1.Summary of the nation-wide autoantibodies test quality survey in 2006

objeetive To consecutively understand the current national clinical testing quality and enforce quality-control of auto-antibody detection.Methods Hospitals or departments were recruited by letters or telephone communications:The autoantibodies examined for quality control survey included anti-nuclear antibodies (ANA),anti-double-stranded DNA (A-dsDNA)antibody,anti-extractable nuclear antigens(A-ENA)antobodies,anti-mitochondria antibody(AMA)/anti-smooth muscle antibody(ASMA),and anti-CCP antibody.Each autoantibody was tested in 3 samples, and altogether 15 samples in total for testing.Sample designation and testing results data analysis were double-blinded.Results Fifltv-five hospitals/departments participated in this survey.The accuracy rates for this survey were 92%,89%,96%,72%respectively for ANA,A-dsDNA,AMA/ASMA,and anti-CCP.Anti-ENAs were further divided into anti-RNP,Sm,SSA,SSB and Scl-70 subgroups,and the accuracy rates were 98%,89%,92%,75%and 77% respectively.Conelusion Compared to the previous 3 national surveys.accuracy rates in our country's autoantibody testing is increaseing steadly with more testing items included each year.This indicats that the quality of auto-antibody testing is improving across the country.
2.Chemical constituents from stems of Lonicera japonica.

OBJECTIVETo investigate the chemical constituents from the ethanol extract of the stems of Lonicera japonica.
METHODThe constituents were isolated and purified by repeated column chromatography on silica gel, Sephadex LH-20 and MCI HP-20. Their structures were identified by phsicochemical properties and spectroscopic analysis.
RESULTThirteen compounds were isolated and identified as protocatechuic acid (1), caffeic acid (2), macranthoin G (3), esculetin (4), luteolin (5), quercetin (6), apigenin (7), luteolin-7-O-beta-D-glucopyranoside (8), isorhamnetin-7-O-beta-D-glucopyranoside (9), diosmetin-7-O-beta-D-glucopyranoside (10), rhoifolin (11), lonicerin (12), hydnocarpin D (13).
CONCLUSIONCompound 4, 7, 9-11 were isolated from this plant for the first time, while compound 13 was first reported flavanolignan from this genus Lonicera.

3.Role of immune response and inflammatory injury in the pathogenesis of liver failure

Liver failure is a syndrome of severe liver diseases commonly seen in clinical practice, and it has a high mortality rate and thus becomes one of the critical diseases in internal medicine. Massive hepatocyte death and the extent of hepatocyte death exceeding the liver’s regenerative capacity are considered the core events in the development and progression of liver failure, and direct injury and immune-mediated inflammatory injury are the two main factors in this process. An increasing number of evidence has shown that host immune response and inflammatory cascade play an important role in the pathogenesis of liver failure. This article reviews the mechanism of action of immunoregulation (congenital and adaptive) and inflammatory injury (inflammation inducers, receptor cells, and inflammatory mediators) in the process of hepatic failure, as well as the interactions between immune response and immune cells and between inflammatory response and inflammatory factors, in order to help understand the pathogenesis of liver failure and provide new ideas for the diagnosis and treatment of liver failure and drug research and development.
4.Comparison of Rotational Techniques between Chinese and World Elite Men’s Shot Put Athletes

Objective To compare rotational techniques used by Chinese and world elite men’s shot put athletes, so as to provide scientific references for Chinese male shot putters to improve their sports performance and results in international competitions. Methods Three-dimensional (3D) kinematics data from Chinese male shot putters in actual competitions were obtained, and kinematic parameters of Chinese and world elite men’s shot put athletes were calculated and compared. Namely, shot velocities and hip-shoulder separation angles at five critical instants of rotational techniques: right foot off, left foot off, right foot touchdown, left foot touchdown and release, as well as phase durations and shot travel distances during four critical phases: first single support, air-born flight, second single support and final delivery. ResultsCompared with world elite athletes, Chinese male shot putters using rotational techniques had significantly lower vertical release velocity, longer air-borne duration, smaller hip-shoulder separation angles at left foot off and touchdown instant, and longer shot travel distance during air-borne flight, but shorter shot travel distance during final shot delivery. Conclusions The difference in lower extremity strength is a primary casue leading to different sports performance between Chinese and world elite male shot putters. The technique differences in Chinese and world elite male shot putters mainly lie in different phase timing and shot travel distances during different techinique phases.
5.Application and Analysis of High-quality Evaluation for Jianwei Xiaoshi Tablets

Jianwei Xiaoshi tablets, as a common variety of Chinese patent medicine with "one product with many manufacturers", have many manufacturers and huge market sales. However, the phenomenon about uneven quality and discrepant price is prominent. Based on this, this study was carried out for the quality evaluation of Jianwei Xiaoshi tablets by applying the high-quality evaluation criteria with the quality as core for Chinese patent medicine, which was based on the full production cycle, from the multi-dimension including raw material selection, production process, quality control, post-marketing research and so on. The evaluation results showed that the quality evaluation scores of Jianwei Xiaoshi tablets from different manufacturers varied greatly (ranging from 35 to 66), indicating that the quality was significantly different. In the actual production, generally inadequate attention was paid to the quality of raw materials, and the quality of raw materials was insufficient with the score ratio of 43%, especially the poor consistency control of them. The role of good manufacturing practice was obvious, and the scores of production process were generally high with the average score ratio of 62%, and the maximum up to 80%. The technological advancement of the manufacturer was outstanding. The score ratio of quality control was only 31% that the internal quality standard of each manufacture almost stayed at the qualified line, which was equal to the national standard, and the consistency of products was insufficient. The post-marketing research was lacking with the score ratio of 37%. Manufacturers with high brand awareness and market share were upper scores, while the others lagged far behind. The results of this evaluation are in line with the overall prediction, which can provide a reference for the high-quality evaluation of Chinese patent medicine, and supply the scientific data for high-quality and high-price application.
6.Early clinical experience of transcatheter aortic valve implantation via apical approach for high-risk aortic valve disease in single-center

Objective To summarize the clinical experience in the treatment of high-risk patients with severe aortic valve disease by transcatheter aortic valve implantation (TAVI) via heart apex approach and to evaluate the early efficacy. Method Five patients who underwent TAVI via heart apex approach from September 2017 to February 2019 in Henan Thoracic Hospital were retrospectively analyzed, including 3 males and 2 females, aged 65-84 (74.6±4.5) years. Result All operations were performed through a small left incision into the thoracic cavity (3-5 cm), and then through the J-Valve transport system, the aortic valve was successfully released via heart apex after precise positioning under digital subtraction angiography. One patient developed ventricular fibrillation during the operation, and the operation was completed with the assistance of emergency femoral arteriovenous catheterization cardiopulmonary bypass; one patient underwent percutaneous coronary intervention first because of severe coronary stenosis; one patient had paroxysmal atrial fibrillation during the perioperative period, and had hepatorenal insufficiency and thrombocytopenia after the operation, and was improved after medical treatment; one patient had perivalvular leak during the operation, and was improved after re-implantation of the valve; one patient was in stable condition during operation and recovered smoothly after operation. Surgery was successful in all 5 patients. The follow-up time was 2-19 months, and the early clinical effect was good. Conclusion The short-term clinical efficacy of TAVI via heart apex approach in the treatment of high-risk severe aortic valve disease is definite and safe, but the long-term and medium-term effects need to be further evaluated.
