Objective: To investigate the effect of astragalus on the serum levels of hepatocyte growth factor (HGF), renal function and the damage of renal morphology in ischemic acute renal failure (IARF). To study the protective mechanism of astragalus in IARF. Methods: The rats were pretreated with astragalus or vehicle for 3 days. IARF was induced by left renal artery clipping for 60 min, after the right kidney was removed. Biochemical parameters for serum HGF,blood urea nitrogen(BUN),creatinine(Cr) were measured and histological analyses of the kidneys were performed by tubular grading. Results: There have no significant difference between the serum HGF levels of I/R controls and the levels observed in sham-operated group (P > 0.05). Serum HGF levels in astragalus treated group was significantly increased than those of I/R control group(P < 0.01) .After 4 h of I/R, serum levels of BUN and Cr were increased in I/ R control rats compare with Sham-operated rats(P < 0.01). Astragalus treatment reduced the levels elevation(P < 0.05). Simultaneously,. I/R-induced severe damage in renal morphology were significantly ameliorated by astragalus treatment(P < 0.01). Conclusions: Astragalus pretreatment can alleviate the damage of renal function in IARF. It is may be one of the mechanisms of astragalus for protecting renal tissue in IARF that astragalus can induce the expression of HGF.

Objective To explore the clinical characteristics of localized peritoneal mesothelioma by the retrospective analysis of the clinical data and its relationship with asbestos exposure.Methods A total of 22 cases with localized peritoneal mesothelioma confirmed by pathological test and they were selected as our subjects in the Central Hospital of Cangzhou from Jan.2007 to Dec.2012.The information of all cases was collected.The incidence,asbestos exposure history,clinical manifestations,imaging studies,pathological type,immunohistochemistry and tumor markers of peritoneal mesotheliom patients were recorded or measured.Results Of 22 cases,female accounted for 68.18％.The periods from onset symptoms to treatment time was from 2 days to 1 year with an average of 83 days.Clinical symptoms were verified including localized abdominal pain (11 cases,50.00％),abdominal mass (8 cases,36.36％),abdominal distension (6 cases,27.27％),ascites (10 cases,45.45％).Patient was with increased platelet and carcinoma antigen 125.Abdominal computerized tomography showed that local mass was seen and 12 cases were with asbestos spot.Ultrasound-guided peritoneal biopsy was confirmed as the main diagnostic method followed by Laparotomy.Epithelial type was the main pathological type (19 cases,86.36％),following the fleshy tumor type and mixed type.Eighteen cases had asbestos exposure history.Conclusion Localized peritoneal mesothelioma is a rare disease.However,the incidence is high in the current region due to asbestos exposure.Abdominal pain and local mass are the main clinical symptoms,and the main pathology is epithelial typeas well as surgery is the main therapy.

Objective To observe the effect of Wenxiao Decoction in treating the patients with carotid atherosclerostic (CAS) plaque and assess the safety. Methods Sixty patients with CAS plaque were randomly allocated to treatment group (33 cases) and control group (27 cases). The control group was given routine therapy, and the treatment group was given Wenxiao Decoction additionally. And they were further divided into half-year treatment group and half-year control group, 1-year treatment group and 1-year control group. Carotid artery color Doppler ultrasound and hepatorenal function were examined before and after treatment. Results Compared with the control groups after therapy, the average numbers of plaques, maximal plaque thickness, Crouse plaque integral were decreased both in 1-year and half-year treatment groups (P<0.05, P<0.01). The plaque stability of 1-year treatment group was better than that of half-year treatment group. The average numbers of plaques, plaques size and Crouse plaque integral increased in control groups after routine therapy (P<0.05, P<0.01). Bilateral common carotid artery diameter, intima-medial thickness (IMT) and hepatorenal function had no statistical difference between before and after treatment both in treatment and control groups (P>0.05). Conclusion Wenxiao Decoction is effective and safe in treatment of CAS plaque, and plaque stability is more obvious with the extension of treatment time.

Objective To investigate the clinical manifestation of convulsive status epilepticus (CSE) associated with the outcome of children contracted this illness in order to make an effort to prevent and treat those children. Method The patients with CSE admitted from October 1998 to October 2008 were prospectively and consecutively enrolled. All the patients met the diagnostic criteria set by International Anti-epilepsy Union (ILAE) in 1993. The prognosis predictors of outcome were analyzed with the Rank sum test, Chi-square analysis and Logistic regression analysis. Results A total of 169 patients with CSE were enrolled. Of them, 92 patients were from rural areas (54.4% ). The primary cause was central nervous system infection, and the minor ones were a reduction of dose or withdrawal of the antiepileptics, and cerebral hemorrhage. The duration of CSE before treatment was longer in rural patients than in urban residents ( P < 0.01) . The duration of CSE before treatment and the total persistence time of CSE was significantly associated with the mortality 14.2% . The source of patients, the duration before treatment, and respiratory depression were independent prognostic predictor variables of CSE (P < 0.05) .Conclusions The rural patients account for the major ratio of convulsive status epilepticus. Central nervous system infection is the primary cause. The duration before treatment and total persistence time of CSE are highly related to the mortality and morbidity.

