1.Manufacture and application study of the portable,pollution-prevention and high-pressure-vapor disinfection basket

Objective The high-pressure-vapor disinfection basket was made to take precautions against the sterile article pollution after disinfection from boiler disinfection and also for convenience. Methods Items after disinfection from 100 pots were randomly allocated to the observation group and the control group, each group had 50 pots.The observation group used the high-pressure-vapor disinfection basket to take the items from the sterilization room to the disinfection room directly. The control group used conventional disinfection cart, re-moved the items to the cart,thon took them to the disinfection room. Time cost for convey of the disinfection items was observed,and surface sampling for gormiculture was obtained from the last item conveyed to the disinfection room. Results After germiculture,2 pollution packets were found in the observation group,the polluting rate was 4%;49 pollution packets in the control group,the polluting rate was 98%,which was statistically different from that of the observation group(P<0.01). Time cost of the observation group was 8 minutes,of the control group was 13 minutes. Conclusions Use of the portable,pollution-prevention and high-pressure-vapor disinfection basket can prevent the surface of disinfection items from pollution,besides,it is convenient and costs less labor and time.
2.Nosocomial Infections in Central Supply Unit in Hospital:Hidden Dangers and Administration Strategy

OBJECTIVE To prevent and control nosocomial infection in the central supply unit.METHODS Hidden dangers of nosocomial infection of central supply unit were investigated and analyzed to formulate the strategy of precaution and control.RESULTS Management status in central supply unit was not standardized and several administration shortages such as supervising system,architectural composition,and precaution consciousness were occurred.CONCLUSIONS Adopting comprehensive management,establishing regulations and carrying out strictly are essential for prevention and control against nosocomial infections in central supply unit.
4.HLA-B locus genotyping with reference strand mediated conformation analysis (RSCA) system in clinical usage

Objective To establish and stabilize a new HLA-B locus typing strategy, reference strand mediated conformation analysis(RSCA)system and to conform its advantages.Methods Of 27 hematopoitic stem cell transplant patients and 63 potential donors DNA samples were extracted from peripheral blood cells and HLA-B loci were both typed by RSCA and PCR-SSP methods. The (ambi)-(guous) results were identified by using DNA sequencing. Results Among 90 samples, 87/90 ((96.7) %) cases could be designated definitely, but one of them was disagreement with the SSP result which was confirmed by sequencing, and 67/90 ((74.4) %) cases could be typed to allelic level. 1/90 ((1.1) %) case could not be identified by RSCA for its bad PCR results. Only one allele of each 2 samples could be designated and the other could not be identified by RSCA, which were further confirmed by sequencing method, and the results confessed which were known as alleles, but there were no corresponding data in RSCA database. Twenty samples were randomly selected to identify the replication rate of RSCA, and the results demonstrated that the replication rate was 100 %. Among PCR-SSP typing results, about 10 % samples might be typed for 1-3 times to confirm their types. Conclusion RSCA had some advantages such as high resolution, high sensitivity, high accuracy, high replication, finding new alleles, large scale and low cost, and was especially suitable for unrelated donor-recipient (screening).
5.Isomers of amygdalin in BUYANG HUANWU TANG and its production

Object To study the isomers of amygdalin in BUYANG HUANWU TANG and its production Methods Amygdalin was isolated from both peach seeds and BUYANG HUANWU TANG by using various column chromatography Their structures were identified by the various spectral data Results Amygdalin had been isolated from n BuOH fraction of aqueous extract of BUYANG HUANWU TANG and found to be a pair of D , L epimers and their ration was 1∶1 It was also found that the structures and the ration of D , L epimers of amygdalin in decoction of single peach seed were similar to that in BUYANG HUANWU TANG. The peach seed only gave D amygdalin when it was extracted in 95% EtOH at reflux temperature, and D amygdalin cannot be isomerized when it was treated in water at 100 ℃ Conclusion Isomerization of D amygdalin results from interaction between it and other compounds of peach seed in water at high temperature, and has no evident relation to other constituents in BUYANG HUANWU TANG L amygdalin is a new compound generated due to decoction of peach seed
6.Isolation and identification of four isoflavones and one pterocarpan from BUYANGHUANWUTANG

