temperatures to investigate the relationship between the initial average rate (V0) of the glucose degradation reaction and Kelvin's temperature (1/T), and the activation energy of reaction, the drug reaction velocity constant and shelf life were computed as well. RESULTS: The regression equation was lgV0=18.15—6 774.6?1/T(r=—0.937 5), the reaction activation energy was 31.0Kcal?mol-1, the drug reaction velocity constant at 20℃ was 1.1?10-5h-1, and the shelf life was 1.09 years. CONCLUSION: The method is simple and it can accurately predicate the shelf life of compound sodium citrate injection.
4.Clinic significance of four clinical scoring systems in evaluating prognosis of acute pancreatitis

Objective To compare the clinic significance of four clinical scoring systems in evaluating prognosis of acute pancreatitis: bedside index for severity in acute pancreatitis(BISAP), acute physiology and chronic health evaluation (APACHEⅡ), Ranson’s scoring system, computed tomography severity index (CTSI) in AP. Methods Patients visited our clinic with AP (n=114) in recent 2 years were retrospectively analyzed. BISAP and APACHEⅡscores were obtained at 24 hours after admission; Ranson ’s score was obtained at 48 hours after admission and CTSI are obtained was obtained at 72 hours after admission. Results of four scoring system were compared under different causes and different severity of the dis?ease. Correlation between BISAP score and the other three scores were analyzed and the predicative value of all four scoring systems for severity of AP and death were also compared. Results The mean values of four scoring systems show no signifi?cant difference in AP patients with different etiology (P>0.05). The BISAP score is positively correlated with APACHE-Ⅱ, Ranson ’s score and CTSI score (P<0.01). The four scoring systems all present good predictive value on the severity of AP and death (P<0.01). Conclusion The four scoring systems can all be applied to grading and prognosis for AP of various causes. BISAP is a simple, prompt, economical scoring system in clinical practice.
5.Preparative Method of CRRT Displacement Liquid

OBJECTIVE:To investigate the feasibility and accuracy of the current preparative method of CRRT displacement liquid in hospital preparation manufacturing laboratory.METHODS:The electrolytes in 8 CRRT displacement liquid samples were determined and the RSD values were calculated by comparison with theoretic value;the effects of ionic concentration and volume of raw material on the preparing results as well as the RSD of volume error were investigated.RESULTS:The RSD between theoretical and practical ionic concentrations was lower than 4.14%.The maximum change of ionic concentration caused by concentration of solution was 5.32 mmol?L-1;The maximum volume change caused by volume loading was 62 mL and the displacement liquid ionic concentration lower than theoretical value was 0.4 mmol?L-1.The RSD of volume difference was 2.46%.CONCLUSION:The preparing results of CRRT displacement liquid by the preparative method adopted in our hospital can meet the clinical standard on this preparation.
6.Increased expression of ROCK-Ⅰ, p-MBS Thr-697 and α-SMA during hepatic fibrogenesis

Objective To explore the dynamic expression of ROCK-Ⅰ,p-MBS Thr-697,α-SMA protein and their mRNA in the hepatic fibrogenesis and the changes of actin cytoskeleton.Methods ROCK-Ⅰ and p-MBS Thr-697 protein in liver were determined by Western blot and their mRNA was examined by reverse transcription-polymerase chain reaction (RT-PCR),while the distribution of ROCK-Ⅰ and α-SMA in liver was assessed immunohistochemis-tically.The change of actin cytoskeleton was shown by fluorescence.Results With the development of hepatic fi-brosis,the positive areas in model groups at week 1 to 4 of ROCK-Ⅰ and α-SMA of the rat livers were larger than that in control group respectively(P <0.05).ROCK-Ⅰ,p-MBS Thr-697 ,α-SMA protein and mRNA were increased than that in control group respectively.ROCK-Ⅰ mRNA expression correlated with α-SMA (r =0.718,P <0.05).With the development of liver fibrosis,the images of fluorescence were inhanced.Conclusion With the develop-ment of liver fibrosis,both protein and mRNA of ROCK-Ⅰ increased.

Objective:? N oxalo L ?,? diaminopropionic acid were isolated from the powder of red ginseng. its physiological characteristics were studied. Method:This material is obtained by CM Sephadex C 25 and FPLC MonoQ. We have examined the effect of ? N oxalo L ?,? diaminopropionic acid on contraction of artery of guinea pig. Result:When histamin and adrenalin do not exist, there is neither contraction nor relaxation. It can strengthen induction of contraction of artery of guinea pig, but it has no such effect on adrenalin. Besides, it doesn't induce plaque coagulation through adrenalin, ADP blood coagulation enzyme and collagen. It doesn't suppress the transformation from tension peptide of blood vessel to enzyme.Conclusion:Hemostatic effect of ? N oxalo L ?,? dlaminopropinomie acid may caused by vesseles contraction through promoting Histamine.

