Objective　To search a simple, safe and effective osteotomy for hypertrophied mandibular angle.Methods　Hypertrophied mandibular angles were classified as lateral bulge, posteroinferior projection and combination based on their appearances. With local nerve blocking and infiltrating anesthesia, 162 cases with hypertrophied mandibular angel were respectively operated by intraoral approach with sagittal split, curved or combinative osteotomy method according to their types. Hypertrophied masseters were partially resected simutaneouly in 75 cases combined with masseter hypertrophy, and buccal fat pads were extracted in 52 cases associated with round faces.Results　Postoperative follow-up of 3～12 months to 60 cases of them showed 94.3 % of them were satisfactory.Conclusion　This operative method is simple and safe.It has minimal influence on the overall body of patients, and it is accord with the rules of aesthetic surgery.

Using PCR technique, the vp3 gene of chicken anemia virus (CAV) was cloned into the eukaryotic expression vector pcDNA3 to construct a recombinant pcDNA-vp3. Restriction enzyme digestion and sequencing analysis revealed that CAV vp3 gene was correctly inserted into the blank vector pcDNA3. After LipofectAMINE-mediated transfection in vitro with pcDNA-vp3 and pcDNA3 respectively, the total mRNA was extracted from liver carcinoma cell lines HepG2 and diploid cell line L-02, and RT-PCR was performed afterward. The results of RT-PCR suggested that vp3 gene was expressed in these two cell lines. At the same time, using in situ apoptotic detection assay, TUNEL kits, the apoptotic cells were found in pcDNA-vp3 transfected HepG2, but not in mock transfected cell lines. VP3 could induce cell death by apoptosis in cancer cell lines, but not in diploid cell lines. All the results indicated that CAV vp3 gene, a potential therapeutic agents, has the potential of being used for cancer treatment.

Objective: To investigate the clinical effect of ω-3 polyunsaturated fatty acids (ω-3 PUFA) on elder patients after gastric cancer radical operation. Methods: Forty patients were randomly divided into experimental group (EG) and control group(CG). The patients of CG were treated with hypocaloric and lower nitrogen parenteral nutrition, and those in EG received ω-3 PUFA additionally. The nutrition index, immune function, liver function, inflammatory reaction and complications were compared. Results: The rate of complications significantly decreased in the EG (20% vs 5% , χ~2 = 3. 958, P < 0. 05). There were no significant differences in albumn and hemoglobin between the two groups (P > 0. 05). The level of prealbumin was significantly increased in the EG. The humoral immunity and cellular immunity were higher in the EG than those in the CG. IL-6, TNF-α and CRP levels on postoperative d5 were significantly depressed in the EG. Conclusion: Postoperative supplementation of ω-3 PUFA may have a favorable effect on the outcomes in elder gastric cancer patients.

Summary: Receptor mediated gene delivery is a new gene transfer strategy. Asialoglycoprotein receptor (ASGP-R), the receptor of asialoorosomucoid (Asor), is specially expressed on the surface of hepatocyte. In this paper, the nuclide 131I was combined with Asor to form a kind of soluble nuclide-protein complex, which can be specifically endocytosed into hepatocyte by ASGP-R. After intravenous injection of the complex into experimental animals, the deposition of Asor in vivo and the targeting quality of hepatocyte was detected by ECT. This research testified the feasibility of targeting Asor complex delivery to hepatocyte mediated by ASGP-R in vivo, and provided foundation for the genetic diagnosis and gene therapy of hepatic cell-related diseases.

Objective To study the clinical efficacy of autologous fat transplantation combined with plasma fiber de-fatting in improving the facial contour.Methods A lot of 110 cases were collected.The surgical designation was according to the patients' facial features and their wishes.Facial fiber and autologous fat filling technology were combined in clinical application.The satisfaction and postoperational complications were evaluated.Results A lot of 110 cases were followed-up for 6 to 24 months postoperatively.95 cases were well satisfied.15 cases were satisfied.All cases had no hematoma,induration,infection and other complications.The effect was stable.Conclusions Autologous fat transplantation combined with plasma fiber de-fatting can effectively improve the facial contour and skin texture.

Objective To investigate the value of iFlow color-coding technique in quantitative real-time analysis of hemodynamic changes after transarterial chemoembolization (TACE) for hepatocellular carcinoma (HCC) . Methods A total of 31 patients who were diagnosed with HCC in Shanghai Fifth People's Hospital from December 2015 to January 2017 were enrolled. No patient underwent surgical operation or ablation. All patients underwent TACE with the same contrast agent, high-pressure injector parameters, and place of angiographic catheter. The iFlow technique was used to generate two-dimensional color-coded images and time-density curve (TDC) before and after surgery and measure the opening of the angiographic catheter and the time to peak (TTP) of the starting and ending points of the major tumor feeding arteries, as well as the ratio of the areas under the curve (AUC) of TDC of tumor tissue and the opening of the angiographic catheter. The paired t-test was used for comparison of continuous data between groups. Results TTP of the major tumor feeding arteries was 4.64 ± 0. 49 s before TACE and 5. 97 ± 0. 84 s after TACE (t = 11. 57, P ＜ 0. 01), and there was a significant difference in AUC between the tumor tissue and the opening of the angiographic catheter (0. 53 ± 0. 15 vs 0. 16 ± 0. 12, t = 25. 85, P ＜ 0. 01) . There was no significant difference in TTP between the opening of the angiographic catheter and the major tumor feeding arteries before and after TACE (P＞ 0. 05) .Before TACE, the TDC of tumor feeding arteries had a shape of"rapid increase-rapid reduction"with relatively high slope and peak value, while after TACE, the TDC had a shape of "increase-flat-reduction"with reductions in slope and peak value. Conclusion The iFlow technique can perform real-time measurement of TTP and TDC of the region of interest and helps with quantitative evaluation of hemodynamic changes in HCC. Therefore, it can provide objective quantitative indices for evaluating the degree of tumor embolism.

