1.Experience of Peri-operation Treatment for Senile Diabetic Hip Fracture

[Objective] To investigate the characteristics of senile diabetic hip fracture in the peri-operation stage and to summarize the experience for its treatment. [Methods] A retrospective study was conducted in 21 cases of senile hip fracture complicated with diabetic mellitus which had received operation. [Results] No case was dead and no case was infected after operation; the incision was healed well and the patients went through the peri-operation stage safely. [Conclusion] On the basis of effectively controlling the blood sugar level, operation can be taken into consideration and pre-operative and post-operative treatment should also be emphasized.
2.Regulative Effect of microRNA let-7c on Myotrophin Gene Expression in Rat’s H9c2 Cardiac Myocytes

Objective: To explore weather microRNA let-7c (miR-let-7c) could regulate myotrophin gene expression in rat’s H9c2 cardiac myocytes with possible mechanisms.
Methods: Recombinant plasmid carrying 3′ untranslated region (3′-UTR) of myotrophin and miRNA precursor of let-7c was co-transfected into Hela cells to construct the luciferase reporter system in order to measure luciferase activity. Rat’s H9c2 cardiac myocytes were cultured. The let-7c gene expression was detected by Taqman probe-based real-time PCR after let-7c miRNA precursor or let-7c miRNA inhibitor transfection respectively; protein expressions of myotrophin and nuclear factor-κB (NF-κB) were examined by Western blot analysis.
Results: Luciferase activity examination indicated that compared with recombinant luciferase gene expression carrier (pMIR-MTPN)+miR precursor negative control group, pMIR-MTPN+miR-let-7c miRNA precursor group showed reduced luciferase activity (59.30±9.90) % vs (98.10±15.10) %. Western blot analysis presented that compared with miR negative control group, miR-let-7c precursor group had decreased protein expressions of myotrophin (0.28±0.05) vs (0.90±0.09) and NF-κB (0.25±0.06) vs (0.75±0.07); in contrast, compared with Negative inhibitor group, miR-let-7c inhibitor group had increased protein expressions of myotrophin (1.14±0.09) vs (0.44±0.09) and NF-κB (1.09±0.05) vs (0.71±0.06), allP<0.05.
Conclusion: miR-let-7c could inhibit myotrophin expression via acting on its 3′-UTR domain and may also inlfuence NF-κB signaling pathway in rat’s H9c2 cardiac myocytes.
3.Thoughts of Establishing Comprehensive Therapeutic Protocols for Hypertensive Middle- and Large-Amount Cerebral Hemorrhage

Up to now, advantages and disadvantages in the treatment of hypertensive middle_ and large_amount cerebral hemorrhage (HMLCH) by western medicine or traditional Chinese medicine,or by medical treatment or surgical treatment still exist. Mortality and disablement rate remain high. Western medicine combined with Chinese herbal medicine shows better effect in treating HMLCH, but the related research is preliminary. Therefore, thoughts of establishing comprehensive therapeutic protocols for HMLCH are presented as follows. Western medicine combined with Chinese herbal medicine is the fundamental therapeutic principle. Proper surgical operation should be employed promptly to clear away hemotoma and relieve the symptoms first according to the situation of the patient, hospital and the surgeon. Clearing heat and calming the liver, removing blood stasis and phlegm, relaxing the bowels and restoring consciousness are the fundamental therapeutic methods for HMLCH with yang_excess syndrome, and reinforcing Qi and activating blood circulation, removing phlegm and dredging channels, relaxing the bowels and restoring consciousness for yin_excess syndrome. Routine treatment and symptomatic treatment also should be taken into account.
4. Clinical value of echocardiography in differential diagnosis of segmental wall motion abnormalities in coronary heart disease

Objective:
To explore the clinical value of echocardiography in differential diagnosis of segmental wall motion abnormalities in coronary heart disease(CHD), and to provide help for clinical treatment.
Methods:
From January 2017 to November 2018, 56 patients with segmental abnormal wall motion of CHD in Yuyao People's Hospital of Zhejiang Province were selected in the study.All patients were diagnosed by echocardiography and coronary angiography.The diagnostic coincidence rate, specificity, sensitivity, false negative rate, false positive rate and accuracy of echocardiography were evaluated.
Results:
Coronary angiography showed abnormal segmental wall motion in 56 patients, with 100.00% anastomosis rate.Echocardiography revealed abnormal segmental wall motion in 54 patients with CHD, 2 patients showed normal segmental wall motion, anastomosis rate was 96.43%.There was no statistically significant difference between the two diagnostic methods(χ2=0.635,
5.Efficacy of micro steel plate and Kirschner needle oblique and transverse internal fixation of adjacent metacarpal bone in the treatment of metacarpal diaphyseal oblique fracture.