Object To study the antithombotic portion and bioactive fraction of BUYANGHUANWUTANG (BHT), a decoction of astragalus in combination with peony and several other ingredients. Methods The EtOAc portion of BHT extract was repeatedly separated by chromatography and identified by means of spectral analysis. Results One pterocarpan, (6aR, 11aR) 9, 10-dimethoxypterocarpan-3-O-?-D-glucopyanoside (Ⅴ) and four isoflavones, formononetin (Ⅰ), calycosin (Ⅱ), formononetin-7-O-?-D-glucopyanoside (Ⅲ), and calycosin-7-O-?-D-glucopyranoside (Ⅳ) were identified. Conclusion All five compounds were isolated for the first time from BHT.
7.Alkaloids isolated from Buyang Huanwu Decoction and their structure identification

Objective To study the antithrombotic compounds and bioactive fraction from Buyang(Huanwu) Decoction(BHD).Methods Four alkaloids have been isolated from BHD by repeated chromatographies.Their structures were elucidated by means of spectral and single-crystal Xray diffraction analysis.Results They are N-(3′-maleimidyl)-5-hydroxymethyl-2-pyrrole formaldehyde(Ⅰ),4-carbamoyl-2-pyrrole carboxylic acid(Ⅱ),N-(1′-D-deoxyxylitolyl)-6,7-dimethyl-1,4-dihydro-2,3-quinoxalinedione(Ⅲ),and 2,3,4,9-tetrahydro-1H-pyrido indole-3-carboxylic acid(Ⅳ).Conclusion All these compounds are isolated from BHD for the first time and compounds Ⅰ-Ⅲ are new alkaloids.
9.Inhibition of EAU by anterior chamber injection with antigen

Objective:To study the effect of injection bovine retinal soluble antigen (S-Ag)into anterior chamber(AC)in inducing immunological tolerance.Methods:S-Ag was extracted from fresh bovine retina and inoculated into the AC of Wistar rats,One week later,the S-Ag mixed with complete fruend's adjuvant(CFA)was injected into footpad intradermaly.The clinical symptoms and the pathological changes were observed.The IL-4 and IFN-? concentrations in the supernatant of splenic cell culture were measured by ELISA.Results:The incidence of EAU in pretreated group(group A) was markedly higher than that of positive controls(gropu B).There were histopathological changes in all rats with clinical symptoms.The IL-4 level of splenic cell culture in group A were significantly higher than those in group B ( P
10.Value of Tei index in predicting cardiopulmonary exercise capacity in patients with congestive heart failure

Objective To measure Tei index in patients with idiopathic or ischemia dilated cardiomyopathy, and to evaluate its correlation with other conventional indexes of echocardiography of left ventricular(LV) function,and its applicability in assessing cardiopulmonary exercise capacity in those patients.Methods Seventy consecutive patients (53?14 years) with heart failure(New York Heart Association [NYHA] class, Ⅱ to IV) who had received echocardiographic diagnosis, and believed to be suffering from dilated or ischemia cardiomyopathy were studied. Echocardiography were performed; LV volume as well as ejection fraction were measured from the apical view of the two-dimensional echocardiogram using a modified Simpson’s rule algorithm in all patients. The following variables: peak early transmitral filling velocity (E), late transmitral filling velocity (A), their ratio (E/A), the deceleration time of E (DT), and Tei index were measured by spectral Doppler echocardiography. Furthermore, all patients underwent a cardiopulmonary exercise using 6min walk test. Results Advanced NYHA class was associated with higher Tei index values. A positive correlation was found between Tei index and early /late filling velocity ratio, while a negative correlation was found between Tei index and A wave, E wave, DT, and LV EF. Tei index showed no correlation with heart rate, blood pressure or age. Compared with the parameters of left ventricular function Tei index had a closer relation to the distance of 6min walk. Stepwise regression analysis revealed that the Tei index and the late LV filling velocity were the only independent predictors for cardiopulmonary exercise capacity.Conclusion Tei index correlates inversely with LV performance and reflects disease severity, which is a useful complimentary variable in the assessment of cardiopulmonary exercise performance in patients with heart failure.
