1.ENDOGENOUS EXPRESSION AND HLA STABILIZATION ASSAY OF PLASMODIUM FALCIPARUM CTL EPITOPE MINIGENE IN HUMAN HLA-A2.1 AND HLA-B51 CELLS

Objective. To evaluate the Plasmodium falciparum CTL epitope vaccines in HLA class I allele specific human cell lines that have high frequency among Chinese population. Methods. Synthesized oligonucleotides encoding for P.f. CTL epitope genes, constructed eukaryotic expression plasmids, transfected the minigenes into HLA class I allele specific human cell lines and identified endogenous expressing of the minigenes by RT-PCR and HLA stabilization assay. Results. Two mini-genes encoding Plasmodium falciparum CTL epitopes were designed and cloned, respectively, into an eukaryotic expressing vector to form TR26 which was restricted to HLA-B51, SH6 which was restricted to HLA-A2.1, and TS, which had the two aforementioned mini-genes fused in tandem. All of these CTL epitope genes were transfected and endogenously expressed in respective cell lines containing appropriate HLA molecules. The obviously increased expressions of HLA class I molecules were detected in the transfected cell lines. It was demonstrated that the two discrete Plasmodium falciparum epitope genes were effectively processed and presented, and the close proximity of the two epitope genes in one chain as in mini-gene TS did not interfere with the processing and presenting of each epitope gene in corresponding cell line. Conclusion. A successful expression and presentation of multiple CTL epitope mini-gene in MHC class I allele specific human cell lines were demonstrated by an in vitro assay, which could be corresponding to the vaccination of CTL vaccines in people with different MHC I molecules. This work also suggested the possibility of constructing a multiple CTL epitope plasmodium falciparum DNA vaccine that could cover most of Chinese population.
2.The clinicopathologic characterizations and prognosis in pafients of basal-like breast cancer

Objective To analyze the clinicopathologic characterizations and patients prognosis of basal-like breast cancer. Methods The clinicopathologic eharacterizations,patient 5 year disease-free survival rate and overall survival rate of 25 basal-like breast cancer patients were compared with 34 CerbB2 overexpressing subtype and 37 ER positive subtype breast cancer patients admitted in our hospital in the same period. Results Patients of basal-like breast cancer accounted for 15.7% of all patients admitted.Compared with CerbB2 over-expressing subtype(29.4%)and ER positive subtype(35.1%),basal-like breast cancer patients were eider with 56.0% being≥50 years old(P<0.05).Basal-like subtype breast cancer was larger in tumor size than ER positive subtype(P<0.05),56% of basal-like subtype were of poor-differentiated grade pathologically compared with 18% in CerbB2 overexpression type and 16% in ER positive subtype respectively. Meanwhile, axillary lymph node metastasis was hishbar in CerbB2 overexpressing and ER positive subtype(64.7%,64.9%)than that of basal-like subtype(40.0%).Most visceral metastases were found in basal-like and CerbB2 subtype,but there were more local lymph node and bone metastases in ER positive subtype.Five year overall survoval rate of CerbB2 overexpressing subtype (48.5%)and basal-like subtype breast cancer patients(44.1%)were poorer than that of ER positive subtype(83.8%).respectively;However,there was no difference in 5 year disease-free surviVal rate between the three subtypes(42.7%,40.4%,58.3%,respectively).Conclusions Similar to CerbB2 over-expressing subtype.tlle clinicopathologic characterization and prognosis in basal-like breast cancer are poorer than that in ER positive subtype.
3.Study on the Construction of a Question-Answer Corpus Dataset for Chinese Medical Knowledge Large Language Models

Purpose/Significance To construct a Chinese medical knowledge Q&A corpus dataset as a standardized evaluation bench-mark for large language models(LLMs)in the medical domain,so as to improve the accuracy and efficiency of LLMs in handling Chinese medical questions.Method/Process Chinese medical paper knowledge,medical terminology explanations and supplementary questions are acquired from the Chinese medical licensing examination,and open-source Chinese medical Q&A datasets are encompassed in the developed Q&A datasets.Result/Conclusion The Chinese medical knowledge Q&A corpus datasets enrich the sources of existing datasets and promote the objective and comprehensive quantitative evaluation of large models in the medical field.In the near future,additional data such as electronic medical records and those from online health communities will be used to strengthen the support of artificial intelli-gence for the Healthy China strategy.
4.Diagnosis and treatment of 11 cases of liver injury due to percutaneous nephrolithotripsy

【Objective】 To explore the diagnosis and management of liver injury caused by percutaneous nephrolithotripsy (PCNL), so as to provide reference for the diagnoise and treatment of similar patients. 【Methods】 The clinical data of 926 patients who underwent PCNL during Oct.2017 and Oct.2022 were searched, and the data of those complicated with liver injury were analyzed. 【Results】 A total of 11 cases were collected, including 6 males and 5 females, average age (55.00±13.25)years.All injuries were confirmed with CT.The average decrease of hemoglobin after operation was (14.00±11.97)g/L.One patient needed blood transfusion due to pyonephrosis and multiple operations, and all patients were cured and discharged after delaying the removal of nephrostomy tube [an average of (6.73±1.27)days] . 【Conclusion】 In the absence of obvious signs of peritonitis and hemodynamic stability, conservative treatment of liver injury caused by PCNL is safe and effective.
