1.Observation about the effects of fluid management for patients with severe heart failure under the guidance of PICCO

Objective To guide fluid management for sever heart failure patients by using PICCO indicators, in order to direct clinical fluid management and nursing care. Methods Sixty-four heart failure patients with level IV cardiac function were randomly divided into the control group and the experimental group according to random number table, and each one had 32 patients. Fluid management for patients in the control group was implemented with CVP monitoring technology, while the patients in the experimental group accepted PICCO monitoring technology as fluid management. Then compare these indicators between the two groups--length of stay in ICU, mortality rate of 28 days, daily fluid intake, output and time of achieving negative fluid balance, and observe the change of cardiac function index (CFI) and capacity indicators (ITBVI, GEDVI, EVLWI) in the experiment group before and after treatment. Results Indicators of ITBVI、GEDVI、EVLWI in the experiment group recovered to normal state and CFI improved. The indicators which had mentioned above was (1 203.41±111.08) ml/m2, (1 087.78±66.91) ml/m2, (12.91±3.54) ml/kg, (2.91±0.29) L·min-1·m-2 respectively when before the treatment, while the values after the treatment was (895.50 ± 50.27) ml/m2, (728.19 ± 73.33) ml/m2, (6.51 ± 0.75) ml/kg, (4.61 ± 0.69) L · min-1 · m-2, the difference was significant (t=-18.52-54.42, P<0.05). The length of stay in ICU, mortality rate of 28 days, daily fluid intake, output and time of achieving negative fluid balance of the experimental group were significantly lower than those in the control group(t=-17.19,-76.80,-12.38, χ2=3.26, P<0.05). Conclusions PICCO monitoring indicators are better than CVP method in the aspect of fluid management for patients with sever heart failure, which can increase the rescue success rate, promote the treatment effect, improve prognosis, and promote the rehabilitation of patients.
2.Bone marrow mononuclear cell transplantation for cerebral hemorrhage

BACKGROUND:It has been proved that bone marrow mononuclear cel transplantation can obviously improve neurological function of rats with cerebral hemorrhage. OBJECTIVE:To investigate the effects of transplanted bone marrow mononuclear cel s on the neurological function and apoptosis in perihematomal brain tissues fol owing cerebral hemorrhage in a rat model. METHODS:Twenty-four Sprague-Dawley rats were given stereotaxical injection of col agenase IV into the caudate nucleus to establish cerebral hemorrhage models in transplantation group (n=12) and model group (n=12), and then at 6 hours after cerebral hemorrhage, rats in these two groups were administrated 3x1010/L al ograft bone marrow mononuclear cel s and the same amount of PBS, respectively. Another 12 rats were given no interventions as control group. Neurological functions of rats were assessed at 1, 4, 8, 16 days after cerebral hemorrhage;pathological changes of the injury sites were observed at 16 days after transplantation;neuronal apoptosis rates in the perihematomal brain tissue were detected by flow cytometry at 2 and 4 days after transplantation. RESULTS AND CONCLUSION:The modified neurologic severity scores in the transplantation group were significantly lower than those in the model group at 8 and 16 days after cerebral hemorrhage (P<0.05). In the control group, cel s in each layer arranged closely with complete structure, and neurons and glial cel s were in good shape;in the model group, perihematomal brain tissues were loose with intercel ular gap, in which most neurons and glial cel s became necrotic;in the transplantation group, cel s in each layer arranged closely and regularly, and glial cel proliferation occurred. Besides, compared with the model group, the neuronal apoptosis rate in the transplantation group was significantly lower (P<0.05). To conclude, bone marrow mononuclear cel s can significantly enhance the neurological function recovery and reduce neuronal apoptosis in the brain of cerebral hemorrhage rats.

Migraine is a kind of nerve vascular disease, characterized by hemialgia, moderate and severe, pulsating pain. Acupuncture as a safe and effective analgesic therapy, has been widely used in more than 160 countries and regions around the globe. Treating migraine with acupuncture has certain good features as effective, non-toxic side effects and untoward effect.But its mechanism is still not very clear.The cerebral blood flow study ( CBF study) is currently the most credibility of migraine pathogenesis theory.Now based on the theory of CBF study, this paper discusses the acupuncture treatment of migraine which can provide the basis for clinical treats.
4.Analysis of quality of life of 115 parents with cleft lip and/or palate children.