OBJECTIVE:
To compare the clinical efficacy of micro steel plate and Kirschner needle oblique and transverse internal fixation of adjacent metacarpal bone in the treatment of metacarpal diaphyseal oblique fracture.
METHODS:
Fifty-nine patients with metacarpal diaphyseal oblique fractures admitted between January 2018 and September 2021 were selected as the study subjects and divided into the observation group (29 cases) and the control group (30 cases) based on different internal fixation methods. The observation group was treated with Kirschner wire oblique and transverse internal fixation of adjacent metacarpal bones, while the control group was treated with micro steel plate internal fixation. Postoperative complications, operation time, incision length, fracture healing time, treatment cost, and metacarpophalangeal function were compared between the two groups.
RESULTS:
No incision or Kirschner wire infections occurred in the 59 patients, except for one in the observation group. No fixation loosening, rupture, or loss of fracture reduction occurred in any of the patients. The operation time and incision length in the observation group were (20.5±4.2) min and (1.6±0.2) cm, respectively, which were significantly shorter than those in the control group (30.8±5.6) min and (4.3±0.8) cm (P<0.05). The treatment cost and fracture healing time in the observation group were (3 804.5±300.8) yuan and (7.2±1.1) weeks, respectively, which were significantly lower than those in the control group (9 906.9±860.6) yuan and (9.3±1.7) weeks (P<0.05). The excellent and good rate of metacarpophalangeal joint function in the observation group was significantly higher than that in the control group at 1, 2, and 3 months after operation (P<0.05), but there was no significant difference between the two groups at 6 months after operation (P>0.05).
CONCLUSION
Micro steel plate internal fixation and Kirschner wire oblique and transverse internal fixation of adjacent metacarpal bones are both viable surgical methods for treating metacarpal diaphyseal oblique fractures. However, the latter has the advantages of causing less surgical trauma, shorter operation time, better fracture healing, lower cost of fixation materials, and no need for secondary incision and removal of internal fixation.

6.A retrospective evaluation of the performance of 2 blood culture media in detection of bloodstream infection: clinical comparison of BACTEC and BacT/Alert systems

Objective To evaluate the clinical performance of the BACTEC Plus aerobic,BACTEC Lytic anaerobic,BacT/Alert aerobic and anaerobic blood culture media in detection of bloodstream infections.Methods Retrospective study was conducted.A total of four blood culture bottles from each inpatient with suspected bloodstream infections were collected and analyzed from June 2013 to September 2015 in Peking University People's Hospital.The four bottles,including BACTEC Plus aerobic,BACTEC Lytic anaerobic,BacT/Alert FA aerobic and BacT/Alert FN anaerobic media,and was incubated for 5 days in the BacT/Alert 3D and BACTEC FX instruments,respectively.Time to detection (TTD) and positive rate in detecting bacteria of the two systems were evaluated by Wilcoxon test and Chi-square test.Results Among 2 189 total cultures collected,20 were excluded because of blood shortage and 201 (9.27%) were positive for pathogens.The positive rates of BACTEC Plus aerobic media and BacT/Alert FA aerobic media were 75.3% (140/186) and 69.4% (129/186) (x2 =1.625,P=0.202),respectively.While,the positive rates of BacT/Alert FN anaerobic media and BACTEC Lytic anaerobic media were 81.8% (99/121) and 63.6% (77/121) for total organisms,respectively (x2 =10.083,P =0.001).A significant difference in TTD was detected in BACTEC Plus aerobic media[11.0 (8.0-16.0) h] and BacT/Alert FA aerobic media[13.9 (10.4-18.7) h] (Z =-5.240,P < 0.001).BACTEC Lyric anaerobic media[8.0(7.0-10.0) h] had a shorter TTD (Z =-4.299,P < 0.001) than BacT/Alert FN anaerobic media[11.3(9.3-12.7) h].The positive rates of BACTEC and BacT/Alert system were 74.13% (149/201) and 74.63% (150/201),respectively,compared with taking one set from each system.Conclusions BACTEC media has a shorter TTD and almost the same bacterial recovery,and lower false positive rate than the BacT/ Alert media.
7.Relationship among motivated forgetting,child trauma and depressive symptoms in adolescents with depression