Objective To study clinical and laboratory features of children with brucellosis and to provide a theoretical basis for clinical diagnosis and treatment.Methods Using the retrospective analysis method,the clinical diagnosed information of child patients with brucellosis from January 2013 to January 2016 in Heilongjiang Agricultural Reclamation Bureau General Hospital were selected to analysis the general characteristics,clinical symptoms and laboratory examination results.At the same time,follow the case control method,dadult brucellosis laboratory results were selected,and the laboratory examination results were compared between child and adult patients.Results The children of > 12-15 years of age had the highest incidence in 65 cases of brucellosis,and the rate was 47.7% (31/65).Clinical symptoms were fever and joint pain,which accounted for 83.1% (54/65) and 84.6% (55/65),respectively.The mean values of γ-glutamyl transpeptadase (γ-GT),α-hydroxybutyric dehydrogenase (HBDH),creatine kinase isoenzyme (CK-MB) and lactate dehydrogenase (LDH) were compared between child brucellosis and adult brucellosis,and the differences were statistically significant [(30.66 ± 27.05) vs (82.80 ± 36.34) U/L,(287.40 ± 35.02) vs (180.08 ± 40.90)U/L,(20.88 ± 13.01) vs (11.70 ± 4.13)U/L,(328.69 ± 37.86) vs (217.88 ± 45.67)U/L,t =-4.243,3.396,3.756,3.135,all P < 0.05].After treatment,the test results of children with aspartate transaminase (AST),alanine aminotransferase (ALT),γ-GT,total protein (TP),albumin (ALB),HBDH,CK-MB and LDH were compared with those of before the treatment,the differences were statistically significant [(56.95 ± 29.50) vs (23.00 ± 12.26) U/L,(57.60 ± 35.65) vs (23.94 ± 13.38) U/L,(30.66 ± 27.05) vs (18.45 ± 12.62) U/L,(63.77 ± 5.51) vs (67.42 ± 3.50) g/L,(39.15 ± 3.89) vs (43.06 ± 3.11) g/L,(287.40 ± 35.02) vs (139.23 ± 51.70) U/L,(20.88 ± 13.01) vs (11.61 ± 10.26) U/L,(328.69 ± 37.86) vs (187.77 ± 42.32) U/L,t =4.848,2.717,2.182,-5.442,-13.752,6.989,4.749,5.993,all P < 0.05].Conclusions Liver and heart of children with brucellosis are injured.By monitoring the changes of myocardial enzymes,medicines for children might be cautiously used in clinical trial,and myocardial damages of children with brucellosis can be protected and prevented.
9.Improvement of the current routine method of WBC counting in cerebrospinal fluid

Objective To improve the reliability and accuracy of WBC counting in cerebrospinal fluid (CSF) ,this article is stud-ying the improved method of WBC counting in CSF by finding out the optimum percentage of CSF specimen with the most suitable concentration of acetic acid .Methods CSF specimen was mixed with different acetic acid at different ratio respectively .WBC counts were performed in 5 minutes on diluted samples of various concentrations .A series of 20 CSF specimens were analyzed via the proposed assay and conventional method .The average value and coefficient of variation (CV) of WBC count of each sample were c compared and analyzed .Results The optimum percentage of CSF sample was obtained at 60∶40 ratio .In this percentage , the maximal WBC count (189/μL) was obtained compared that of conventional method (161/μL) .Moreover ,the CV of the WBC counts in this percentage (7% ) was also lower than that of the conventional method (18% ) .Conclusion The reliability and accur-ancy of WBC counting in CSF was the optimum percentage of CSF specimen and 5% acetic acid was 60 :40 .It may lead to a more reliable ,accurate and standard way of WBC counts in CSF .
10.A comparison between the rapid enzyme-linked immunosorbent assay and the traditional serological methods in diagnosis of human brucellosis

Objective To investigate the differents of rapid enzyme-linked immunosorbent assay(IELISA) and the traditional serological methods in diagnosis of human brucellosis.Methods Brucella antibody was detected by IELISA,rose bangel plate test (RBPT),standard agglutination test (SAT),Coomb's test and cysteine test of serological methods in 430 confirmed and suspected patients and 300 healthy controls during the same period.The consistency was analyzed between IELISA and other tests.Results Positive rates of patients were significandy higher than those in healthy controls (IELISA:83.49% vs.0;RBPT:86.97% vs.0;SAT:76.27% vs.0;Coomb's:65.58% vs.0.02%;and cysteine test:67.91% vs.0,x2 =535.05,412.47,437.66,339.22,489.49,all P < 0.01).Positive consistency rates between IELISA and other tests were 87.19%,79.39%,71.59% and 76.60%,the highest was between IELISA and RBPT,the lowest was between IELISA and Coomb's.Conclusions Brucella antibody is detected conveniently,quickly,and accurately by the joint application of RBPT and IEHSA,which provides important technical support for prevention and control of brucellosis.It is worthy of application extensively.