OBJECTIVE:To systematically review the efficacy and safety of Bushen huoxue herb and celecoxib in the treat-ment of knee osteoarthritis, and provide evidence-based reference for clinical treatment. METHODS:Retrieved from CNKI, CBM,VIP and Wanfang Database,Medline,PubMed and Cochrane Library,randomized controlled trials (RCT) about Bushen huoxue herb(test group)and celecoxib(control group)in the treatment of knee osteoarthritis were collected. Meta-analysis was per-formed by using Rev Man 5.3 software after data extraction and quality evaluation. RESULTS:Totally 15 RCTs were included,in-volving 1 129 patients. Results of Meta-analysis showed,the total effective rate [RR=1.09,95%CI(1.04,1.14),P<0.01] and visu-al ache(VAS)score [MD=-0.49,95%CI(-0.94,-0.03),P<0.05] in test group were significantly higher than control group, the differences were statistically significant;there was no significant difference in knee function score [Lequesne:MD=0.24,95%CI (-1.20,1.68),P>0.05;Lysholm:MD=2.32,95%CI(-1.95,6.58),P>0.05;WOMAC:MD=-2.87,95%CI(-6.38,0.64), P>0.05] and the incidence of adverse reactions in 2 groups[RR=0.49,95%CI(0.22,1.09),P=0.08]. CONCLUSIONS:Bushen huoxue herb shows better efficacy and analgesic effect than celecoxib in the treatment of knee osteoarthritis,and is similar to cele-coxib in terms of improving knee function score and safety.

Objective To investigate the effect of APE1 on differentiation of peripheral blood mononuclear cells into osteoclast-like cells(OCL) which induced by macrophage colony stimulating factor (RANKL) and macrophage colony stimulating factor (M-CSF) .Methods Human peripheral blood mononuclear cells (PBMCs) were collected by density gradient separation ;Constructed APE1 siRNA expression vector Ad5v-APE1 siRNA was used to transfect PBMCs .Tartrate-resistant acid phosphatase (TRAP) method was conducted to identify the cells ,the expression level of APE1 was detected by Western blot ,the mRNA expression levels of Cathepsin K(CK) and V-ATPase were detected by RT-PCR .Results PBMCs transfected with APE1 siRNA had significantly lower protein expression of APE1 than untransfected cells (P< 0 .05) ;PBMCs could differentiate into OCL under the stimulation of RANKL and M-CSF ,the mRNA expression levels of CK and V-ATPase increased ;After APE1 siRNA treatment ,the number of OCL was reduced and the levels of CK and V-ATPase mRNA decreased .Conclusion PBMCs can differentiate into a large number of OCL induced by RANKL and M-CSF ,APE1 siRNA significantly inhibited differentiation of PBMCs into osteoclast-like cells , APE1 may be involved in the regulation of osteoclast-like differentiation process .

Objective To detect mechanism of action mode of Jiuxieling Granules in spleen and kidney yang deficiency ulcerative colitis. Methods The perfusion of Rhei Radix et Rhizoma Decoction plus intramuscular injection of hydrocortisone and combined with TNBS and ethanol enema were employed to establish UC animal model. Ninety rats were randomly divided into blank group, model group, SASP group and Jiuxieling Granules 7 days, 14 days and 21 days groups. All treatment groups received relevant medicine intervention. The levels of IL-1, IL-6, TNF-α, and IFN-γin serum and colon tissue were detected by ELISA. Results Compared with the blank group, the levels of IL-1, IL-6, TNF-α, and IFN-γ in serum and colon tissues of rats in model group increased (P<0.05);compared with the model group, the levels of IL-1, IL-6, TNF-α, and IFN-γin serum and colon tissues of rats in treatment groups were reduced greatly (P<0.05), among which Jiuxieling Granules 21 days group showed the most obvious effects (P<0.05). Conclusion Jiuxieling Granules can regulate the normal secretion of the levels of IL-1, IL-6, TNF-α, and IFN-γin serum and colon tissue of model rats, and inhibit inflammation and protect colonic mucosa.