Objective:To compare the motivated forgetting ability between adolescents with depression and normal adolescents and to explore the relationship of this ability with childhood trauma and symptoms of depression.Methods:Totally 141 adolescents diagnosed with depression according to the Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition(DSM-5),and 42 normal controls participated in the study.The directed forgetting(DF)task was employed,using emotional images as memory stimuli,to compare the recognition per-formance between the two groups.The presence of the DF effect was identified when the recognition score under the"to-be-remembered"condition was significantly higher than the"to-be-forgotten"condition.The Childhood Trauma Questionnaire Short Form(CTQ-SF)and Beck Depression Inventory-Ⅱ(BDI-Ⅱ)were utilized to assess the severi-ty of participants'childhood trauma and depression symptoms.Results:Unlike the control group,the adolescents with depression only exhibited the DF effect with positive stimuli(P<0.001).In the depression group,the DF effect value for negative stimuli partially mediated the relationship between CTQ-SF scores and BDI-Ⅱ scores(effect size=0.10,95%confidence interval 0.05-0.17,accounting for 21.3%of the total effect).Conclusion:The motivated forgetting ability is partially impaired in adolescents with depression.The ability,especially regarding negative memories,plays a partial mediating role between childhood trauma and depressive symptoms in adolescents with depression.

9.Re-positive nucleic acid detection in COVID-19 patients after discharge from hospital

Cases of 2019-nCoV nucleic acid and antibody (IgM and IgG total antibody) after discharge from a hospital in Chongqing were continuously monitored. It was found that 5 cases of "re-positive" phenomenon, 5 cases of antibody were positive, and there was a trend of increasing with time. "Re-Positive" may be related to the following three factors. Children with asymptomatic infection had a long time of fecal detoxification.There were two consecutive nucleic acid tests "false negative" caused by various reasons.The virus clearance in patients was not complete, and the discharge standard was not conservative enough. The analysis of the causes of "Re-Positive" patients and the discussion of its infection will help us reveal more characteristics of this virus, and to provide a new basis for the discharge standard in the constantly updated diagnosis and treatment programme.
10.Prevalence of comorbidity of hypertension, diabetes and dyslipidemia and the association between comorbidity and cardiovascular mortality in population aged 40 years and over in Liaoning Province

Objective:To investigate the comorbidity status of hypertension, diabetes, and dyslipidemia (the"three diseases") among residents aged≥40 in Liaoning Province, and to explore the correlation between the comorbidity and cardiovascular disease mortality.Methods:This investigation was a prospective cohort study. From February 2017 to March 2019, a multi-stage stratified cluster random sampling method was used to carry out a baseline survey of 18 758 permanent residents aged≥40 years in Liaoning Province. Demographic information and history of hypertension, diabetes, and dyslipidemia were collected and followed up every year. Death was mainly identified by linkage to the Population Death Information Registration Management System. Cox proportional hazard regression model was used to analyze the association between the comorbidity of the "three diseases" and cardiovascular disease mortality risk.Results:A total of 18 758 residents aged≥40 in Liaoning Province were included, with an age of (60.3±9.9) years and 7 325 males (39.1%). The comorbidity rate of hypertension, diabetes, and dyslipidemia was 6.7% (1 256/18 758), and the standardized prevalence rate was 5.4%. The comorbidity rate increased with age (P<0.001), which was higher in women than in men, and more significant in urban areas than in rural areas (all P<0.001). The comorbidity of "three diseases" accounted for 39.3% (1 256/3 198), 18.7% (1 256/6 710), and 11.8% (1 256/10 653) in patients with diabetes, dyslipidemia, and hypertension, respectively. With a follow-up of (4.3±0.6) years, 463 people died of cardiovascular disease. The mortality rate of cardiovascular disease in the comorbidity of hypertension, diabetes, and dyslipidemia was 8.74/1 000 person-years. After adjusting potential confounders, Cox proportional hazard regression model analysis showed that compared with normal individuals, the hazard ratio of cardiovascular disease mortality in patients with the "three diseases" was 2.55 (95% CI: 1.63-3.99). Conclusion:The prevalence of comorbidity of hypertension, diabetes, and dyslipidemia among residents aged≥40 in Liaoning Province was relatively high, and the risk of cardiovascular disease mortality in patients with the "three diseases" was increased.