Objective To establish a method for the determination of perchlorate in food by ultra performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS).Methods The perchlorate residue in spices and condiments was extracted with water,that in vegetables and fruits was extracted with acetonitrile-water (1∶ 1,V/V),and that in meat,poultry,eggs,milk and aquatic products was extracted with acetonitrile-water (2∶ 1,V/V).The supernatant was cleaned up with C18 SPE (3 ml,200 mg),and the detection was carried out by UPLC-MS/MS with internal standardmethod for quantification.Results The calibration curve was linear in the concentration range of 0.3-20.0 pg/L (R2 ≥0.999),the recovery was in the range of 82.6％-108.6％,the relative standard deviation (RSD) was in the range of 1.0％-9.9％,and the limit of detection was 2.0 μg/kg for milk,and 10.0 μg/kg for other food.Conclusion The method was simple,accurate and highly sensitive,and suitable for the determination of perchlorate in food.

Primary lesion removal and lymph node dissection are the main constituents of radical gastrectomy. However, the high recurrence rate after D2 radical gastrectomy for advanced gastric cancer has not improved. Recently, studies have found that discrete tumor deposits in the mesogastrium may be an important factor affecting the prognosis of gastric cancer after surgery. With the development of laparoscopic equipment, the ever-expanding "submicroscopic vision" makes it possible to completely remove the mesogastrium. Professor Gong Jianping advocated "membrane anatomy" to optimize the concept of radical gastrectomy: D2- based complete mesenteric resection (CME), namely D2+CME procedure. To prevent the leakage of tumor cells into the surgical field, as histological barrier, the intact mesogastrium should be located. The essential difference between D2+CME and previous D2/D2+systematic mesogastrium excision (SME), en-bloc mesogastric excision (EME) is as follow: double-factor guiding (lymph nodes and discrete tumor deposits) vs. single factor guiding (lymph nodes only). After practicing dozens of radical gastrectomy (D2+CME) authors believe that its conceptual connotation (double factor guiding) and operational extension (above mesentery bed) cover D2. In D2+CME surgery, depending on the anatomical identification under the magnified field of view, the conformal space between gastric mesentery and mesenteric beds is unique operational plane with repeatability. These findings and considerations address one problem: where is the precise boundary of en bloc principle in radical gastrectomy? In author′s opinion, with laparoscopy and "sub-microsurgery" progression and detection of discrete tumor deposit metastasis, survival benefit from definition of en bloc boundary in radical gastrectomy will be widely recognized. Meanwhile, D2+CME procedure is an appropriate way for study. Although the development of the "membrane anatomy" concept for gastric cancer still requires many further clinical and basic researches, it is reasonable to foresee that D2+CME surgery will guide a concept-optimized era for gastric cancer surgery.

In order to testify the antitumor effect, especially its effect against liver carcinoma in vivo, of VP3 protein, one kind of protein coded by chicken anemia virus, recombinants pcDNA-vp3 containing chicken anemia virus vp3 gene, and control vector pcDNA3 were mixed with murine liver carcinoma cell lines H22 respectively. The mixture was injected subcutaneously into Balb/C mice. Some days later, the mice were killed and the solid tumor weighed. The antitumor efficiency was evaluated. The manners of VP3 protein in vivo inducing tumor cell death were identified by using TUNEL assay. All the results suggested that the injection of pcDNA-vp3 and H22 mixture resulted in a significant reduction of tumor growth in mice when compared with the results of control groups. TUNEL assay revealed that VP3 induced apoptosis in vivo. All these indicated that CAV vp3 might be a potential new gene in reducing the growth rate of tumor cells in liver carcinoma or in other kind of solid tumors in vivo.

Objective The aim of this study was to investigate and analyze the etiology, seizure type and anti-epileptic drugs (AEDs) utilization of in-patients with epilepsy Methods The study included 5563 cases in-patients with epilepsy. The etiology and seizure type and the date of type, quantity of AEDs in-patient department were collected and their usage frequency were analyzed statistically. Results The most common etiology of epilepsy was traumatic brain injury(13.64%), followed by hippocampal sclerosis (11.52%), stroke (5.24%), nervous system infection (4.98%), perinatal injury ( 5 . 28 % ) and undefined etiology ( 40 . 80 % ) . The most common seizure type was partial seizures (45.43% ). The AEDs of carbamazepine and valproate were the most common used drugs in the clinical and their average usage frequency were 36.88% and 30.80%, respectively. The newer AEDs of Lamotrigine and Levetiracetam were used more frequently. The use of Lamotrigine increased from 16.16% to 28.44% and the Levetiracetam from 0.61% to 20.87% whereas the use of Oxcarbazepine and Topiramate remained a stable level of 15.07% and 9.42%. Conclusion The etiology of epilepsy is complicated and the seizure type of epilepsy was diverse. Among a great variety of anti-epileptic drugs, the newanti-epileptic drugs are being increasingly used.
