1.Adverse Drug Reactions Induced by Antibiotics:Literature Analysis
China Pharmacy 2007;0(32):-
OBJECTIVE:To study the incidence, types, constitution ratio of adverse drug reactions(ADR)induced by antibiotics and the correlation factors of the ADR so as to promote rational drug use in our hospital.METHODS:The literature about antibiotics-induced ADR from Jan.1980 to May 2008 retrieved from CNKI,Wanfang Data and VIP full-text data base was analyzed quantitatively by dividing the literature into different categories:case study or typical case reports, clinical trial or observational reports, and regional ADR monitoring data base or hospital ADR analytical reports.RESULTS:There were 2 310 case studies or typical case reports and in which 3 794 ADR cases were reported, which manifested chiefly as allergic reaction(35.58%),nervous system reaction(17.82%)and disulfiram-like reaction(in 546 cases).928 papers were literature about clinical trial or observational reports and in which 9 434 ADR cases were reported and the average ADR incidence was 9.14%,with anti-tuberculosis drugs showing the highest proportion(25.57%),followed by nitromidazole(24.29%)and carbapenem antibiotics(23.52%);and the ADR manifested chiefly as gastrointestinal tract reaction and allergic reaction, and there were ADR cases manifested as pathoglycemia induced by quinolones. There were 300 papers were literature about regional ADR monitoring data base or hospital ADR analytical reports,in which almost all the major variety of antibiotics were involved, leading the list in terms of ADR cases were quinolones, penicillins and cephalosporins.CONCLUSION:Great importance should be attached to the rational use of antibiotics so as to avoid or reduce ADR incidence and ensure safe and effective use of antibiotics in patients.

Objective To examine the cognitive status and influencing factors of spiritual care among neurosurgery nur-ses,providing a basis for developing a spiritual care management plan for neurosurgery nurses. Methods A convenience sam-pling method was used to select 741 neurosurgical nurses from 35 tertiary hospitals across 14 prefecture-level cities in Guangdong Province from October 2022 to March 2023.A self-developed general information questionnaire and the Chinese version of the Spiritual Care-Giving Scale(C-SCGS)were utilized for the questionnaire survey. Results The average cognitive score for spirit-ual care among neurosurgery nurses was 169.07±27.66.Factors influencing the total score and various dimensions of spiritual care cognition included educational background, receipt of spiritual care education, position, number of night shifts, marital sta-tus,and employment type(P<0.05).Conclusion Neurosurgical nurses'spiritual care cognition is at a moderate level,indi-cating a need for targeted training programs to enhance their understanding and practice of spiritual care based on their status and characteristics.

Objective:To construct a scientifically standardized and promotable pneumonia prevention and nursing plan for acute stroke patients to guide clinical nursing practice. Methods:According to the evidence-based retrieval strategy, we systematically searched the domestic and foreign clinical decision support system, guideline networks, professional association websites and relevant databases. All databases were searched from January 2012 to November 2022. A preliminary pneumonia prevention and nursing plan for acute stroke patients was formulated after literature quality evaluation, evidence extraction, evidence integration and evaluation. The plan was further revised and improved through three rounds of Delphi expert consultation. Results:In the three rounds of expert consultation, the enthusiasm of experts was 100.0%, 100.0%, 83.3%, the authority coefficient of experts were 0.967, 0.967, 0.937, the coefficient of variation were 0.048 to 0.363, 0 to 0.177, 0 to 0.144, and the Kendal harmony coefficient were 0.221, 0.139, 0.339 ( P<0.001) . The final nursing plan included 8 modules, including pneumonia risk prediction, general nursing, swallowing and dietary management, airway management, oral management, position and exercise, condition observation, and health education, with a total of 30 items. Conclusions:The evidence and Delphi method based pneumonia prevention and nursing plan for acute stroke patients is scientific, reliable and applicable, which can provide a basis for nursing practice.

Objective To explore the best time for indwelling gastric tube after tracheal stenosis in infants with congenital heart disease, so as to shorten the indwelling time and relieve the pain of children with tracheal tube. Methods A total of 50 infants with congenital heart disease combined with tracheal stenosis were divided into observation group (26 cases) and control group (24 cases) by the admission order. The observation group accepted swallowing function evaluation combined with physiological indicators to determine the timing of removal of indwelling gastric tube while the control group just conventionally evaluating swallowing function. Results The ICU retention time and total hospitalization days was (4.35 ± 0.94), (23.15 ± 4.92) d in the observation group, and (6.27 ± 1.42), (27.42 ± 6.43) d in the control group, and the difference between the two groups was statistically significant (Z=5.589, 2.621, P<0.05). The extubation time was (2.85 ± 0.23), (4.50 ± 0.27) days in the observation group and the control group, and the difference was statistically significant (χ2= 15.595, P<0.01). Conclusions Deglutition function assessment combined with physiological indicators for evaluating the time of retention of gastric tube after tracheal stenosis in infants with congenital heart disease could be more effective to reduce the occurrence of secondary intubation and postoperative complications, shorten the course of the disease, improve infants comfort level, promote postoperative recovery.