Objective To evaluate the contrast enhanced perfusion pattern of PTC micro-vascular imaging (MVI) quantitatively.Investigate the correlation between PTC MVI features and CD34 micro-vascular density (MVD).Methods Thirty-nine pathological and clinical confirmed sporadic PTCs were evaluated with real-time gray-scale contrast-ernhanced micro-vascular imaging under a low mechanical index.The micro-bubble agent was SoneVue.Of the 39 PTCs,33 were classical PTCs,6 were PTC with follicular variant (FVPTC).The △ ROI,which is the subtraction of peak echo intensity between the lesion region of interest (ROI) and normal thyroid parenchyma ROI,was used to evaluate the perfusion characteristics of PTC MVI quantitatively.The paraffined specimens were selected for immunohistochemical (IHC) staining for CD3,and the correlation between △ ROI and the CD34 were evaluated.Results △ ROI was strongly correlated with the CD34 expression (P=0.000),significant differences were detected in the distribution pattern of △ ROI value among different CD34 expression levels,no overlapping of the mean △ ROI values and the 95％ confidence intervals was found among the 3 CD34 expression levels.The PTC MVI perfusion was classified into 3 patterns,low perfusion,focal perfiusion and high perfusion,on the basis of combining△ ROI values with the peak ehco pattem in time-intensity curve.Conclusions The △ ROI is strongly correlated with the CD34 expression in papillary thyroid cancer.It can be used for the quantitative evaluation ofPTC MVI pattem and intensity as an objective indicator.

Objective: To learn the mutation types and hearing screening results in local newborns of Zhoushan,in order to provide evidence for prevention and early detection of deafness. Methods: The newborns in Zhoushan Maternal and Child Health Hospital from August 2015 to May 2018 were recruited and detected by matrix-assisted laser desorption ionization time of flight mass spectrometry（MALDI-TOF-MS）for twenty-two mutation sites of GJB2,SLC26A,GJB3 and 12SrRNA genes. The results of genotyping and hearing screening were analyzed and the hearing condition of abnormal newborns was followed up. Results: Among 4 029 newborns,180（4.47%）newborns were identified to carry mutations,including 94 males（4.66%）and 86 females （4.28%）. There was no statistically significant difference in the rate of carrying mutations between male and female infants （P>0.05）. Totally 135 （3.35%）newborns failed in primary hearing screening,13（9.63%）of whom carried the deafness genes;3 894（96.65%）newborns passed,167（4.29%）of whom carried the deafness gene. There was statistically significant difference in the the rate of carrying mutations between newborns who passed and failed in primary hearing screening （P<0.05）. Eleven newborns were diagnosed with hearing loss,with a rate of 2.73‰. Among 180 mutations identified,there were 91 GJB2 mutations（2.26%）,57 SLC26A4 mutations（1.41%）,14 GJB3 mutations （0.35%）,15 mtDNA 12SrRNA mutations （0.37%）and 3 with mutations of two genes （0.07%）. Sixteen mutation sites （184 cases）were found,and the detection rate was 4.57%. Conclusion The rate of carrying deafness genes in Zhoushan newborns was 4.47%. The deafness genes found were mainly GJB2 and SLC26A4,the carrying rate of mtDNA 12SrRNA gene mutation was also high.

Objective To observe the clinical efficacy of Huoxue Bitong Plaster on lumbar osteoarthritis with blood stasis and cold-dampness blocking collaterals syndrome. Methods Totally 120 patients were randomly divided into treatment group (60 cases) and control group (60 cases). Huoxue Bitong Plaster was applied externally in the treatment group, while Goupigao Plaster was used externally in the control group, once a day, for two weeks. The clinical efficacy was evaluated by observing TCM symptom scores, VAS, Oswestry dability index (ODI), and onset time in both groups before and after treatment. Results The total effective rate was 86.67%(52/60) in treatment group, and 85.00% (51/60) in the control group, with no statistical significance (P>0.05). TCM symptom scores of both groups decreased after treatment (P<0.05). For the effect of relieving back pain and morning stiffness, the treatment group was superior to control group (P<0.05). For the effect of relieving waist heaviness, the control group was better than the treatment group (P<0.05). Besides, VAS and ODI of the treatment group were lower than the control group after treatment (P<0.05). The onset time of lightening back pain, morning stiffness in the treatment group was quicker than control group (P<0.05). For waist heaviness, the onset time in control group was quicker than treatment group (P<0.05). Conclusion Huoxue Bitong Plaster has good efficacy for lumbar osteoarthritis with blood stasis and cold-dampness blocking collaterals syndrome.
