Objective To summarize the clinical experience of nasal endoscopic treatment for refractory epistaxis. Methods .A retrospective analysis was made on 258 patients with refractory epistaxis treated with intranasal endoscopic exploration and hemostatic treatment from January 1998 to December 2004 in this hospital. Results .Hemorrhage was found on the nasal septum and the lateral nasal wall in 118 and 47 patients, respectively. Nose bleeding resulted from intranasal tumor in 8 patients. The site of bleeding was not clearly identified in 85 patients. Of the 258 patients, 257 responded to the treatment (99.6%) and 1 died ( 0.4%).Recurrent bleeding occurred in 43 patients. The incidence of recurrent bleeding was significantly higher in patients with unknown bleeding sites than in clearly diagnosed patients (?2=17.689,P=0.000). Conclusions .The emphasis of nasal endoscopic treatment for refractory epistaxis should be laid on the initial management, during which the bleeding site should be carefully identified. For patients with repeated nose bleeding, a combination treatment, including endoscopic filling-up of the anterior nasal cavity, endoscopic embolization of anterior and posterior nose, or embolization of the internal maxillary artery, is necessary.

OBJECTIVE:
To investigate the perioperative treatments of endoscopic sinus surgery for nasal diseases in patients with coronary heart disease after PCI.
METHOD:
Clinical data of 12 cases of endoscopie-assised surgery such as nasal tumors resection, functional sinus surgery, correction of deviated nasal septum,low-temperature plasma hemostasis in patients with coronary heart disease after PCI were analyzed retrospectively.
RESULT:
Serious bleeding did not take place with the 12 cases during surgery, and surgery progressed smoothly; one of patients had heavy nosebleed after surgery, however her condition was stable when received active treatment. Follow-up 3 months to 2 years, nasal diseases of 12 patients recovered well and symptoms were relieved; cardiovascular events such as hemorrhage, thrombosis and so on did not occur.
CONCLUSION
Due to physiological function of the heart dysfunction in patients with coronary heart disease after PCI,they often accompany a number of trouble issues such as medical disorders, oral antiplatelet drugs, surgery affordability loss and increase surgical risk. Correct and effective perioperative treatments, strictly surgical indications are really necessary which can keep patients safe through perioperative period.

Objective To investigate the correlation between cytomegalovirus infection and heart rate variability (HRV) in elderly patients with atherosclerosis. Methods 160 patients with coronary heart disease who met World Health Organization diagnostic criteria for coronary heart disease in 1979 were collected.According to the IMTHCMV PP65 antigen test results, patients were divided into positive group (observation group,n=103) and negative group (control group,n=57).We detected the levels of of HRV,metalloprotease-9 (MMP 9) and tumor necrosis factor α (TNF-α) in the two groups in order to access the plaque stability. Results The all sinus standard deviation of RR interval (SDNN),standard deviation of the average NN interval (SDANNI),mean value of sinus standard deviation of RR interval (SDNNI) were lower in observation group than in control group (P <0.05).There were no significant differences in square root of the mean squared differences of successive NN intervals (RMSSD) level and percentage of differences exceeding 50ms between adjacent normal number of intervals (PNN50) between the two groups (P>0.05).The levels of total cholesterol and low density lipoprotein were higher and the levels of MMP 9 AND TNF α were lower in observation group than in the control group (all P<0.05).Compared with control group, the plaque stability was decreased in observation group [20.4% (21/103) vs.61.4% (35/57),x2=4.273,P=0.015].Conclusions Patients with atherosclerosis combined with cytomegalovirus infection have a greater heart rate variability and poorer plaque stability.

Chinese Journal of Health Management 2011;05(2):95-98
Objectives To learn the effect of self-recognition-based healthy management on community diabetics. Methods A total of 40 community diabetics were evaluated and trained for 3 months.SPSS11.5 software was used for data analysis. Results At 3 months, some risk factors of diabetes, including body weight, body mass index (BMI), waist circumstance (WC), systolic blood pressure (SBP), diastolic blood pressure (DBP) and 2 h postprandial blood sugar (PBS), were improved (P <0. 05). Male patients showed statistically significant reduction in body weight, BMI and WC ( P < 0. 05 ).However, female subjects were found to have significant decline in body weight, BMI, WC, SBP and DBP (P<0. 05). Those >60 year-old had significantly decreased body weight, BMI, WC, SBP, DBP and 2 h PBS ( P < 0. 05). Lower levels of WC, SBP and triglyceride were seen in individuals < 60 year-old ( P <0. 05 ). Conclusion Self-recognition-based healthy management could effectively reduce diabetic risk factors and prevent the development of diabetic complications.
