1.Therapeutic effects of modified Bushen Huoxue granules on ovariecto-mized osteoporosis rats

AIM: To observe the therapeutic effect of modified Bushen Huoxue granules (MBHG) on ovariec-tomized osteoporosis rats.METHODS: Female SD rats were randomly given sham operation (n =10) and ovariectomy, and then the model rats were further randomly divided into model group, MBHG treatment groups at doses of 200 mg/kg, 100 mg/kg and 50 mg/kg respectively, and positive control (estradiol valerate) group, with 10 rats in each group by intra-gastric administration for 12 weeks.The morphology, area, thickness, spacing and area percentage of trabecular bone in the rats were observed.The serum levels of calcium (Ca), phosphorus (P) and alkaline phosphatase (ALP) were mea-sured by automatic analyzer.Bone mineral density (BMD) was analyzed.Serum estradiol (E2 ), osteocalcin (BGP), os-teoprotegerin (OPG), receptor activator of nuclear factor-κB (RANK) and receptor activator of nuclear factor κB ligand (RANKL) levels were detected by ELISA.RESULTS: Compared with model group, trabecular bone significantly widened in all treatment groups with large number, and net-like structure restored partially.The thickness, area and area percenta-ges of trabecular bone in treatments groups were higher than those in model group,and trabecular spacing was less than that in model group (P <0.05).The serum Ca, P, E2 and OPG, and femoral BMD were significantly higher in treatment groups than those in model group, and the levels of ALP, BGP, RANK and RANKL were significantly lower than those in model group (P <0.01).CONCLUSION: MBHG has a significant therapeutic effect on ovariectomized osteoporosis rats. The mechanism may be related to the regulation of OPG and inhibition of RANKL secretion.
2.Finite element analysis of prosthesis position during hip arthroplasty

BACKGROUND:Three-dimensional finite element studies found that the instal ation location of the hip prosthesis is closely related to the effects of hip replacement, but studies on different anteversion and abduction angle of the hip prosthesis mounting position on the effects of hip biomechanics are not much. OBJECTIVE:To analyze the biomechanical characteristics of different prosthetic positions in hip arthroplasty using three-dimensional finite element analysis. METHODS:One healthy volunteer was selected and data of pelvis and femur were col ected. Three-dimensional geometric model of this volunteer was established and received finite element network partitioning. CT scan was used to obtain prosthesis model data and accurate prosthesis model was established. 16 kinds of different prosthesis position model of hip replacement were established. Vertical y downward force of 780 N was applied on the top of sacroiliac joint and pubic symphysis. The acetabular stress peak, femoral stress peaks, polyethylene liner peak stress and polyethylene liner damage zone volume of 16 kinds of model were observed. RESULTS AND CONCLUSION:(1) The acetabular stress peak, femoral stress peaks, polyethylene liner peak stress and polyethylene liner damage zone volume of 16 kinds of different prosthesis position model of hip replacement, anteversion angle 10° abduction angle 50° got the best results. The acetabular stress peak was 51.23 MPa;femoral stress peak was 26.34 MPa. Polyethylene liner peak stress was 5.288 MPa and polyethylene liner damage zone volume was 2.239×10-7 m3. (2) These results indicated that anteversion angle 10° abduction angle 50° is the ideal hip replacement degree. The peak stresses of acetabulum, femur, and polyethylene liner at this instal ation angle are minimum;polyethylene liner damage zone volume is also minimum. This can provide data for clinical reference.
3.The role of bone marrow mesenchymal stem cells transfected with neurotrophin-3 in treatment of spinal cord ischemia-reperfusion injury

