1.Observation of bathing posture in neonates

Objective:To explore the best bathing posture in neo nates.Methods:Normal mature neonates 664 total were randomly seperated into two groups .The bathing posture was the traditional upwards from face to back in the control group,but upwards from back to face in the experimental group .The noise and comfort levels of the neonates were observed.Results:Uncomfortableness in the experimental gr oup was markedly lowerthan in the control group(χ2=45.78,P<0.0001).Bathing style,i.e.in basin or in shower,did not have an effect on the postures (P> 0.05).Conc lusion:The bathing posture of upwards from back to face is better than upwards from face to back.In this position,the neonates feel safer and more comfortable,and can be stroked while being bathed by the neonates' parents.
2.Test of validity and reliability of Nursing Professional Values Scale in registered nurses

Objective To explore the validity and reliability of Nursing Professional Values Scale in registered nurses.Methods Demographic questionnaires and the Revised Nursing Professional Values Scale were used among 280 registered nurses.Results The Cronbach's α was 0.94.The factor analysis extracted 4 factors which could account for 59.55% variances.These four factors were defined as professional characteristic,providing care,behaviorist,and trust.The Cronbach's α for these four factors were 0.89,0.87,0.86 and 0.70 respectively.Conclusions The Revised Nursing Professional Values Scale possesses desirable validity and reliability in registered nurses,and is a psychometric good instrument of professional value.
3.Efficacy of precise sequential therapy for primary liver cancer

Objective To investigate the construction and implementation of the concept of precise sequential therapy for primary liver cancer.Methods The clinical data of 207 patients with resectable liver cancer who were admitted to the Air Force General Hospital from May 2005 to June 2010 were retrospectively analyzed.Of all the patients,81 received conventional therapy (conventional therapy group),and the other 126 patients received precise sequential therapy (precise sequential therapy group).The conditions of the patients in perioperative phase,during postoperative sequential treatment and the period of follow-up between the 2 groups were compared.All data were analyzed by using the covariance analysis,analysis of variance,chi-square test or rank sum test.Results The detection rates of lesions with a diameter less than 1 cm,operation time,liver resection volume,intraoperative blood loss,rate of perioperative blood transfusion,duration of postoperative hospital stay,incidences of postoperative complications were 81% (22/27),( 186 ± 36) minutes,(75 ± 29) ml,( 189 ± 60) ml,24%(30/126),(21 ± 12)days and 13% (17/126) in the precise sequential therapy group,and 18% (2/11),(222 ± 30)minutes,(133 ±88)ml,(327 ±46)ml,51% (41/81),(26 ± 17)days and 20% (16/81) in the conventional therapy group,respectively,with significant differences between the 2 groups (F =10.876,7.390,46.996,31.025,14.556,6.315,4.017,P < 0.05).No significant difference was observed on the levels of alanine transaminase,albumin and the Child-Pugh score before and after the intervention in the precise sequential therapy group,but significant differences were observed in the conventional therapy group.The 1-,2-,3-year tumor recurrence rates and the 1-,2-,3-year survival rates were 17% (21/126),22% (17/76),26% (8/31) and 87% (110/126),87% (66/76),84% (26/31) in the precise sequential therapy group,and 31% (25/81),38% (27/71),48%(31/65 ) and 77% (62/81),75% (53/71 ) and 60% (39/65) in the conventional therapy group,respectively.There were significant differences in the prognosis of the patients in the 2 groups ( x2 =4.958,4.292,4.168,4.062,3.640,5.470,P < 0.05 ).Conclusion Through accurate assessment of the patients' condition before surgery,precise hepatectomy and precise postoperative intervention,the goal of effective control of tumor recurrence,maximum protection of the liver function and improvement of the survival rate can be achieved.
4. Effect of curcumin on the protein and mRNA expression of osteopontin in asthmatic airway remodeling rats

OBJECTIVE: To observe the effects of curcumin on the protein and mRNA expression of osteopontin (OPIN) in asthmatic airway remodeling rats. METHODS: Twenty-four SD rats were randomly divided into 3 groups; normal control group (N group), asthma group (A group) and curcumin group (C group), with each 8 rats. The asthmatic airway remodeling model was established by sensitization and challenge with ovalbumin (OVA). The total brochial wall thickness (Wat) and the brachial smooth muscle thickness (Warn) were measured by image analysis system. The protein expression of OPN in lung homogenats were detected by immunohisto-chemistry (IHC) and the mRNA expression measured by reverse transcription-polymerase chain reaction (RT-PCR). Besides, correlative analysis between Warn/Wat and the expression of OPN was studied. RESULTS: Wat and Warn of asthma group were significant higher than those of normal control group. Compared with asthma group, those of curcumin group were significant lower (P<0.01, respectively). The protein expression of OPN detected by IHC in asthma group were significant higher than those in normal control group. Compared with asthma group, those in curcumin group were significant lower (P<0.01, respectively). The mRNA expression of OPN detected by RT-PCR in asthma group were significant higher than those of normal control group. Compared with asthma group, those in curcumin group were significant lower (P<0.01, respectively). There was significantly positive correlation between Wat and the protein/mRNA expression of OPN, also between Warn and the protein/mRNA expression of OPN. CONCLUSION: Curcumin may inhibit asthmatic rat airway remodeling partly through the down regulation of OPN.
5.Effect of Sodium ferulate on glucocorticoid-induced osteoporosis

Aim To study the influence of Sodium fer-ulate ( SF) on bone metabolism in glucocorticoid–in-duced osteoporosis rats. Methods Thirty cases of fe-male Wistar Rats(3-month-old) were divided into con-trol group, model group and SF group ( low-dose group, middle-dose group, high-dose group ) by ran-domized block design. Double fluorochrome labeling with calcein was performed before necropsy. The left tibia was taken for bone histomorphometry. Results In static parameters, the proximal tibia cancellous bone trabecular thickness, trabecular quantity and area ratio were significantly reduced in model group compared with control group;while compared with model group, those were increased in middle and high-dose SF group. Trabecular separation degree was increased in model group compared with control group, while it was decreased in middle and high-dose SF group compared with model group. In dynamic parameters, the calcula-tion parameters of cancellous bone mark perimeter rate and the bone formation rate were increased in model group compared with control group, in middle and high-dose SF group the bone formation rate was in-creased compared with model group. In bone cells, os-teoclast number per mm, osteoblast number per mm, percent osteoblast surface perimeter and percent osteo-clast surface perimeter were increased in model group compared with control group. In growth-plate, the thickness of growth-plate was increased in model group compared with control group. In bone cells and growth-plate there was no statistical significance between treat-ment group and model group. Conclusion This study demonstrates that SF can increase bone mass and im-prove bone structure,which may be related to the im-provement of bone formation. SF is effective for GIOP in rats.
6.Effects of edaravone on the expression of interleukin-1beta, nuclear factor-kappaB and neuron apoptosis in juvenile rat hippocampus after status convulsion.

OBJECTIVETo investigate the effect of edaravone on expression of interleukin-1beta (IL-1beta), nuclear factor-kappaB (NF-kappaB) and neuron apoptosis in the juvenile rat hippocampus after status convulsion (SC).
METHODSOne hundred and ninety-five juvenile male Sprague-Dawley (SD) rats were randomly divided into normal saline (NS) control group, status convulsive group and edaravone treatment group. Each group was further divided into five subgroups for different time points. The rats in status convulsive group were kindled into epilepsy by lithium-pilocarpine chemical method. Expressions of IL-1beta and NF-kappaB proteins were detected with immunohistochemistry methods. Expression of NF-kappaB mRNA was detected with reverse transcription-polymerase chain reaction (RT-PCR). The neuron apoptosis was observed by TdT-mediated dUTP nick end labeling (TUNEL).
RESULTS(1) Measured by immunohistochemistry the value of IOD of IL-1beta (30.83 +/- 3.81, 41.00 +/- 5.61, 36.32 +/- 6.78 and 28.48 +/- 4.61, respectively, 12-72 h points) and NF-kappaB (67.60 +/- 5.81, 74.61 +/- 7.94, 82.43 +/- 10.67, 70.70 +/- 5.85 and 68.22 +/- 9.67, respectively, 4-72 h points) positive cells in the SC group increased,there was significant difference compared with NS group (IL-1beta: 11.74 +/- 2.32, 12.93 +/- 2.49, 13.02 +/- 2.83 and 12.98 +/- 5.29, respectively, 12-72 h points. NF-kappaB: 48.67 +/- 16.14, 44.62 +/- 7.82, 53.16 +/- 114.45, 54.27 +/- 5.25 and 55.56 +/- 7.56, respectively, 4-72 h points) (P < 0.01, or P < 0.05). By ED intervention in IL-1beta (22.01 +/- 4.45, 28.28 +/- 4.50 and 26.00 +/- 5.34, respectively, 12-48 h points) and NF-kappaB (58.56 +/- 6.37, 59. 86 +/- 6.73, 70.00 +/- 10.09, 64.78 +/- 7.56 and 64.45 +/- 6.51, respectively, 4-72 h points) positive cells value of the IOD decreased as compared with SC group (P < 0.01, or P < 0.05). (2) Measured by RT-PCR, the expression of NF-KB mRNA and protein trend was similar. (3)The TUNEL positive cells in hippocampus, CA1 of SC group (11.41 +/- 2.37) were more than that of NS group 12 h after the SC (P < 0.01), reached its highest level at48 h (28.78 +/- 5.11), after the intervention with edaravone (8.98 +/- 2.22, 13.09 +/- 2.54 and 20. 57 +/- 4.89, respectively, 12-48 h points) ,TUNEL positive cells showed a significant drop in SC group at 12-48 h time points (P < 0.01, or P < 0.05), but still significantly higher than that of the NS group (6.22 +/- 1.50, 6.57 +/- 1.61 and 6.72 +/- 1.14, respectively) (P < 0.01, or P < 0.05), at the 4 h time point(NS group 6.29 +/- 1.49, SC group 6.61 +/- 1.71, ED group 5.75 +/- 1.41) among the three groups, no significant difference in TUNEL positive cells was found (P = 0.759).
CONCLUSIONSEdaravone inhibited expression of IL-1beta and NF-kappaB in pilocarpine-induced seizures in rat hippocampus, reduced the number of neuronal apoptosis. These results suggest that edaravone may have protective effect against the damage caused by status convulsion.

7.Comparative studies of perioperative period correlation factor of hepatectomy treatment for liver malignant tumor

Objective To compare superiorities of liver malignant tumors underwent regular and irregular hepatectomies through approaching perioperative factors.Methods 1019 consecutive hepatectomies of liver malignant tumor from 1986 to 2009 at Air Force General Hospital and General Hospital of Chinese PLA were investigated retrospectively according to their medical documentation.Results Multivariate analysis showed that liver malignant tumors on which regular hepatectomy and irregular hepatectomy were performed,there was no significant difference in the blood loss,complications,mortality related to operation,hospital stay,and so on.But the operating time of regular hepatectomy was obvious more than that of irregular hepatectomy (P< 0.001,OR=1.004).Conclusions Although for liver malignant tumor,regular hepatectomy seems to be superior to irregular hepatectomy based on oncological theory,in clinical practice,there were no significant difference between the perioperative risk of regular hepatectomy and that of irregular hepatectomy.
8. Heterogeneity of viral bronchiolitis

Bronchiolitis is the most common lower respiratory tract infection and the leading cause of hospitalization in infants. It has become one of the public health problems worldwide. The pathogenesis is unclear and there are currently no targeted therapies that have been proven to be effective in bronchiolitis. There exists a poor understanding of the mechanisms underlying the variable disease severity and the inconsistent manifestation of long-term respiratory morbidity. Nor can these outcomes be reliably predicted in patients on initial presentation. Bronchiolitis is generally considered a single disease,however,more and more researches have shown that it is a heterogeneous disease that needs individualized and specific treatment,with differences in etiology,clinical charicteristics,immunology,microecology and long-term respiratory outcomes. The study on heterogeneity of viral bronchiolitis will contribute to individualized and targeted therapy and may reduce the development of recurrent wheezing and asthma.
9.The role of hepatic arteriography before precise hepatectomy in primary liver cancer

ObjectiveTo study the role of hepatic arteriography before precise hepatectomy in primary liver cancer,Methods207 patients with primary liver cancers resected from 2005 to 2010 at Air Force General Hospital of Chinese PLA were studied retrospectively.There were 81 patients who received routine hepatectomy and 126 who received precise hepatectomy with hepatic arteriography before surgery.ResultsAnalysis showed that precise hepatectomy was superior to routine hepatectomy for lesions less than 1.0 cm (81.48% vs 18.18%),with less operative time (168.20±35.81 min vs 221.75±30.33 min),volume of resection,blood loss (168.20±35.81 min vs 221.75±30.33 min),transfusion rate (256.13±185.24 ml vs 436.97±590.12 ml),and hospital stay (20.53± 12.41 d vs 25.72±17.27 d),(all P<0.05).Hepatic arteriography did not affect liver function before precise hepatectomy.ConclusionsHepatic arteriography was significantly better in showing blood vessels of the primary liver cancer,and in detecting liver metastases or satellite lesions of less than 1.0 cm before surgery.
10. Practice of clinical thinking and clinical judgement in the standardized resident training

The standardized resident training is an important step for training high-level professionals in clinical medicine and it is an important part of medical education after graduation. The training of residents' clinical thinking and clinical judgement should be emphasized in all resident training programs. Some resident programs in China still focus on the teaching of important knowledge that are originated from the medical colleges. Therefore,it is urgent for faculty to adopt new teaching methods aiming at the training of clinical thinking and clinical judgement. The paper introduces a mode for teaching clinical thinking and clinical judgement in the resident training programs. The goal is to improve the level of resident physicians in clinical diagnosis and treatment in China.