OBJECTIVETo investigate the quality of life of cleft lip and/or palate children's parents and discuss the factors to provide the oretical basis for improving the quality of life of these parents and promoting the healthy growth of children with cleft lip and/or palate.
METHODSA total of 115 parents whose children had cleft lip and/or palate surgery treatment were selected as the experiment group, and another 198 parents (with healthy children having a similar age with those in the experiment group) as the control group. The experiment group was divided into three subgroups according to different types of cleft lip and/or palate: cleft Lip (CL), cleft palate (CP), cleft lip and palate (CLP). The experiment group and the control group were both divided into four subgroups according to age: 0-1, 1-3, 3-6 years old, and more than 6 years old. The experiment group and the control group were both divided into three subgroups according to education: junior middle school and the following, high school and technical secondary school, junior college degree or above. The GQOLI-74 scale was selected to assess the experiment group and the control group. SPSS 16.0 software was used to analyze data.
RESULTS1) The experiment group had no significant difference with the control group in terms of the overall score and the scores of various children ages. 2) The scores of every item had no significant difference in CL, CP, CLP subgroup (P > 0.05). 3) The quality of life scores and scores of psychological function dimension and social function dimension of parents with 3-6 years old patients were obviously lower than those of parents with more than 6 years old patients (P<0.05). The scores of social function dimension of parents with 0-1, 1-3, 3-6 years old patients were obviously lower than those of parents with more than 6 years old patients (P < 0.05). The other items had no significant difference. 4) The scores of material life dimension and social function dimension of parents with junior college degree or above were higher than those of parents with junior middle school degree and the following (P < 0.05). The scores of social function dimension of parents with high school and technical secondary school degree were higher than those of parents with junior middle school degree and the following (P < 0.05).
CONCLUSIONNo difference was observed in the quality of life between cleft lip and/or palate children's parents and normal group. The parents with the low age children with cleft lip and/or palate and low-levels of education need more help and support to improve quality of life.

5.Cloning and expression of Dengue virus nonstructural protein 4A gene and affinity purification of its interacting proteins

Objective To clone and express Dengue virus nonstructural protein 4A (NS4A) gene and express in eukaryotic cells.Then,to isolate and purify and isolate cellular proteins interacted with NS4A.Methods With specific primers,NS4A gene fragment tagged with FLAG and HA (FLAG-NS4A-HA) was amplified by PCR and cloned into an expression vector,pSG5 vector.Recombinant plasmid was transfected into A549 cells by LipofectAMINETM2000.Transient expression of FLAG-NS4A-HA was detected by Western blot.The NS4A interacting proteins were isolated and purified by tandem affinity purification (TAP) system using HA and FLAG antibodies,and then assayed by silver stained SDS-PAGE.Results Dengue virus NS4A gene tagged with FLAG and HA was successfully constructed into pSG5 vector and expressed in A.549 cells.Silver stained SDS-PAGE showed that the expressed NS4A and two potential interacting proteins that interact with NS4A were isolated after TAP purification and SDS-PAGE.Conclusion Cellular proteins that potentially interacted with Dengue virus NS4A were successfully purified and isolated,which provided a basis for further research.
6.Establishment and application of a tandem affinity purification system of innate immune regulatory protein PKR

Objective To establish a tandem affinity purification(TAP) system of innate immune-regulatory protein PKR and analyze PKR function, for the future screen and identification of novel PKR-interaction proteins. Methods PKR gene was amplified by PCR, and then cloned into a mammalian expression vector pcTAP-A. Recombinant pcTAP-PKR was transfected into PKR knock-down(PKRkd) HeLa cells by LipofectAMINE 2000,and the PKR overexpressed HeLa cells were harvested for mitogen-activated protein kinases(MAPK) activation analysis. Cell extracts of PKR overexpressed cells were purified using TAP kit and examined by Western blot. Results Cal modulin resin(CBP) and streptavidin resin(SBP) tagged PKR was detected in PKRkd HeLa cells as early as 24 h upon transfection with pcTAP-PKR, and its expression decreased at later time points. The overexpression of PKR was autophosphorylated, and thus involved in the regulation of MAPK actviation. After small-scale TAP kit purification, PKR protein was detectable by Western blot. Conclusion We have successfully established a TAP system that over-expresses functional PKR, providing a useful tool for the future study on the identification of PKR interacting proteins.
7.Preparation and Quality Evaluation of Norcantharidin Albumin Nanoparticles

Objective: To prepare norcantharidin albumin nanoparticles and evaluate the physical characteristics of the albumin nanoparticles. Methods:Norcantharidin albumin nanoparticles were prepared by ultra-high pressure microfluidization technology. The average particle size and the drug entrapment efficiency of albumin nanoparticles were used as the evaluation indices. Firstly,Plackett-Burman experimental design was used to screen the formula and process variables which had significant effects on the properties of albu-min nanoparticles,and then Box-Behnken experimental design was used to optimize the variables range. The morphology,particle size distribution,zeta potential and in vitro drug release behavior were investigated. Results:The average particle size of norcantharidin al-bumin nanoparticles was (105.2 ± 30.1) nm,the PdI was 0.127,and the zeta potential was( -24.7 ± 1.9) mV. In 0.5% Tween-80 phosphate buffered saline (pH 7.4),the in vitro cumulative release of norcantharidin albumin nanoparticle suspension reached up to 81.4% in 24 h. Conclusion:The preparation technology of norcantharidin albumin nanoparticles by ultra-high pressure microfluidi-zation technology is simple and feasible. The preparation technology can be used in industrial production.
8.Analysis on clinical therapeutic effects of endoscopic retrograde cholangiopancreatography for acute biliary pancreatitis