Objective To observe the mechanisms of bone marrow mesenchymal stem cells (BMSCs) transfected with neurotrophin-3 in treating nanoparticle repaired spinal cord ischemia-reperfusion (I/R) injury of adult rat. Methods BMSCs were transfected into NT-3 with nanoparticle carrier and transplanted into 80 adult rats on the spinal cord I/R injury model. Then the rats were divided into four groups: sham group, NS group, BMSCs and BMSCs+NT-3. The BBB scoring system, the neurons cell apoptosis, levels of malondialdehyde (MDA) and myeloperoxidase (MPO) were observed and compared between the four groups. Results The BBB score of the BMSCs+NT-3 group was significantly higher than those of BMSCs and NS group , but was lower obviously than the Sham group. TUNEL-apoptosis cells were significantly decreased in the BMSCs + NT-3 group compared to BMSCs and NS groups, but was more significantly lower than the Sham group. The contents of MDA and MPO of the NS group were significantly higher than the BMSCs group. The contents of the BMSCs + NT-3 group were lower than those of the BMSCs group, but lower significantly than the Sham group (P < 0.05). Conclusion BMSCs transfected with NT-3 with nanoparticle carrier could be induced each other. When transplanted into SCI , they can repair spinal cord by alleviating neuron cells apoptosis and inhibiting the lipid peroxidation of free radicals.
4.Studies on correlativity between in vivo and in vitro release of XIANGHE SUPPOSITORY

Object To study the correlativity between the in vivo release of healthy volunteers and the in vitro dissolution of XIANGHE SUPPOSITORY(XS). Methods The electromagnetic stirring method was used to determine the in vitro dissolution of XS, and the in vivo release was got by the indirect method, assaying the content of the left XS after it had released in administering system, HPLC was used for the determination of XS with the index of variant content of berberine hydrochloride. Results A good correlativity was shown between the percentage of in vivo release and in vitro dissolution of XS. Conclusion In vitro dissolution test under the given conditions could indicate the in vivo absorption of XS.
5.Application of Different Routes of Administration of Tranexamic Acid in One Stage Posterior Surgery of Thoracic Tuberculosis

OBJECTIVE:To observe the effects of different routes of administration of tranexamic acid on coagulation function and amount of bleeding in patients with one stage posterior surgery of thoracic tuberculosis. METHODS:40 patients suffered from thoracic tuberculosis in our hospital from Jan. 2011 to Dec. 2013 were randomly divided into intravenous group(5% Glucose injec-tion 100 ml+tranexamic acid 10 mg/kg,through an intravenous drip at 30 min before closing the wound) and topical application group(5% Glucose injection 10 ml and tranexamic acid 10 mg/kg,through soaking the wound before closing the wound)with 20 cases in each group. Other 15 cases suffered from the thoracic tuberculosis in our hospital from Jan. 2009 to Dec. 2010 were includ-ed in control group. 3 groups received one stage posterior surgery of thoracic tuberculosis,interbody fusion and internal fixation. The difference of hemoglobin,coagulation function and the amount of suction drainage were observed before and after surgery, and followed up. Bone graft fusion and therapeutic condition of tuberculosis were observed in the study. RESULTS:There was no statistical significance in postoperative suction drainage between intravenous group and topical application group (P>0.05),but their decrease was more significant than control group,with statistical significance(P<0.05). There was no statistically difference in fiber protease,prothrombin time or activated partial thromboplastin time among 3 groups(P>0.05). The difference value of he-moglobin in control group before and after operation was significantly higher than in intravenous group and topical application group,with statistical significance (P<0.05);there was no statistical significance between intravenous group and topical applica-tion group(P>0.05). 55 patients were all followed up and bone graft of all cases were fused,and all patients were cured and no case recurred. CONCLUSIONS:Tranexamic acid by intravenous application or topical application can reduce hemorrhage and ane-mia after operation of thoracic tuberculosis,and has no effect on blood coagulative system.
6.Osteogenic differentiation of adipose-derived stem cells and the effect of melatonin on the bio-viability of differentiated cells

