1.Influence of chitosan on skin and soft tissue expansion

Objective To observe the influence of chitosan on the skin and soft tissue expansion.Methods Twenty-five patients were selected,who were suitable to be embedded soft tissue expanders in the face,a 100-milliliter expander was implanted in one side of the face,and other side was used as control.A 100-milliliter expander was implanted in each group,and a slender silicon duct was embedded between the expander and subcutaneous tissue in the experimental group.About five to seven days after the operation,the negative drainage tube was removed,and then two-milliliter medical chitosan injected with the silicon duct in the experimental group,but not in the control group.Two groups were injected with normal saline in the second day.The center of expanded skin was pressed and skin elasticity and relaxation were compared between the two groups during the injection interval.The time of injection interval,the quantity of normal saline inside the expanders after two weeks and three weeks and the total time of expansion to 100 milliliters were recorded.After injection was completed in the two groups and maintained for two weeks.In the stage Ⅱ operation,the expanders were taken out,1 cm × 1 cm fibropeplos was removed from the center of the expanded skin flap from the two groups,and pathological section was prepared to measure the thickness of fibropeplos,average gray scale of collagen and the quantity of blood capillaries.The fibroblasts,collagen fiber and capillaries were observed and compared under light microscope.A matched-pairs t analysis was used to analyze the data.Results Compared with the control group,the quantity of normal saline inside the expanders in the experimental group was increased at the same time; the water injection period was shorten and tissue expansion was significantly accelerated.The number of fibroblasts in the fibropeplos decreased with the influence of chitosan.The fibroblasts were restrained to mature period and collegan decreased.The fibropeplos became thinner but the capillaries were not affected.Conclusions Chitoson can inhibit fibroblast proliferation and collagen production,and the effect of accelerating tissue expansion is significant and deserves to be recommended.
2.Influence of different anesthetic methods on postoperative serum excitatory amino acid(EAA) levels and cognitive function of elderly patients operated with transurethral resection of prostate (TURP)

Objective To discuss influence of different anesthetic methods on postoperative serum excitatory amino acid (EAA) levels and cognitive function of elderly patients operated with transurethral resection of prostate (TURP). Methods A total of 100 cases of elderly patients with benign prostatic hyperplasia (BPH), in Department of Anesthesiol-ogy of the hospital to be given the TURP operation at certain time were selected, and divided into general anesthesia group (n=50) and epidural group (n=50) in accordance with different anesthetic methods, who were given intubation anesthesia and epidural anesthesia respectively. The changes of serum EAA levels and cognitive function before anes-thesia, 4 hours, 8 hours, one day and four days after the operation of patients in two groups were observed and record-ed. Results Serum EAA levels 4 hours, 8 hours and one day after the operation of patients in two groups rose than be-fore (P<0.05), and the rising rates in epidural group were much lower than those in general anesthesia group (P<0.05). Meanwhile, MMSE scores of patients in two groups 4 hours, 8 hours and one day after the operation declined than be-fore (P<0.05), and the declining rates in epidural group were much lower than those in general anesthesia group (P<0.05). Four days after operation, the serum EAA levels and MMSE scores of patients in two groups returned to the levels before anesthesia (P>0.05). Conclusion Both intubation anesthesia and epidural anesthesia have effect on early cogni-tive function of elderly patients operated with TURP, and the former has less influence on early cognitive function of patients, which may have close effect on the lower rising rate of serum EAA level by epidural anesthesia.
3.Study on the diagnostic value of imageology of giant cell tumour of tendon sheath.

OBJECTIVETo investigate the diagnostic value of imageology of giant cell tumour of tendon sheath (GCTS) including X-ray, CT and MRI.
METHODSThirty-five patients with GCTTS confirmed by operation and pathology were retrospectively analyzed. There were 16 males and 19 females. The average age was 39.4 years, ranged from 7 to 66 years. All the patients underwent X-ray examination, 8 patients underwent CT examination, and 16 patients underwent MRI examination.
RESULTSThere were 2 patients in knee joint, 6 patients in ankle joint, 1 patient in capitulum radius, 2 patients in wrist joint, 14 patients in hand and 10 patients in foot. Ten cases were the diffuse form, and 25 cases were the focal form. The X-ray results: the slightly high density soft tissue mass surrounding the bone were shown in 32 cases, 3 cases were normal. The bone erosion were shown in 9 cases, the obvious destruction of bone were shown in 5 cases. CT results: The soft tissue mass and the destruction of bone were shown clearly. MRI results: On T1WI, the signal intensity of GCTTS almost was similar to those of skeletal muscle in 9 cases and was slightly lower than those of skeletal muscle in 7 cases. On T2WI, the signal intensity presented mainly hypointensity with patchy isointensity or hyperintensity signal. A little of fluid was shown in 6 cases.
CONCLUSIONX-ray can demonstrate the lesion and erosion of bone, destruction of bone can clearly be shown on CT. The low intensity signal on MRI T1WI and T2WI is the characteristic appearance of GCTTS. And it can clearly show the lesion range and type of GCTTS.

4.Fetal Costello syndrome caused by a de novo mutation in HRAS gene: a case report

This article reports a case of prenatal diagnosis of fetal Costello syndrome. At 11 weeks of gestation, the fetal nuchal translucency thickness was 2.5 mm. At 26 +1 weeks of gestation, ultrasound indicated that the fetal abdominal circumference was significantly enlarged, suggesting fetal overgrowth. Subsequent regular ultrasound follow-ups revealed polyhydramnios, enlarged fetal kidneys, and macroglossia after 30 +5 weeks of gestation. Whole-exome sequencing of the family detected a c.34G>A(p.Gly12Ser) mutation in the fetal HRAS gene, which was pathogenic and not present in either parent. Based on clinical manifestations, the fetus was diagnosed with Costello syndrome. After genetic counseling, the pregnant woman opted for termination of the pregnancy.
5. Clinical significance of exosomal miR-1231 in pancreatic cancer

Objective:
To investigate the expression and clinical significance of exosomal miR-1231 in plasma of pancreatic cancer (PC) patients and pancreatic cancer cells.
Methods:
A total of 16 patients who were diagnosed with pancreatic cancer in Hunan Cancer Hospital were collected from April 2016 to August 2017. Meanwhile, 16 healthy volunteers were recruited as the healthy control group at the same period. The plasma exosomes were extracted, and the levels of miR-1231 were detected by qRT-PCR in PC and healthy control groups. Moreover, the clinicopathological significance of exosomal miR-1231 expression was analyzed. Furthermore, the expression of exosomal miR-1231 was detected in several pancreatic cancer cells (MIA PaCa-2, PANC-1, SW1990, AsPC-1 and BxPc-3) and two normal pancreatic epithelial cells (HPDE and human primary pancreatic epithelial cell).
Results:
