1.Expression of endostatin in ischemic myocardium of myocardial infarction rats

AIM: To study the expression of endostatin in ischemic myocardium of myocardial infarction(MI) rats in various periods and the correlation with VEGF expression and microvascular density(MVD).METHODS: Thirty-two male Sprague-Dawley rats after myocardial infarction were randomly divided into 7,14,21 and 28 days group.The sham group was normal control group(eight rats in each group).The expression of endostatin,VEGF and MVD in ischemic myocardium were observed by immunohistochemistry.RESULTS: The expression of endostatin significantly increased in the ischemic myocardium after MI,peaked at 7 days,then gradually decreased at 14,21 and 28 days.The endostatin level at 28 days was the same as the shams.The changing trends of expression of endostatin in ischemic myocardium after MI were similar to that of VEGF and were significantly correlated with the MVD.CONCLUSION: The expression of endostatin increased in ischemic myocardium of myocardial infarction rats.The changing trends of endostatin were similar to that of VEGF and positively correlated with the MVD.These data suggest that endostatin may modulate ischemic myocardium angiogenesis after myocardial infarction.

3.Accuracy of intraocular lens power prediction by using six different formulas

Objective To compare the accuracy of intraocular lens(IOL)power calculations by using 6 formulas(SRK-T,Haigis,Binkhorst-Ⅱ,Hoffer Q,Holladay-1,SRK-Ⅱ)in different axial lengths of the eye,in order to improve the accuracy of predicating IOL powers for patients after cataract surgery.Design Retrospective comparative clinical analysis.Participants 169 patients(169 eyes)with age-related cataract underwent regular phacoemulsification and posterior chamber IOL implantation.Methods 169 eyes with cataract were divided into five groups according to the axial lengths:AXL≤22mm,22mm28.4mm.The axial lengths and keratometry of the 169 eyes were measured with A-mode ultrasound before operation,and the actual postoperative refraction was measured with the methods of computer and retinoscopy optometry when patients could gain best corrected visual acuity one month after surgery or later.Inputting the axial lengths,keratometry and IOL powers into A-mode ultrasound,the predicted postoperative refraction was calculated with SRK-T,Haigis,Binkhorst-Ⅱ,Hoffer Q,Holladay-1,SRK-Ⅱ formulas,and the differences of predicted and actual postoperative refractions in different axial lengths were compared.Main Outcome Measures The absolute value with the differences of the predicted and actual postoperative refractions,i.e.absolute predicted errors.Results(1)Comparing the difference of predicted and actual postoperative refraction,there was no statistical significance between the five formulas in the groups of axial length(AXL)≤22mm,22mm28.4mm,but the average absolute predicted error of Haigis and SRK-T formulas is significantly smaller than SRK-Ⅱand Binkhorst-Ⅱ formulas.Conclusions In existing sample size,there was no obvious influence on postoperative refraction when using other five formulas except Binkhorst-Ⅱ.But in the extremely long AXL group,Haigis and SRK-T formulas have some ascendancy over the other formulas.
4.Strengthen Management for Rinse and Disinfection of Endoscope

OBJECTIVE To analyze the factors influencing endoscope disinfection,in order to take the effective measure,improve endoscope disinfection work and prevent nosocomial infection.METHODS According to Instructions for Technical Procedures of Endoscope Cleaning and Disinfection published by Ministry of Health,42 samples of disinfected endoscopes from 18 000 were randomly selected.RESULTS The work for rinse and disinfection of endoscope in our hospital came to be standardized procedure gradually.We already achieved qualified by 100 percent according to Instructions.CONCLUSIONS The standardized management for rinse and disinfection of endoscope is the assurance for preventing nosocomial infection.

Aim To determine the roles of peripheral ? 1 and ? 2 -adrenoceptors (? 1-AR,? 2-AR) in inhibition of carotid sinus barore ceptor reflex(CSR) induced by intracerebroventricular injection (icv) of histami ne (HA).Methods The left and right carotid sinus regions were i solated from the systemic circulation in 22 male Sprague-Dawley rats anesthetiz ed with pentobarbital sodium.The intracarotid sinus pressure (ISP) was altered in a stepwise ma nner in vivo.ISP-mean arterial pressure (MAP) relationship curve and its ch aracteristic parameters were constructed by fitting to the logistic function wit h five parameters.The changes in CSR performance induced by icv HA and the effec ts of pretreatment with ? 1-AR or ? 2-AR selective antagonist into the per ipheral vein on the responses of CSR to HA were examined.Results icv microinjection of HA (60 ?mol?L -1 in 5 ?l) significantly shifted the ISP-MAP relationship curve upwards (P0.05).Conclusion The intracerebroventricular administration of HA results in a rapid resetting of CSR and a decrease in reflex sensitivity, and the functions of both the peripheral ? 1-AR and ? 2-AR may attenuate CSR resetting induced by icv microinjection of HA. Furthermore,the peripheral ? 1-AR might play an important role in mediating the responses of CSR to central HA.
6.Noninvasive functional evaluation of the left atrium in hypertensive patients by strain rate imaging

Objective To investigate the effects of hypertension on left atrial myocardial properties by strain and strain rate(SR)imaging.Methods A total of 56 consecutive patients with hypertension were divided into 2 groups:33 with normal LVMI and 23 with left ventricular hypertrophy.These patients and 20 controls underwent transthoracic echocardiography,strain and SR imaging examination.Atrial strain and SR values of hypertensive patients were compared with those of controls.Results When compared with the controls,early diastolic SR(ESR)was significantly decreased in hypertensive patients(P<0.01-0.001),with time to peak late diastolic SR corrected for heart rate(TASRe)significantly increased in normal LVMI subgroup(P<0.05).Moreover,late diastolic SR(ASR)were significantly decreased(P<0.05)in hypertensive patients with left ventricular hypertrophy as compared with normal LVMI.Conclusions Enlargement of left atrium leads to increased reservoir function in hypertensive patients,which compensates for the deterioration of conduit function.The duration of atrial systole may be prolonged to meet the need of ventricular filling in consequence of lower left atrial booster pump function.Noninvasive quantification of left atrial function by SR imaging enables evaluation of left atrial dysfunction due to hypertension.
7.Value of serum IL-33 and its soluble receptor ST2 in patients with chronic hepatitis B

Objective To explore the clinical value of serum interleukin(IL)-33 and its soluble receptor ST2(sST2) level in patients with chronic hepatitis B.Methods A total of 65 cases with chronic hepatitis B were recruited into study group,meanwhile 60 healthy persons were enrolled in the control group from January 2014 to October 2016 in our hospital.IL-33,sST2 and alanine aminotransferase(ALT) were detected and compared in the two groups.Results The level of IL-33,sST2 and ALT were significant higher than those of the control group(t=6.542,7.218,6.324;P<0.05).IL-33 and sST2 levels in chronic hepatitis B patients with abnormal ALT level were significant higher than those with normal ALT(t=16.328,9.874,P<0.05).Conclusion The detection of IL-33 and sST2 in patients with chronic hepatitis B could help to understand the immune status of patients,and provide a theoretical basis for the treatment of chronic hepatitis B.
8.Comfort analysis of colostomy waste water using multilayer soft silicone dressings in the treatment of patients with dermatitis

Objective To explore the treatment effect and comfort of colostomy surgery with fecal water der-matitis around stoma after application of multilayer soft silicone dressings.Methods Approved by the Hospital Ethics Committee and the patients'informed consent,120 patients were divided into the hydrocolloid dressings group ( the control group) and multi-layer soft silicone dressing group (the treatment group),60 cases in each group.Before treatment,after treatment 24h and 36h,DET score,GCO score and the treatment effect were evaluated.Results After treated for 24h,the DET,GCO scores and treatment effects ( in the control group,cured in 3 cases,effective 45 cases, in the treatment group 5 patients were cured,effective in 46 cases) had no significant differences between the two groups (t=-2.624,P>0.05,t=-0.429,P>0.05,χ2 =0.519,P>0.05).After treated for 36h,the DET,GCO scores and treatment effects ( in the control group:cured in 29 cases,effective in 25 cases,in the treatment group:51 patients were cured,effective in 9 cases) had significant differences between the two groups ( t=-5.022,P<0.01,t=12.784,P<0.01,χ2 =6.316,P<0.05).Conclusion Colostomy fecal water dermatitis around stoma after application of multilayer soft silicone dressings can significantly improve the patients'comfort.
9.Drug resistance and molecular epidemiological analysis of KPC-producing Escherichia coli strains isolated in Hangzhou

Objective To analyze the epidemiological characteristics of Klebsiella pneumoniae car-bapenemase(KPC)-producing Escherichia coli(E. coli)strains isolated in Hangzhou,China. Methods A total of 25 KPC-producing Escherichia coli strains were collected from four hospitals in Hangzhou from July 2012 to January 2014. Antibiotic susceptibility of the isolates to 22 common antimicrobial agents was deter-mined by using Kirby-Bauer(K-B)disk diffusion method. PCR analysis and gene sequencing were used for bla KPC gene screening. The modified Hodge test was performed to detect the production of carbapenemase. Pulsed-field gel electrophoresis(PFGE)and multi-locus sequence typing(MLST)were used for homology analysis. Results All of the 25 clinical isolates were confirmed to be KPC-producing E. coli strains,harbo-ring the blaKPC-2 gene. These KPC-producing isolates showed high drug resistance rates and were resistant to almost all β-lactam antibiotics. PFGE typing classified the 25 isolates into three main homologous clone groups,including clone group A(4 isolates),clone group B(5 isolates)and clone group C(2 isolates), and some single clones(14 isolates). MLST typing classified the isolates into eight ST types,including ST131(14 isolates),ST167(3 isolates),ST2003(3 isolates),ST410(1 isolate),ST457(1 isolate), ST1463(1 isolate),STnew1(1 isolate)and STnew2(1 isolate). The typing results of PFGE and MLST were consistent with each other. Conclusion The prevalent KPC-producing E. coli strains in Hangzhou, China were ST131 type,which were resistant to multiple antibiotics and had been detected in several hospi-tals. The epidemic of KPC-producing E. coli strain often occurred at some special wards,such as Intensive Care Unit(ICU)and emergency ICU.
10.Effect of Internet+ hand hygiene management model on the hand hygiene compliance of medical staff

Objective To explore the effect of the Internet technology′s application on the hand hygiene compliance of medical staff. Methods Establishing Internet+hand hygiene management model to implement Internet based hand hygiene supervisory and reminding, Internet based hand hygiene publicity and education, and Internet based hand hygiene feedback and communication. This model was applied in Guangdong Provincial Dermatology Hospital to intervene the hand hygiene related activities. Before and after the intervention, the hand hygiene compliance of medical staff in the trial hospital was observed with WHO hand hygiene monitoring table by directly manual observation. Thereafter, the changes of hand hygiene compliance before and after the intervention were compared based on the statistical analysis of test results. Results With the application of Internet+hand hygiene management model, the hand hygiene compliance of medical staff increased from 41.75% (324/776) before the intervention to 72.14% (554/768) after the intervention, and the difference was statistically significant (χ2=145.264, P<0.01). The hand hygiene compliance of both doctors and nurses was significantly increased (doctors: χ2=94.784, P<0.01;nurses:χ2=58.798, P<0.01). In all the five indications, i.e., before patient care, before an aseptic task, after exposure to blood/body fluids, after patient care, and after environmental contact, the hand hygiene compliance was 35.42%(68/192), 63.33%(38/60), 84.38%(54/64), 44.83%(130/290), and 20.00%(34/170) respectively before the intervention;it was 65.38%(102/156), 83.33%(120/144), 95.45%(105/110), 68.52%(148/216), and 55.63%(79/142) after the intervention; and the difference before and after the intervention was statistically significant (χ2=6.305-42.529;P<0.01 or 0.05). Most of the medical staff of the trial department supported this model. Conclusions Internet+ hand hygiene management model could significantly improve the hand hygiene compliance of medical staff and were very popular. And, the model is worth of application in more hospitals.