Objective To investigate the clinical therapeutic effect of endoscopic retrograde cholangiopancreatography(ERCP) for treating acute biliary pancreatitis (ABP). Methods 220 cases with ABP admitted in General Hospital of Northern Theater Command from January 2016 to January 2019 were enrolled. Patients were divided into endoscopy group (n=107) and non-endoscopy group (control group n=113) based on the different treatments. Blood WBC, amylase (AmyL), TBiL and ALT levels were recorded before, 1 day and 3 days after treatment. Time of abdominal pain and distention alleviation, length of stay and cost of hospitalization were compared between two groups. Results AmyL levels 1 day and 3 days after treatment in endoscopy group was significantly lower than that before treatment [( 197. 45 ± 149. 36 ), (67.39 ±42.30)U/L vs (394.45 ±582.33)U/L], and the difference was statistically different(P<0.05), but AmyL in control group was not greatly different before and after ERCP. ALT level 3 days after treatment in endoscopy and control group was significantly lower than that before treatment [(60. 94 ± 62. 44) U/L vs (162.07 ±62.53) U/L, (46.96 ± 46.75) U/L vs (186.86 ± 89.04) U/L]], and the difference was statistically different(P<0. 05). Tbil level after treatment in endoscopy group was significantly lower than that before treatment[(32. 63 ± 26. 84)μmol/L vs (164. 18 ± 64. 93)μmol/L], and the difference was statistically different(P<0. 05), but Tbil in control group was not greatly different before and after ERCP. Time of abdominal pain and distention relief [(6. 24 ± 1. 65)d vs (10. 00 ± 2. 91)d], length of stay[(8. 53 ± 5. 40)d vs (15. 84 ± 5. 05)d] and cost of hospitalization[(42685. 75 ± 14462. 10) Yuan vs (63246. 29 ± 18040. 02) Yuan] in endoscopy group were obviously decreased compared with those in control group, and the difference was statistically different(P<0. 05). Conclusions The clinical therapeutic effect of ERCP was obvious in the treatment of ABP.
9.Research progress of Fengweiqi from"Qiyao"in Qinling Mountains

Fengweiqi is the whole plant of Rhodiola dumulosa.It is a kind of natural and precious folk medicinal plant,mainly distributed on hillside rocks and crevasses at the altitude of 1 600-4 100 m.Fengweiqi mainly contains flavonoids,volatile oil,polysaccharides,various amino acids and trace elements.Modern pharmacological studies have shown that Fengweiqi has many significant pharmacological activities,such as anti-oxidation,anti-fatigue,anti-hypoxia and bacteriostasis.In this paper,the textual research,chemical constituents,pharmacological actions and artificial cultivation of Fengweiqi were reviewed in order to provide reference for further research and development of Fengweiqi resources.
10.Characteristics of respiratory syncytial virus infections in children with severe acute respiratory tract infections in Huzhou City, Zhejiang Province from 2018 to 2022

ObjectiveTo understand the prevalence of respiratory syncytial virus (RSV) infections among children hospitalized with severe acute respiratory infections (SARI) in Huzhou, Zhejiang Province, and to explore the epidemiological characteristics and influencing factors of RSV. MethodsNasopharyngeal swab samples from children under 15 years of age, newly admitted to the Huzhou First People's Hospital from 2018 to 2022 and who met the SARI surveillance definition, were collected for respiratory virus nucleic acid testing. SPSS 25.0 and Excel 2016 software were used for statistical analysis of the data. ResultsA total of 1 076 samples were tested for nucleic acids, with an overall positivity rate of 11.2%. The positivity rate of infants under 5 years old was relatively high (16.7%). RSV infection showed a seasonal distribution, mainly concentrated in autumn and winter, with the highest positivity rate in December (22.3%). A total of 23 cases of respiratory mixed infection were detected, with a mixed infection rate of 2.13%, among which 16 were RSV-related mixed infections. Logistic regression analysis indicated that season and age were risk factors for RSV infection. ConclusionFrom 2018 to 2022, RSV showed different epidemiological characteristics across different ages and seasons in SARI cases in Huzhou, with significant differences, providing a scientific basis for future prevention of RSV infections.
