1.Influences of exercise rehabilitation pathway on postoperative rehabilitation effect in patients with craniocerebral injury

Objective To explore the influences of exercise rehabilitation pathway on postoperative rehabilitation exercise effect in patients with craniocerebral injury.Methods A total of 1 52 patients with craniocerebral injury,who admitted in Tangshan Gongren Hospital from July 201 2 to June 201 4,were selected by convenience sampling as objects and were divided according to random number table method into observation group and control group,each with 76 cases.In the control group,patients were given routine postoperative nursing and rehabilitation guidance,while in the observation group,patients,on the basis of routine nursing, were given exercise rehabilitation pathway,which took the patients as the center and develop individualized rehabilitation exercise program.The scores of postoperative rehabilitation exercise compliance scale (PRECS), functional independence measure scale (FIA),modified Barthel index (MBI),the quality of life after traumatic injury (QOLIBRI)were compared between the two groups 4 weeks after the interventions.Results After the interventions,the three dimensions scores of the (observation group in knowledge mastery of PRECS,exercise compliance,actively seeking advice)were (93.5 ±5.3),(92.2 ±5.7)and (91 .6 ±6.1 )points,higher than (87.2 ±6.7),(84.2 ±7.0)and (83.5 ±7.8)points in the control group (t =6.429,7.726,7.1 31 ;P <0.01 );after the interventions,scores in the observation group in motor,cognition,total score of FIA,and in MBI were respectively (78.2 ±7.3),(32.3 ±2.5),(1 1 0.5 ±7.9)and (88.2 ±7.3)points,higher than that in the control group,(73.4 ±9.0),(30.8 ±3.1 ),(1 04.2 ±9.7)and (81 .9 ±9.1 )points,with (t =3.61 1 ,3.284,4.390,4.708;P <0.01 );after the interventions,scores in the observation group in overall feeling,thinking ability,emotion and self-rating,independence and daily life,and interpersonal relationship of QOLIBRI were (65.7 ±6.5),(60.7 ±5.2),(62.1 ±5.8),(61 .4 ±4.5)and (55.7 ±4.5)points,higher than that in the control group,(61 .2 ±7.2),(557.7 ±6.1 ),(59.0 ±6.1 ),(57.6 ±6.3),and (52.9 ± 6.0)points (t =4.044,3.263,3.21 1 ,4.279,3.255;P <0.01 );scores in the observation group in negative feeling,physical condition and distress of QOLIBRI were (38.9 ±5.4),(44.7 ±6.1 ),and (38.2 ± 5.9)points,lower than (43.1 ±6.5),(49.4 ±7.5)and (41 .8 ±7.3)points in the control group (t =4.333,4.238,3.344;P <0.01 ).Conclusions Exercise rehabilitation pathway can improve the compliance of postoperative rehabilitation exercise for patients with craniocerebral injury.The individualized rehabilitation exercise program is beneficial to the rehabilitation of motor function.Quality of life is also improved from aspects of physical condition and mental state.It should be promoted.
2.Application of group health education on the prevention of disuse atrophy of muscle in patients with craniocerebral trauma

Objective To explore the effect of group health education on the prevention of disuse atrophy of muscle in patients with craniocerebral trauma.Methods From June 2014 to May 2016 at the root of craniocerebral injury patients in the treatment of 148 cases of our hospital as the research object, according to the order of admission number, were randomly divided into observation group and control group, 74 cases in each. The patients of control group were given routine health education, and the patients of observation group were given the group health education. At admission and intervention for 3 months were determined in patients with neurological function, motor function and muscle strength.Results 3 months after the intervention, the CSS score of the observation group (16.45±4.65) was significantly lower than that of the control group, the difference was statistically significant (t=2.646,P<0.01). The score of upper limb motor function (53.58±10.31) and the score of lower extremity motor function (28.46±4.75) of the observation group were significantly higher than those in the control group, the difference was statistically significant (t=3.015,2.699;P<0.01). The total effective rate of muscle recovery in observation group was 84.38%, and the total effective rate of muscle recovery in control group was 53.97%, there was significant difference between two groups (χ2=6.783,P<0.01). Conclusions Comparing with conventional health education, group health education can improve the recovery of limb motor function and muscle strength in patients with craniocerebral trauma. Meanwhile, it can effectively prevent the disuse atrophy of muscle so as to improve the quality of life of patients.
3.Preventive effect of early rehabilitation nursing on disuse amyotrophy in patients with craniocerebral injury

Objective To explore the preventive effect of path-type early rehabilitation nursing on disuse amyotrophy in patients with craniocerebral injury.Methods A total of 63 patients with craniocerebral injury in Tangshan Workers Hospital from June 2013 to May 2014 were treated as control group, and received routine rehabilitation nursing. Another 64 patients with craniocerebral injury hospitalized in the same hospital from June 2014 to May 2015, were selected as observation group, and received path-type rehabilitation nursing intervention on the basis of routine rehabilitation nursing. Motor function, muscle strength and quality of life were measured at the patient's admission and intervention for 3 months.Results After intervention, evaluation score of upper limb and lower extremity motor function of the observation group were higher than that of the control group (t=3.215, 2.965;P<0.01). After 3 months' intervention, there were 54 cases having elevated muscle Ⅱ level and above in the observation group, compared with 34 cases in the control group; the total effective rate of the observation group was 84.38%,and which was higher than that of the control group, which was 53.97% (χ2=7.326,P=0.003). Psychological function, social function, healthy self-awareness ability and overall quality of life score improvement of the observation group were higher than those of the control group (t=5.247, 4.678, 4.268, 4.184, 3.857;P<0.01).Conclusions The path-type early rehabilitation nursing model can improve the physical function and muscle recovery of patients with craniocerebral injury, effectively prevent disuse amyotrophy and improve the quality of life of patients.
4.Relationship Between Cardiac Myosin-binding Protein c.G772A Gene Mutation and Familial Hypertrophic Cardiomyopathy

To investigate the mutation site of pathogenic gene in patients with hypertrophic cardiomyopathy (HCM) and to analyze the relationship between the genotype and clinical phenotype. Methods: Targeted exon capture sequencing was conducted in a HCM proband for 30 coding exons related HCM gene by all exon amplification and high-throughput sequencing. Furthermore, Sanger sequencing was performed in other family member and in 200 healthy volunteers for verification. The familial investigation included in clinical presentation, physical examination, electrocardiogram and echocardiography. Results: There were 3/6 blood relatives carrying cardiac myosin-binding protein gene MyBPC3 G772A heterozygous mutation, the mutation site was at 258 amino acid of MyBPC3 as glutamic acid (Glu) was substitute to lysine (Lys), such mutation was not found in rest of family member and not in healthy volunteers. The onset of proband and her daughter was rather late, they had palpitation and chest tightness; echocardiography showed interventricular septum basal segment thickening (16-18) mm. Proband was complicating paroxysmal ventricular tachycardia, malignant arrhythmia and heart failure, the maximum pressure gradient of left ventricular outflow was 56 mmHg, which with the high risk for sudden death. Conclusion: Comprehensive gene test has been helpful for clinical stratification, early diagnosis and treatment. MYBPC3 site mutation c.G772A might be the pathogenic mutation in that specific HCM family.