BACKGROUND:Studies have indicated that melatonin can promote the differentiation of adipose-derived stem cels into neurons, and the effect of melatonin on the osteoblasts form adipose-derived stem cels is rarely reported.
OBJECTIVE:To observe the osteogenic differentiation of adipose-derived stem cels and the effect of melatonin on the bio-viability of differentiated cels.
METHODS:(1) Adipose-derived stem cels were isolated and purified from the inguinal fat of Kunming mice by type I colagenase digestion and differential adhesion method, respectively. Immunohistochemical staining of CD44 was used as a quality control. (2) Osteogenic induction medium was added to induce osteogenic differentiation of passage 2 adipose-derived stem cels. Alkaline phosphatase staining and von Kossa method were combined to evaluate differentiation condition. (3) Melatonin at variable concentrations was added to treat mature osteocytes originated from adipose-derived stem cels and MTT was applied to determine their viability at 24 and 48 hours after culture respectively to find out optimal condition of melatonin treatment. (4) Melatonin at the optimal concentration was used to treat differentiated cels and detect alkaline phosphatase activity after 3 days and 6 days respectively.
RESULTS AND CONCLUSION: (1) After seeding for 48 hours, most cels were adherent, and after 4 days, the cels displayed multiple shapes and colonies of different sizes formed. After subculture, cel morphology homogenized as spindle shape. Cels positive for CD44 were brownish yelow, and localized mainly on the cel membrane. (2) Differentiated cels were positive for von Kossa staining and black sediments scattered in the extracelular matrix. Alkaline phosphatase expressed positively, and brown-black particles, appeared within cels. (3) Melatonin supplement improved the viability of differentiated cels; and 1, 10 and 100 μmol/L was observed as the optimal concentrations both at 24 and 48 hours. (4) The intracelular alkaline phosphatatse activity was increased with time in al the groups (P < 0.05). Compared with the blank group, the intracelular alkaline phosphatase activity in Melatonin groups (1, 10 and 100 μmol/L) had nochanges at 3 days, but significantly increased at 6 days (P < 0.05). These findings indicate that melatonin can enhance the proliferation of osteocytes differentiated from adipose-derived stem cels, and improve the activity of intracelular alkaline phosphatase.
7.Poly-IC protects the spinal cord in inflammatory response to ischemia-reperfusion injury in rats

Objective Polyinosinic-polycytidylic acid (poly IC) plays an important role in the central nervous system damage and repair.This study was to investigate the effect of poly IC on inflammatory response after spinal cord ischemia-reperfusion injury (SCIRI) in rats.Methods A total of 72 healthy adult male SD rats were equally randomized into a sham-operation, an ischemia-reperfusion (IR), and a poly IC group.The abdominal cavities of the rats were cut open and closed again in the sham-operation group and SCIRI models were established in the IR and poly IC groups by clamping the abdominal aorta, followed by reperfusion 60 minutes later and intraperitoneal injection of saline (0.1 mL) and poly IC (1.25 μg/g), respectively.At 6, 24, and 48 hours after modeling, BBB scores were obtained and the contents of TNFα, IL-1β and IFN-β were measured by ELISA.At 48 hours, the expressions of NF-κB and IL-10 were determined by immunohistochemistry, the area of ischemic necrosis in the spinal cord tissue was calculated by TTC staining, and its morphological changes were observed under the optical microscope.Results At 48 hours after modeling, the BBB scores were significantly lower in the IR and poly IC groups than in the sham-operation group (3.80±0.75 and 9.40±0.49 vs 20.00±0.00, P<0.01), though higher in the poly IC than in the IR group (P<0.01).The rats of the IR group showed extensive degenerated neurons in the gray substance of the spinal cord, with scattered foci of bleeding and blood coagulation, while those of the poly IC group exhibited fewer necrotic neurons and basically normal nuclear morphology, though with a few swelling cells.The ischemic necrosis area of the spinal cord tissue was significantly reduced.The expression of NF-κB was decreased while that of IL-10 increased markedly.Compared with the IR group, the poly IC group showed a significant increase in the expression of IFNβ (117.23±6.06 vs 55.65±4.02, P<0.01) and a remarkably decrease in the expressions of TNFα (190.45±4.16 vs 201.82±2.18, P<0.01) and IL-1β (39.27±2.48 vs 50.59±1.47, P<0.01) at 48 hours.Conclusion Poly IC can protect the spinal cord and reduce inflammatory response after spinal cord ischemia-reperfusion injury.
8.Zero-profile interboby fixation system for multi-level cervical spondylotic myelopathy

BACKGROUND: Anterior cervical decompression and fusion has been widely used in the treatment of multi-level cervical spondylotic myelopathy, but accompanied with lots of complications.OBJECTIVE: To investigate the efficacy of zero-profile interboby fixation system for multi-level cervical spondylotic myelopathy.METHODS: Seventy-one patients with multi-level cervical spondylotic myelopathy were randomly divided into two groups, and the patients in group A accepted zero-profile interboby fixation system, and group B accepted cage interboby fixation system. The Japanese Orthopaedic Association score, fusion rate, as well as the incidence of dysphagia and esophageal fistula were detected to compare the efficacy between two groups.RESULTS AND CONCLUSION: (1) All cases were followed-up for 3-34 months, average of 17.5 months. The excellent and good rate at the last follow-up showed no significant difference between two groups. (2) The final fusion rate did not differ significantly between two groups, but the fusion rate in the group A was significantly higher than that in the group B at 6 and 9 months postoperatively (P < 0.05). (3) There was one patient with mild dysphagia in the group A (3%), three mild, five medium, and two severe dysphagia in the group B (29%), which showed significant difference between two groups (P < 0.05). No internal fixation loosening occurred in the group A, but three cases in the group B. The blood loss, operation time and radiology times in the group A were significantly lower than those in the group B (P < 0.05). (4) These results suggest that the effect of these two surgical methods in promoting functional recovery of spinal cord and final fusion rate show no significant differences; however, the zero-profile interboby fixation system exhibits better postoperative stability and interim fusion rate, with lower incidence of dysphagia.
9.Analysis of the Utilization and Per Capita Burden of Antihypertensive Drugs Based on the Characteristics of the University Teaching Staff

OBJECTIVE:To understand the proportion of university faculty suffering from hypertension,and the regularity,ratio-nality and affordability of antihypertensive drugs under closed medical environment. METHODS:Empirical research and research methods were adopted to collect indicators during 2011-2014,such as average daily wage in Wuhan,the total number of staff in Wuhan University,the number of staff with hypertension,antihypertensive drugs amount/DDDs ranking ratio,affordability ratio,relative av-erage affordability, etc. The rationality and affordability of antihypertensive drug prices were evaluated. RESULTS:During 2011-2014,the prevalence rate of hypertension in the staff were about 23%,and the proportion increased year by year;the annual in-crease of antihypertensive drug amount was larger than that of the number of patients;the type of antihypertensive drugs with affordabil-ity ratio>1 in 4 years was ARB varieties,and affordability ratio of other drugs was less than 1. CONCLUSIONS:More than half of drugs is high in price,that result in heavy burden of patients receiving ARB;on the whole,antihypertensive drugs are affordable of the teaching staff of Wuhan University.
10.Mean color vessel density with contrast-enhanced ultrasound and microvessel density: comparative study

Objective To investigate the correlation between quantitative parameters with contrast-enhanced ultrasound-power Doppler(CEUS-PD) and mierovessel density (MVD) in hepatoeellular carcinoma (HCC). Methods Thirty-four cases with HCC underwent CEUS before operation. The satisfactory images were selected through CEUS-PD and analyzed with computer to calculate mean color vessel density(MCVD). The blood flows of focal lesions were graded into 0-Ⅲ based on the numbers of color vessels with CEUS-PD. The correlation between those parameters and MVD counted with immunohistochemistry was analyzed after operation. Results In 34 cases with HCC, the MCVD of focal lesions with CEUS-PD was 0. 17±0.09, the pathologic MVD was (62.59 ±23.96)/400 × after operation. The MCVD was positively correlated to MVD( r = 0. 56, P <0.05). In 34 cases HCC,3 cases were in grade Ⅰ , 12 cases were in grade Ⅱ , 19 cases were in grade Ⅲ. There was significant differences in pathologic MVD of every grade ( F = 8.06, P <0.05). But there was no linear tendency in the grades( F = 7.57, P<0. 05). Conclusions In focal lesions of 34 HCC, MCVD calculated with CEUS-PD was positively correlated to pathologic MVD. It can be used to evaluate tumor blood perfusion exactly before operation.
